OBJECTIVE: High-dose methotrexate (MTX) exposure during pregnancy is associated with embryopathy. The teratogenic potential of MTX at dosages typically used in the treatment of rheumatic diseases remains uncertain. The aim of this study was to evaluate the risk of spontaneous abortion, major birth defects, elective termination of pregnancy, shortened gestational age at delivery, and reduced birth weight in women exposed to MTX. METHODS: Pregnancy outcome in women taking MTX (≤30 mg/week) either after conception or within the 12 weeks before conception was evaluated in a prospective observational multicenter cohort study. Pregnancy outcomes in the MTX group were compared to outcomes in a group of disease-matched women and a group of women without autoimmune diseases (neither group was exposed to MTX). RESULTS: The study sample included 324 MTX-exposed pregnancies (188 exposed post-conception, 136 exposed pre-conception), 459 disease-matched comparison women, and 1,107 comparison women without autoimmune diseases. In the post-conception cohort, the cumulative incidence of spontaneous abortion was 42.5% (95% confidence interval [95% CI] 29.2-58.7), which was significantly higher than the incidence of spontaneous abortion in either comparison group. The risk of major birth defects (7 of 106 [6.6%]) was elevated compared to both the cohort of women without autoimmune diseases (29 of 1,001 [2.9%]) (adjusted odds ratio [OR] 3.1 [95% CI 1.03-9.5]) and the disease-matched cohort (14 of 393 [3.6%]) (adjusted OR 1.8 [95% CI 0.6-5.7]). None of the malformations were clearly consistent with MTX embryopathy. Neither the cumulative incidence of spontaneous abortion (14.4% [95% CI 8.0-25.3]) nor the risk of major birth defects (4 of 114 [3.5%]) was increased in the pre-conception cohort. Elective termination rates were increased in both of the MTX-exposed cohorts. There were no other significant differences among groups in other study end points. CONCLUSION: Post-conception administration of MTX at dosages typically used in the treatment of rheumatic diseases was associated with an increased risk of major birth defects and spontaneous abortion. Such evidence was not found among women in our pre-conception cohort.

Apamè, Stratonice et Laodice(s) sont les différents protagonistes de cette thèse qui s'attache à saisir comment les souveraines séleucides participent au pouvoir. Principalement centré sur les identités politiques des reines que l'épigraphie grecque et akkadienne nous permet de re-construire, ce travail se développe chronologiquement. Il débute en 324, lors des noces Suse - qui virent Séleucos épouser la bactrienne Apamè, pour se terminer en 177 par la mention de la reine Laodice, la mère du roi Séleucos IV. Nous avons, par ce travail, démontré que les reines séleucides participent pleinement au pouvoir et ce dès le début de la constitution du royaume. Les souveraines mobilisent l'administration, écrivent aux cités, possèdent une autonomie financière grâce aux domaines qu'elles détiennent et disposent de réseaux d'influence personnels. Les identités qu'elles se construisent les lient notamment à la fécondité et leur permettent d'avoir un domaine d'action politique particulier inaccessible au roi. Elles sont en ce sens des partenaires politiques indispensables qui soutiennent à la fois la perpétuation du pouvoir royal et le maintien du territoire séleucide. Si par ce travail nous avons focalisé notre attention sur les basilissai, le cas de la première épouse d'Antiochos II nous a permis de mettre en évidence la polygamie des souverains, l'organisation « hiérarchique » des différentes épouses du roi ainsi que les stratégies développées par le pouvoir séleucide pour désigner celle dont sera issus le successeur. Enfin, le pouvoir séleucide, tel que nous l'avons décrit, ne permet plus de défendre les hypothèses de répudiation ou de régence des souveraines séleucides avancées pour les cas des épouses d'Antiochos II et d'Antiochos III.

Um dos principais fatores de produção para as culturas é a disponibilidade de água no solo, de forma que a quantificação das entradas e saídas dessa água, por meio do método do balanço hídrico, pode ser um instrumento valioso para melhorar o manejo dado a ela. Desse modo, este trabalho teve como objetivo analisar os componentes do balanço hídrico (variação do armazenamento de água no solo, drenagem, ascensão capilar e evapotranspiração) e o perfil do sistema radicular em Brachiaria decumbens, em nove meses do ano de 2011 (março a novembro). Também foi avaliada a razão entre evapotranspiração (ET) e evapotranspiração de referência (ETo). Esta pesquisa foi realizada na fazenda Riacho do Papagaio, em São João, na mesorregião do Agreste do Estado de Pernambuco, onde foram instalados sensores automatizados para medição da umidade volumétrica do solo nas profundidades de 0,10; 0,20; 0,30; e 0,40 m. A precipitação pluvial foi monitorada por meio de um pluviógrafo automatizado instalado numa torre no centro da área. Para a determinação da drenagem e da ascensão capilar, foram realizados ensaios de infiltração para obtenção da condutividade hidráulica do solo, além da determinação da curva de retenção, em laboratório. A ET foi obtida como termo residual da equação do balanço hídrico e a ETo, pelo método de Penman-Monteith. O volume de controle utilizado para o balanço hídrico teve como limite superior a superfície do solo e como limite inferior a profundidade de 0,30 m. Também foi analisado o perfil do sistema radicular da braquiária, mensalmente. Observou-se que os períodos com elevadas pluviosidades resultaram num maior armazenamento de água no solo, em maiores valores de drenagem e de evapotranspiração. O fluxo de água no limite inferior (z = 0,30 m) do solo ocorreu somente no sentido descendente, sendo perdidos 103,14 mm de água por drenagem, o que representa 24,12 % de toda a água fornecida à cultura. A evapotranspiração total da Brachiaria decumbens Stapf foi de 324,96 mm, com valor médio de 1,2 mm d-1. A pastagem sofreu estresse hídrico, em quase todo período experimental, tendo em vista os valores da relação ET/ETo serem inferiores a 1. Os baixos valores da razão ET/ETo indicaram a necessidade de se realizar irrigação para se diminuir o déficit hídrico. No entanto, em razão dos elevados valores de drenagem, recomendam-se práticas de manejo que aumentem a retenção de água pelo solo.

OBJECTIVE: To better assess the diagnosis of an infection in patients presenting at an emergency department with peripheral blood leukocytosis (>10 x 10(9) cells/l) on laboratory testing. METHODS: We prospectively evaluated serum procalcitonin concentration (PCT), C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR). Patients were divided into two groups according to their final diagnosis: patients with infection and those without infection. PCT, CRP, and ESR were compared between these groups. Sensitivity, specificity, positive predictive values, negative predictive values, receiver operating characteristic curves, and areas under the curves were calculated for each biological measurement. RESULTS: Out of 173 patients, 99 (57%) had a final diagnosis of systemic infection. If a cutoff point of 0.5 ng/ml is considered, procalcitonin concentration had a sensitivity of 0.57, a specificity of 0.85, a negative predictive value of 0.59, and a positive predictive value of 0.84 for the diagnosis of a systemic infection. Adding CRP or ESR to PCT gave no more information (p=0.84). CONCLUSIONS: Only about half of the patients attending the emergency department with leukocytosis were suffering from an infection. Determination of the procalcitonin level may be useful for these patients, particularly in the case of a value higher than 0.5 ng/ml.

In a worldwide collaborative effort, 19,630 Y-chromosomes were sampled from 129 different populations in 51 countries. These chromosomes were typed for 23 short-tandem repeat (STR) loci (DYS19, DYS389I, DYS389II, DYS390, DYS391, DYS392, DYS393, DYS385ab, DYS437, DYS438, DYS439, DYS448, DYS456, DYS458, DYS635, GATAH4, DYS481, DYS533, DYS549, DYS570, DYS576, and DYS643) and using the PowerPlex Y23 System (PPY23, Promega Corporation, Madison, WI). Locus-specific allelic spectra of these markers were determined and a consistently high level of allelic diversity was observed. A considerable number of null, duplicate and off-ladder alleles were revealed. Standard single-locus and haplotype-based parameters were calculated and compared between subsets of Y-STR markers established for forensic casework. The PPY23 marker set provides substantially stronger discriminatory power than other available kits but at the same time reveals the same general patterns of population structure as other marker sets. A strong correlation was observed between the number of Y-STRs included in a marker set and some of the forensic parameters under study. Interestingly a weak but consistent trend toward smaller genetic distances resulting from larger numbers of markers became apparent.

The use of ecological momentary assessment (EMA) for studying parenting has been rare. We examined the psychometric properties and structural validity of an EMA Parenting Scale based on 32 mothers' reports of their parenting over a period of 10 consecutive days, and explored the acceptance of the scale and compliance with the procedure. The results suggested that the EMA Parenting Scale was well accepted for the assessment of daily parenting, and that it consistently captured the overreactive and lax dimensions of parenting across different episodes of child misbehavior. Moreover, multilevel analyses suggested that the scale was sensitive to change across different parenting episodes, and that it reliably assessed the dimensions at both the personal and situational levels. In chicken dorsal root ganglia, calretinin immunoreactivity is expressed by a subpopulation of large A-neurons, most of which co-express calbindin D-28k. The myelinated axons of these neurons selectively innervate all muscle spindles and most Herbst corpuscles associated to feathers in hindlimbs. It is suggested that the presence of calretinin in primary afferents may be correlated with the electrophysiological properties of rapidly adapting mechanoreceptors.

The identification of hypothermia as the cause of death has always been somewhat problematic in forensic pathology because of unspecific, inconstant, or even negative macroscopic and microscopic findings. Though the simultaneous presence of frost erythema, Wischnewski spots, hemorrhages into the synovial membrane, bloody discoloration of synovial fluid of the knee, and basal vacuolization of the renal tubular epithelial cells has been indicated as strongly supportive of fatal hypothermia, their absence does not allow the diagnosis of hypothermia to be ruled out. Postmortem biochemical investigations are valuable in detecting adaptation responses to cold stress and metabolic changes that occur following cold exposure. However, ethanol intoxication prevents appearance of adaptation responses to cold, rendering the diagnosis less obvious. Immunohistochemistry, postmortem imaging, and molecular pathology have shown promising results, although at present, they do not provide pathognomonic signs of fatal hypothermia. The aim of this article is to present a review of the literature covering the significance of different postmortem investigations that are associated with hypothermia fatalities.

BACKGROUND: Neuropsychological deficits (NPD) are common in patients with aneurysmal subarachnoid haemorrhage (aSAH). NPD are one of the major limiting factors for patients with an otherwise acceptable prognosis for sustained quality of life. There are only a few studies reporting outcome after aSAH, which used a standardized neuropsychological test battery as a primary or secondary outcome measure. Aim of this study was to determine the current practice of reporting NPD following aSAH in clinical studies. METHODS: A MEDLINE analysis was performed using the search term "subarachnoid haemorrhage outcome". The latest 1,000 articles were screened. We recorded study design, number of patients, and the presence of neuropsychological outcome report. Additionally, the time of testing after aSAH, the neuropsychological tests administered, as well as the percentage of patients with NPD were analyzed. RESULTS: A total of 324 publications between 2009 and 2012 were selected for further review. Of those, 21 studies (6.5%) reported neuropsychological outcome, in 2,001 of 346,666 patients (0.6%). The assessment of NPD differed broadly using both subjective and objective cognitive evaluation, and a large variety of tests were used. CONCLUSION: Neuropsychological outcome is underreported, and there is great variety in assessment in currently published clinical articles on aSAH. Prospective randomized trials treating aSAH may benefit from implementing more comprehensive and standardized neuropsychological outcome measures. This approach might identify otherwise unnoticed treatment effects in future interventional studies of aSAH patients.
