Mayaro virus (MAYV) is an arbovirus (Togaviridae: Alphavirus) enzootic in tropical South America and maintained in a sylvan cycle involving wild vertebrates and Haemagogus mosquitoes. MAYV cases occur sporadically in persons with a history of recent activities inside or around forests. This paper reports three cases of MAYV fever detected in men infected in Camapuã, MS, Brazil. Serum samples collected at four days and two months after the onset of the symptoms and examined by hemagglutination inhibition test, revealed monotypic seroconversion to MAYV. Isolation of the virus was obtained from one of the samples by inoculation of the first blood samples into newborn mice. A suspension of the infected mouse brain was inoculated into C6/36 cells culture and the virus was identified by indirect immunofluorescent assay with alphavirus polyclonal antibodies. RT-PCR, performed with RNA extracted from the supernatant of C6/36 infected cells in the presence of alphavirus generic primers as well as specific MAYV primers, confirmed these results. The reported cases illustrate the importance of laboratory confirmation in establishing a correct diagnosis. Clinical symptoms are not always indicative of a disease caused by an arbovirus. Also MAYV causes febrile illness, which may be mistaken for dengue.

A Via Verde do Acidente Vascular Cerebral (AVC) tem como objectivo o tratamento rápido dos doentes com AVC isquémico com terapêuticas de repermeabilização, a fim de diminuir o grau de incapacidade por sequelas neurológicas Existem, contudo, critérios rígidos de selecção dos candidatos que beneficiam de terapêuticas de fase aguda o que origina a exclusão de um grande número de doentes. Estes doentes devem realizar de forma célere um estudo neurovascular de forma a permitir uma estratificação de risco e uma orientação terapêutica adequada. No nosso Centro, este estudo neurovascular tem sido feito frequentemente no Serviço de Urgência. Apresentação e discussão do resultado dos exames neurovasculares, neste caso Exame Ultrassonográfico dos Grandes Vasos do Pescoço (ECODVP), realizado no Serviço de Urgência do Hospital de São José - Centro Hospitalar Lisboa Central, pelo Laboratório de Neurossonologia – Unidade Cerebrovascular, durante o ano de 2011. Análise retrospectiva de todos os doentes que realizaram ECODVP no Serviço de Urgência durante o período de 1 de Janeiro a 31 de Dezembro de 2011. Estes exames foram executados com recurso a um Ecógrafo Toshiba com sonda linear (7-14MHz). Os exames foram classificados em “Normal”, “Estenose Carotídea ou Vertebral > 50%”, “Trombo na Carótida Interna”, “Dissecção Carotídea ou vertebral”, “Oclusão Alta da Carótida Interna” e “Oclusão Proximal da Carótida Interna”. Foram avaliados 164 indivíduos (93 sexo masculino e 71 sexo feminino) com uma média de idades = 64,40 anos. Dos 164 indivíduos avaliados foram documentados 134 exames Normais correspondendo a 82% do total de exames. Dos restantes 18%, 30 tinham as seguintes alterações: Estenoses Carotídeas ou Vertebrais (> 50%): 17; Trombo Carotídeo: 2; Dissecções Carotídeas ou Vertebrais: 3; Oclusão Alta da Carótida Interna: 6; Oclusão Proximal da Carótida Interna: 2. A realização de exames Neurovasculares no Serviço de Urgência do Hospital de São José traduz um serviço de qualidade, permitindo redireccionar e encaminhar os doentes, adequando as devidas medidas diagnósticas e terapêuticas a instituir.

A Declaração de Bolonha (1999) obrigou a mudanças várias, reconfigurando os modelos formativos no espaço europeu do ensino superior, até 2010. A partir de 2006, em Portugal, com a criação e adequação dos cursos superiores existentes ao modelo de Bolonha, verificou-se uma generalizada redução da duração média dos diferentes ciclos de estudo e a definição de competências gerais e específicas para os cursos e estudantes. Reflecte-se sobre a importância da literacia da informação, conceito evolutivo e abrangente, que se pode traduzir, sumariamente, em saber quando e porquê se tem uma necessidade informacional, onde encontrar a informação, como avaliá-la, usá-la e comunicá-la de forma ética, incluindo as competências tecnológicas, definição que se inscreve na interdisciplinar Ciência da Informação e no comportamento informacional. Destaca-se a vantagem de uma formação para a literacia da informação no ensino superior, a qual contribuirá, certamente, para dotar os estudantes das referidas competências e melhorá-las. Defende-se a necessidade de uma desejável inter-acção entre múltiplos agentes educativos, com destaque para a trilogia estudantes, bibliotecários e professores, sendo os primeiros encarados como protagonistas activos das suas aprendizagens e devendo ser dotados de competências de literacia da informação, factor determinante para o seu sucesso. Quanto ao Bibliotecário, dotado de novas competências, entre as quais as tecnológicas, deve ser um facilitador do processo de formação para a literacia - preferencialmente integrada num projecto pedagógico e no currículo - articulando a sua acção educativa com estudantes e docentes. Corroborando a extensão educativa das Bibliotecas e aliando-a ao uso inevitável das novas tecnologias da informação e comunicação, sublinha-se o papel das Bibliotecas Digitais, que podem corresponder eficientemente aos anseios dos utilizadores no acesso a uma informação de qualidade, de forma cómoda, rápida, a baixo custo, com personalização dos serviços online, com inter-acção e socialização, através de ferramentas de edição colaborativa, típicas da Web 2.0.

Rocio virus (ROCV) was responsible for an explosive encephalitis epidemic in the 1970s affecting about 1,000 residents of 20 coastland counties in São Paulo State, Brazil. ROCV was first isolated in 1975 from the cerebellum of a fatal human case of encephalitis. Clinical manifestations of the illness are similar to those described for St. Louis encephalitis. ROCV shows intense antigenic cross-reactivity with Japanese encephalitis complex (JEC) viruses, particularly with Ilheus (ILHV), St. Louis encephalitis, Murray Valley and West Nile viruses. In this study, we report a specific RT-PCR assay for ROCV diagnosis and the molecular characterization of the SPAn37630 and SPH37623 strains. Partial nucleotide sequences of NS5 and E genes determined from both strains were used in phylogenetic analysis. The results indicated that these strains are closely related to JEC viruses, but forming a distinct subclade together with ILHV, in accordance with results recently reported by Medeiros et al. (2007).

Durante os últimos anos a Internet continua a expandir-se e novos websites de interesse são acrescentados diariamente. A nova era da informação fez com que a informação médica disponível na World Wide Web (WWW) crescesse a uma taxa e a um ritmo consideráveis, o que exige um alto nível de literacia da informação aos médicos que desejam manter-se atualizados na sua especialidade sem terem de investir muito tempo. A aptidão para pesquisar e filtrar informação de forma eficaz e eficiente (consumindo pouco tempo) representa uma parte crítica importante da prática clínica, podendo ser uma experiência frustrante e demorada, a menos que se saiba quais são os recursos que oferecem melhor informação e como estruturar o método de pesquisa.

There is a positive relationship between learning music and academic achievement, although doubts remain regarding the mechanisms underlying this association. This research analyses the academic performance of music and non-music students from seventh to ninth grade. The study controls for socioeconomic status, intelligence, motivation and prior academic achievement. Data were collected from 110 adolescents at two time points, once when the students were between 11 and 14 years old in the seventh grade, and again 3 years later. Our results show that music students perform better academically than non-music students in the seventh grade (Cohen’s d = 0.88) and in the ninth grade (Cohen’s d = 1.05). This difference is particularly evident in their scores in Portuguese language and natural science; the difference is somewhat weaker in history and geography scores, and is least pronounced in mathematics and English scores (η2 p from .09 to .21). A longitudinal analysis also revealed better academic performance by music students after controlling for prior academic achievement (η2 p = .07). Furthermore, controlling for intelligence, socioeconomic status and motivation did not eliminate the positive association between music learning from the seventh to the ninth grade and students’ academic achievement (η2 p = .06). During the period, music students maintained better and more consistent academic standing. We conclude that, after controlling for intelligence, socioeconomic status and motivation, music training is positively associated with academic achievement.

Background Musicians are frequently affected by playing-related musculoskeletal disorders (PRMD). Common solutions used by Western medicine to treat musculoskeletal pain include rehabilitation programs and drugs, but their results are sometimes disappointing. Objective To study the effects of self-administered exercises based on Tuina techniques on the pain intensity caused by PRMD of professional orchestra musicians, using numeric visual scale (NVS). Design, setting, participants and interventions We performed a prospective, controlled, single-blinded, randomized study with musicians suffering from PRMD. Participating musicians were randomly distributed into the experimental (n = 39) and the control (n = 30) groups. After an individual diagnostic assessment, specific Tuina self-administered exercises were developed and taught to the participants. Musicians were instructed to repeat the exercises every day for 3 weeks. Main outcome measures Pain intensity was measured by NVS before the intervention and after 1, 3, 5, 10, 15 and 20 d of treatment. The procedure was the same for the control group, however the Tuina exercises were executed in points away from the commonly-used acupuncture points. Results In the treatment group, but not the control group, pain intensity was significantly reduced on days 1, 3, 5, 10, 15 and 20. Conclusion The results obtained are consistent with the hypothesis that self-administered exercises based on Tuina techniques could help professional musicians controlling the pain caused by PRMD. Although our results are very promising, further studies are needed employing a larger sample size and double blinding designs.

BACKGROUND: Musicians are a prone group to suffer from working-related musculoskeletal disorder (WRMD). Conventional solutions to control musculoskeletal pain include pharmacological treatment and rehabilitation programs but their efficiency is sometimes disappointing. OBJECTIVE: The aim of this research is to study the immediate effects of Tuina techniques on WRMD of professional orchestra musicians from the north of Portugal. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: We performed a prospective, controlled, single-blinded, randomized study. Professional orchestra musicians with a diagnosis of WRMD were randomly distributed into the experimental group (n=39) and the control group (n=30). During an individual interview, Chinese diagnosis took place and treatment points were chosen. Real acupoints were treated by Tuina techniques into the experimental group and non-specific skin points were treated into the control group. Pain was measured by verbal numerical scale before and immediately after intervention. RESULTS: After one treatment session, pain was reduced in 91.8% of the cases for the experimental group and 7.9% for the control group. CONCLUSION: Although results showed that Tuina techniques are effectively reducing WRMD in professional orchestra musicians of the north of Portugal, further investigations with stronger measurements, double-blinding designs and bigger simple sizes are needed.

Temporomandibular disorders (TMD) consist of a group of pathologies that affect the masticatory muscles, temporomandibular joints (TMJ), and/or related structures. String instrumentalists, like many orchestra musicians, can spend hours with head postures that may influence the biomechanical behavior of the TMJ and the muscles of the craniocervicomandibular complex (CCMC). The adoption of abnormal postures acquired during performance by musicians can lead to muscular hyperactivity of the head and cervical muscles, with the possible appearance of TMD. Medical infrared thermography is a non-invasive procedure that can monitor the changes in the superficial tissue related to blood circulation and may serve as a complement to the clinical examination. The objective of this study was to use infrared thermography to evaluate, in one subject, the cutaneous thermal changes adjacent to the CCMC that occur before, during, and after playing a string instrument.

o autor fundamenta a tratamento psicanaalítico numa concepção interactiva e intersubjectiva do desenvolvimento psíquico normal e palológico. Peia internalização - durante a infância - de relações patológicas e patogénicas, o indivíduo estrutura um modelo relacional interno perturbado que vai determinar as suas escolhas sequentes, perpetuando a patologia. O estilo relacional patológico repete-se, também, na relação psicanalítica, permitindo a sua análise e dissolução. Paralelamente, enceta-se uma nova relação - proposta e promovida - pelo psicanalista - que vai no sentido do desenvolvimento e da saúde mental. Esta nova relação - desenvolutiva e sanígena - vai sendo transportada para o quotidiano do paciente. O modelo interno de relação, ele próprio, e através das novas vivências, transforma-se. A psicanálise termina quando o novo modelo relacional interno esta consolidado. A cura psicanalítica é, portanto, um processo de transformação; e o psicanalista o agente transformacional. Neste sentido, o autor defende que a contratransferência precede a transferência e é o motor do processo de cura. O que resume na injunção: precessão e primazima da contratransferência.

After detecting the death of Howlers monkeys (genus Alouatta) and isolation of yellow fever virus (YFV) in Buri county, São Paulo, Brazil, an entomological research study in the field was started. A YFV strain was isolated from newborn Swiss mice and cultured cells of Aedes albopictus - C6/36, from a pool of six Haemagogus (Conopostegus) leucocelaenus (Hg. leucocelaenus) mosquitoes (Dyar & Shannon) collected at the study site. Virus RNA fragment was amplified by RT-PCR and sequenced. The MCC Tree generated showed that the isolated strain is related to the South American I genotype, in a monophyletic clade containing isolates from recent 2008-2010 epidemics and epizootics in Brazil. Statistical analysis commonly used were calculated to characterize the sample in relation to diversity and dominance and indicated a pattern of dominance of one or a few species. Hg. leucocelaenus was found infected in Rio Grande do Sul State as well. In São Paulo State, this is the first detection of YFV in Hg. leucocelaenus.
