The aim of this IRB-approved study was to analyze prospectively quality of life (QOL) and psychological changes in 30 ESRD patients before and after kidney transplantation (KT). Semi-structured interviews were conducted after inclusion on the waiting list (A). Follow-up interviews were performed 6 months later with patients still awaiting KT (B6, n= 15), and with transplant recipients 6, 12 and 24 months after KT (C6, n=15; C12, n=15; C24, n=14). Qualitative thematic analysis was performed. A: All patients reported loss of freedom, 87% tried to maintain normality; 57% modified medical directives. All mentioned emotional fragility, negative thoughts (43%), and suicidal thoughts (20%) related to loss of QOL from dialysis (D), and professional tension (26%). B6: 40% reported no change compared to baseline, while 60% mentioned increase of illness intrusiveness, 46% D side effects, 40% communication problems, and 33% concerns about the waiting list handling. Fear of emotional breakdown (40%), couple problems (47%), and worsened professional difficulties (20%) were reported. C6: All patients reported recovery of QOL and concerns about acute rejection. 73% were anxious about laboratory results. 93% felt dependent on immunosuppressants (IS), 47% reported difficulties coping with their regimen, and 47% were concerned about side effects; 67% had resumed work, but medical constraints led 40% to professional stigmatization. C12: All enjoyed good QOL. Adherence to IS was mandatory (100%). All were aware of the limited long-term graft survival and 47% anxious about a possible return to D. 60% underlined positive life value; 47% resumed a full time job; 40% were on social security. C24: Good QOL was underlined (86%). Patients stated they would prefer re-TX to resuming D (71%). Post-TX health problems were mentioned (64%); increase of creatinine levels induced fear (36%). 79% complained about side effects. 64% reported changes in life values. This study reveals positive QOL and psychological transformations after KT, which are associated with positive changes related to graft survival and freedom from D. Psychological follow-up should be offered to patients who face relapsing ESRD or post-TX co-morbidities.

Demographic ageing is increasing pensions, health and social services spending and threatening the future balance of public budgets. Providing home care can help to curb health expenditure and it may improve elderly welfare also, but EU states have chosen different policies in providing home are. Main differences are related with source of financing and eligibility criteria but also with the kind of benefits (benefits in cash or in kind). How these different options affect welfare and carers’ employment opportunities is the core of this research. Home care growth is going to be more efficient as far as it pro motes employment and, public revenues consequently. Using microdata from the European Community Household Panel, British and Spanish means tested programs are compared with German and Austrian ‘in cash’ benefits, and with Danish ‘in kind’ benefits also. The results show that Danish policies are the most efficient and equitable while the British and Spanish ones are the worst.

Introduction : Au CHUV, les contrôles microbiologiques des préparations pharmaceutiques stériles produites par la pharmacie de l'hôpital se basent sur l'essai de stérilité de la Pharmacopée Européenne. Avant 2000, une méthode en circuit ouvert était utilisée, puis, dès l'année 2000, une nouvelle méthode développée par H. Ing du service de pharmacie des HUG a été adoptée (méthode « Ing »). Cette dernière permet d'opérer en circuit fermé et de filtrer le milieu de culture. De plus, elle utilise du matériel bon marché (trousse de perfusion, filtres à usage unique). Objectifs : Le présent travail avait pour but : 1) l'évaluation préliminaire de cette méthode (validation). 2) l'évaluation du bénéfice apporté en terme d'incidence de faux positifs sur les préparations stériles filtrables. Matériel et méthode : La validation a été effectuée en analysant des flexs de NaCl 0.9% préalablement inoculés avec 10-100 CFU de 6 souches microbiennes décrites dans la Pharmacopée pour le test de validation, ainsi qu'un flex « contrôle » non inoculé. Le bénéfice de la méthode a été évalué à partir des résultats des essais de routine effectués au laboratoire. Un taux de faux positifs imputable à chaque méthode a ainsi pu être déterminé (i.e. croissance microbienne due à une contamination lors de l'essai et non à une contamination initiale de la préparation pharmaceutique) et la comparaison a été effectuée à l'aide du test statistique de Fisher. Résultats : Une croissance a été observée dans toutes les préparations préalablement inoculées par des micro-organismes. La méthode a donc pu être implantée dans le laboratoire pour les analyses de routine dès février 2000. L'analyse rétrospective des résultats des essais de stérilité effectués sur une période de plus de 4 ans (2 ans avec l'ancienne méthode (système ouvert) et de presque 3 ans avec la nouvelle méthode) montre que l'ancienne méthode produisait un taux de faux positifs de 1.57 %, alors que ce taux n'est que de 0.21% avec la méthode « Ing ». Cette dernière se caractérise donc par un taux de faux positifs significativement plus bas que celui de l'ancienne méthode (p < 0.0001). Conclusion : La méthode « Ing » constitue une technique bien adaptée à l'essai de stérilité pour l'hôpital, suffisamment sensible et conforme aux recommandations de la Pharmacopée. En maintenant le produit dans un espace clos, elle permet de diminuer les risques de contamination susceptibles de se produire lors de l'essai.

By taking care of cancer patients in their process of end of life, nursing experience situations of suffering before the anguish of others. This study aimed to understand the meaning and significance attributed by the nurses from the palliative care cancer hospital. This is a phenomenological research, grounded in Heidegger’s thinking, performed with 13 nurses, who work at Oncology hospitalward, through semi-structured interviews, which were analyzed according to the steps recommended by Josgrilberg. From understanding the statementsof the subjects, two ontological themesemerged: Feeling satisfaction and love in the care offered and Feeling anger and inabilitytowards terminally ill patients.We inferred that working in Oncology Ward is something rewarding for these professionals, but it entails physical and mental suffering, from feeling helpless before the death-dying process. Thus, we showedthat nursing professionals need to be recognized as human beings and as such, also deserving of care.

1) Etat des connaissances : Actuellement, les indications à la transplantation pulmonaire sont bien décrites et validées. Par contre, lors d'une retransplantation, les résultats restent encore discutés : la retransplantation pulmonaire reste la seule option thérapeutique dans certains cas de rejet du greffon, de syndrome de bronchiolite oblitérante avancée et aussi lors de graves affections respiratoires, principalement de sténose cicatricielle. Malgré cela, son utilité a été mise en doute en raison de la pénurie de donneurs d'organes et des rapports indiquant des résultats insatisfaisants. 2) But de l'étude : Ce travail est une étude d'évaluation méthodologique dans lequel on utilisera un questionnaire DELPHI auprès de 10 soignants engagés dans le programme Vaud-Genève de transplantation pulmonaire afin de déterminer les critères de sélection qui mèneraient à la pratique d'une retransplantation. 3) Objectifs : Ce projet a pour but d'élaborer un consensus local sur les critères et les contre-indications à une retransplantation pulmonaire, dans le cadre du programme Vaud-Genève et du système d'allocation d'organes en vigueur en Suisse. D'où la question : ''Quelles sont les indications potentielles à une retransplantation pulmonaire ?'' 4) Justification : Dans une situation de pénurie explicite de ressources (manque de donneurs), l'utilisation des greffons doit être basée à la fois sur des critères d'équité et d'efficacité médicale. En l'absence de données claires dans la littérature sur les indications à la retransplantation pulmonaire, la recherche d'une attitude pragmatique au sein du programme local est justifiée. 5) Méthode : Cette étude monocentrique multisite est une étude clinique ouverte où les personnes interrogées restent en insu: un questionnaire DELPHI sera envoyé à 10 soignants spécialisés dans le domaine de la transplantation pulmonaire issus du programme Vaud-Genève. La méthode Delphi a pour but de mettre en évidence des convergences d'opinions afin de trouver des consensus grâce à l'interrogation de différents experts par le biais de questionnaires successifs. L'objectif des études Delphi est d'apporter l'éclairage des experts sur des zones d'incertitude en vue d'une aide à la décision. 6) Sélection des sujets : Les ''sujets'' éligibles sont un groupe de 10 experts anonymes spécialisés dans le domaine de la retransplantation pulmonaire du canton de Vaud et de Genève. 6.1 Critères d'inclusion : Personnel médical et paramédical directement impliqué dans le cadre du programme de transplantation Vaud-Genève. 6.2 Critère d'exclusion : Toute personne qui est elle-même directement impliquée dans l'analyse de l'étude. 7) Déroulement de l'étude et investigations prévues: L'étude aura lieu au CHUV et au HUG pour une durée de 2 ans. Les investigations seront effectuées par le biais de questionnaires envoyés par courrier électronique et seront analysées au CHUV.

Evidence of a sport-specific hierarchy of protective factors against doping would thus be a powerful aid in adapting information and prevention campaigns to target the characteristics of specific athlete groups, and especially those athletes most vulnerable for doping control. The contents of phone calls to a free and anonymous national anti-doping service called 'ecoute dopage' were analysed (192 bodybuilders, 124 cyclists and 44 footballers). The results showed that the protective factors that emerged from analysis could be categorised into two groups. The first comprised 'Health concerns', 'Respect for the law' and 'Doping controls from the environment' and the second comprised 'Doubts about the effectiveness of illicit products, 'Thinking skills' and 'Doubts about doctors'. The ranking of the factors for the cyclists differed from that of the other athletes. The ordering of factors was 1) respect for the law, 2) doping controls from the environment, 3) health concerns 4) doubts about doctors, and 5) doubts about the effectiveness illicit products. The results are analysed in terms of the ranking in each athlete group and the consequences on the athletes' experience and relationship to doping. Specific prevention campaigns are proposed to limit doping behaviour in general and for each sport.

Objective To assess the effectiveness of Problem-Solving Therapy (PST) on family caregivers through the use of scales to measure anxiety, depression and emotional distress; and to explore facilitating factors and obstacles for its use based on the narrative of nurses. Method A clinical trial and an exploratory focus group with the use of mixed analysis methodology. The study was conducted in a primary health care center in Tarragona, Spain, and the sample consisted of 122 family caregivers who were included in the home care service, and 10 nurses who participated in the intervention group. Family caregivers with evident symptoms of anxiety, depression and emotional distress received PST in the intervention group. The intervention group also consisted of a discussion with eight nurses, which was transcribed and submitted to content analysis. Conclusion Problem-Solving Therapy proved to be effective in reducing perceived anxiety, depression and emotional distress. We identified its strong points and obstacles as described by nurses.

The aim of this IRB-approved study was to prospectively analyze psychological transformations in ESRD patients before and after transplantation (KT). Semi-structured interviews were conducted in 30 patients (mean age = 53±10) after their inclusion on the waiting-list (Gr. A). Follow-up interviews were performed 6 months later in 15 patients still awaiting KT (Gr. B6), and in 15 patients 6 months (Gr. C6) and 12 months (Gr. C12) after KT. Qualitative analysis was performed. Gr: A:All patients underlined loss of freedom, 87% devoted much energy to maintain normality, 57% modified medical directives. All reported emotional fragility related to dialysis and loss of quality of life (QOL), negative (43%) or suicidal thoughts (20%). Professional stigma was underlined (26%). Gr: B6:40% reported no change, 60% mentioned increase of illness intrusiveness, 46% dialysis side-effects, 40% communication problems, 33% tension with medical staff and waiting list handling. Fear of emotional breakdown (40%), couple problems (47%) and worsened professional difficulties (20%) were reported. Gr: C6:All patients mentioned improved QOL and freedom recovery (87%). All expressed concerns about possible acute rejection, 73% were anxious about laboratory results, 93% experienced dependence to immunosuppressants, 47% reported difficulties in handling medication, 21% feared to forget them, 47% were concerned about side-effects, 67% had resumed work but medical constraints led to professional tension (40%). Gr: C12:All mentioned recovered QOL. Medical controls were accepted as a routine (87%) and adherence to medication was mandatory (100%). All mentioned the limited long-term graft survival and 47% were anxious about possible return to dialysis, especially younger patients (27%). Positive identity and existential changes were reported (60%). This prospective qualitative study identifies psychological modifications in the course of KT. It provides a basis to adequately address concerns, but it shows also that KT is clearly associated with positive psychological transformations.

Objective To identify the difficulties of families with children and/or adolescents with mental disorder. Method This is an integrative review. In December 2013, an electronic search was performed on Latin American Caribbean Literature on Health Sciences databases (LILACS) and on Electronic Medicus Index of the National Library of Medicine (MEDLINE) indexed in the Health Virtual Library (BVS) using a combination of descriptors and boolean operators as follows: mental disorders and child or adolescent and caregivers and/not health staff. Results 557 studies were identified, of which 15 were selected for this study. The findings indicated difficulties related to the care for or to interaction with children/adolescents with mental disorder. Conclusion The studies revealed difficulties related to everyday practices of care and feelings expressed during care practices, as well as in relationships with children or adolescents with mental disorder.
