“Poor teaching performance is tolerated, whereas poor quality in research or substandard patient care is not. It is time to professionalize teaching and education” (Steinert, 2005). A Faculdade de Medicina da Universidade de Coimbra (FMUC) é uma escola médica clássica, sem qualquer tradição na avaliação do ensino e com uma considerável resistência à introdução de mecanismos de auditoria de qualidade. Com o objectivo de contribuir para ultrapassar estas dificuldades, o regente da disciplina de Fisiopatologia, e membro do Departamento de Educação Médica (DEM) da FMUC, aplicou um questionário nessa disciplina. Este questionário foi concebido para cobrir dimensões relevantes na avaliação da qualidade de ensino: objectivos; conteúdos; materiais; métodos de ensino; instrumentos de avaliação; apresentação e preparação da aula. Foi pedido aos alunos que respondessem ao questionário no fim de cada aula teórica e cada aula teórico-prática. Foi recolhido um total de 2401 questionários (aulas teóricas=1210; aulas teórico-práticas= 1191). Os resultados são discutidos em termos das suas implicações para o desenvolvimento do pessoal docente, das políticas da Escola e para a promoção da qualidade de ensino, como uma contribuição tendente ao estabelecimento de um clima institucional que encoraje a liderança educacional, a inovação e a excelência.

Estudo descritivo que teve como objetivo avaliar a atuação do enfermeiro como gestor da Unidade de Terapia Intensiva do Hospital Regional do Baixo Amazonas Dr. Waldemar Pena – HRBA, o qual obteve o selo de Acreditado em Nível 1 em 2012. Para a obtenção dos dados utilizou-se um questionário elaborado com 10 perguntas subjetivas e aplicado aos enfermeiros gerentes que atuam nas Unidades de Terapia Intensiva e CTI do HRBA. Verificou-se nesse estudo que são indivíduos na faixa etária de 25 a 45 anos, com predominância do gênero feminino, com especialização em áreas diversas da enfermagem. Na atuação no setor de gerência, os enfermeiros estabelecem prioridades, direcionando sua equipe para os atendimentos assistenciais complexos aos pacientes, ficando o trabalho burocrático para ser realizado por último. Os entraves relatados são a insatisfação e o desgaste no exercício da função de gerente da unidade devido ao excesso de funcionários sem experiência em UTI, falta de autonomia, dupla jornada de trabalho, excesso de burocracia entre outras situações que se tornam motivos de estresse. Concluiu-se que a atuação do enfermeiro como gerente se respalda na função assistencial ao paciente através de cuidados diretos e indiretos e na organização dos trabalhos burocráticos na Unidade de Terapia Intensiva.

A acreditação hospitalar é um método de avaliação dos recursos institucionais, voluntário e periódico, que busca garantir a qualidade da assistência prestada, através da padronização, organização e segurança nos processos de trabalho. A certificação hospitalar é um reconhecimento da Organização Nacional de Acreditação - ONA que garante alto nível de prestação de serviço em todos os setores de uma organização de saúde, introduzindo a gestão da qualidade como ferramenta permanente de aprimoramento institucional. Destacam-se como uns de seus objetivos, a busca contínua pela qualidade nos cuidados aos pacientes, proporcionando um ambiente livre de riscos para todos aqueles que circulam na instituição de saúde, abrangendo todos os seus serviços e segmentos existentes. O caminho para a implantação bem sucedida de um programa baseado nos padrões exigidos pelo Manual Brasileiro de Acreditação - MBA da ONA passa pela estruturação de um processo educativo permanente e abrangente, que contemple todo o quadro funcional, desde o operacional até a alta administração da instituição, compartilhando princípios, metas e objetivos a serem alcançados. O presente trabalho tem como objetivo analisar cultura de melhoria contínua de gestão da qualidade na perspectiva de profissionais de saúde de um hospital privado em Macapá, sendo classificada como um estudo quanti-qualitativo, de abordagem descritiva exploratória. A pesquisa foi desenvolvido com 198 profissionais de saúde alocados nas seguintes categorias: liderança, equipe multiprofissional, equipe técnica e administrativo/apoio, por meio de aplicação de questionário.

Purpose: Increasing costs of health care, fuelled by demand for high quality, cost-effective healthcare has drove hospitals to streamline their patient care delivery systems. One such systematic approach is the adaptation of Clinical Pathways (CP) as a tool to increase the quality of healthcare delivery. However, most organizations still rely on are paper-based pathway guidelines or specifications, which have limitations in process management and as a result can influence patient safety outcomes. In this paper, we present a method for generating clinical pathways based on organizational semiotics by capturing knowledge from syntactic, semantic and pragmatic to social level. Design/methodology/approach: The proposed modeling approach to generation of CPs adopts organizational semiotics and enables the generation of semantically rich representation of CP knowledge. Semantic Analysis Method (SAM) is applied to explicitly represent the semantics of the concepts, their relationships and patterns of behavior in terms of an ontology chart. Norm Analysis Method (NAM) is adopted to identify and formally specify patterns of behavior and rules that govern the actions identified on the ontology chart. Information collected during semantic and norm analysis is integrated to guide the generation of CPs using best practice represented in BPMN thus enabling the automation of CP. Findings: This research confirms the necessity of taking into consideration social aspects in designing information systems and automating CP. The complexity of healthcare processes can be best tackled by analyzing stakeholders, which we treat as social agents, their goals and patterns of action within the agent network. Originality/value: The current modeling methods describe CPs from a structural aspect comprising activities, properties and interrelationships. However, these methods lack a mechanism to describe possible patterns of human behavior and the conditions under which the behavior will occur. To overcome this weakness, a semiotic approach to generation of clinical pathway is introduced. The CP generated from SAM together with norms will enrich the knowledge representation of the domain through ontology modeling, which allows the recognition of human responsibilities and obligations and more importantly, the ultimate power of decision making in exceptional circumstances.

Multiple subclonal populations of tumor cells can coexist within the same tumor. This intra-tumor heterogeneity will have clinical implications and it is therefore important to identify factors that drive or suppress such heterogeneous tumor progression. Evolutionary biology can provide important insights into this process. In particular, experimental evolution studies of microbial populations, which exist as clonal populations that can diversify into multiple subclones, have revealed important evolutionary processes driving heterogeneity within a population. There are transferrable lessons that can be learnt from these studies that will help us to understand the process of intra-tumor heterogeneity in the clinical setting. In this review, we summarize drivers of microbial diversity that have been identified, such as mutation rate and environmental influences, and discuss how knowledge gained from microbial experimental evolution studies may guide us to identify and understand important selective factors that promote intra-tumor heterogeneity. Furthermore, we discuss how these factors could be used to direct and optimize research efforts to improve patient care, focusing on therapeutic resistance. Finally, we emphasize the need for longitudinal studies to address the impact of these potential tumor heterogeneity-promoting factors on drug resistance, metastatic potential and clinical outcome.

BACKGROUND: Using continuing professional development (CPD) as part of the revalidation of pharmacy professionals has been proposed in the UK but not implemented. We developed a CPD Outcomes Framework (‘the framework’) for scoring CPD records, where the score range was -100 to +150 based on demonstrable relevance and impact of the CPD on practice. OBJECTIVE: This exploratory study aimed to test the outcome of training people to use the framework, through distance-learning material (active intervention), by comparing CPD scores before and after training. SETTING: Pharmacy professionals were recruited in the UK in Reading, Banbury, Southampton, Kingston-upon-Thames and Guildford in 2009. METHOD: We conducted a randomised, double-blinded, parallel-group, before and after study. The control group simply received information on new CPD requirements through the post; the active intervention group also received the framework and associated training. Altogether 48 participants (25 control, 23 active) completed the study. All participants submitted CPD records to the research team before and after receiving the posted resources. The records (n=226) were scored blindly by the researchers using the framework. A subgroup of CPD records (n=96) submitted first (before-stage) and rewritten (after-stage) were analysed separately. MAIN OUTCOME MEASURE: Scores for CPD records received before and after distributing group-dependent material through the post. RESULTS: Using a linear-regression model both analyses found an increase in CPD scores in favour of the active intervention group. For the complete set of records, the effect was a mean difference of 9.9 (95% CI = 0.4 to 19.3), p-value = 0.04. For the subgroup of rewritten records, the effect was a mean difference of 17.3 (95% CI = 5.6 to 28.9), p-value = 0.0048. CONCLUSION: The intervention improved participants’ CPD behaviour. Training pharmacy professionals to use the framework resulted in better CPD activities and CPD records, potentially helpful for revalidation of pharmacy professionals. IMPACT: • Using a bespoke Continuing Professional Development outcomes framework improves the value of pharmacy professionals’ CPD activities and CPD records, with the potential to improve patient care. • The CPD outcomes framework could be helpful to pharmacy professionals internationally who want to improve the quality of their CPD activities and CPD records. • Regulators and officials across Europe and beyond can assess the suitability of the CPD outcomes framework for use in pharmacy CPD and revalidation in their own setting.

Essa pesquisa procurou investigar o processo de composição narrativa pela dupla estagiário-terapeuta/paciente, em uma situação de psicoterapia psicanalítica, a partir do contexto de uma prática supervisionada de estágio em Psicologia Clínica. Participaram da pesquisa duas acadêmicas de Psicologia que realizaram o estágio em um abrigo municipal. O trabalho clínico desenvolvido pelas estagiárias foi acompanhado pela supervisão acadêmica, cuja responsável na época era a pesquisadora. Também participaram dessa pesquisa três meninas de seis, nove e dez anos de idade, acolhidas temporariamente na instituição e em acompanhamento psicoterapêutico pelas estagiárias. Os atendimentos foram realizados uma vez por semana, individualmente, na própria instituição. As estagiárias relataram cada entrevista preliminar realizada com as crianças sob a forma escrita de entrevista dialogada, cujo objetivo é a memorização do desenvolvimento da entrevista. Essa memorização associada às reflexões acerca do estágio produzidas no espaço de supervisão acadêmica formaram as fontes dos dados. Para atingir o objetivo dessa pesquisa, três estudos foram realizados e, em cada um deles, três casos, constituídos por diferentes duplas terapêuticas, foram analisados. Os resultados dos três estudos demonstram, inicialmente, que o discurso elaborado pelas duplas terapêuticas, em cada entrevista preliminar isoladamente, estrutura-se narrativamente porque esse discurso apresenta os dois princípios da narrativa, que são a sucessão e a transformação, como propõe Tzvetan Todorov. A análise conjunta dessas entrevistas denota, entretanto, que as narrativas constituídas nesse processo não podem ser reduzidas a uma lógica de sucessão linear como formula esse autor. A seqüência narrativa é regida pela lógica de causalidade semântica, que é de natureza polifônica, como propõe Paul Ricoeur. As intervenções das estagiárias sob a forma de construções, conforme conceito estabelecido por Freud, mesmo que guiadas pelo princípio da associação livre, são demarcadas, em sua maioria, pela repetição de uma versão já conhecida da história da vida de seu paciente, geralmente àquela que versa sobre o motivo do abrigamento. Assim, essas intervenções, cujo efeito possível seria que o paciente pudesse desconstruir os sentidos dados a priori, reconstruindo novas versões para os acontecimentos de sua vida e, com isso, ocupasse o lugar de autor de sua história, acabam insistindo no trauma. Dessa forma, fica explicitado um dos paradoxos do processo de formação da escuta clínica: o estagiário, ao procurar abrir os sentidos para o seu paciente, construindo junto com ele uma versão possível para a sua história, acaba, muitas vezes, fechando o sentido, construindo uma única versão para os eventos narrados pelo paciente.

This study aimed to understand the typical ideal of the nursing technician about the systematization of nursing care in the light of the theoretical framework of Alfred Schutz. It is a comprehensive phenomenological research, using the theoretical framework of Alfred Schutz. For the unveiling of the phenomenon (the typical ideal of the nursing technician about the systematization of nursing care), the search process was configured from the proposed guiding principles for a research methodology based on the work itself of Schutz held by Zeferino (2010) in his PhD. For data collection, we used the focus group technique, counting on the collaboration of thirteen practical nurses working in a university hospital in Rio Grande do Norte, who responded positively to the inclusion criteria: working in the study hospital, performing care direct to patients. Forty-four subjects showed interest in participating, being held a draw for selection of the research sample, consisting of 14 professionals, one of whom did not attend the gathering of data collection. The focus group, entitled "What I think about the systematization of nursing care", took place on February 15th, 2013, totaling 101 minutes. It was performed according to the Experiential Education Humanescent using building posters as projective technique, from the key question: "What is the systematization of nursing care for you?". In order to understand some of the biographical situation of the participants, a questionnaire was administered to study participants. From the agreement of the subjects, the focus group was recorded and photographed with the cooperation of one reporter and two other employees. We used Microsoft Word 2010 to perform the transcript of the meeting and Microsoft Excel 2010 for synthesizing the results via a spreadsheet. The study followed the ethical and legal principles that govern scientific research on humans, recommended in Resolution nº 196/96, it was approved by Opinion Embodied Ethics Committee in Research of UFRN (Federal University of Rio Grande do Norte), nº 98 424, of August 31th, 2012, CAAE No. 05906912.0.0000.5537. The analysis of the nursing staff speeches, along with the contemplation of their posters and their written descriptions, allowed from the guiding principles of Zeferino (2010), in light of the reference of Alfred Schutz, unveiling the typical ideal of nursing technicians about the systematization of nursing care, passing four themes: typing of the concept of systematization of nursing care; benefits, which resulted in the reasons to believe in the positivity of this working tool; experienced problems, revealing the world of everyday life of nursing professionals, and possibilities for improvement. It was concluded that the nursing technicians are unaware of the systematization of nursing care. However, they typify a very positive perception about the same, especially with regard to improvements that may foster care

Trasnversal study, with the objective of evaluating the accuracy of clinical indicators of nursing diagnosis excessive fluid volume in patients undergoing hemodialysis. The study occurred in two stages, the first consisted of the evaluation of the diagnostic indicators in study; and the second, the diagnostic inference conducted by nurse diagnosticians. The first stage occurred from december 2012 to april 2013, in a University Hospital and a Hemodialysis Clinic in Northeastern of Brazil, with a sample of 100 chronic renal failure patients on hemodialysis. The data were selected through an interview form and a physical examination, organized into spreadsheets and analyzed as to the presence or absence of the indicators of diagnosis excessive fluid volume. In the second step, the spreadsheets were sent to three nurses diagnosticians, who judged the presence or absence of diagnosis in the clientele searched. This step was conducted from july to september 2013. For analysis of the data, we used descriptive and inferential statistics. In the descriptive analysis, we used measures of central tendency and dispersion. In inferential analysis, we used the tests Chi- square, Fisher and prevalence ratios. The accuracy of the clinical indicators pertaining to the diagnosis were measured as to the specificity, sensitivity, predictive values, likelihood ratios and Diagnostic Odds Ratio. Also developed a logistic regression. The results were organized in tables and discussed with literature. This study was approved by the Ethics Committee in Research of the Federal University of Rio Grande do Norte, with Presentation Certificate for Ethics Appreciation nº 08696212.7.0000.5537. The results revealed that the diagnosis studied was present in 82% of patients. The characteristics with prevalence above 50 % that stood out were: azotemia, decreased hematocrit, electrolyte imbalance, intake exceeds output, anxiety, edema, decreased hemoglobin, oliguria and blood pressure changes. Eight defining characteristics were presented statistically significant association with the nursing diagnosis investigated: pulmonary congestion, intake exceeds output, electrolytes imbalance, jugular vein distension, edema, weight gain over short period of time, agitation and adventitious breath sounds. Among these, the 10 characteristics which showed higher prevalence ratios were: edema and weight gain over short period of time. The features with the highest sensitivity were edema, electrolytes imbalance and intake exceeds output and the standing out with greater specificity were: anasarca, weight gain over short period of time, change in respiratory pattern, adventitious breath sounds, pulmonary congestion, agitation and jugular vein distension. The indicators jugular vein distension, electrolytes imbalance, intake exceeds output, increased central venous pressure and edema, together, were identified in the logistic regression model as the most significant predictors. It is concluded that the identification of accurate clinical indicators allow a good prediction of the nursing diagnosis of excessive fluid volume in patients undergoing hemodialysis in order to assist the nurse in the inference process, which will contribute to the success of patient care. In addition, nurses will consider for diagnostic inference not only his clinical experience, but also scientific evidence of the occurrence of excessive fluid volume, contributing to the control of volemia in these patients

Hypertension is a dangerous disease that can cause serious harm to a patient health. In some situations the necessity to control this pressure is even greater, as in surgical procedures and post-surgical patients. To decrease the chances of a complication, it is necessary to reduce blood pressure as soon as possible. Continuous infusion of vasodilators drugs, such as sodium nitroprusside (SNP), rapidly decreased blood pressure in most patients, avoiding major problems. Maintaining the desired blood pressure requires constant monitoring of arterial blood pressure and frequently adjusting the drug infusion rate. Manual control of arterial blood pressure by clinical personnel is very demanding, time consuming and, as a result, sometimes of poor quality. Thus, the aim of this work is the design and implementation of a database of tuned controllers based on patients models, in order to find a suitable PID to be embedded in a Programmable Integrated Circuit (PIC), which has a smaller cost, smaller size and lower power consumption. For best results in controlling the blood pressure and choosing the adequate controller, tuning algorithms, system identification techniques and Smith predictor are used. This work also introduces a monitoring system to assist in detecting anomalies and optimize the process of patient care.

Pesquisa qualitativa orientada pelos referenciais teórico-metodológicos: Interacionismo Simbólico e Grounded Theory para compreender o processo planejamento-implementação da Sistematização da Assistência de Enfermagem (SAE), segundo dois grupos amostrais: enfermeiros e auxiliares/técnicos de enfermagem de um hospital universitário, e desenvolver uma síntese dos modelos teóricos representativos dessas experiências. A saturação teórica configurou-se mediante a análise da 24ª entrevista não diretiva de 12 enfermeiros e de 12 técnicos de enfermagem, lotados em unidades de internação. da análise emergiram dois modelos teóricos, cuja síntese originou o terceiro, intitulado Entre o êxito e a frustração com a operacionalização da SAE: recursos humanos como componente determinante para a visibilidade do enfermeiro no processo de trabalho. Este modelo desvela o déficit de recursos humanos, impulsionando o enfermeiro a realizar uma SAE ilusória, e perpetuando um processo cíclico de sofrimento, por vivenciar a invisibilidade de sua práxis no processo de trabalho.

Este estudo objetivou compreender o significado do trabalho em equipe dos profissionais de reabilitação em anomalias craniofaciais. Realizou-se análise fenomenológica por contemplar a compreensão e interpretação do sentido considerando o sujeito. Entrevistou-se 12 profissionais de diferentes áreas, norteadas pela questão: O que significa para você trabalhar em equipe na reabilitação de anomalias craniofaciais? Resgatando-se os temas: Capacitação para o trabalho, Dificuldade para trabalho em equipe, Relação com paciente e família, Condições de trabalho e A inserção do profissional na equipe. A análise visou refletir o fenômeno engendrando convergências e divergências destacando a explicitação das diferenças e aprendizado contínuo.

O estudo propõe-se a analisar as dificuldades e facilidades dos Agentes Comunitários de Saúde (ACS) frente ao trabalho em equipe. A análise pautou-se na perspectiva hermenêutica-dialética, tendo como referência os princípios do método de interpretação dos sentidos. As dificuldades e facilidades apontadas por eles revelam que trabalhar em equipe demanda relações efetivas, com ênfase na comunicação, respeito e cooperação, sendo as reuniões de equipe estratégia importante para isso. Depreende-se a necessidade de constantes investimentos nas relações entre os membros da equipe.

This study investigated the level of knowledge held by dentists about the possible treatment plan procedures for periodontal ligament injuries after dentoalveolar trauma. A 5-item self-applied questionnaire was prepared with questions referring to the professional profile of the interviewees and to the treatment plan they would propose for periodontal ligament injuries secondary to dentoalveolar trauma. The questionnaires were filled out by 693 dentists attending the 23rd Annual Meeting of the Brazilian Society for Dental Research, and the data obtained were subjected to descriptive analysis. Either the chi-square test or Fisher's exact test was applied to assess associations among variables, at a 5% level of significance. The results revealed that dentists experienced difficulty in establishing a treatment plan for subluxation, and for extrusive, lateral and intrusive luxations. In general, holding a dental specialty degree had no influence on the knowledge about treatment plan procedures for the most severe injuries. It could be concluded that the dentists participating in this study, whether specialists or not, did not have sufficient knowledge to treat most of the periodontal ligament injuries resulting from dentoalveolar trauma adequately.
