Contexte: L'évolution démographique est un facteur essentiel pour la planification hospitalière. Le scenario démographique moyen de l'OFS ne répond que partiellement aux exigences de la planification pour le canton du Valais ; il ne permet pas de faire des projections par région hospitalière. De plus, la croissance en 2010 et 2011 selon le scenario OFS est inferieure a celle observée. Nous avons développe un scenario ajuste en tenant compte de l'évolution récente de la population valaisanne, et ce par région hospitalière. Méthode: La population de départ et les taux d'accroissement du au solde naturel et migratoire 2012 ont été déterminés à partir des données réelles 2011. Entre 2002 et 2011, le taux d'accroissement migratoire a fortement varie. Les taux 2011 ont donc été ajustés en fonction des taux moyens d'accroissement migratoire 2002-2011 calcules pour chaque région hospitalière. Pour les années 2013-2025, les écarts de taux d'accroissement naturel et migratoire du scenario OFS ont été repris tel quel. Résultats: Entre 2000 et 2010, la population valaisanne est passée de 276'170 a 312'684 habitants (+13.2%). Entre 2012 et 2025, les deux scenarios prévoient une poursuite de la croissance de la population, mais de manière moins soutenue, particulièrement selon le scenario OFS. Ainsi, en 2020, la population devrait atteindre 348'783 habitants (+11.5%) selon le scenario ajuste et 330'616 (+5.7%) selon le scenario OFS. La part de la population âgée de 65 ans et plus passera de 17.1% en 2010 a 20.5% en 2020 selon le scenario ajuste et a 21.4% selon le scenario OFS. Conclusion: Au vu des limites du scenario moyen de l'OFS, il est pertinent de développer des alternatives tenant compte au mieux des spécificités démographiques cantonales. Le scenario ajuste prédit une croissance plus forte de la population, ce qui a un impact important sur les besoins en soins hospitaliers et la planification hospitalière.

Proton magnetic resonance spectroscopy (1H-MRS) has been used in a number of studies to noninvasively assess the temporal changes of lactate in the activated human brain. However, the results have not been consistent. The aim of the present study was to test the sensitivity of 1H-MRS during functional experiments at the highest magnetic field currently available for human studies (7 T). Stability and reproducibility of the measurements were evaluated from LCModel analysis of time series of spectra measured during a visual stimulation paradigm and by examination of the difference between spectra obtained at rest and during activation. The sensitivity threshold to detect concentration changes was 0.2 micromol/g for most of the quantified metabolites. The possible variations of metabolite concentrations during visual stimulation were within the same range (+/-0.2 micromol/g). In addition, the influence of a small line-narrowing effect due to the blood oxygenation level-dependent (BOLD) T2* changes on the estimated concentrations was simulated. Quantification of metabolites was, in general, not affected beyond 1% by line-width changes within 0.5 Hz.

Studies exploring the effect of calcium supplementation on cardiovascular risk suggest that systolic blood pressure decreases with supplementation. A lower calcium intake has been associated with an increased risk of stroke. By contrast, calcium supplementation may increase the risk of myocardial infarction. The effect of vitamin D supplementation on blood pressure is still unclear and no effect of vitamin D supplementation on coronary heart disease or stroke has been clearly demonstrated. There is a lack of randomized clinical trials primarily addressing the effect of these parameters on CVD. Currently, the use of calcium and vitamin D supplementations for the prevention of cardiovascular disease is not justified.

OBJECTIVE: We examined the analytic validity of reported family history of hypertension and diabetes among siblings in the Seychelles. STUDY DESIGN AND SETTING: Four hundred four siblings from 73 families with at least two hypertensive persons were identified through a national hypertension register. Two gold standards were used prospectively. Sensitivity was the proportion of respondents who indicated the presence of disease in a sibling, given that the sibling reported to be affected (personal history gold standard) or was clinically affected (clinical status gold standard). Specificity was the proportion of respondents who reported an unaffected sibling, given that the sibling reported to be unaffected or was clinically unaffected. Respondents gave information on the disease status in their siblings in approximately two-thirds of instances. RESULTS: When sibling history could be obtained (n=348 for hypertension, n=404 for diabetes), the sensitivity and the specificity of the sibling history were, respectively, 90 and 55% for hypertension, and 61 and 98% for diabetes, using clinical status and, respectively, 89 and 78% for hypertension, and 53 and 98% for diabetes, using personal history. CONCLUSION: The sibling history, when available, is a useful screening test to detect hypertension, but it is less useful to detect diabetes.

Molecular monitoring of BCR/ABL transcripts by real time quantitative reverse transcription PCR (qRT-PCR) is an essential technique for clinical management of patients with BCR/ABL-positive CML and ALL. Though quantitative BCR/ABL assays are performed in hundreds of laboratories worldwide, results among these laboratories cannot be reliably compared due to heterogeneity in test methods, data analysis, reporting, and lack of quantitative standards. Recent efforts towards standardization have been limited in scope. Aliquots of RNA were sent to clinical test centers worldwide in order to evaluate methods and reporting for e1a2, b2a2, and b3a2 transcript levels using their own qRT-PCR assays. Total RNA was isolated from tissue culture cells that expressed each of the different BCR/ABL transcripts. Serial log dilutions were prepared, ranging from 100 to 10-5, in RNA isolated from HL60 cells. Laboratories performed 5 independent qRT-PCR reactions for each sample type at each dilution. In addition, 15 qRT-PCR reactions of the 10-3 b3a2 RNA dilution were run to assess reproducibility within and between laboratories. Participants were asked to run the samples following their standard protocols and to report cycle threshold (Ct), quantitative values for BCR/ABL and housekeeping genes, and ratios of BCR/ABL to housekeeping genes for each sample RNA. Thirty-seven (n=37) participants have submitted qRT-PCR results for analysis (36, 37, and 34 labs generated data for b2a2, b3a2, and e1a2, respectively). The limit of detection for this study was defined as the lowest dilution that a Ct value could be detected for all 5 replicates. For b2a2, 15, 16, 4, and 1 lab(s) showed a limit of detection at the 10-5, 10-4, 10-3, and 10-2 dilutions, respectively. For b3a2, 20, 13, and 4 labs showed a limit of detection at the 10-5, 10-4, and 10-3 dilutions, respectively. For e1a2, 10, 21, 2, and 1 lab(s) showed a limit of detection at the 10-5, 10-4, 10-3, and 10-2 dilutions, respectively. Log %BCR/ABL ratio values provided a method for comparing results between the different laboratories for each BCR/ABL dilution series. Linear regression analysis revealed concordance among the majority of participant data over the 10-1 to 10-4 dilutions. The overall slope values showed comparable results among the majority of b2a2 (mean=0.939; median=0.9627; range (0.399 - 1.1872)), b3a2 (mean=0.925; median=0.922; range (0.625 - 1.140)), and e1a2 (mean=0.897; median=0.909; range (0.5174 - 1.138)) laboratory results (Fig. 1-3)). Thirty-four (n=34) out of the 37 laboratories reported Ct values for all 15 replicates and only those with a complete data set were included in the inter-lab calculations. Eleven laboratories either did not report their copy number data or used other reporting units such as nanograms or cell numbers; therefore, only 26 laboratories were included in the overall analysis of copy numbers. The median copy number was 348.4, with a range from 15.6 to 547,000 copies (approximately a 4.5 log difference); the median intra-lab %CV was 19.2% with a range from 4.2% to 82.6%. While our international performance evaluation using serially diluted RNA samples has reinforced the fact that heterogeneity exists among clinical laboratories, it has also demonstrated that performance within a laboratory is overall very consistent. Accordingly, the availability of defined BCR/ABL RNAs may facilitate the validation of all phases of quantitative BCR/ABL analysis and may be extremely useful as a tool for monitoring assay performance. Ongoing analyses of these materials, along with the development of additional control materials, may solidify consensus around their application in routine laboratory testing and possible integration in worldwide efforts to standardize quantitative BCR/ABL testing.

Resumo:O objetivo deste trabalho foi determinar a concentração de caulim mais eficiente e econômica para a proteção de algodoeiro contra os danos causados pelo bicudo-do-algodoeiro (Anthonomus grandis). Utilizou-se o delineamento experimental de blocos ao acaso, com cinco tratamentos de pulverizações com caulim nas concentrações: 20, 40, 60, 80 e 100 g L-1. Determinaram-se os seguintes parâmetros: percentagem de botões florais com orifícios de oviposição por fêmeas do bicudo; resíduos de caulim depositados sobre as folhas e as brácteas do algodoeiro; média da produção; e receita líquida do algodão, por hectare, a partir da pesagem da pluma de algodão com caroço. As percentagens de botões florais com orifício de oviposição variaram de 13,6 a 30,8%; o resíduo médio de caulim depositado nas folhas, de 0,0010 a 0,0034 mg mm-2, e nas brácteas, de 0,0010 a 0,0034 mg mm-2; a produção variou de 348,1 a 717,8 kg ha-1; e a receita líquida de algodão de R$ 1.033,88 a R$ 2.098,86 por hectare. As concentrações de caulim mais eficientes para a proteção de algodoeiro contra o bicudo são as de 60, 80 e 100 g L-1; no entanto, a de 60 g L-1foi a mais econômica.

BACKGROUND: To ensure vaccines safety, given the weaknesses of the national pharmacovigilance system in Cameroon, there is a need to identify effective interventions that can contribute to improving AEFI reporting. OBJECTIVE: To assess the effect of: (i) sending weekly SMS, or (ii) weekly supervisory visits on AEFI reporting rate during a meningitis immunization campaign conducted in Cameroon in 2012 using the meningitis A conjugate vaccine (MenAfriVac?). METHODS: Health facilities that met the inclusion criteria were randomly assigned to receive: (i) a weekly standardized SMS, (ii) a weekly standardized supervisory visits or (iii) no intervention. The primary outcome was the reported AEFI incidence rate from week 5 to 8 after the immunization campaign. Poisson regression model was used to estimate the effect of interventions after adjusting for health region, type of health facility, type and position of health workers as well as the cumulative number of AEFI reported from weeks 1 to 4. RESULTS: A total of 348 (77.2%) of 451 health facility were included, and 116 assigned to each of three groups. The incidence rate of reported AEFI per 100 health facility per week was 20.0 (15.9-24.1) in the SMS group, 40.2 (34.4-46.0) in supervision group and 13.6 (10.1-16.9) in the control group. Supervision led to a significant increase of AEFI reporting rate compared to SMS [adjusted RR=2.1 (1.6-2.7); p<0.001] and control [RR=2.8(2.1-3.7); p<0.001)] groups. The effect of SMS led to some increase in AEFI reporting rate compared to the control group, but the difference was not statistically significant [RR=1.4(0.8-1.6); p=0.07)]. CONCLUSION: Supervision was more effective than SMS or routine surveillance in improving AEFI reporting rate. It should be part of any AEFI surveillance system. SMS could be useful in improving AEFI reporting rates but strategies need to be found to improve its effectiveness, and thus maximize its benefits.

Resumo: O objetivo deste trabalho foi avaliar o efeito da alimentação suplementar de resíduos de feijão no desempenho produtivo e nos parâmetros sanguíneos de vacas leiteiras em pastejo. Foram utilizadas oito vacas mestiças, com peso médio de 500 kg, distribuídas em duplo quadrado latino 4×4, alimentadas com alimentação suplementar de 134, 240, 348 e 449 g kg-1 de resíduos de feijão na ração concentrada. Os animais receberam a suplementação após a ordenha no período da manhã e da tarde. A inclusão de resíduos de feijão não alterou os pesos corporais médios e os escores corporais; no entanto, reduziu a produção de leite. Os teores de gordura, proteína bruta, lactose e extrato seco total não foram alterados pela inclusão dos resíduos de feijão. A concentração sanguínea de glicose e colesterol não foi alterada pela adição do resíduo antes do fornecimento de ração concentrada e nem quatro e seis horas após essa alimentação. A inclusão dos resíduos de feijão na ração concentrada de vacas leiteiras em pastejo, com produção média diária de 13 kg, reduz a produção de leite. Todavia, a utilização desses resíduos pouco influencia a composição do leite e os parâmetros sanguíneos.
