A total of 479 diarrhoeic children and 337 children without diarrhoea (controls) less than 5 years old were investigated in a two-year study in the city of S. Luís (MA), with the purpose to determine the incidence, the age distribution and the seasonality of rotaviruses, as well as to establish the severity of the disease in this region between the North and the Northeast of Brazil. rotavirus incidence was highest in children of the 1st. year of life, showing an average of 25% per year among the diarrhoeic patients attending the two main hospitals and three health units at the periphery of the city. It was shown that rotaviruses are significant enteropathogens in children less than 18 months old. Frequency of rotaviruses droped in diarrhoeic patients 18 to 23 months old to only 4%, the same percentage observed in children of the control group. A typical seasonal distribution of rotaviruses was not seen during the two years of study. There was a peak in the incidence of rotaviruses in 1986, during the rainy season, and two peaks in 1987, one in the rainy season and one in the dry season. It was also shown that severity of diarrhoea in rotavirus positive cases was higher than in the negative cases. Rotavirus diarrhoeic patients had more loose stools per day, and higher frequencies of vomiting and fever, resulting more often (> 2 times) in moderate or severe dehydration. Finally, it is concluded that the introduction of immunoprophylaxis may reduce significantly the high mortality rates in early childhood observed in S. Luís.

Background: Mammography is considered the best imaging technique for breast cancer screening, and the radiographer plays an important role in its performance. Therefore, continuing education is critical to improving the performance of these professionals and thus providing better health care services. Objective: Our goal was to develop an e-learning course on breast imaging for radiographers, assessing its efficacy , effectiveness, and user satisfaction. Methods: A stratified randomized controlled trial was performed with radiographers and radiology students who already had mammography training, using pre- and post-knowledge tests, and satisfaction questionnaires. The primary outcome was the improvement in test results (percentage of correct answers), using intention-to-treat and per-protocol analysis. Results: A total of 54 participants were assigned to the intervention (20 students plus 34 radiographers) with 53 controls (19+34). The intervention was completed by 40 participants (11+29), with 4 (2+2) discontinued interventions, and 10 (7+3) lost to follow-up. Differences in the primary outcome were found between intervention and control: 21 versus 4 percentage points (pp), P<.001. Stratified analysis showed effect in radiographers (23 pp vs 4 pp; P=.004) but was unclear in students (18 pp vs 5 pp; P=.098). Nonetheless, differences in students’ posttest results were found (88% vs 63%; P=.003), which were absent in pretest (63% vs 63%; P=.106). The per-protocol analysis showed a higher effect (26 pp vs 2 pp; P<.001), both in students (25 pp vs 3 pp; P=.004) and radiographers (27 pp vs 2 pp; P<.001). Overall, 85% were satisfied with the course, and 88% considered it successful. Conclusions: This e-learning course is effective, especially for radiographers, which highlights the need for continuing education.

Pinworm infection was prospectively studied during one year in 469 children attending three day care centers. Each child was examined at six months intervals using up to three perianal swabs with adhesive tape. Those found infected were treated with mebendazole. At the beginning of the study we found a prevalence of 28% that dropped to 13% and 12% in the following study periods. The reinfection rate was twice the incidence rate in both study periods. We also found a small percentage (10%) of the children reinfected in most or all study periods. There was a high correlation between reinfection and perianal itching. Our results add further knowledge to the epidemiology of intestinal parasites in day care centers.

Drug development represents a highly complex, inefficient and costly process. Over the past decade, the widespread use of nuclear imaging, owing to its functional and molecular nature, has proven to be a determinant in improving the efficiency in selecting the candidate drugs that should either be abandoned or moved forward into clinical trials. This helps not only with the development of safer and effective drugs but also with the shortening of time-to-market. The modern concept and future trends concerning molecular imaging will assumedly be hybrid or multimodality imaging, including combinations between high sensitivity and functional (molecular) modalities with high spatial resolution and morphological techniques.

A 26 year-old female was admitted with abdominal pain, fever and weight loss. The clinical and laboratory investigations led to the diagnosis of paracoccidioidomycosis. Gallium-67 whole body images correlated well with the clinical course of the disease and with the patient's prognosis.

Actualmente a inflamação é considerada uma componente importante na aterosclerose, desde o seu início até à ruptura da placa seguida de trombose e da progressiva obstrução do vaso. A ruptura da cápsula fibrótica da placa expõe factores de tecido presentes no seu núcleo necrótico que induzem o processo inflamatório, promovendo a adesão celular e a coagulação e que conduzem à formação do trombo. Por seu turno, várias citocinas e moléculas de adesão celular contribuem activamente para o desenvolvimento da placa. Em particular a citocina TNF-a e a molécula de adesão intercelular (ICAM-1) poderão ser indicadoras de inflamação enquanto que as formas solúveis de P-selectina e de CD40 ligando (sCD40L) poderão dar a magnitude da activação plaquetária. Neste trabalho foram estudados 17 doentes com enfarte de miocárdio submetidos a angioplastia (grupo AMI) e 16 doentes com confirmação angiográfica de ausência de doença coronária. Os doentes do grupo AMI foram seguidos nas primeiras 24h de evolução do enfarte agudo de miocárdio antes da administração de medicação e da intervenção angiográfica e ao longo do período de recuperação, 2 e 40 dias após enfarte. Foram medidas no soro por imunoensaio as concentrações de TNF-a e das formas solúveis de CD40L, ICAM-1 e P-selectina. Foram observadas variações significativas de sP-selectina relativamente aos controlos. Imediatamente após o enfarte de miocárdio verificou-se um aumento de sP-selectina, seguido de uma descida brusca dos seus níveis às 48h, e de um incremento para valores idênticos aos observados no grupo de controlo ao 40º dia. As variações observadas nas concentrações de sCD40L não foram significativas relativamente aos controlos. No entanto, verificou-se uma tendência de diminuição da concentração até 48h após o enfarte de miocárdio, seguindo-se um aumento que atingiu valores ligeiramente superiores ao do grupo controlo no 40º dia. As concentrações de TNF-a medidas foram sistematicamente superiores às verificadas no grupo controlo, tendo-se ainda observado uma subida gradual desde o enfarte de miocárdio até ao 40º dia, sendo este incremento significativo. Os valores de sICAM-1 não apresentaram quaisquer variações após o enfarte nem relativamente ao grupo controlo. As variações observadas sugerem um papel importante destes marcadores no processo inflamatório e na evolução do enfarte de miocárdio. O aumento brusco da concentração de sP-selectina após o enfarte de miocárdio evidencia a activação plaquetária e trombose. Na evolução do enfarte, e à medida que as variáveis hemodinâmicas retornam a valores estáveis, devido à medicação aplicada, o aumento de sCD40L e TNF-a em circulação pode reflectir o papel destas moléculas na recuperação endotelial e do miocárdio.

Background: Performing magnetic resonance imaging (MRI) on women with gynecological devices is a completely accepted practice. The goal of our review is to assess how safe it is to perform MRI on women using contraceptive implants or devices. Study Design: Literature review, searching in PubMed-Medline/Ovid for the following keywords: magnetic resonance imaging, intrauterine devices, Implanon® and Essure®. Results: Though plastic devices do not represent a contraindication to the use of the technique, those including metallic components have been submitted to several tests, after which they were classified as MR Conditional (devices presenting no risks in MR-specific environments) by the Food and Drug Administration. Thus, the use of MRI can be safely advised to women with this type of device as long as the magnetic resonance equipment is ≤3.0 T. Conclusions: Presently, there is no scientific evidence that contraindicates performing MRI on women with any kind of gynecological device. Therefore, this procedure is safe as long as it is performed under previously tested conditions.

PURPOSE: To describe the anatomy and imaging findings of the prostatic arteries (PAs) on multirow-detector pelvic computed tomographic (CT) angiography and digital subtraction angiography (DSA) before embolization for symptomatic benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: In a retrospective study from May 2010 to June 2011, 75 men (150 pelvic sides) underwent pelvic CT angiography and selective pelvic DSA before PA embolization for BPH. Each pelvic side was evaluated regarding the number of independent PAs and their origin, trajectory, termination, and anastomoses with adjacent arteries. RESULTS: A total of 57% of pelvic sides (n = 86) had only one PA, and 43% (n = 64) had two independent PAs identified (mean PA diameter, 1.6 mm ± 0.3). PAs originated from the internal pudendal artery in 34.1% of pelvic sides (n = 73), from a common trunk with the superior vesical artery in 20.1% (n = 43), from the anterior common gluteal-pudendal trunk in 17.8% (n = 38), from the obturator artery in 12.6% (n = 27), and from a common trunk with rectal branches in 8.4% (n = 18). In 57% of pelvic sides (n = 86), anastomoses to adjacent arteries were documented. There were 30 pelvic sides (20%) with accessory pudendal arteries in close relationship with the PAs. No correlations were found between PA diameter and patient age, prostate volume, or prostate-specific antigen values on multivariate analysis with logistic regression. CONCLUSIONS: PAs have highly variable origins between the left and right sides and between patients, and most frequently arise from the internal pudendal artery.

de imagem em RM, caracterizada pela sua alta resolução espacial e grande sensibilidade a diferenças de susceptibilidade magnética dos tecidos, acentuando as propriedades paramagnéticas de produtos como a desoxihemoglobina, hemossiderina, ferro e cálcio e sendo particularmente útil na avaliação das estruturas venosas. Objectivos: O objectivo deste trabalho é fazer uma breve revisão das aplicações clínicas da sequência SWI em neuropediatria e demonstrar a sua grande utilidade, nomeadamente em comparação as a sequência T2*. Material e Métodos: Os exames foram realizados a crianças com idades compreendidas entre o período neonatal e os 16 anos, internadas ou seguidas em consulta no Hospital Pediátrico D. Estefânia; as imagens SWI foram efectuadas em equipamento Siemens 1.5 T, Avanto, com os seguintes parâmetros: TR 49, TE 40, flip angle 15, espessura 1,6mm. Resultados: Apresentamos vários casos ilustrativos de patologias em que o SWI demonstra a sua utilidade e mais-valia, nomeadamente na detecção de lesões hemorrágicas recentes ou antigas em diferentes contextos particulares em neuropediatria (patologia hipoxico-isquémica, vascular, trauma não acidental), detecção de cavernomas e anomalias venosas de desenvolvimento, avaliação de tumores e doenças neurodegenerativas. Conclusão: A sequência SWI é bastante útil na avaliação imagiológica de várias patologias e variantes venosas em neuropediatria, fornecendo uma informação adicional com implicações diagnósticas e prognósticas comparativamente com o T2*, obviando também a administração de contraste para avaliação de estruturas venosas.

Temporomandibular disorders (TMD) consist of a group of pathologies that affect the masticatory muscles, temporomandibular joints (TMJ), and/or related structures. String instrumentalists, like many orchestra musicians, can spend hours with head postures that may influence the biomechanical behavior of the TMJ and the muscles of the craniocervicomandibular complex (CCMC). The adoption of abnormal postures acquired during performance by musicians can lead to muscular hyperactivity of the head and cervical muscles, with the possible appearance of TMD. Medical infrared thermography is a non-invasive procedure that can monitor the changes in the superficial tissue related to blood circulation and may serve as a complement to the clinical examination. The objective of this study was to use infrared thermography to evaluate, in one subject, the cutaneous thermal changes adjacent to the CCMC that occur before, during, and after playing a string instrument.
