OBJETIVOS: Estudamos os efeitos de alta dose de fentanil (F) em atributos de função renal (FR) do cão. DESENHO: Anestesiamos com pentobarbital sódico (PS) 16 cães divididos aleatoriamente em 2 grupos: manutenção com PS (Gi) e PS com F (0,05 mg.kg-1) (G2). INTERVENÇÃO: os cães foram ventilados artificialmente e tiveram cateterizadas as veias femorais esquerda e direita e a artéria femoral esquerda para infusão de drogas e coleta de dados hemodinâmicos e de sangue para dosagens laboratoriais. Coletou-se urina durante todo experimento. MENSURAÇÃO: Determinaram-se os valores de atributos de FR.. RESULTADOS: PS não mudou a FR e o comportamento de G1 deveu-se à expansão do volume extracelular. O F diminuiu significativamente freqüência cardíaca, pressão arterial média, clearance de creatinina, volume urinário, clearance osmolar e excreção fracionária de sódio e potássio. CONCLUSÕES: A diminuição da FR foi provavelmente devida às alterações hemodinâmicas induzidas pelo F, não se descartando possível ação da aldosterona.

Na fase de tolerância à glicose diminuída (TGD) já estão presentes vários distúrbios metabólicos característicos do diabetes mellitus tipo 2 e fatores de risco que predispõem à vasculopatia. Nosso objetivo foi o de verificar se indivíduos de nossa população, com TGD e normotensos, apresentam elevação da excreção urinária de albumina (EUA) e da depuração de creatinina (D Cr). Teste oral de tolerância à glicose padrão com dosagem de insulina foi realizado, classificando os participantes em dois grupos: com TGD e com tolerância à glicose normal (TGN). Urina do período noturno foi utilizada para a determinação da EUA (imunoturbidimetria) e da D Cr. Os dois grupos não diferiram quanto à EUA e à D Cr. A freqüência de microalbuminúria foi de 21,1 e 3,3%, respectivamente, para os grupos com TGD e TGN (NS). Concluímos que, nesta amostra da população brasileira, indivíduos com TGD, normotensos e com resistência à insulina não apresentaram microalbuminúria e hiperfiltração glomerular.

Diversos fatores podem contribuir para a formação de cálculos urinários, dentre estes, o desequilíbrio nutricional e a dureza da água consumida pelos ruminantes. O objetivo deste estudo foi identificar as características de propriedades que predispõem à urolitíase, através da avaliação da água, da dieta e determinações séricas e urinárias de cálcio, fósforo, magnésio, cloretos, sódio, potássio, cálculo da excreção fracionada (EF) dos eletrólitos, e da creatinina, proteína total, albumina e globulinas séricas. Foram colhidas amostras de sangue e urina de bovinos, Guzerá, criados semi intensivamente, distribuídos por dois grupos. O primeiro denominado grupo urolitíase (Gu), composto de animais com histórico, sinais clínicos e confirmação ultrassonográfica que apresentavam urolitíase; o segundo: grupo controle (Gc), sem histórico, nem sintomas da doença. Os bovinos do grupo urolitíase consumiam água com dureza total na concentração de 166,0mg CaCO3/L. A dieta dos animais do Gu apresentava maior concentração de fósforo e relação Ca:P inadequada. Os teores de fósforo sérico e urinário dos animais do Gu foram maiores do que os do Gc, assim como a concentração sérica de magnésio (p<0,05). Não houve aumento nas concentrações de creatinina no grupo urolitíase, mas ocorreu hipoproteinemia por hipoglobulinemia (p<0,05). As EFs de cálcio, fósforo e sódio não diferiram entre os grupos (p>0,05), mas houve diminuição significativa nas EFs de magnésio, cloretos e de potássio do grupo urolitíase (p<0,05). A união destes fatores contribuiu para a ocorrência da urolitíase, sendo dureza total da água e a alta concentração de fósforo na dieta os principais fatores na gênese dos cálculos em bovinos.

The present study is aimed to determine serum and urine interleukin-8 (IL-8) levels in premature infants with late onset sepsis (LOS) and to evaluate if urine IL-8 is a useful test for LOS diagnosis. Fifty-six premature infants admitted to the NICU over 1 year had serum and urine IL-8 determined by ELISA. They were divided into three groups: I definite sepsis, II probable sepsis and III non-infected. Results were expressed as mean or median. Differences between groups were assessed by ANOVA, Kruskal-Wallis ANOVA and Dunns Method. Sensitivity, specificity and positive and negative predictive values were calculated and a receiver operator characteristic curve was constructed to determine serum and urine IL-8 accuracy. There were no differences between groups for birth weight, and gestational and post-natal age. Median serum and urine IL-8 levels were significantly higher in GI and GII: 929 x 906 x 625pg/ml; P=0.024, and 249 x 189 x 42pg/mgCr; P< 0.001. Optimal cut-off point was 625pg/ml for serum IL-8 with 69 sensitivity and 75pg/mgCr for urine IL-8 with 92 sensitivity. IL-8 can be determined in urine from premature infants with LOS and is an accurate and feasible diagnosis method.

Background: Elevated sodium excretion in urine resulting from excessive sodium intake can lead to hypercalciuria and contribute to the formation of urinary stones. The aim of this study was to evaluate salt intake in patients with urinary lithiasis and idiopathic hypercalciuria (IH).Methods: Between August 2007 and June 2008, 105 lithiasic patients were distributed into 2 groups: Group 1 (n = 55): patients with IH (urinary calcium excretion > 250 mg in women and 300 mg in men with normal serum calcium); Group 2 (n = 50): normocalciuric patients (NC). Inclusion criteria were: age over 18 years, normal renal function (creatinine clearance >= 60 ml/min), absent proteinuria and negative urinary culture. Pregnant women, patients with intestinal pathologies, chronic diarrhea or using corticoids were excluded. The protocol of metabolic investigation was based on non-consecutive collection of two 24-hour samples for dosages of: calcium, sodium, uric acid, citrate, oxalate, magnesium and urinary volume. Food intake was evaluated by the three-day dietary record quantitative method, and the Body Mass Index (BMI) was calculated and classified according to the World Health Organization (WHO). Sodium intake was evaluated based on 24-hour urinary sodium excretion.Results: The distribution in both groups as regards mean age (42.11 +/- 10.61 vs. 46.14 +/- 11.52), weight (77.14 +/- 16.03 vs. 75.99 +/- 15.80), height (1.64 +/- 0.10 vs. 1.64 +/- plusorminus 0.08) and BMI (28.78 +/- 5.81 vs. 28.07 +/- 5.27) was homogeneous. Urinary excretion of calcium (433.33 +/- 141.92 vs. 188.93 +/- 53.09), sodium (280.08 +/- 100.94 vs. 200.44.93 +/- 65.81), uric acid (880.63 +/- 281.50 vs. 646.74 +/- 182.76) and magnesium (88.78 +/- 37.53 vs. 64.34 +/- 31.84) was significantly higher in the IH group (p < 0.05). There was no statistical difference in calcium intake between the groups, and there was significantly higher salt intake in patients with IH than in NC.Conclusions: This study showed that salt intake was higher in patients with IH as compared to NC.

A avaliação do cálcio e do fósforo nutricional é difícil quando se usam concentrações séricas desses elementos, pois estas permanecem dentro de valores normais apesar de variações do conteúdo desses minerais na dieta, pela alta eficiência do processo de homeostasia corporal. Alterações significativas serão observadas em processos carenciais crônicos, quando então as perdas econômicas já terão significado relevante. A técnica da excreção fracionada permite avaliação quantitativa sem a necessidade de determinação do volume urinário produzido. Com o objetivo de analisar a excreção fracionada de Ca e P, foram utilizadas 22 novilhas criadas extensivamente. As amostras de soro e urina foram colhidas a cada 45 dias, antes (duas coletas) e durante a gestação (seis coletas). A excreção fracionada do Ca e do P apresentou medianas variando entre 1,06% e 3,72% e 1,09% e 2,70%, respectivamente. Houve tendência na diminuição dos valores da mediana da excreção fracionada do Ca, comparando-se os momentos em que os animais ainda não estavam gestantes com os valores obtidos durante a gestação. Este experimento demonstrou que existem amplas variações na excreção fracionada de Ca e P em diferentes animais em um mesmo momento, dificultando a utilização deste método na avaliação do estado nutricional desses elementos em bovinos criados extensivamente.

Para avaliar tratamentos na intoxicação por amônia, 15 novilhos, infundidos com solução de cloreto de amônio, foram distribuídos em três grupos de cinco animais cada e tratados, como: 1 - grupo-controle (C), infundido com solução salina fisiológica (H); 2 - grupo O+H, medicado com aminoácidos do ciclo da ureia (O) e H; 3 - grupo O+F+H, o mesmo protocolo do grupo 2 acrescido de furosemida (F). Os animais foram monitorados, colhendo-se amostras sanguíneas e todo volume urinário. Os tratamentos O+F+H e O+H promoveram melhora clínica pronunciada, em relação ao grupo H, isto é, permaneceram em estação, retornaram o movimento de rúmen e o apetite e recuperaram-se do edema pulmonar mais rapidamente. Observaram-se, nos grupos O+F+H e O+H, teores plasmáticos mais baixos de amônia e lactato-L, urina mais abundante, excreção pela urina de maior quantidade de amônio e ureia, depuração mais intensa de amônia do sangue e pH urinário mais baixo. Concluiu-se que os tratamentos propostos apresentaram ação efetiva principalmente por diminuírem a hiperamonemia, por meio da maior excreção renal de amônio e maior transformação hepática de amônia em ureia

The circumventricular structures and the lateral hypothalamus (LH) have been shown to be important for the central action of angiotensin II (ANGII) on water and electrolyte regulation. Several anatomical findings have demonstrated neural connection between circumventricular structures and the LH, the present experiments were conducted to investigate the role of the alpha-adrenergic antagonists and agonistic injected into the LH on the water intake, sodium and potassium excretion elicited by injections of ANGII into the lateral ventricle (LV), the water intake was measured every 30 min over a period of 120 min. The sodium, potassium and urinary volume were measured over a period of 120 min in water-loaded rats. The injection of ANGII into the LV increased the water intake, which was reduced by previous injection of clonidine (an alpha-2-adrenergic agonist) into the LH. The injection of yohimbine (an alpha-2-adrenergic antagonist) and prazosin (an alpha-l-adrenergic antagonist) into the LH, which was done before injecting ANGII into the LV, also reduced the water intake induced by ANGII. The injection of ANGII into the LV reduced the sodium, potassium and urinary volume. Previous treatment with clonidine attenuated the action of ANGII in reducing the sodium, potassium and urinary volume, whereas previous treatment with yohimbine attenuated the effects of ANGII but with less intensity than that caused by clonidine. Previous treatment with prazosin increased the inhibitory effects of ANGII in those parameters. The injection of yohimbine and prazosin, which was done before the injection of clonidine, attenuated the effect of clonidine on the ANGII mechanism. The results of this study led us to postulate that when alpha-2-adrenergic receptors are blocked, the clonidine may act on the imidazoline receptors to produce its effects on the ANGII mechanism. We may also conclude that the LH is involved with circumventricular structures, which present excitatory and inhibitory mechanisms. Male Holtzman rats weighting 200-250 g were anesthetized with zoletil 50 mg/Kg (tiletamine chloridrate 125.0 mg and zolazepan chloridrate 125.0 mg) into quadriceps muscle and stainless steel cannulas were implanted into their supraoptic nucleus (SON). We investigated the effects of the injection into the supraoptic nucleus (SON) of FK 409, a nitric oxide donor, and N(W-)nitro-L-arginine methyl ester (L-NAME), a nitric oxide synthase inhibitor (NOS), on the salivary secretion, arterial blood pressure, sodium excretion and urinary volume induced by pilocarpine, which was injected into SON. The drugs were injected in 0.5 mul volume over 30-60 s. Controls was injected with a similar volume of 0.15 M NaCl. FK 409 and L-NAME were injected at doses of 20 mug/0.5 mul and 40 mug/0.5 mul. respectively. The amount of saliva secretion was studied over a five-minute period after injection of pilocarpine into SON. Injection of pilocarpine (10, 20, 40, 80, 160 mug/mul) into SON produced a dose-dependent increase in salivary secretion. L-NAME was injected into SON prior to the injection of pilocarpine into SON, producing an increase in salivary secretion due to the effect of pilocarpine. FK 409 injected into SON attenuating the increase in salivary secretion induced by pilocarpine. Mean arterial pressure (MAP) increase after injections of pilocarpine into the SON. L-NAME injected into the SON prior to injection of pilocarpine into SON increased the MAP. FK 409 injected into the SON prior to pilocarpine attenuated the effect of pilocarpine on MAP. Pilocarpine (0.5 mumol/0.5 mul) injected into the SON induced an increase in sodium and urinary excretion. L-NAME injected prior to pilocarpine into the SON increased the urinary sodium excretion and urinary volume induced by pilocarpine. FK 409 injected prior to pilocarpine into the SON decreased the sodium excretion and urinary volume induced by pilocarpine. All these roles of pilocarpine depend on the release of nitric oxide into the SON. In summary the present results show: a) SON is involved in pilocarpine-induced salivation; b) that mechanism involves increase in MAP, sodium excretion and urinary volume. (C) 2003 Elsevier B.V. The aim of the present study was to analyze the role of alpha(1),alpha(2)-adrenoceptors, and the effects of losartan and PD123319 (selective ligands of the AT(1) and AT(2) angiotensin receptors, respectively) injected into the paraventricular nucleus (PVN) on the diuresis, natriuresis, and kaliuresis induced by administration of adrenaline into the medial septal area (MSA). Male Holtzman rats with a stainless steel cannula implanted into the MSA and bilaterally into the PVN were used. The administration of adrenaline into the MSA increased in a dose-dependent manner the urine, sodium, and potassium excretions. The previous administration of prazosin (an alpha(1)-adrenoceptor antagonist) injected into the PVN abolished the above effects of adrenaline, whereas yohimbine (an a-adrenoceptor antagonist) doesn't affect the diuresis, natriuresis, and kaliuresis induced by adrenaline. Pretreatment with losartan into the PVN decreased in a dose-dependent manner the urine, sodium, and potassium excretions induced by MSA administration of adrenaline (50 ng), while PVN PD123319 was without effect. These results indicate that urinary and electrolyte excretion effects induced by adrenaline into the MSA are mediated primarily by PVN AT, receptors. However, the doses of losartan were more effective when combined with the doses of PD123319 than given alone, suggesting that the urinary, natriuretic, and kaliuretic effects of MSA adrenaline may involve activation of multiple angiotensin II receptors subtypes into the PVN. The 5-hydroxytryptamine (5-HT)(1A) receptor system plays a prominent role in a variety of physiological functions and behavior and regulation of this responsiveness of the receptor system has been implicated in the central regulation of water intake and urinary excretion. The lateral septal area (LSA) exhibits a high density of 5-HT1A receptors, as well as a subpopulation of oxytocin (OT) receptors. Here we report the effects of pMPPF (a selective 5-HT1A antagonist), d(CH2)(5)[Tyr(Me)(2)Thr(4), Orn(5), Tyr(NH2)(9)]-vasotocin (an OT antagonist), and that 5-HT1A receptor system is regulated as a consequence of activation of the Na+ channel by veratridine. Cannulae were implanted into the LSA of rats to enable the introduction of the drugs. Injections of 8-OH-DPAT (a 5-HT1A agonist) blocked water intake and increased urinary excretion, while pMPPF or the OT antagonist injected bilaterally before 8-OH-DPAT blocked its inhibitory effect on water intake and its diuretic effect. In contrast, increases in extracellular sodium levels induced by the sodium channel modulator, veratridine, enhanced 5-HT1A responsiveness for water intake and reduced the diuretic effects induced by 8-OH-DPAT. These trials demonstrated that the responsiveness of the 5-HT1A receptor system in the LSA can be enhanced or depressed as a consequence of an induced rise in extracellular sodium. The paraventricular nucleus (PVN) may be considered as a dynamic mosaic of chemically-specified subgroups of neurons. 5-HT1A is one of the prime receptors identified and there is expressed throughout all magnocellular regions of the PVN. Several reports have demonstrated that a subpopulation of the magnocellular neurons expressing 5-HT1A receptors are oxytocin (OT) neurons and activation of 5-HT1A receptors in the PVN increases the plasma OT. Increasing evidence shows that OT inhibits water intake and increases urinary excretion in rats. The aim of this study was to investigate the role of serotonergic 5-HT1A receptors in the lateral-medial posterior magnocellular region of the PVN in the water intake and diuresis induced by 24 h of water deprivation. Cannulae were implanted in the PVN of rats. 5-HT injections in the PVN reduced water intake and increased urinary excretion. 8-OH-DPAT (a 5-HT1A agonist) injections blocked the water intake and increased urinary output in all the periods of the observation. pMPPF (a 5-HT1A antagonist) injected bilaterally before the 8-OH-DPAT blocked its inhibitory effect on water intake and its diuretic effect. We suggest that antidipsogenic and diuretic responses seem to be mediated via 5-HT1A receptors of the lateral-medial posterior magnocellular region of the PVN in water-deprived rats.
