There is an increased interest in constructing Pre-Cast (PC) Twin and Triple Reinforced Concrete Box (RCB) culverts in Iowa due to the efficiency associated with their production in controlled environment and decrease of the construction time at the culvert sites. The design of the multi-barrel PC culverts is, however, based on guidelines for single-barrel cast-inplace (CIP) culverts despite that the PC and CIP culverts have different geometry. There is scarce information for multiplebarrel RCB culverts in general and even fewer on culverts with straight wingwalls as those designed by Iowa DOT. Overall, the transition from CIP to PC culverts requires additional information for improving the design specifications currently in use. Motivated by the need to fill these gaps, an experimental study was undertaken by IIHR-Hydroscience & Engineering. The goals of the study are to document flow performance curves and head losses at the culvert entrance for a various culvert geometry, flow conditions, and settings. The tests included single-, double- and triple-barrel PC and CIP culverts with two span-to-rise ratios set on mild and steep slopes. The tests also included optimization of the culvert geometry entrance by considering various configurations for the top bevel. The overall conclusion of the study is that by and large the current Iowa DOT design specifications for CIP culverts can be used for multi-barrel PC culvert design. For unsubmerged flow conditions the difference in the hydraulic performance curves and headloss coefficients for PC and CIP culverts are within the experimental uncertainty. Larger differences (specified by the study) are found for submerged conditions when the flow is increasingly constricted at the entrance in the culvert. The observed differentiation is less important for multi-barrel culverts as the influence of the wingwalls decreases with the increase of the number of barrels.

Résumé En suisse, trois inhibiteurs de l'acétylcholinestérase sont disponibles pour le traitement symptomatique de la maladie d'Alzheimer légère à modérée. Il s'agit du donépézil (Aricept®), de la galantamine (Reminyl®) et de la rivastigmine (Exelon®). Leur prescription est maintenant largement répandue, sur la base d'études ayant confirmé leur efficacité et leur tolérance. Le but de la présente étude a été d'évaluer si de tels résultats se retrouvent dans notre hôpital. Nous avons analysé les dossiers hospitaliers et interviewé les proches et les médecins traitants de 103 patients chez qui un tel traitement a été proposé ou initié dans notre centre de gériatrie, entre janvier 2001 et juillet 2003. Nos résultats montrent que le donépézil fut la molécule la plus fréquemment prescrite (50,6% des cas), suivie par la galantamine (44,2%). Après une année de traitement, seulement 54% des patients (n = 51) prenaient encore leur traitement. Une faible majorité des patients (52% selon les médecins traitants, 62% selon les proches) a favorablement répondu au traitement. Nos résultats confirment l'efficacité de ces molécules mais dans une proportion moindre que celle publiée dans la littérature et sans dégager de supériorité d'une molécule sur une autre. Sur le long terme (une année), le pourcentage d'abandon a été élevé et supérieur aux données de la littérature. Nous estimons que le grand âge des patients de notre collectif explique en partie cette discordance de résultats.

Deficits in memory consolidation have been reported in adult patients with epilepsy but, not to our knowledge, in children. We report the long-term follow-up (9 y. o. to 18 y. o.) of a boy who suffered from temporal lobe epilepsy and underwent a left temporal lobectomy with amygdalo-hippocampal resection at the age of 10. He showed an abnormal forgetting rate when trying to encode new information and a significant deficit for retrieving remote episodic memories (when compared with his twin brother), both consistent with a consolidation disorder. His memory condition slightly improved after cessation of the epilepsy, nevertheless did not normalize. No standard memory assessment could pinpoint his memory problem, hence an adapted methodology was needed. We discuss the nature of the memory deficit, its possible causes and its clinical implications.

La présente recherche analyse le concept mental d'identité personnelle et en propose une interprétation critique, qui se développe à partir des cas de troubles psychologiques se manifestant chez certains patients transplantés. Nous pensons que l'étude de ces cas offre la possibilité de repenser, et par là critiquer, certains présupposés qui conditionnent le débat philosophique contemporain sur l'identité personnelle de matrice essentiellement analytique. Nous proposons de ce concept mental une interprétation inusuelle, reposant sur une certaine compréhension de l'être (humain), que nous caractérisons par la formule «identité personnelle transductive ». Cette expression véhicule l'idée d'une «ontologie autre » ; autre que l'ontologie classique qui pense l'être comme déterminité, à l'instar d'un individu substantiel, stable et persistant. Nous nous éloignons ainsi de la conception de la déterminité telle qu'elle se développe depuis l'Antiquité grecque, parce que nous croyons que le changement ou le devenir réside dans la nature des choses. Nous avons ainsi essayé de présenter une interprétation de l'être différente, en empruntant à Gilbert Simondon la notion de « transductivité » -dans le sens d'« individuation en progrès »que cet auteur lui attribue. En comparant les témoignages des patients transplantés avec les cas imaginaires de transplantation de cerveau qui dominent les discussions philosophiques, nous avons montré la nécessité de refonder l'ontologie sur laquelle reposent les théories contemporaines de l'identité personnelle. Cette tentative répond à l'exigence de résoudre et tempérer les apories qui se font jour au sein des conceptions déterministes de la personne -soient-elles réductionnistes (à l'instar des partisans du critère de continuité psychologique et physique) ou non réductionnistes (à l'instar des conceptions simples ou Théories de l'ego basées sur une approche à la première personne, où la personne, comprise comme persistance substantielle d'un sujet, se fonde d'elle-même). Au coeur de notre démarche se trouve la notion d'« image du corps », véritable outil d'unification épistémologique et de renouvellement ontologique. Cette notion s'est révélée extrêmement utile non seulement pour comprendre et démêler les composantes qui interviennent dans le surgissement des troubles psychologiques lors des transplantations d'organes, ainsi que la variabilité des réactions individuelles, mais aussi, plus fondamentalement, pour rendre manifeste l'opération de «transduction perpétuée »qui est à la base de l'ontogenèse de l'être humain en tant que personne, donc du moi. La notion d'image du corps nous a donc permis de mettre en évidence ce que l'expérience de la transplantation d'organes bien comprise pourrait ou devrait apporter à la théorie de l'identité personnelle. C'est ainsi que, par approximation, s'est précisée l'idée d'une identité personnelle transductive, où le moi (la personne), étant interprété comme l'aspect subjectif d'une opération d'«individuation perpétuée », se donne sous le signe de la discordance.

OBJECTIVE: To determine risk of Down syndrome (DS) in multiple relative to singleton pregnancies, and compare prenatal diagnosis rates and pregnancy outcome. DESIGN: Population-based prevalence study based on EUROCAT congenital anomaly registries. SETTING: Eight European countries. POPULATION: 14.8 million births 1990-2009; 2.89% multiple births. METHODS: DS cases included livebirths, fetal deaths from 20 weeks, and terminations of pregnancy for fetal anomaly (TOPFA). Zygosity is inferred from like/unlike sex for birth denominators, and from concordance for DS cases. MAIN OUTCOME MEASURES: Relative risk (RR) of DS per fetus/baby from multiple versus singleton pregnancies and per pregnancy in monozygotic/dizygotic versus singleton pregnancies. Proportion of prenatally diagnosed and pregnancy outcome. STATISTICAL ANALYSIS: Poisson and logistic regression stratified for maternal age, country and time. RESULTS: Overall, the adjusted (adj) RR of DS for fetus/babies from multiple versus singleton pregnancies was 0.58 (95% CI 0.53-0.62), similar for all maternal ages except for mothers over 44, for whom it was considerably lower. In 8.7% of twin pairs affected by DS, both co-twins were diagnosed with the condition. The adjRR of DS for monozygotic versus singleton pregnancies was 0.34 (95% CI 0.25-0.44) and for dizygotic versus singleton pregnancies 1.34 (95% CI 1.23-1.46). DS fetuses from multiple births were less likely to be prenatally diagnosed than singletons (adjOR 0.62 [95% CI 0.50-0.78]) and following diagnosis less likely to be TOPFA (adjOR 0.40 [95% CI 0.27-0.59]). CONCLUSIONS: The risk of DS per fetus/baby is lower in multiple than singleton pregnancies. These estimates can be used for genetic counselling and prenatal screening.

OBJECTIVE: To establish the genetic basis of Landau-Kleffner syndrome (LKS) in a cohort of two discordant monozygotic (MZ) twin pairs and 11 isolated cases. METHODS: We used a multifaceted approach to identify genetic risk factors for LKS. Array comparative genomic hybridization (CGH) was performed using the Agilent 180K array. Whole genome methylation profiling was undertaken in the two discordant twin pairs, three isolated LKS cases, and 12 control samples using the Illumina 27K array. Exome sequencing was undertaken in 13 patients with LKS including two sets of discordant MZ twins. Data were analyzed with respect to novel and rare variants, overlapping genes, variants in reported epilepsy genes, and pathway enrichment. RESULTS: A variant (cG1553A) was found in a single patient in the GRIN2A gene, causing an arginine to histidine change at site 518, a predicted glutamate binding site. Following copy number variation (CNV), methylation, and exome sequencing analysis, no single candidate gene was identified to cause LKS in the remaining cohort. However, a number of interesting additional candidate variants were identified including variants in RELN, BSN, EPHB2, and NID2. SIGNIFICANCE: A single mutation was identified in the GRIN2A gene. This study has identified a number of additional candidate genes including RELN, BSN, EPHB2, and NID2. The 1935 Iowa-Illinois Memorial Bridge is being documented at this time to fulfill the requirements of the Memorandum of Agreement regarding the removal of the Iowa-Illinois Memorial Bridge and the Iowana Farms Milk Company Building for the proposed improvements to Interstate 7 4 in Bettendorf, Iowa, and Moline, Illinois.1 The 1959 twin suspension bridge will be removed as well, but it was determined to be ineligible for the National Register of Historic Places. Discussion of the history of the 1959 twin span is included, however, in the current report as part of the overall history of the Iowa-Illinois Memorial Bridge. Fieldwork for the documentation occurred in November 2009 and October 2010 (Fig. 1). Limitations on photography included limited shoreline access on the Illinois side, making good views of the bridge from the south somewhat challenging. Also, photographs on the bridge deck were not possible because of interstate traffic and prohibitions on pedestrian traffic. Within the last few years, online primary sources have proliferated, along with historical materials regarding the Iowa-Illinois Memorial Bridge. Sources available online for this report included numerous historical photographs, as well as historical Davenport, Iowa, and U.S. newspapers that document the bridge planning and construction. Additional primary source material was found at the University of Iowa Libraries, the State Historical Society of Iowa in Iowa City, the Bettendorf Public Library, the Richardson-Sloane Special Collections Center at the Davenport Public Library, and the Iowa State University Special Collections in Ames.

The 3-year FREEDOM trial assessed the efficacy and safety of 60 mg denosumab every 6 months for the treatment of postmenopausal women with osteoporosis. Participants who completed the FREEDOM trial were eligible to enter an extension to continue the evaluation of denosumab efficacy and safety for up to 10 years. For the extension results presented here, women from the FREEDOM denosumab group had 2 more years of denosumab treatment (long-term group) and those from the FREEDOM placebo group had 2 years of denosumab exposure (cross-over group). We report results for bone turnover markers (BTMs), bone mineral density (BMD), fracture rates, and safety. A total of 4550 women enrolled in the extension (2343 long-term; 2207 cross-over). Reductions in BTMs were maintained (long-term group) or occurred rapidly (cross-over group) following denosumab administration. In the long-term group, lumbar spine and total hip BMD increased further, resulting in 5-year gains of 13.7% and 7.0%, respectively. In the cross-over group, BMD increased at the lumbar spine (7.7%) and total hip (4.0%) during the 2-year denosumab treatment. Yearly fracture incidences for both groups were below rates observed in the FREEDOM placebo group and below rates projected for a "virtual untreated twin" cohort. Adverse events did not increase with long-term denosumab administration. Two adverse events in the cross-over group were adjudicated as consistent with osteonecrosis of the jaw. Five-year denosumab treatment of women with postmenopausal osteoporosis maintained BTM reduction and increased BMD, and was associated with low fracture rates and a favorable risk/benefit profile.

O objetivo deste trabalho foi avaliar a deposição das gotas de pulverização, dotadas de carga elétrica (eletrostática), em comparação à técnica de pulverização convencional em crisântemo, com uso de diferentes pontas de pulverização. O experimento foi conduzido em delineamento inteiramente ao acaso, com oito tratamentos: combinação das pontas TXVK-3, AXI 110015, AXI 12002 TWIN e AXI 11003, com duas técnicas de pulverização (com e sem eletrostática), e quatro repetições. Cada repetição foi representada por 12 plantas, às quais foram afixados papéis do tipo mata-borrão na superfície abaxial e adaxial dos folíolos, e em duas posições da planta: ápice e base. Um corante marcador (Rodamina B) foi pulverizado na proporção de 5 g por 100 L d'água em cada um dos tratamentos. Os depósitos do marcador foram quantificados por fluorometria. As pontas com gotas de menor diâmetro mediano volumétrico (TXVK-3 e AXI 110015) apresentaram maiores depósitos na superfície abaxial da folha, quando se utilizou a pulverização eletrostática. Esse fato não foi observado, quando foram pulverizadas gotas com maior diâmetro mediano volumétrico e desprovidas de carga elétrica, nas diferentes partes da planta.

Anorectal anomalies occurring with other anomalies or as part of syndromes were analyzed to determine how their epidemiological characteristics differed from those of isolated anal anomalies. Almost 15% of cases were chromosomal, monogenic or teratogenic syndromes, whereas the rest were of unknown cause including sequences (9.3%), VACTERL associations (15.4%) and multiple congenital anomalies (MCA) (60.2%). Almost half of babies with MCA had one or two VACTERL anomalies with distribution frequencies that did not differ significantly from those in babies with the full VACTERL association. There were considerable differences in the frequency of the VACTERL association among babies with different types of anorectal anomaly. Babies with anal anomalies occurring with sequences, VACTERL or MCA showed the same sex differences as babies with isolated anal anomalies, namely male predominance in anal atresia without fistula or cloaca, no sex difference in anal atresia with fistula, and female predominance in ectopic anus and congenital anal fistula. These anomalies, however, were associated with significantly lower mean gestational lengths and birth weights, and higher frequencies of fetal death and pregnancy termination than babies with isolated anal anomalies. Twins were more frequent in sequences, VACTERL and MCA than in isolated anomalies, monogenic syndromes or chromosome anomalies. Five cases were conjoined twins, representing 15% of all cases of twin pregnancies with an anal anomaly. Indeterminate sex was more frequent in babies with anal atresias without fistula than in those with fistula. Anal anomalies are defects of blastogenesis attributable to disorders in expression of pattern determining genes. The differential sex involvement in different types of anal anomaly may be manifestations of expression of the HY/SRY genes during blastogenesis or of X-linkage.

On August 3, 2009, a road safety audit was initiated for the intersection of IA 1 and County Road F-67 in Johnson County, Iowa. Due to the high volume of traffic accessing the cheese producing plant (Twin County Dairy, Inc.), a grocery store east of the intersection, and a large Amish community with horse-drawn wagons and carriages frequently sharing the roads with motorized vehicles, this intersection has developed a crash history that concerns the Iowa Department of Transportation (Iowa DOT), Iowa State Patrol, and local agencies. Considering this, Johnson County and the Iowa DOT requested that a road safety audit be conducted to address the safety concerns and recommend possible mitigation strategies.

Meiotic drive has attracted much interest because it concerns the robustness of Mendelian segregation and its genetic and evolutionary stability. We studied chromosomal meiotic drive in the common shrew (Sorex araneus, Insectivora, Mammalia), which exhibits one of the most remarkable chromosomal polymorphisms within mammalian species. The open question of the evolutionary success of metacentric chromosomes (Robertsonian fusions) versus acrocentrics in the common shrew prompted us to test whether a segregation distortion in favor of metacentrics is present in female and/or male meiosis. Performing crosses under controlled laboratory conditions with animals from natural populations, we found a clear trend toward a segregation distortion in favor of metacentrics during male meiosis, two chromosome combinations (gm and jl) being significantly preferred over their acrocentric homologs. Apart for one Robertsonian fusion (hi), this trend was absent in female meiosis. We propose a model based on recombination events between twin acrocentrics to explain the difference in transmission ratios of the same metacentric in different sexes and unequal drive of particular metacentrics in the same sex. Pooled data for female and male meiosis revealed a trend toward stronger segregation distortion for larger metacentrics. This is partially in agreement with the frequency of metacentrics occurring in natural populations of a chromosome race showing a high degree of chromosomal polymorphism.

The objective of this work was to evaluate the genomic behavior of hybrid combinations between elephant grass (Pennisetum purpureum) and pearl millet (P. glaucum). Tetraploid (AAA'B) and pentaploid (AA'A'BB) chromosome races resulting from the backcross of the hexaploid hybrid to its parents elephant grass (A'A'BB) and pearl millet (AA) were analyzed as to chromosome number and DNA content. Genotypes of elephant grass, millet, and triploid and hexaploid induced hybrids were compared. Pentaploid and tetraploid genomic combinations showed high level of mixoploidy, in discordance with the expected somatic chromosome set. The pentaploid chromosome number ranged from 20 to 34, and the tetraploid chromosome number from 16 to 28. Chromosome number variation was higher in pentaploid genomic combinations than in tetraploid, and mixoploidy was observed among hexaploids. Genomic combinations 4x and 5x are mixoploid, and the variation of chromosome number within chromosomal race 5x is greater than in 4x.

BACKGROUND: The geographic distribution of evolutionary lineages and the patterns of gene flow upon secondary contact provide insight into the process of divergence and speciation. We explore the evolutionary history of the common lizard Zootoca vivipara (= Lacerta vivipara) in the Iberian Peninsula and test the role of the Pyrenees and the Cantabrian Mountains in restricting gene flow and driving lineage isolation and divergence. We also assess patterns of introgression among lineages upon secondary contact, and test for the role of high-elevation trans-mountain colonisations in explaining spatial patterns of genetic diversity. We use mtDNA sequence data and genome-wide AFLP loci to reconstruct phylogenetic relationships among lineages, and measure genetic structure RESULTS: The main genetic split in mtDNA corresponds generally to the French and Spanish sides of the Pyrenees as previously reported, in contrast to genome-wide AFLP data, which show a major division between NW Spain and the rest. Both types of markers support the existence of four distinct and geographically congruent genetic groups, which are consistent with major topographic barriers. Both datasets reveal the presence of three independent contact zones between lineages in the Pyrenean region, one in the Basque lowlands, one in the low-elevation mountains of the western Pyrenees, and one in the French side of the central Pyrenees. The latter shows genetic evidence of a recent, high-altitude trans-Pyrenean incursion from Spain into France. CONCLUSIONS: The distribution and age of major lineages is consistent with a Pleistocene origin and a role for both the Pyrenees and the Cantabrian Mountains in driving isolation and differentiation of Z. vivipara lineages at large geographic scales. However, mountain ranges are not always effective barriers to dispersal, and have not prevented a recent high-elevation trans-Pyrenean incursion that has led to asymmetrical introgression among divergent lineages. Cytonuclear discordance in patterns of genetic structure and introgression at contact zones suggests selection may be involved at various scales. Suture zones are important areas for the study of lineage formation and speciation, and our results show that biogeographic barriers can yield markedly different phylogeographic patterns in different vertebrate and invertebrate taxa.

But: L'échauffement local augmente le flux sanguin cutané (SkBF) en induisant une vasodilatation. Cette réponse de la microcirculation dermique est dénommée hyperémie thermique. Dans une précédente étude, nous avons montré qu'un stimulus thermique local appliqué une première fois atténue la réponse hypérémique à un second stimulus appliqué ultérieurement sur le même site cutané, un phénomène que nous appelons désensibilisation. Cependant, d'autres études n'ont pas mis en évidence une désensibilisation dans des conditions similaires. Le but du présent travail est d'explorer les raisons de cette discordance qui pourrait relever d'une différence au niveau de l'instrumentation. Méthode: vingt-huit jeunes hommes en bonne santé sont étudiés. Deux paires de corps de chauffe, l'une faite sur mesure (notre étude) et l'autre produite commercialement (autres groupes d'investigateurs), sont fixées sur l'avant bras. SkBF est mesuré par fluxmétrie laser Doppler ponctuelle (LDF) (780 nm) sur une paire, et par imagerie laser Doppler (LDI) (633 nm) sur l'autre paire. Des palliers de température de 34 à 41 °C sont appliqués pendant 30 minutes et reproduits après deux heures. Résultats : Durant le second échauffement, le plateau SkBF est plus bas que lors du premier stimulus thermique et ce phénomène est observé avec chacune des quatre combinaisons entre différents instruments de mesure du SkBF et de chauffage (p< 0.05 pour chacune des conditions, différence de -9% à -16% de la valeur initiale). Conclusion : La désensibilisation lors d'une hypérémie thermique n'est pas spécifique à des conditions opératoires particulières.
