Objective: This study aimed to evaluate the accessibility to oral health services in Santa Cruz (RN), focusing on the organizational aspect which is characterized by variables like delay in getting consultation, waiting lines, difficulty scheduling appointments, lack of material, among others. Material and Methods: For such purpose, questionnaires were addressed to 194 users’ houses based on census tract. Results: It has been found that 12.9% of the population has never gone to the dentist’s office, and that the service demand has not been affected by users’ individual and socioeconomic characteristics, except for gender. Data have revealed that 36.1% of users went to the dentist’s in less than a year. In addition, the youngest users are among those who seek the dentist’s office more frequently (p<0.05). 63.3% of the interviewees have reported they find some kind of difficulty when seek for dental care, and 43.2% of users have waited three weeks or more for assistance. It is also noteworthy that 7.4% of the interviewees have been finding difficulties in getting access to urgency care, and that long wait for the consultation is the most frequently cited matter. Conclusion: It could be verified that accessibility to oral health services in the municipality of Santa Cruz (RN) has been hampered by factors related to public policies organization, especially with regards to the work process.

In the early 1990s, a major milestone in the treatment of Acquired Immune Deficiency Syndrome was the development of highly active combination antiretroviral therapy. The great benefit generated by the use of this therapy was prolonging the survival of the people who got this disease, since it is no longer considered fatal, becoming a chronic condition. Despite improvements generated by this therapy, there are still many difficulties to be overcome. One is the patient adherence to their treatment, bringing challenges to services and health professionals. Hence the need for early identification of nursing diagnosis Lack of Accession so that solutions are sought by the nurse with the patient and his family. With this problem, adds to the difficulty of hospital nurses in inferring that diagnosis, especially in identifying their defining characteristics. In this context, the objective was to evaluate the accuracy of clinical indicators of nursing diagnosis Lack of Adherence to antiretroviral treatment for people living with the Acquired Immunodeficiency Syndrome. The research took place in two stages. The first consists of the evaluation of the diagnostic indicators in the study; and second, the diagnostic inference performed by specialist nurses. The first step took place in a referral hospital in the treatment of infectious diseases in the Northeast of Brazil, and data were collected through an instrument for carrying out history and physical examination and analyzed for the presence or absence of the diagnostic indicators. In the second stage, the data were sent to experts, who judged the presence or absence of the diagnosis in the studied clientele. The project was submitted to the Ethics Committee of the Federal University of Rio Grande do Norte, obtaining approval with the General Certificate for Ethics Assessment (CAAE) No 46206215.3.0000.5537. Data were analyzed using descriptive and inferential statistics. Test were used Fisher's exact, chi-square test of Pearson and logistic regression. Since the accuracy of clinical indicators was measured by sensitivity, specificity, predictive values, likelihood ratios. As a result, we identified the presence of diagnosis Lack of Accession on 69% (n = 78) of the study patients. The defining characteristics that showed statistically significant association with the diagnosis studied were: lack of adherence behavior, complications related to development, missing scheduled appointments, failure to achieve results, and exacerbation of symptoms. The characteristic with greater sensitivity was missing scheduled appointments and the highest specificity behavior of noncompliance. The logistic regression showed as predictors for the diagnosis Lack of Accession: lack of adherence behavior, missing scheduled appointments, failure to achieve results, and exacerbation of symptoms. It was concluded that the identification of clinical indicators accurately enabled a good prediction of the nursing diagnosis Lack of Accession on people living with the Acquired Immune Deficiency Syndrome, helping nurses develop early on strategies for promoting adherence to the use of antiretrovirals.

Importance:
Follow-up after trabeculectomy surgery is important to surgical success, but little is known about the effect of interventions on improving follow-up in low-resource areas.
Objective:
To examine whether text message reminders and free eye medications improve follow-up after trabeculectomy in rural southern China.
Design, Setting, and Participants:
This randomized clinical trial studied 222 consecutive patients undergoing trabeculectomy from October 1, 2014, through November 31, 2015, at 4 rural hospitals in Guangdong and Guangxi Provinces, China. Data from the intention-to-treat population were analyzed.
Interventions:
Patients undergoing trabeculectomy were randomized (1:1) to receive text message reminders 3 days before appointments at 1 and 2 weeks and 1 month after surgery and free topical corticosteroid medication (US$5.30) at each visit or to standard follow-up without reminders or free medication.
Main Outcomes and Measure:
Follow-up at 1 month postoperatively.
Results:
Among 222 eligible patients, 13 (5.9%) refused and 209 (94.1%) were enrolled, with 106 (50.7%) randomized to the intervention group (mean [SD] age, 64.4 [12.7] years; 56 women [52.8%]) and 103 (49.3%) to the control group (mean [SD] age, 63.0 [12.7] years; 53 women [51.5%]). A total of 6 patients (2.9%) were unavailable for follow-up. Attendance at 1 month for the intervention group (59 of 102 [57.8%]) was significantly higher than for the control group (34 of 101 [33.7%]) (unadjusted relative risk [RR], 1.72; 95% CI, 1.13-2.63; P = .01). Factors associated with 1-month attendance in multiple regression models included intervention group membership (RR, 1.65; 95% CI, 1.08-2.53; P = .02) and being told to return for suture removal (RR, 1.80; 95% CI, 1.06-3.06; P = .03). One-month attendance among controls not told about suture removal was 3 of 31 (9.7%), whereas it was 44 of 68 (64.7%) among the intervention group with suture removal (unadjusted RR, 6.69; 95% CI, 2.08-21.6; P = .001).
Conclusions and Relevance:
In this setting, low-cost interventions may significantly improve postoperative follow-up after glaucoma surgery, a potential opportunity for interventions known to improve surgical success.

O medo e a ansiedade dentária são importantes fatores condicionantes do tratamento dentário. Ao interferirem na condição psicológica do paciente, condicionam o seu comportamento na consulta e a atitude que apresentam em relação aos cuidados de saúde oral. Os pacientes ansiosos, medrosos ou fóbicos adiam a consulta de medicina dentária, evitam os tratamentos e só recorrem ao médico dentista quando surgem os sintomas dolorosos. Este adiar dos procedimentos dentários resultará num agravamento dos problemas de saúde oral, e em maiores necessidades de tratamento, tratamento esse que será mais intensivo, mais invasivo e potencialmente mais traumático, levando a um reforço do medo e da ansiedade dentária já existente. As crianças, pela menor maturidade psico-emocional têm menor capacidade de lidar com as suas emoções perante diversos acontecimentos, nomeadamente, em contexto médico-dentário. Tornam-se assim mais suscetíveis ao desenvolvimento de medo e ansiedade dentária, e exibindo, com alguma frequência, comportamentos negativos na consulta, que dificultam a adequada prestação de cuidados de saúde oral. Existem ainda outros fatores etiológicos predisponentes e desencadeantes de ansiedade dentária na criança e que condicionam o seu comportamento na consulta (idade, género, faixa etária, número de consultas anteriores, entre outros). Objetivo: Neste trabalho pretendeu-se avaliar os fatores determinantes do comportamento infantil na consulta de medicina dentária da Unidade de Saúde da Ilha Terceira, em crianças com idades entre os 4 e os 16 anos. Métodos: Foi realizado um estudo descritivo observacional transversal onde se pretendeu avaliar a ansiedade dentária da criança antes da consulta dentária através da Facial Image Scale (FIS); avaliar a ansiedade dentária dos acompanhantes através da. Corah Dental Anxiety Scale, Revised (DAS-R). e o comportamento das crianças durante o tratamento dentário usando a Escala de Frankl. O estudo decorreu de 30 de Abril a 8 de Maio na Unidade de Saúde da Ilha Terceira, Região Autónoma dos Açores, tendo sido observadas 53 crianças de idades compreendidas entre os 4 e os 16 anos. Resultados: Numa amostra de 53 crianças, verificou-se que 11,3% das crianças apresentavam ansiedade dentária antes da consulta dentária, que os pais eram mais ansiosos que as crianças, 49,1% apresentavam ansiedade dentária e que a percentagem de crianças com um comportamento negativo durante a consulta médico-dentária foi muito baixa, correspondendo a 1,9%. Verificou-se que a ansiedade dentária parental não interfere com a ansiedade dentária da criança, quando comparadas, ao contrário do que alguns estudos sugerem. Não houve relação entre a ansiedade dentária da criança e o género, idade, número de vezes que veio ao médico dentista ou estatuto social. Conclusão: Neste estudo pôde-se concluir que as crianças que frequentaram a consulta de medicina dentária da Unidade de Saúde da Ilha Terceira entre 30 de Abril a 8 de Maio apresentaram baixa prevalência de ansiedade dentária e elevada prevalência de comportamento positivo na consulta. Já os seus pais ou acompanhantes apresentaram uma prevalência de ansiedade dentária parental elevada. Conhecer os fatores que condicionam o comportamento infantil na consulta dentária como a ansiedade dentária da criança e a ansiedade dentária parental e medi-los antes da consulta poderá ajudar a equipa dentária na abordagem comportamental da criança durante os tratamentos dentários. Envolver a comunidade escolar e a população infantil em ações de promoção da saúde oral, promovendo rastreios dos problemas orais nas escolas, e consultas dentárias de acompanhamento logo desde muito jovens, poderá ter um efeito benéfico na diminuição da ansiedade dentária nas crianças e no desenvolvimento de comportamentos positivos nas consultas.

Purpose The purpose of this paper was to review the effectiveness of telephone interviewing for capturing data and to consider in particular the challenges faced by telephone interviewers when capturing information about market segments. Design/methodology/approach The platform for this methodological critique was a market segment analysis commissioned by Sport Wales which involved a series of 85 telephone interviews completed during 2010. Two focus groups involving the six interviewers involved in the study were convened to reflect on the researchers’ experiences and the implications for business and management research. Findings There are three principal sets of findings. First, although telephone interviewing is generally a cost-effective data collection method, it is important to consider both the actual costs (i.e. time spent planning and conducting interviews) as well as the opportunity costs (i.e. missed appointments, “chasing participants”). Second, researchers need to be sensitised to and sensitive to the demographic characteristics of telephone interviewees (insofar as these are knowable) because responses are influenced by them. Third, the anonymity of telephone interviews may be more conducive for discussing sensitive issues than face-to-face interactions. Originality/value The present study adds to this modest body of literature on the implementation of telephone interviewing as a research technique of business and management. It provides valuable methodological background detail about the intricate, personal experiences of researchers undertaking this method “at a distance” and without visual cues, and makes explicit the challenges of telephone interviewing for the purposes of data capture.

A la hora de buscar responsables de las dificultades del proceso democrático en el país, es recurrente que muchos de los señalamientos se dirijan en contra de las organizaciones partidistas. La desconfianza frente a estas ha sido alimentada no solo por aquellos sectores sociales y políticos que durante tiempo estuvieron marginados del escenario político electoral por la vía del cierre bipartidista, sino que de manera creciente sectores de elite, académicos, creadores de opinión y líderes gubernamentales han aportado críticas y distanciamiento frente a los partidos. Las razones para ello parecen cada vez más obvias. No obstante, los partidos siguen ahí, aunque ahora con un pedazo mucho menor de la torta. Podemos decretar finalmente la muerte del bipartidismo, del bipartidismo que hemos conocido, pero no de los partidos tradicionales. Con cierto pesimismo, se los declara como males necesarios, dejando traslucir el carácter instrumental de la relación que sostienen muchos sectores con las diversas expresiones partidistas. Pero su importancia no se agota en esta instrumentalización. A pesar de los males en que se ven implicados, los partidos con dinámicas, estrategias y vínculos muy diversos siguen jugando un papel importante de articulación política. El esfuerzo que se reclama, y que se intenta en este libro, es el de comprender y tratar de explicar cómo son estas organizaciones, sobre todo cuando ponen en funcionamiento su batería electoral.

El tema social se halla actualmente en el centro del escenario histórico de América Latina. Se suceden desde las más variadas fuentes los llamados de alerta sobre la magnitud y profundidad de los problemas que sacuden a la región en el campo social. La mayor reunión de Presidentes del Continente, la cumbre hemisférica (Santiago de Chile, 1998) consignó en su declaración final, suscrita por todos los mandatarios, que “superar la pobreza continúa siendo el mayor desafío confrontado por nuestro Hemisferio”. Caracterizando algunos de los principales problemas existentes, los Presidentes indicaron “estamos decididos a remover las barreras que deniegan a los pobres el acceso a nutrición adecuada, servicios sociales, un medio ambiente saludable, créditos y títulos legales sobre su propiedad”. El Secretario General de la CEPAL, José A. Ocampo resaltó (1998) sobre la situación que “siguen aumentando los niveles de pobreza absoluta, los niveles de desigualdad no muestran mejoría y sigue aumentando el empleo en el sector informal”. El Presidente del BID, Enrique V. Iglesias ha destacado (1997) que “el proceso de cambio ha dejado sin resolver en la gran mayoría de los países un tema central: la pobreza crítica y la mala distribución del ingreso”. El Banco Mundial ha hecho continuos señalamientos sobre la gravedad del problema: “América Latina es notable como una región en la que la pobreza, particularmente la pobreza absoluta, no registra mejora alguna” (Burki, 1996), e indicó en reciente conferencia internacional sobre la región (Chile, 1999), los riesgos que corría la democracia en tales condiciones.
