Although the performance of the Swiss health system is high, one out of ten patients in general practitioner's (GP) office declares having foregone care in the previous twelve months for economic reasons. Reasons for foregoing care are several and include a lack of knowledge of existing social aids in getting health insurance, unavailability of GPs and long waiting lists for various types of care. Although long term knowledge of patients or a psychosocial history of deprivation or poverty may help identify individuals at risk of foregoing care, many may remain undetected. We propose then a few instruments to help GPs to identify, in a simple and structured approach, patients at risk of forgoing care for economic reasons; these patients are frequently deprived and sometimes poor.

Grevées d'une mortalité comparable à celle du choc septique (40%-60%), les candidoses invasives sont une complication nosocomiale rare, mais particulièrement redoutée. Elles sont cependant difficiles à diagnostiquer, car si près de 50% des patients sont colonisés par des levures du genre Candida au cours d'un séjour prolongé en réanimation, seule une minorité d'entre eux développent une candidose sévère. En dehors des cas de candidémie, aucun test diagnostique ne permet de distinguer les patients colonisés de ceux qui sont infectés. Chez les patients présentant des facteurs de risque, la pratique de cultures de surveillance systématiques permet de déceler précocement une colonisation et d'en quantifier le degré. Un traitement préemptif peut être envisagé lorsque le degré de colonisation dépasse un seuil critique prédictif d'infection. De nouvelles classes d'antifongiques sont sur le point de révolutionner les schémas thérapeutiques actuels.

Trust is essential to foster and preserve a long-term relationship between primary care physicians and their patients suffering from chronic diseases. However, this relation remains insufficient to successfully manage more complex situations, such as those of older patients with multiple diseases and disability. For the primary care physician, a significant limitation is the time required to plan and coordinate interventions supplied by different health and social care providers. This article describes a structured approach to support primary care physicians in this difficult task and help them to identify vulnerable older patients requiring to mobilize and coordinate health and social care resources. Current and future resources available to family physicians to complete this challenging task are further described.

Thyroid substitution is generally considered easy as well by general practitioners as by specialists, considering that a single hormone levothyroxin is recommended and that laboratory tests are readily available for measurement of free T4 and TSH. However cross sectional studies have shown that about 45% of patients are over-treated and under-treated. This paper summarizes the critical information useful to facilitate a better management of hypothyroid patients by promoting long lasting euthyroidism.

Recurrent aphthous stomatitis (RAS) is the most common oral mucosa ailment. This condition is frequently considered as idiopathic due to the doubts about its etiology, probably related to a minor immunological dysregulation in a context of genetic predisposition. However, ulcers that resemble recurrent aphthous stomatitis in some respects can be found in systemic disorders that must be ruled out for the differential diagnosis of SAR, particularly when they appear after adolescence and/or when associated lesions exist out of the oral cavity. SAR management lies on the elimination of predisposing factors (drugs, oral trauma, food allergies...) and if needed, topical corticosteroids are the first choice regimen. More severe cases may require systemic regimens.
