Becoming a researcher, like any process of cultural initiation, is a complicated negotiation of processes and identity. While it is tempting to imagine that a researcher is just 'one who researches', the reality is far more complex and can be thought of as a journey rather than a sudden change of role. In practice-led research there is often a final and difficult assessment that needs to be made of that journey, and this requires the collation and integration of seemingly schizophrenic elements into one coherent body of findings. This paper attempts to outline the underlying “system of methods used in a particular area of study or activity” that have defined my research journey. It discusses the various elements that have informed and directed this enquiry, which include, but are not limited to: practice-led research, reflexivity, and post/academic writing, all underpinned by a feminist approach.

Background The MAGIC/UK Medical Research Council (MRC) trial set the standard of care for treatment of resectable gastric and junctional adenocarcinoma, demonstrating that perioperative chemotherapy with epirubicin, cisplatin and 5-fluorouracil (ECF) confers a survival benefit over surgery alone. The randomized ECF for advanced and locally advanced esophagogastric cancer (REAL-2) trial showed that, in the metastatic setting, the EOX regimen (epirubicin, oxaliplatin and capecitabine) is as effective as ECF, with a favourable toxicity profile. Methods Consecutive patients with resectable gastric or junctional adenocarcinoma treated with perioperative EOX, between 2007 and 2012, were retrospectively analysed. Results Fifty-nine patients (12 female, 47 male), commenced EOX therapy; 47 underwent surgery. A good pathological response was seen in 34 %, (16/47). Disease recurrence occurred in 19 patients (19/47, 40 %). Median overall survival was 22 months, with 4-year survival of 47 %. Chemotoxicities were consistent with those previously reported for this regimen. Conclusion This study in a high-volume centre demonstrates that EOX in resectable gastric and junctional adenocarcinoma is associated with a reasonable safety profile, and efficacy consistent with that reported for ECF.

Various policies, plans, and initiatives have been implemented to provide safe, quality, and culturally competent care to patients within Queensland’s healthcare system. A series of models of maternity care are available in Queensland that range from standard public care to private midwifery care. The current study aimed to determine whether identifying as Culturally or Linguistically Diverse (CALD) was associated with the perceived safety, quality, and cultural competency of maternity care from a consumer perspective, and to identify specific needs and preferences of CALD maternity care consumers. Secondary analysis of data collected in the Having a Baby in Queensland Survey 2012 was used to compare the experiences of 655 CALD women to those of 4049 non-CALD women in Queensland, Australia, across three stages of maternity care: pregnancy, labour and birth, and after birth. After adjustment for model of maternity care received and socio-demographic characteristics, CALD women were significantly more likely than non-CALD women to experience suboptimal staff technical competence in pregnancy, overall perceived safety in pregnancy and labour/birth, and interpersonal sensitivity in pregnancy and labour/birth. Approximately 50% of CALD women did not have the choice to use a translator or interpreter, or the gender of their care provider, during labour and birth. Thirteen themes of preferences and needs of CALD maternity care consumers based on ethnicity, cultural beliefs, or traditions were identified, however, these were rarely met. Findings imply that CALD women in Queensland experience disadvantageous maternity care with regards to perceived staff technical competence, safety, and interpersonal sensitivity, and receive care that lacks cultural competence. Improved access to support persons, continuity and choice of carer, and staff availability and training is recommended.

Many women creative practice-led researchers appear inhibited by a number of factors directly connected to their gender. This paper discusses these factors, including the culture of visual arts professional practice, the circumstances surrounding women postgraduate students, and unproductive self-theories about intelligence and creativity. A number of feminist strategies are discussed as potential interventions that may assist women creative practice-led researchers and their supervisors to reap more personal and professional rewards from their postgraduate research.

Background To investigate potential cardiovascular and other effects of long-term pharmacological interleukin 1 (IL-1) inhibition, we studied genetic variants that produce inhibition of IL-1, a master regulator of inflammation. Methods We created a genetic score combining the effects of alleles of two common variants (rs6743376 and rs1542176) that are located upstream of IL1RN, the gene encoding the IL-1 receptor antagonist (IL-1Ra; an endogenous inhibitor of both IL-1α and IL-1β); both alleles increase soluble IL-1Ra protein concentration. We compared effects on inflammation biomarkers of this genetic score with those of anakinra, the recombinant form of IL-1Ra, which has previously been studied in randomised trials of rheumatoid arthritis and other inflammatory disorders. In primary analyses, we investigated the score in relation to rheumatoid arthritis and four cardiometabolic diseases (type 2 diabetes, coronary heart disease, ischaemic stroke, and abdominal aortic aneurysm; 453 411 total participants). In exploratory analyses, we studied the relation of the score to many disease traits and to 24 other disorders of proposed relevance to IL-1 signalling (746 171 total participants). Findings For each IL1RN minor allele inherited, serum concentrations of IL-1Ra increased by 0·22 SD (95% CI 0·18–0·25; 12·5%; p=9·3 × 10−33), concentrations of interleukin 6 decreased by 0·02 SD (−0·04 to −0·01; −1·7%; p=3·5 × 10−3), and concentrations of C-reactive protein decreased by 0·03 SD (−0·04 to −0·02; −3·4%; p=7·7 × 10−14). We noted the effects of the genetic score on these inflammation biomarkers to be directionally concordant with those of anakinra. The allele count of the genetic score had roughly log-linear, dose-dependent associations with both IL-1Ra concentration and risk of coronary heart disease. For people who carried four IL-1Ra-raising alleles, the odds ratio for coronary heart disease was 1·15 (1·08–1·22; p=1·8 × 10−6) compared with people who carried no IL-1Ra-raising alleles; the per-allele odds ratio for coronary heart disease was 1·03 (1·02–1·04; p=3·9 × 10−10). Per-allele odds ratios were 0·97 (0·95–0·99; p=9·9 × 10−4) for rheumatoid arthritis, 0·99 (0·97–1·01; p=0·47) for type 2 diabetes, 1·00 (0·98–1·02; p=0·92) for ischaemic stroke, and 1·08 (1·04–1·12; p=1·8 × 10−5) for abdominal aortic aneurysm. In exploratory analyses, we observed per-allele increases in concentrations of proatherogenic lipids, including LDL-cholesterol, but no clear evidence of association for blood pressure, glycaemic traits, or any of the 24 other disorders studied. Modelling suggested that the observed increase in LDL-cholesterol could account for about a third of the association observed between the genetic score and increased coronary risk. Interpretation Human genetic data suggest that long-term dual IL-1α/β inhibition could increase cardiovascular risk and, conversely, reduce the risk of development of rheumatoid arthritis. The cardiovascular risk might, in part, be mediated through an increase in proatherogenic lipid concentrations. Diversification and expansion of global higher education in the 21st century, has resulted in Learning Landscapes for architectural education that can no longer be sustained by the traditional model. Changes have resulted because of surging student numbers, extensions to traditional curricula, evolving competency standards and accreditation requirements, and modified geographical and pedagogical boundaries. The influx of available new technology has helped to democratise knowledge, transforming when, where and how learning takes place. Pressures on government funded higher education budgets highlight the need for a critical review of the current approach to the design and use of learning environments. Efficient design of physical space contributes significantly to savings in provision, management and use of facilities, while also potentially improving pedagogical quality. The purpose of this research is to identify emerging trends in the design of future Learning Landscapes for architectural education in Australasia; to understand where and how students of architecture are likely to learn, in the future context. It explores the important linkages between space, place, pedagogy, technology and context, using a multi methodological qualitative research approach. An Australasian context study will explore the Learning Landscapes of 23 Schools of Architecture across Australia, New Zealand and Papua New Guinea. The focus of this paper is on the methodology which is being employed to undertake dynamic data collection for the study. The research will be determined through mapping all forms of architectural learning environments, pedagogical approaches and contextual issues, to bridge the gap between academic theory, and architectural design practice. An initial understanding that pedagogy is an intrinsic component imbedded within the design of learning environments, will play an important role. Active learning environments which are exemplified by the architectural design studio, support dynamic project based and collaborative connected learning models. These have recently become a lot more common in disciplines outside of design and the arts. It is anticipated, therefore, that the implications for this research may well have a positive impact far beyond the confines of the architectural studio learning environment.

Biological control techniques attract increasing attention as one of the sustainable alternatives to pesticide use in integrated pest management programs. In order to develop sustainable pest management methods for arable crops based on entomopathogenic nematodes (EPN), their efficacy and persistence needed to be investigated, and an economically feasible delivery system had to be developed. In this study, first a survey of entomopathogens was conducted, and a system approach was tested, using the oilseed Brassica (OSB) growing system (OSB, spring wheat, and red clover) as a model. The system approach aimed at determining the potential of Steinernema feltiae (Filipjev) for the control of OSB pests, developing OSB rotation schemes that support EPN persistence, and investigating the impact of the selected biotic and abiotic factors on efficacy and persistence of EPN. This study employed abductive logic (which employs constant interplay between the theory and empirical observation), quantitative methods, and a case study on OSB. Laboratory and field experiments were carried out, and two types of pathogen surveys. A horizontal survey included OSB fields across Estonia, Germany, Poland, Sweden and the UK, while a vertical survey included sampling from two sets of differently managed experimental fields during three years. A new approach was introduced for measuring occurrence, where the prevalence and relative intensity of entomopathogens, biotic agents, and unidentified insect antagonists were determined. The effect of dose, timing, and the application method on S. feltiae in the control of pests in OSB, and the potential of a controlled release delivery system (CRS) were evaluated in the field. Studies on the impact of selected biotic and abiotc factors (Brassica plant, bait insects, developmental stages of Meligethes aeneus Fab., Isaria fumosorosea Wize (Ifr), and organic and synthetic fertilizers) on the efficacy of S. feltiae were conducted in the laboratory. Persistence of S. feltiae in the OSB growing system, and the effect of dose, timing, and the application method, was assessed in the field as part of the efficacy experiments. The impact of selected biotic and abiotic factors on S. feltiae persistence was assessed in laboratory experiments. The pathogen survey showed that the occurrence of entomopathogens is low in the OSB growing system, and that a management system causing less disturbance (ICM) to the soil increases the relative intensity of insect parasitic nematodes and other insect antagonists. A longer study period is required to show any possible impact of ICM on the relative intensity of entomopathogenic fungi, or on the prevalence of entomopathogens. Two different measures of the occurrence yielded different results: the relative intensity revealed the difference between the two different crop management methods, while prevalence did not. The highest efficacy of S. feltiae was achieved by using a low dose and targeting all stages of M. aeneus. When only the larval stage was targeted, the application method and dose had no significant effect. The CRS decreased the pest abundance significantly more than the surface application method. S. feltiae persisted in the OSB fields in Finland for several months, but did not survive the winter. The strain survived for 7 months when it was applied in autumn in Germany, but its populations declined rapidly after winter. The examined biotic and abiotic factors had variable impacts on S. feltiae efficacy and persistence. The two measures, prevalence and relative intensity of entomopathogens, gave valuable information for their use in biocontrol programs. The recommended biocontrol strategy for OSB growing in Finland is inundation and seasonal inoculation of EPN. The impact of some biotic and abiotic factors on S. feltiae efficacy and persistence is significant, and can be used to improve the efficacy of EPN. The CRS is a novel alternative for EPN application, and should also be considered for use on other crops. The objective of this qualitative study is to reveal the discursive reality of Finnish women, who have chosen to follow their husbands to expatriate assignments. The research material includes interviews of seventeen (17) Finnish housewives who lived in Singapore in 1999. The primary theoretical framework of the study is a social-constructive view, in which the reality is seen to have been constructed through meanings that create and maintain cultural practices, social roles and institutions. Gender is interpreted as produced socially, politically and in language. Human beings in the study exist as cultural and social beings with sex. An underlying assumption of the analysis is that the women in the study do not recognize their experience from their position that has been located to the prompt box in a play of expatriate assignment a role that is offered to them by the business sciences. Referring to Somers (1994) these women suffer of narrative silence as they lack public narratives that correspond to their circumstances. According to Williams (1983), an experience is an evidence of conditions. Therefore, the experiences of the women, who have followed their husbands to expatriate assignments, include information of their conditions. The analysis of the interview material has been performed in two phases: First, the women's experiences are identified from the research material. Thereafter, by means of discourse analysis the experiences are revealed analytically. This revealing process can also be regarded as an articulation of a counter-narrative. The research results can be found from three such discursive interpretations that are offered to the women and from eight such discursive interpretations that can be drawn from the women's own experiences. One of the discursive interpretations, which came out from the women's own experiences, is named as cultural dysphasia. In the study, cultural dysphasia is defined as a condition, in which the women have a difficulty in making their lived reality understood by others outside the sphere of their situation. Finnish women do not only lack public narrative, but the absence of a housewife-culture in Finland prevents them from any public narrative that would have a positive tone. To avoid dependence on the housewife concept, a woman's decision to follow her husband to an expatriate assignment is interpreted as a demonstration of solidarity to the relationship. In this connection, these Finnish women are re-named as 'siirtonainen' (Finnish, literally 'transfer woman'). This is the first scientific study to make visible the lived reality of Finnish women who have followed their husbands to expatriate assignments. The study will help the women in similar circumstances to find their marks among the current narratives. By reducing dichotomy between housewives and career women, which is damaging to women, and by creating the concept of 'siirtonainen' it will be possible to expand the cultural space of Finnish women. Finally, the study argues that a Finnish career woman, grown together with the imperative of self-support, has been lifted up as one symbol of modern Finland. Este estudo buscou identificar os critérios utilizados pelas enfermeiras obstétricas para empregar as tecnologias não-invasivas de cuidado no suporte físico à parturiente e quais são seus efeitos esperados. Para tanto, se realizou uma pesquisa quantitativa exploratória do tipo survey, que abordou as práticas/cuidados fornecidas pelas enfermeiras obstétrica durante a assistência ás parturientes que tinham relação com o suporte físico. Estas foram agrupadas em: suporte relacionado ao ambiente; suporte ao posicionamento (livre movimentação e adoção de posturas verticais, deambulação, movimentos pélvicos, posição de cócoras, e posição de quatro apoios); suporte aos estímulos táteis (massagens, compressas frias/mornas, banho morno de aspersão e imersão); e suporte energético (oferta de alimento). Para tanto, utilizou-se um questionário, que foi disponibilizado via internet, após a criação de um domínio e web site próprio para tal finalidade. A população-alvo constituiu-se de enfermeiras obstétricas que atuam no cuidado da parturiente em território nacional, sendo que estas foram convidadas a participar, entre os meses de julho e setembro de 2009, através de e-mails survey individuais ou coletivos. Participaram do estudo120 profissionais, sendo que 45,8% foram excluídos automaticamente pelo sistema, pois não possuíam os critérios de inclusão. Dos participantes elegíveis (65), 33,8% responderam somente o teste de elegibilidade, 4,6% responderam parcialmente e 61,6% responderam completamente o questionário. Os resultados demonstraram que alguns aspectos relacionados ao conceito ainda encontram-se pouco compreendidos pelas profissionais da área, entretanto este não é um entrave para que práticas/cuidados relacionados a esta nova terminologia sejam utilizadas durante a assistência à parturiente. De acordo com os objetivos propostos, conseguiu-se determinar os critérios e efeitos esperados pelas enfermeiras obstétricas ao utilizarem as tecnologias não-invasivas de cuidado estudadas, entretanto também se evidenciou brechas no conhecimento científico. Assim conclui-se que as enfermeiras obstétricas utilizam práticas/cuidados relacionadas as tecnologias não-invasivas de cuidado de enfermagem obstétrica no suporte físico à parturiente, pautadas em critérios e efeitos esperados que em sua maioria possuem bases científicas que os comprove. Considera-se que a utilização destas são uma ferramenta importante para a desmedicalização do processo de parto e consequentemente, para a diminuição dos índices de morbimortalidade materna e neonatall. Portanto, é necessário estimular à assistência ao parto por enfermeiras obstétricas, de modo a suplantar o modelo de assistência tecnocrático, ainda hegemônico no país.

Tap water is not sterile; it contains organisms which grow in water distribution systems or inside taps and their fittings. The absence of known pathogenic bacteria is assured by the absence of the indicator organisms but concerns have been raised in the past few years that drinking water fulfilling the standards laid down in the EC Directive ECC 80/778 may still cause disease. These concerns have arisen from several sources: the fact that a cause has been identified in only half of all suspected waterborne outbreaks of disease; reports have suggested that heterotrophic bacteria possessing single pathogenic mechanisms such as haemolysin may cause disease; reports of heterotrophic organisms causing water contact diseases in hospitals. These concerns led to a reappraisal of the pathogenic potential of heteretrophic bacteria, by carrying out an extensive literature search and review commissioned by the UK Water Research Company. This research identified many papers showing an association between drinking water and heterotrophic bacteria but only very few reports of suspected waterborne disease associated with the heterotrophs. The organisms demonstrating potential to cause disease were species of Aeromonas and Yersinia, but typing of organisms identified in patients and isolated from the water revealed very few similarities. The potential of Aeromonas and Yersinia to cause waterborne disease is thought to be very low and the Communicable Disease Surveillance Centre database of laboratory infections due to these two genera of organisms was analysed to produce population-related incidences for each health region in England and Wales. Additionally a laboratory questionnaire revealed different levels of ascertainment of these two organisms in different laboratories of the Public Health Laboratory Service.

O presente estudo analisa a rede pública de assistência em saúde mental no município de Piraí no estado do Rio de Janeiro. A proposta da reforma psiquiátrica em curso no Brasil, gerou a constituição de uma rede extra hospitalar de cuidados, que têm nos CAPS, os Centros de Atenção Psicossocial, os serviços de saúde mental de base territorial, um dos principais representantes deste percurso de mudanças. Utiliza-se o CAPS de Piraí, como referencial, já que esta Unidade é designada como prestadora de assistência direta, através de procedimentos em saúde, bem como referência para as demais ações de saúde mental no município, vinculadas à atenção básica e o hospital geral. O resgate da história da reorientação de ações no município, através da pesquisa documental e o levantamento de dados acerca do perfil de assistência prestada no CAPS permite a análise acerca da demanda e oferta de serviços , em sua relação com a normatização proposta pelo ministério da saúde para as ações nesta área e os paradigmas da integralidade e da desinstitucionalização. O produto desta análise destaca as mudanças significativas ocorridas nos últimos anos no cotidiano da assistência, a potência do CAPS na ruptura de paradigmas em saúde e a necessidade do avanço em práticas avaliativas em saúde mental, como instrumento de qualificação de ações para a área.

Background: Neonatal trials remain difficult to conduct for several reasons: in particular the need for study sites to have an existing infrastructure in place, with trained investigators and validated quality procedures to ensure good clinical, laboratory practices and a respect for high ethical standards. The objective of this work was to identify the major criteria considered necessary for selecting neonatal intensive care units that are able to perform drug evaluations competently. Methodology and Main Findings: This Delphi process was conducted with an international multidisciplinary panel of 25 experts from 13 countries, selected to be part of two committees (a scientific committee and an expert committee), in order to validate criteria required to perform drug evaluation in neonates. Eighty six items were initially selected and classified under 7 headings: "NICUs description - Level of care'' (21), "Ability to perform drug trials: NICU organization and processes (15), "Research Experience'' (12), "Scientific competencies and area of expertise'' (8), "Quality Management'' (16), "Training and educational capacity'' (8) and "Public involvement'' (6). Sixty-one items were retained and headings were rearranged after the first round, 34 were selected after the second round. A third round was required to validate 13 additional items. The final set includes 47 items divided under 5 headings. Conclusion: A set of 47 relevant criteria will help to NICUs that want to implement, conduct or participate in drug trials within a neonatal network identify important issues to be aware of.
