Objective : To characterize patients with chronic subdural hematoma undergoing surgery and to identify prognostic indicators. Methods : We conducted a retrospective analysis of patients diagnosed with chronic subdural hematoma (CSDH) undergoing surgical treatment. We analyzed: age, period from trauma to diagnostic imaging, pre and postoperative Glasgow coma scale, type of surgery, associated comorbidities, use of postoperative drainage and outpatient treatment. Results : The sample consisted of 176 patients, 126 male and 50 female patients (ratio 2.5 : 1), ages ranged from six months to 97 years, with an average of 59.3 years. CSDH was caused by trauma in 52% of patients, with the time from trauma to imaging averaging 25.05 days; 37.7% were hypertensive patients and 20% had a neurological disease. Eighty-five (48.3%) patients were elderly and altered consciousness was present in 63% of cases. Of the 91 (51.7%) non-elderly patients, 44% presented with headache, altered consciousness occurred in 40% and motor abnormalities in 27.5%. The CSDH was located on the right in 41%, left in 43% and bilaterally in 16% of patients. Conclusion : the change of consciousness was the most common clinical alteration in the elderly and headache in non-elderly. The most associated comorbidity was the arterial hypertension and the most frequent cause, head trauma. The trepanation with two oriffices associated with a closed drainage system was the most used operating, with high efficacy and low complication rate.

Objective: to analyze the epidemiology, clinical features and survival rate of patients undergoing orbital exenteration (OE) in a tertiary referral hospital. Methods : we conducted a retrospective study of all patients undergoing OE at the Hospital das Clínicas, FMUSP between January 2007 and December 2012. We collected data records related to gender, age, origin, length of stay, duration of the disease, other treatments related to the disease, number of procedures outside of the face related to the disease, follow-up and histological diagnosis. Results : we treated 37 patients in the study period. The average survival in one year was 70%, in two years, 66.1%, and 58.3% in three years. There was no significant difference in the one-year survival related to histological diagnosis (p=0.15), days of hospitalization (p=0.17), gender (p=0.43), origin (p=0.78), disease duration (p=0.27) or the number of operations for the tumor (p=0.31). Mortality was higher in elderly patients (p=0.02). The average years of life lost was 33.9 in patients under 60 years, 14.7 in patients in the 61-80 years range and 11.3 in patients over 80 years. Conclusion : the present series of cases is significant in terms of prevalence of orbital exenteration; on the other hand, it shows one of the lowest survival rates in the literature. This suggests an urgent need for improved health care conditions to prevent deforming, radical resections.

Incidentally discovered adrenal masses, or adrenal incidentalomas, have become a common clinical problem owing to wide application of radiologic imaging techniques. This definition encompasses a heterogeneous spectrum of pathologic entities, including primary adrenocortical and medullary tumors, benign or malignant lesions, hormonally active or inactive lesions, metastases, and infections. Once an adrenal mass is detected, the clinician needs to address two crucial questions: is the mass malignant, and is it hormonally active? This article provides an overview of the diagnostic clinical approach and management of the adrenal incidentaloma. Mass size is the most reliable variable to distinguish benign and malignant adrenal masses. Adrenalectomy should be recommended for masses greater than 4.0 cm because of the increased risk of malignancy. Adrenal scintigraphy has proved useful in discriminating between benign and malignant lesions. Finally, fine-needle aspiration biopsy is an important tool in the evaluation of oncological patients and it may be useful in establishing the presence of metastatic disease. The majority of adrenal incidentalomas are non-hypersecretory cortical adenomas but an endocrine evaluation can lead to the identification of a significant number of cases with subclinical Cushing's syndrome (5-15%), pheochromocytoma (1.5-13%) and aldosteronoma (0-7%). The first step of hormonal screening should include an overnight low dose dexamethasone suppression test, the measure of urinary catecholamines or metanephrines, serum potassium and, in hypertensive patients, upright plasma aldosterone/plasma renin activity ratio. Dehydroepiandrosterone sulfate measurement may show evidence of adrenal androgen excess.

Essential hypertension is a disease multifactorially triggered by genetic and environmental factors. The contribution of genetic polymorphisms of the renin-angiotensin-aldosterone system and clinical risk factors to the development of resistant hypertension was evaluated in 90 hypertensive patients and in 115 normotensive controls living in Southwestern Brazil. Genotyping for insertion/deletion of angiotensin-converting enzyme, angiotensinogen M235T, angiotensin II type 1 receptor A1166C, aldosterone synthase C344T, and mineralocorticoid receptor A4582C polymorphisms was performed by PCR, with further restriction analysis when required. The influence of genetic polymorphisms on blood pressure variation was assessed by analysis of the odds ratio, while clinical risk factors were evaluated by logistic regression. Our analysis indicated that individuals who carry alleles 235-T, 1166-A, 344-T, or 4582-C had a significant risk of developing resistant hypertension (P < 0.05). Surprisingly, when we tested individuals who carried the presumed risk genotypes A1166C, C344T, and A4582C we found that these genotypes were not associated with resistant hypertension. However, a gradual increase in the risk to develop resistant hypertension was detected when the 235-MT and TT genotypes were combined with one, two or three of the supposedly more vulnerable genotypes - A1166C (AC/AA), C344T (TC/TT) and A4582C (AC/CC). Analysis of clinical parameters indicated that age, body mass index and gender contribute to blood pressure increase (P < 0.05). These results suggest that unfavorable genetic renin-angiotensin-aldosterone system patterns and clinical risk variables may contribute to increasing the risk for the development of resistant hypertension in a sample of the Brazilian population.

Electrical stimulation of baroreceptor afferents was used in the 1960's in several species, including human beings, for the treatment of refractory hypertension. This approach bypasses the site of baroreceptor mechanosensory transduction. Chronic electrical stimulation of arterial baroreceptors, particularly of the carotid sinus nerve (Hering's nerve), was proposed as an ultimate effort to treat refractory hypertension and angina pectoris due to the limited nature of pharmacological therapy available at that time. Nevertheless, this approach was abandoned in the early 1970's due to technical limitations of implantable devices and to the development of better-tolerated antihypertensive medications. More recently, our laboratory developed the technique of electrical stimulation of the aortic depressor nerve in conscious rats, enabling access to hemodynamic responses without the undesirable effect of anesthesia. In addition, electrical stimulation of the aortic depressor nerve allows assessment of the hemodynamic responses and the sympathovagal balance of the heart in hypertensive rats, which exhibit a well-known decrease in baroreflex sensitivity, usually attributed to baroreceptor ending dysfunction. Recently, there has been renewed interest in using electrical stimulation of the carotid sinus, but not the carotid sinus nerve, to lower blood pressure in conscious hypertensive dogs as well as in hypertensive patients. Notably, previous undesirable technical outcomes associated with electrical stimulation of the carotid sinus nerve observed in the 1960's and 1970's have been overcome. Furthermore, promising data have been recently reported from clinical trials that evaluated the efficacy of carotid sinus stimulation in hypertensive patients with drug resistant hypertension.

The autonomic nervous system plays an important role in physiological and pathological conditions, and has been extensively evaluated by parametric and non-parametric spectral analysis. To compare the results obtained with fast Fourier transform (FFT) and the autoregressive (AR) method, we performed a comprehensive comparative study using data from humans and rats during pharmacological blockade (in rats), a postural test (in humans), and in the hypertensive state (in both humans and rats). Although postural hypotension in humans induced an increase in normalized low-frequency (LFnu) of systolic blood pressure, the increase in the ratio was detected only by AR. In rats, AR and FFT analysis did not agree for LFnu and high frequency (HFnu) under basal conditions and after vagal blockade. The increase in the LF/HF ratio of the pulse interval, induced by methylatropine, was detected only by FFT. In hypertensive patients, changes in LF and HF for systolic blood pressure were observed only by AR; FFT was able to detect the reduction in both blood pressure variance and total power. In hypertensive rats, AR presented different values of variance and total power for systolic blood pressure. Moreover, AR and FFT presented discordant results for LF, LFnu, HF, LF/HF ratio, and total power for pulse interval. We provide evidence for disagreement in 23% of the indices of blood pressure and heart rate variability in humans and 67% discordance in rats when these variables are evaluated by AR and FFT under physiological and pathological conditions. The overall disagreement between AR and FFT in this study was 43%.

La découverte du système des peptides natriurétiques (NP), au début des années 80, fut une découverte majeure qui révéla le rôle endocrinien du cœur. Les connaissances sur la relaxation vasculaire, la diurèse et la natriurèse provoquées par ce système ont évolué vers un niveau de complexité insoupçonné à cette époque. Nous savons à présent que les NP sont impliqués dans plusieurs autres mécanismes dont la prolifération cellulaire, l’apoptose, l’inhibition du système rénine-angiotensine-aldostérone (RAAS) et le métabolisme des adipocytes. Le métabolisme des lipides est maintenant devenu une cible de choix dans la lutte contre l’obésité. Cette condition aux proportions pandémiques est un facteur de risque majeur dans l’apparition de l’hypertension et du syndrome métabolique (MetS). La compréhension des mécanismes et des défauts de la voie des NP pourrait avoir un impact positif sur le contrôle du MetS et de l’hypertension. L’expression du récepteur des peptides natriuretiques de type 1 (NPR1/GCA) est contrôlée par plusieurs agents incluant son propre ligand, le peptide natriurétique de l’oreillette (ANP). La découverte d’une boucle de retro-inhibition, dans les années 90, a été un événement majeur dans le domaine des NP. En effet, suite à une stimulation à l’ANP, le NPR1/GCA peut inhiber l’activité transcriptionnelle de son propre gène par un mécanisme dépendant du cGMP. Notre groupe a identifié un élément cis-régulateur responsable de cette sensibilité au cGMP et mon projet consistait à identifier la ou les protéine(s) liant cet élément de réponse au cGMP (cGMP-RE). Nous avons identifié un clone liant le cGMP-RE en utilisant la technique du simple hybride chez la levure et une banque d’ADN complémentaire (ADNc) de rein humain. Ce clone provient d’un ADNc de 1083-bp dont le gène est localisé sur le chromosome 1 humain (1p33.36) et codant pour une protéine dont la fonction était inconnue jusqu’ici. Nous avons nommé cette nouvelle protéine GREBP en raison de sa fonction de cGMP Response Element Binding Protein. Des essais de liaison à l’ADN ont montré que cette protéine possède une affinité 18 fois plus élevée pour le cGMP-RE que le contrôle, tandis que des expériences de retard sur gel (EMSA) ont confirmé la spécificité des interactions protéine-ADN. De plus, l’immuno-précipitation de la chromatine (ChIP) a prouvé que GREBP lie le cGMP-RE dans des conditions physiologiques. La liaison de GREBP au cGMP-RE inhibe l’expression du gène rapporteur luciférase sous contrôle du promoteur de npr1/gca. L’inhibition de GREBP à l’aide d’ARN interférant active le promoteur de npr1/gca. Dans les cellules NCI-H295R, l’ANP stimule l’expression de grebp de 60% après seulement 3 heures et inhibe l’expression de npr1/gca de 30%. GREBP est une protéine nucléaire surtout exprimée dans le cœur et ayant le facteur eIF3F comme partenaire. Les variations nucléotidiques du gène sont plus fréquentes chez les patients hypertendus que chez des patients normotendus ou hypertendus souffrant de MetS. Nous rapportons ici l’existence d’un gène spécifique à l’humain qui agit comme répresseur transcriptionnel de npr1/gca et potentiellement impliqué dans le développement de l’hypertension.

La sclérose en plaques (SEP) est une maladie inflammatoire du système nerveux central (SNC) caractérisée par une infiltration périvasculaire de cellules mononucléaires, telles que les lymphocytes T CD4+ et CD8+, les lymphocytes B ainsi que les cellules myéloïdes qui comprend les monocytes, les macrophages et les cellules dendritiques (DCs). Ce phénomène d’infiltration est dû à une fragilisation de la barrière hémato-encéphalique (BHE). L’entrée des cellules immunitaires au SNC va mener à la destruction de la gaine de myéline et donc à l’apparition de plaques de démyélinisation. Ainsi, nous avons émis l’hypothèse que la migration des divers sous-types de cellules immunitaires du sang périphérique à travers la BHE est contrôlée par des mécanismes moléculaires distincts et spécifiques à chaque type cellulaire. Afin de répondre à cette hypothèse, quatre différentes études ont été mises sur pieds. En premier lieu, nous démontrons un effet bénéfique des statines sur la BHE en SEP, en diminuant la migration des lymphocytes T et des monocytes, et en diminuant la diffusion de marqueurs moléculaire soluble. Ce phénomène s’opère via la suppression du processus d’isoprenylation, et en empêchant probablement la contraction des cellules endothéliales de la BHE. De plus, nous démontrons que les monocytes qui migrent au SNC en condition inflammé sont en mesures de se différencier en DCs et d’induire une réponse inflammatoire de la part des lymphocytes T CD4+. La migration des monocytes à travers la BHE est contrôlée par une nouvelle molécule d’adhérence nommée Ninjurin-1. Le blocage de Ninjurin-1 conduit à une inhibition spécifique de la migration des monocytes in vitro, ainsi qu’à une amélioration des signes cliniques du modèle animal de la SEP, soit l’encéphalomyélite auto-immune expérimentale (EAE). Finalement, nous démontrons que la migration des lymphocytes T CD8+ au SNC s’effectue via l’intégrine alpha-4. De plus, la majorité des lymphocytes T CD8+ que l’on retrouve dans le liquide céphalo-rachidien de patients SEP, dans le SNC de souris EAE ainsi que dans le SNC de souris infectée au virus de l’hépatite murine portent un phénotype effecteur mémoire. Ces données pourraient expliquer l’émergence de leucoencéphalopathie multifocale progressive observée chez certains patients SEP traités au natalizumab, un anticorps dirigé contre l’intégrine alpha-4. En conclusion, notre étude a permis de démontrer l’importance des monocytes provenant de la périphérie dans le processus inflammatoire prenant part au SNC en SEP. L’inhibition d’entrée de ces cellules pourrait s’avérer bénéfique en SEP tout en permettant l’immuno-surveillance du cerveau, ce que l’anti-alpha-4 intégrine ne permet pas. Les statines pourraient s’avérer une autre option intéressante puisqu’elles agissent sur les processus inflammatoires impliqués dans la SEP.

Introducción: La dismenorrea se presenta como una patología cada vez más frecuente en mujeres de 16-30 años. Dentro de los factores asociados a su presentación, el consumo de tabaco ha revelado resultados contradictorios. El objetivo del presente estudio es explorar la asociación entre el consumo de cigarrillo y la presentación de dismenorrea, y determinar si los trastornos del ánimo y la depresión, alteran dicha asociación. Materiales y métodos: Se realizó un estudio de prevalencia analítica en mujeres de la Universidad del Rosario matriculadas en pregrado durante el primer semestre de 2013, para determinar la asociación entre el consumo de tabaco y la presentación de dismenorrea. En el estudio se tuvieron en cuenta variables tradicionalmente relacionadas con dismenorrea, incluyendo las variables ansiedad y depresión como potenciales variables de confusión. Los registros fueron analizados en el programa Estadístico IBM SPSS Statistics Versión 20.0. Resultados: Se realizaron 538 cuestionarios en total. La edad promedio fue 19.92±2.0 años. La prevalencia de dismenorrea se estimó en 89.3%, la prevalencia de tabaquismo 11.7%. No se encontró una asociación entre dismenorrea y tabaquismo (OR 3.197; IC95% 0.694-14.724). Dentro de las variables analizadas, la depresión y la ansiedad constituyen factores de riesgo independientes para la presentación de dismenorrea con una asociación estadísticamente significativa p=0.026 y p=0.024 respectivamente. El análisis multivariado encuentra como factor determinante en la presentación de dismenorrea, la interacción de depresión y ansiedad controlando por las variables tradicionales p<0.0001. Sin embargo, esta asociación se pierde cuando se analiza en la categoría de dismenorrea severa y gana relevancia el uso de métodos de anticoncepción diferentes a los hormonales, mientras que el hecho de haber iniciado la vida sexual presenta una tendencia limítrofe de riesgo. Conclusiones: No se puede demostrar que el tabaco es un factor asociado a la presentación de dismenorrea. Los trastornos del ánimo y la ansiedad constituyen factores determinantes a la presentación de dismenorrea independientemente de la presencia de otros concomitantes. Las variables de asociación se modifican cuando la variable dependiente se categoriza en su estado más severo. Se necesitan estudios más amplios y detallados para establecer dicha asociación.

Objetivo: Medir y comparar el comportamiento agudo de la presión arterial (TA), la frecuencia cardiaca (FC) y la percepción subjetiva del esfuerzo físico (PSE) durante un circuito de pesas de resistencia variable (CRV) versus ejercicio aeróbico submáximo en cicloergómetro (EA) en hipertensos estados I y II.Métodos: Se evaluaron 21 individuos (8 hombres y 13 mujeres) hipertensos controlados de estados I y II (15 activos y 6 sedentarios, 56±5.9 años). Todos recibieron inducción sobre calentamiento, manejo de escala de PSE, técnica respiratoria y uso de máquinas de pesas durante el ejercicio. Con intervalos de una semana se sometieron a una sesión de CRV en seis estaciones al 50% de 1RM, y a una sesión de EA a intensidades de 70%-80% de la FCmax, en intervalos de una semana. Durante ambas sesiones se realizaron mediciones de FC, PA y PSE.Resultados: Se aplicó una Prueba t pareada para comparar las respuestas a ambos tipos de ejercicio. Se encontró un incremento menor de la FC (p<0.001), PA sistólica (p<0.005) y PSE (p<0.005) durante el CRV. La elevación de la PA diastólica fue mayor con las pesas, pero no significativa (P=0.139). Los sedentarios mostraron incrementos mayores. Entre cada estación de pesas el comportamiento de las variables fue similar (P>0.05).Conclusión: El estudio evidenció que la respuesta cardiovascular aguda y la PSE de hipertensos durante un CRV fueron similares a las observadas con EA. Se observó que la respuesta presora al circuito de pesas fue menor en pacientes con entrenamiento aeróbico previo.

Objectius: a) avaluar la utilitat de la mesura de la pressió arterial a la consulta pel diplomat d'infermeria; b) establir quin és el punt de tall pel valor normal de l'automesura de la pressió arterial al domicili del propi malalt; c) determinar si l'exploració del fons d'ull als malalts hipertensos permet una millor estratificació del risc cardiovascular. Resultats: a) primer objectiu: els valors de la PAC de la infermera són molt similars a l'AMPA i les decisions s'han de prendre en funció d'aquesta mesura. b) segon objectiu: el risc de presentar alguna AOD en els hipertensos recent diagnosticats amb hipertensió clínica aïllada passa de forma progressiva d'una OR de 2.5 per un punt de tall de <125/80 mmHg a una OR de 4.0 pel punt de tall <135/85 mmHg. c) tercer objectiu: un nombre significatiu de pacients canvien de grup de risc quan es consideren els resultats de l'exploració del fons d'ull.

O objetivo deste estudo foi avaliar a utilidade da oftalmoscopia direta realizada por não-oftalmologistas em 400 pacientes hipertensos não diabéticos. Em um estudo transversal, analisou-se a associação entre exame fundoscópico, categorizado em normal e anormal e de acordo com uma classificação de Keith, Wagener e Barker (KW) modificada, com pressão arterial, duração da hipertensão e anormalidades eletrocardiográficas. Associações entre pressão arterial e anormalidades eletrocardiográficas também foram realizadas. As anormalidades de fundo-de-olho foram mais freqüentes em pacientes com pressão arterial diastólica maior que 105 mmHg (P = 0,001), pressão sistólica maior que 180 mmHg (P < 0,001) e com duração da hipertensão maior que 3 anos (P = 0.003). Também houve maior freqüência de anormalidades à fundoscopia em pacientes com estágios mais graves de hipertensão segundo a classificação do V Joint National Committee (V-JNC). Não houve associação entre gravidade da hipertensão e as classes KW 1 e KW 2: 34,5% dos pacientes classificados como KW 1 tinham pressão diastólica acima de 105 mmHg, contra somente 25,3% daqueles classificados como KW 2. Esta categoria também foi mais freqüente em todos os estágios de hipertensão segundo o V-JNC. As anormalidades da classe 3 de KW foram pouco freqüentes, ocorrendo em somente 2,5% dos pacientes da coorte. Em um modelo de regressão logística, o estreitamento arteriolar difuso foi associado com maior pressão diastólica (P = 0,002) e maior idade (P < 0,001). Anormalidades de cruzamentos arteriovenosos foram associados com maior pressão sangüínea sistólica (P = 0,003) e duração da hipertensão maior do que 3 anos (P = 0,024). O valor preditivo positivo de qualquer anormalidade fundoscópica estimar a gravidade da hipertensão foi de 75,20% e o valor preditivo negativo, 41,75%. Observou-se maior proporção de eletrocardiograma anormal (qualquer anormalidade) e de sobrecarga ventricular esquerda em pacientes com pressão diastólica maior que 105 mmHg (P = 0,039 e P = 0,032 respectivamente) e com sistólica maior que 180 mmHg (P = 0,034 e P = 0,001 respectivamente). Anormalidades entre oftalmoscopia e eletrocardiograma não mostraramse associadas. Os resultados deste estudo demostraram que, apesar das anormalidades fundoscópicas terem sido mais freqüentes em hipertensos mais graves, o exame de fundode- olho não proporcionou uma idéia acurada da gravidade da hipertensão na maioria dos pacientes examinados por internistas e cardiologistas e, que a classificação de Keith, Wagener e Barker teve uma aplicação limitada. A observação de um cruzamento arteriovenoso alterado sugere que a pressão sistólica seja elevada. A presença de estreitamento arteriolar difuso é mais freqüente em pacientes mais velhos e naqueles com pressão diastólica elevada.

Introdução: A hipertensão é fator de risco importante para doenças cardiovasculares, mas o controle da pressão arterial é insatisfatório. Um dos motivos para o controle inadequado é a fraca adesão entre os pacientes que recebem antihipertensivos, parcialmente explicada pela ocorrência de eventos adversos. A incidência de eventos adversos chega a 28% em ensaios clínicos, mas a real magnitude do problema na prática assistencial é pouco conhecida. Métodos: Realizou-se um estudo de coorte prospectivamente planejado, acompanhada de 1989 a 2000, no ambulatório de hipertensão arterial do Hospital de Clínicas de Porto Alegre (Divisão de Cardiologia e Farmacologia Clínica do HCPA). Os objetivos foram, determinar a incidência de eventos adversos (EA) relacionadas à terapia anti-hipertensiva, referidos por pacientes hipertensos, descrever os EA mais freqüentes e os fatores de risco para ocorrência de EA. Em cada consulta, os pacientes eram indagados sobre a presença de evento adverso e, no caso de resposta positiva, era aplicada uma lista dirigida a eventos adversos específicos. Resultados: De 1957 pacientes da coorte, 1508 preencheram os critérios de inclusão e foram seguidos por 12,3 ± 12,2 meses (mediana, 10 meses), resultando em 18548 pacientes/mês. Entre todos os pacientes incluídos, 534 (35,4%) apresentaram pelo menos uma queixa de evento adverso durante o acompanhamento, resultando em 28,8 pacientes com EA /1000 pacientes/mês (IC 95% 26,4 a 31,3). Entre os pacientes em tratamento farmacológico (1366), a incidência foi de 31,3 pacientes com EA /1000 pacientes/mês (IC 95% 28,6 a 33,9) e, entre aqueles em uso de monoterapia, de 29,6 pacientes com EA /1000 pacientes / mês ( IC 95% 22,3 a 36,9). Os pacientes em uso de mais de um anti-hipertensivo apresentaram risco relativo bruto para eventos adversos de 2,10 (IC 95% 1,67 a 2,63). Houve associação entre a classe do anti-hipertensivo usado em monoterapia e a ocorrência de eventos adversos em algum momento do seguimento (P < 0,001), os quais foram mais freqüentes com bloqueadores dos canais de cálcio comparados aos tiazídicos e betabloqueadores. Entre as queixas específicas, tontura (P = 0,007), cefaléia (P = 0,003) e problemas sexuais (P= 0,045) foram mais freqüentes no primeiro grupo. Conclusões: O presente estudo descreveu a incidência de eventos adversos em uma coorte de pacientes hipertensos de um ambulatório especializado, confirmando dados de estudos observacionais e ensaios clínicos que indicam que estes são problemas freqüentes. O uso de mais de um anti-hipertensivo aumenta significativamente o risco de eventos adversos e, entre as classes de antihipertensivos usados em monoterapia, os tiazídicos mostram-se os mais seguros.

Objective: To determine the prevalence of auditory manifestations in individuals with hypertension and analyze the association between hearing loss, systemic hypertension and quality of life in hypertensive patients. Method: This was a prospective, observational, case-control study, carried out from June 2010 to December 2013 at the University Hospital Onofre Lopes, in Natal, Brazil, which involved 120 patients of both sexes were analyzed with a diagnosis of hypertension and 120 patients without a diagnosis of hypertension. The audiological function was assessed by tonal and vocal audiometry. The quality of life was defines by the MINICHAL BRASIL questionnaire. Results: The prevalence of hearing loss was high in both groups (82.5 % and 75.8 %, in hypertension group and control, respectively, p=0.003). The sensorineural was the most common type of hearing loss (48.5 %) in hypertension group while conductive hearing loss was predominant (61.5 %) in the control group. There were no difference in the intensity of hearing loss between the groups (p=0,21). The main hearing complaint was hearing loss (51 %), followed by ear pain (14 %). There was worse quality of life in hypertensive individuals with hearing loss (p= 0.0001). Conclusion: Hypertensive individuals showed higher prevalence of auditory events, including hearing loss, sensorineural hearing loss is predominant . Hearing loss is associated with worse quality of life of hypertensive individuals even when these pressure values are within normal limits

Overweight and obesity are defined as abnormal or excessive fat accumulation that may impair health. Obesity significantly increases the risk of developing numerous medical conditions, besides is associated with shortened life span. The objective of this study was to evaluate the anthropometric measurements and lipid profile in adult overweight and obese subjects residents in the state of Rio Grande do Norte. The methodology was the analysis of certain biochemical parameters in serum of individuals and a physical evaluation that consisted of measurements of blood pressure and anthropometric data. In addition, the volunteers of the research answered a questionnaire about personal, social and clinical data. 1741 samples were collected, of which 432 (24,8%) were obese. Of these, 280 were selected to participate in the study, including 118 (42,1%) men and 162 (57,9%) women. Of the remaining 1309 individuals, 731 (55,8%) were overweight. Among them, 179 (24.5%) were analyzed in this study, with 56,4% female. Of the remaining 578 normal individuals, 143 were selected as controls in the research. Amid the obese, overweight and eutrophic, there were respectively 10% vs 5% vs 5,6% of diabetics, 92,1% vs 85,5% vs 68,7% of dyslipidemics and 38,6% vs 22,3% vs 15,6% of hypertensive patients. The degree of obesity presented by the participants was varied, with the majority (72,8%) fall into grade I obese, 22,9% (64) in grade II and only 4,3% (12) in grade III. Mean anthropometric participants showed statistically significant differences between groups eutrophic, overweight and obese on the BMI, WC, WHR and %BF and between sexes in CC, WHR and %BF. As the mean serum biochemical parameters to evaluate the lipid profile showed significant differences between groups eutrophic, overweight and obese, unless the exception of HDL among overweight and obese. The significance level was 5% (p <0.05)
