Context: The complexity of genetic testing in Kallmann syndrome (KS) is growing and costly. Thus, it is important to leverage the clinical evaluations of KS patients to prioritize genetic screening. Objective: The objective of the study was to determine which reproductive and nonreproductive phenotypes of KS subjects have implications for specific gene mutations. Subjects: Two hundred nineteen KS patients were studied: 151 with identified rare sequence variants (RSVs) in 8 genes known to cause KS (KAL1, NELF, CHD7, HS6ST1, FGF8/FGFR1, or PROK2/PROKR2) and 68 KS subjects who remain RSV negative for all 8 genes. Main Outcome Measures: Reproductive and nonreproductive phenotypes within each genetic group were measured. Results: Male KS subjects with KAL1 RSVs displayed the most severe reproductive phenotype with testicular volumes (TVs) at presentation of 1.5 ± 0.1 mL vs 3.7 ± 0.3 mL, P < .05 vs all non-KAL1 probands. In both sexes, synkinesia was enriched but not unique to patients with KAL1 RSVs compared with KAL1-negative probands (43% vs 12%; P < .05). Similarly, dental agenesis and digital bone abnormalities were enriched in patients with RSVs in the FGF8/FGFR1 signaling pathway compared with all other gene groups combined (39% vs 4% and 23% vs 0%; P < .05, respectively). Hearing loss marked the probands with CHD7 RSVs (40% vs 13% in non-CHD7 probands; P < .05). Renal agenesis and cleft lip/palate did not emerge as statistically significant phenotypic predictors. Conclusions: Certain clinical features in men and women are highly associated with genetic causes of KS. Synkinesia (KAL1), dental agenesis (FGF8/FGFR1), digital bony abnormalities (FGF8/FGFR1), and hearing loss (CHD7) can be useful for prioritizing genetic screening.

BACKGROUND: In recent decades, there have been substantial changes in mortality from urologic cancers in Europe. OBJECTIVE: To provide updated information, we analyzed trends in mortality from cancer of the prostate, testis, bladder, and kidney in Europe from 1970 to 2008. DESIGN, SETTING, AND PARTICIPANTS: We derived data for 33 European countries from the World Health Organization database. MEASUREMENTS: We computed world-standardized mortality rates and used joinpoint regression to identify significant changes in trends. RESULTS AND LIMITATIONS: Mortality from prostate cancer has leveled off since the 1990s in countries of western and northern Europe, particularly over the last few years while it was still rising in Bulgaria, Romania, and Russia. In the European Union (EU), it reached a peak in 1995 at 15.0 per 100 000 men and declined to 12.5 per 100 000 in 2006. Mortality from testicular cancer has steadily declined in most countries in western and northern Europe since the 1970s. The declines were later and appreciably lower in central/eastern Europe. In EU, rates declined from 0.75 in 1980 to 0.32 per 100 000 men in 2006, with stronger declines up to the late 1990s and an apparent leveling off in rates thereafter. Over the last 15 years, mortality from bladder cancer has declined in most European countries in both sexes. The major exceptions were Bulgaria, Poland, and Romania. In the EU, bladder cancer mortality was stable until 1992 and declined thereafter from 7.3 to 5.5 per 100 000 men and from 1.5 to 1.2 per 100 000 women in 2006. Mortality from kidney cancer increased throughout Europe until the early 1990s and leveled off thereafter in many countries, except in a few central and eastern ones. Between 1994 and 2006, rates declined from 4.9 to 4.3 per 100 000 in EU men and from 2.1 to 1.8 per 100 000 in EU women. CONCLUSIONS: Over the last two decades, trends in urologic cancer mortality were favorable in Europe, with the exception of a few central and eastern countries.

Testis size and sperm production are directly correlated to the total number of adult Sertoli cells (SCs). Although the establishment of an adequate number of SCs is crucial for future male fertility, the identification and characterization of the factors regulating SC survival, proliferation, and maturation remain incomplete. To investigate whether the IGF system is required for germ cell (GC) and SC development and function, we inactivated the insulin receptor (Insr), the IGF1 receptor (Igf1r), or both receptors specifically in the GC lineage or in SCs. Whereas ablation of insulin/IGF signaling appears dispensable for GCs and spermatogenesis, adult testes of mice lacking both Insr and Igf1r in SCs (SC-Insr;Igf1r) displayed a 75% reduction in testis size and daily sperm production as a result of a reduced proliferation rate of immature SCs during the late fetal and early neonatal testicular period. In addition, in vivo analyses revealed that FSH requires the insulin/IGF signaling pathway to mediate its proliferative effects on immature SCs. Collectively, these results emphasize the essential role played by growth factors of the insulin family in regulating the final number of SCs, testis size, and daily sperm output. They also indicate that the insulin/IGF signaling pathway is required for FSH-mediated SC proliferation.

Small non-coding RNAs act as critical regulators of gene expression and are essential for male germ cell development and spermatogenesis. Previously, we showed that germ cell-specific inactivation of Dicer1, an endonuclease essential for the biogenesis of micro-RNAs (miRNAs) and endogenous small interfering RNAs (endo-siRNAs), led to complete male infertility due to alterations in meiotic progression, increased spermatocyte apoptosis and defects in the maturation of spermatozoa. To dissect the distinct physiological roles of miRNAs and endo-siRNAs in spermatogenesis, we compared the testicular phenotype of mice with Dicer1 or Dgcr8 depletion in male germ cells. Dgcr8 mutant mice, which have a defective miRNA pathway while retaining an intact endo-siRNA pathway, were also infertile and displayed similar defects, although less severe, to Dicer1 mutant mice. These included cumulative defects in meiotic and haploid phases of spermatogenesis, resulting in oligo-, terato-, and azoospermia. In addition, we found by RNA sequencing of purified spermatocytes that inactivation of Dicer1 and the resulting absence of miRNAs affected the fine tuning of protein-coding gene expression by increasing low level gene expression. Overall, these results emphasize the essential role of miRNAs in the progression of spermatogenesis, but also indicate a role for endo-siRNAs in this process.

Este trabalho avaliou o efeito do farelo e da farinha de semente de algodão no ganho de peso e na atividade testicular de alevinos de tilápia-do-nilo (Oreochromis niloticus). Cento e cinqüenta alevinos com peso médio inicial de 1,26 g foram mantidos em aquários de 45 L, com renovação contínua d'água, e alimentados por 120 dias com dietas isoprotéicas (28% PB) e isocalóricas (4.030 kcal EB/kg de ração) contendo 0%, 2%, 4% e 6% de semente de algodão descorticada e moída, e 24% de farelo de algodão. Foi observado que os níveis avaliados interferem na atividade testicular, diminuindo a espermatogênese, embora não tenham comprometido de forma significativa o ganho de peso dos alevinos.

Ejaculados de seis bodes Moxotó e seis ½ sangue Moxotó-Pardo Alpina, adultos, mantidos em confinamento, foram avaliados antes e após a insulação do saco escrotal com bolsa de plástico, de parede dupla, durante 6,5 dias. A degeneração seminal ocorreu em todos os animais na 4ª semana após o início da insulação, ressaltando-se a redução na concentração, o aumento dos defeitos espermáticos e a redução do vigor celular, culminando com a necrospermia. A motilidade individual progressiva (MIP) atingiu os valores mais baixos na 3ª semana após o início da insulação, retornando aos valores normais entre a 8ª e 9ª semanas. Os defeitos espermáticos começaram a aumentar aos sete dias após o início da insulação escrotal. O volume apresentou oscilações atípicas e o aumento da temperatura escrotal afetou os parâmetros físicos e morfológicos do ejaculado de maneira consistente evidenciando-se o efeito negativo sobre a qualidade do sêmen.

INTRODUCTION: The pathogenic mechanism of orthostatic proteinuria has not yet been clearly established. OBSERVATION: In a tall, thin, 21 year-old man, isolated proteinuria was discovered during an urological control conducted one year after a bilateral orchidopexy following left testicular torsion. Proteinuria was orthostatic. Doppler examination of the kidney revealed an entrapment of the left renal vein (nutcracker phenomenon-NCP). COMMENTS: An NCP was diagnosed in a young patient presenting with orthostatic proteinuria. By provoking modifications in intraglomerular haemodynamics, the NCP may, in nearly half of the cases, be at the origin of orthostatic proteinuria. Doppler examination is the diagnostic method of choice in the screening for NCP.

Context: Both biallelic and monoallelic mutations in PROK2 or PROKR2 have been found in Kallmann syndrome (KS). Objective: The objective of the study was to compare the phenotypes of KS patients harboring monoallelic and biallelic mutations in these genes. Design and Patients: We studied clinical and endocrine features that reflect the functioning of the pituitary-gonadal axis, and the nonreproductive phenotype, in 55 adult KS patients (42 men and 13 women), of whom 41 had monoallelic mutations and 14 biallelic mutations in PROK2 or PROKR2. Results: Biallelic mutations were associated with more frequent cryptorchidism (70% vs. 34%, P < 0.05) and microphallus (90% vs. 28%, P < 0.001) and lower mean testicular volume (1.2 +/- 0.4 vs. 4.5 +/- 6.0 ml; P < 0.01) in male patients. Likewise, the testosterone level as well as the basal FSH level and peak LH level under GnRH-stimulation were lower in males with biallelic mutations (0.2 +/- 0.1 vs. 0.7 +/- 0.8 ng/ml; P = 0.05, 0.3 +/- 0.1 vs. 1.8 +/- 3.0 IU/liter; P < 0.05, and 0.8 +/- 0.8 vs. 5.2 +/- 5.5 IU/liter; P < 0.05, respectively). Nonreproductive, nonolfactory anomalies were rare in both sexes and were never found in patients with biallelic mutations. The mean body mass index of the patients (23.9 +/- 4.2 kg/m(2) in males and 26.3 +/- 6.6 kg/m(2) in females) did not differ significantly from that of gender-, age-, and treatment-matched KS individuals who did not carry a mutation in PROK2 or PROKR2. Finally, circadian cortisol levels evaluated in five patients, including one with biallelic PROKR2 mutations, were normal in all cases. Conclusion: Male patients carrying biallelic mutations in PROK2 or PROKR2 have a less variable and on average a more severe reproductive phenotype than patients carrying monoallelic mutations in these genes. Nonreproductive, nonolfactory clinical anomalies associated with KS seem to be restricted to patients with monoallelic mutations.

Soy and soy-based products are widely consumed by infants and adult individuals. There has been speculation that the presence of isoflavone phytoestrogens in soybean cause adverse effects on the development and function of the male reproductive system. The purpose of this study was to examine the influence of dietary soy and phytoestrogens on testicular and reproductive functions. Male mice were fed from conception to adulthood with either a high soy-containing diet or a soy-free diet. Although adult mice fed a soy-rich diet exhibited normal male behaviour and were fertile, we observed a reduced proportion of haploid germ cells in testes correlating with a 25% decrease in epididymal sperm counts and a 21% reduction in litter size. LH and androgens levels were not affected but transcripts coding for androgen-response genes in Sertoli cells and Gapd-s, a germ cell-specific gene involved in sperm glycolysis and mobility were significantly reduced. In addition, we found that dietary soy decreased the size of the seminal vesicle but without affecting its proteolytic activity. Taken together, these studies show that long-term exposure to dietary soy and phytoestrogens may affect male reproductive function resulting in a small decrease in sperm count and fertility.

O objetivo deste trabalho foi comparar uma raça naturalizada brasileira com uma raça européia quanto à adaptação às altas temperaturas. Foram avaliados os efeitos da insulação escrotal, por 48 horas, nas características seminais de touros Curraleiros (n = 5) e Holandeses (n = 6). Uma seqüência de coletas de sêmen e avaliação andrológica dos touros foi realizada antes (duas coletas) e depois (nove coletas semanais) da insulação escrotal. A motilidade espermática progressiva teve queda significativamente menor na raça Curraleira (25%) do que na Holandesa (50%), no período pós-insulação, e retornou aos valores pré-insulação mais precocemente nos touros Curraleiros, aos 37 dias após a insulação escrotal. O vigor espermático nos Curraleiros (15,2%) teve queda menor do que nos Holandeses (47,8%). O número de espermatozóides por ejaculado caiu de forma similar nos dois grupos depois da insulação, respectivamente 67,9 e 79,4%, para Curraleiros e Holandeses, entre 16 e 23 dias após a insulação. A porcentagem de espermatozóides anormais atingiu valores máximos entre 16 e 30 dias após a insulação e foi maior aos 30 dias nos touros Holandeses (41,3±5,9%). Os touros Curraleiros são mais tolerantes aos efeitos da insulação escrotal nas características seminais do que os Holandeses.

O objetivo deste trabalho foi avaliar a presença do DNA pró-viral do lentivírus caprino (LVC) em ejaculados de machos infectados naturalmente, e verificar a influência da lavagem do sêmen e da presença de inflamação testicular sobre a carga viral. Foram realizadas oito coletas de sêmen de sete reprodutores soropositivos para o LVC: quatro antes dos animais sofrerem dano testicular e quatro depois. Entre as coletas realizadas na mesma semana, em uma, o ejaculado era lavado, para retirada do plasma seminal, e na outra, não. O DNA pró-viral do LVC foi identificado pela reação em cadeia da polimerase Nested (PCR Nested), e pelo isolamento viral. O vírus foi isolado em 7,1% das amostras. A PCR identificou o DNA pró-viral em 35,7% do total das amostras: 17,9% nas amostras lavadas e 53,6% das amostras de sêmen integrais. O dano ao testículo permite maior fluxo do vírus para o sêmen, pois antes do dano, 21,4% das amostras foram positivas e pós-dano, 50%. A transmissão do LVC pelo sêmen de reprodutores caprinos é potencializada pela presença de inflamações testiculares e pelo fato de o sêmen criopreservado conter o LVC na forma infectante.

O objetivo deste trabalho foi estimar correlações genéticas e fenotípicas de características reprodutivas e ponderais em 579 touros Nelore, em criação extensiva. As características reprodutivas estudadas foram: circunferência escrotal, consistência, volume e forma testiculares, comprimento dos testículos esquerdo e direito, largura dos testículos esquerdo e direito, motilidade e vigor espermáticos, defeitos espermáticos maiores, menores e totais e classificação andrológica por pontos. As características foram analisadas pelo método de máxima verossimilhança restrita, com algoritmos livres de derivadas, sob modelo animal, com inclusão da matriz de numeradores dos coeficientes de parentesco entre os animais e seus ascendentes. As correlações genéticas entre circunferência escrotal e as características peso corporal, volume testicular, motilidade espermática, vigor espermático, defeitos espermáticos menores, defeitos espermáticos totais e classificação andrológica por pontos foram, respectivamente, 0,72, 0,99, 0,72, 0,60, -0,67, -0,12 e 0,64. As maiores correlações fenotípicas encontradas entre peso e circunferência escrotal, características físicas e morfológicas do sêmen, quando comparadas às correlações entre idade e as mesmas características, são indicativas de que o peso tem maior influência na condição reprodutiva. As correlações genéticas entre classificação andrológica por pontos e as características: peso, circunferência escrotal, volume testicular, defeitos espermáticos maiores e defeitos espermáticos totais foram, respectivamente, 0,19, 0,64, 0,71, -0,47 e -0,58.

Spermatogenesis relies on the precise regulation of the self-renewal and differentiation of spermatogonia to provide a continuous supply of differentiating germ cells. The understanding of the cellular pathways regulating this equilibrium remains unfortunately incomplete. This investigation aimed to elucidate the testicular and ovarian functions of the glucocorticoid-induced leucine zipper protein (GILZ) encoded by the X-linked Tsc22d3 (Gilz) gene. We found that GILZ is specifically expressed in the cytoplasm of proliferating spermatogonia and preleptotene spermatocytes. While Gilz mutant female mice were fully fertile, constitutive or male germ cell-specific ablation of Gilz led to sterility due to a complete absence of post-meiotic germ cells and mature spermatozoa. Alterations were observed as early as postnatal day 5 during the first spermatogenic wave and included extensive apoptosis at the spermatogonial level and meiotic arrest in the mid-late zygotene stage. Overall, these data emphasize the essential role played by GILZ in mediating spermatogonial survival and spermatogenesis.

O objetivo deste trabalho foi avaliar a ação imunoesterilizadora de uma vacina anti-hormônio liberador de hormônio luteinizante (LHRH), composta por ovalbumina-LHRH-7 e tiorredoxina-LHRH-7, em touros mestiços Nelore. Vinte e seis touros, com dois anos de idade, foram distribuídos aleatoriamente em dois grupos de 13 animais. No grupo I, os animais receberam uma dose e dois reforços da vacina nos dias 0, 141, e 287 do experimento. No grupo II, os animais não receberam nenhum tratamento (controle). Para avaliar o efeito da vacina nos touros, foi realizada a mensuração da circunferência escrotal no início do experimento e no dia do abate, 741 dias depois. Por ocasião do abate, também foi coletada uma amostra dos testículos para avaliação histológica. O grupo imunizado apresentou circunferência escrotal ao abate de 22±5,98 cm, menor do que a do grupo controle que foi de 35,6±2,4 cm. Na análise histológica dos animais do grupo imunizado, foi observada degeneração testicular com ausência de espermatozoides em 85% dos animais avaliados, os outros 15% apresentaram redução no número de espermatozoides, em comparação aos animais do grupo controle. A vacina anti-LHRH, com fusão de proteínas, é efetiva na castração imunológica de touros e deve ser considerada como alternativa para utilização na produção bovina extensiva no Brasil.

PURPOSE: The purpose of this study was to analyze prognosis and treatment results for seminoma arising in corrected and uncorrected inguinal cryptorchidism (SCIC and SUIC). METHODS AND MATERIALS: We reviewed 66 patients with inguinal seminomas between June 1958 and December 1991 at the Cancer Hospital and Institute of Chinese Academy of Medical Sciences. Of these patients, 23 had prior orchiopexy and 43 presented with an inguinal form of cryptorchidism. At presentation, 17 of 66 (26%) patients had nodal metastases. This nodal involvement was 30% (7 of 23) for SCIC and 23% (10 of 43) for SUIC, respectively. These numbers are comparable with those in a series of patients treated for scrotal seminoma at our institution (26% vs. 20%). However, 3 of 23 (13%) patients who had prior orchiopexy presented with inguinal nodal metastasis as compared with 0 of 43 patients with SUIC or 4 of 237 patients with scrotal seminoma (p < .05). There were 49 stage I, 5 stage IIA, 8 stage IIB, 3 stage III, and 1 stage IV patients. All patients underwent radical orchiectomy and received further radiotherapy, chemotherapy, or both. Patients with stage I and stage II disease were treated primarily with radiotherapy, whereas patients with stage III and IV disease were treated with chemotherapy. RESULTS: The overall and disease-free survival at 5 and 10 years was 94% and 92%, 89% and 87%, respectively. The overall 5- and 10-year survival by stage was 100% and 100% for stage I, and 77% and 68% for stage II, respectively (p < .05). There was no significant difference in survival between SUIC and SCIC (93% vs. 96% at 5 years). Four patients developed relapse. Two of these four patients experienced relapse at the inguinal area, due to a marginal miss. Three of four patients with relapse were successfully salvaged, and one died of disease. CONCLUSION: Our results indicate that prognosis for inguinal seminoma is excellent and similar to that of scrotal seminoma. Postorchiectomy radiotherapy can be considered as the standard treatment for stage I and IIA inguinal seminoma. We recommend routinely including the para-aortic and ipsilateral pelvic nodes.
