OBJETIVO: o objetivo do estudo foi verificar a prevalência do polimorfismo denominado PROGINS no gene do receptor de progesterona entre mulheres com endometriose em seus diferentes estádios. MÉTODOS: estudo caso-controle desenvolvido entre novembro de 2003 e maio de 2004. Foram analisados os genótipos de 104 mulheres, das quais 66 com endometriose comprovada por videolaparoscopia (26 mulheres nos estádios I-II e 40 nos estádios III-IV) e 38 saudáveis. A inserção Alu de 306 pares de base no intron G do gene do receptor de progesterona denominada PROGINS foi detectada por meio de reação em cadeia da polimerase e analisada em gel de agarose 2% corado com brometo de etídio. Para análise estatística foi utilizado o teste ANOVA paramétrico. RESULTADOS: as amostras pertencentes aos grupos endometriose estádios I-II (grupo EndoI) e estádios III-IV (grupo EndoII) tiveram significativo aumento na incidência do alelo polimórfico do receptor de progesterona em relação ao grupo controle: 27% no grupo EndoI, 38% no EndoII e apenas 18% no grupo controle (p < 0,001). A prevalência da inserção, quando comparamos mulheres com endometriose, independente do estádio, com as do grupo controle, foi estatisticamente superior no grupo das doentes (p = 0,0385). CONCLUSÃO: há associação estatisticamente significante entre o polimorfismo PROGINS e a endometriose pélvica.

OBJETIVO: avaliar as características relativas ao atendimento de parturientes bolivianas e seus desfechos em um hospital do município de São Paulo. MÉTODOS: estudo transversal retrospectivo tipo caso-controle, entre dois grupos de gestantes, atendidas no período de 2003 a 2007. O Grupo Estudo incluiu 312 gestantes bolivianas e o Grupo Controle, 314 gestantes brasileiras. Os grupos foram comparados em relação às variáveis demográficas, à ocorrência de intercorrências maternas e às variáveis perinatais. A análise estatística foi feita com o uso do teste do χ2 e, quando necessário, foi aplicada a correção de Yates. RESULTADOS: comparado às parturientes brasileiras, o número de bolivianas que frequentaram o pré-natal foi menor (16,4 versus 5,1%; p < 0,001) e, dentre as que o fizeram, foi maior a porcentagem das que compareceram a menos do que cinco consultas (50 versus 19,3%; p<0,001). Foi observado menor número de mães bolivianas solteiras (12,1 versus 25,4%; p<0,001) e menor número de gestantes nulíparas (34,1 versus 43,6%; p=0,017). A incidência de sífilis congênita foi maior entre as bolivianas (2,9 versus 0,5%; p<0,05), assim como o número de recém-nascidos classificados como grande para a idade gestacional (14,6 versus 5,8%; p<0,001). CONCLUSÕES: o não comparecimento ao pré-natal, ou o número inadequado de consultas, e o número mais elevado de casos de sífilis congênita observado entre as bolivianas mostram a grande vulnerabilidade desse grupo étnico minoritário frente aos agravos da saúde. Consequentemente, faz-se necessário um planejamento estratégico dos setores responsáveis pela coordenação da assistência em nosso município, com o intuito de reduzir essa disparidade, seja por meio de melhorias socioeconômicas ou da implementação da assistência à saúde, particularizada para esse grupo.

OBJETIVOS:Identificar a frequência do polimorfismo no gene IL-10, rs1800896 (-1082 A/G), em mulheres com pré-eclâmpsia (PE) e em mulheres do grupo controle e associar a presença deste polimorfismo com a proteção contra o desenvolvimento da PE.MÉTODOS:Estudo do tipo caso-controle, no qual foram selecionadas 54 mulheres com PE, classificadas de acordo com os critérios da National High Blood Pressure Education Program e 172 mulheres do grupo controle, com pelo menos duas gestações saudáveis. O polimorfismo proposto foi estudado utilizando-se a técnica de reação em cadeia da polimerase em tempo real (qPCR), com sondas de hidrólise. A análise estatística foi realizada utilizando-se o teste de associação do χ2. Odds ratio e seu intervalo de confiança de 95% foram usados para medir a força de associação entre o polimorfismo estudado e o desenvolvimento da PE.RESULTADOS:Foi observado aumento significativo da frequência do genótipo AG entre mulheres do grupo controle (85 versus 15% nas mulheres com PE). O alelo G é significativamente mais frequente entre as mulheres do grupo controle do que nas com PE (Teste χ2; p=0,01). Oodds ratio para as portadoras do alelo G foi de 2, 13, indicando que apresentam menor risco de desenvolver PE do que as não portadoras.CONCLUSÕES:Sugere-se associação entre a presença do alelo G do polimorfismo no geneIL-10, rs1800896 (-1082 A/G), e a proteção contra o desenvolvimento da PE. Mais estudos sobre a contribuição dessas variações e os mecanismos pelos quais afetam o risco de desenvolver PE ainda necessitam de ser realizadas.

OBJETIVO: Avaliar a presença de podocitúria em gestantes hipertensas crônicas no terceiro trimestre da gestação e a associação com doença renal.MÉTODOS: Estudo observacional descritivo em uma amostra de conveniência de 38 gestantes hipertensas crônicas. Os podócitos foram marcados com técnica de imunofluorescência indireta com antipodocina e diamidino-fenilindol (DAPI). A contagem foi feita a partir de 30 campos analisados de forma aleatória, corrigida pela creatinina urinária (podócitos/mg de creatinina). Foram assumidos dois grupos: grupo GN (função glomerular normal), com até 100 podócitos, e grupo GP (provável glomerulopatia), com mais de 100 podócitos. A dosagem de creatinina foi realizada com uso da técnica do picrato alcalino. As variáveis de análise foram o índice de massa corpórea, a idade gestacional na coleta, a pressão arterial sistólica e a pressão arterial diastólica no momento da coleta. Para a análise dos dados, foi utilizado o programa SPSS - versão 16.0. (IBM - USA). Nas análises estatísticas, foi utilizado o teste do χ2, sendo consideradas diferenças significantes valores de p<0,05.RESULTADOS: A contagem de podócitos no grupo GN teve mediana de 20,3 (0,0 a 98,1), e no grupo GP, de 176,9 (109,1 a 490,6). A média do índice de massa corpórea foi 30,2 kg/m2 (DP=5,6), a média da idade gestacional foi de 35,1 semanas (DP=2,5), a mediana da pressão arterial sistólica foi de 130,0 mmHg (100,0-160,0) e a mediana da pressão arterial diastólica de 80,0 mmHg (60,0-110,0). Não houve correlação significativa entre podocitúria e índice de massa corpórea (p=0,305), idade gestacional na coleta (p=0,392), pressão arterial sistólica (p=0,540) e pressão arterial diastólica (p=0,540).CONCLUSÕES: Não foi identificado um padrão de podocitúria compatível com a presença de glomerulopatia ativa, ainda que algumas das gestantes (15,8%) tenham exibido perda podocitária expressiva. Consideramos ser prematuro recomendar para a prática clínica rotineira a incorporação da pesquisa de podocitúria ao longo do pré-natal de gestantes hipertensas crônicas.

PURPOSE: To investigate the frequencies of polymorphic allele and genotypes for the LT-α gene, position +252 (rs909253), in Brazilian women with preeclampsia.METHODS: This is a case-control study, in which 30 women with preeclampsia, classified according to the criteria of the National High Blood Pressure Education Program, and 115 women in the control group, with at least two healthy pregnancies, were selected. Peripheral blood was collected, and DNA was extracted, followed by genotyping, using specific primers and restriction analysis. The genotypes obtained were AA, AG and GG. Statistical analysis was performed using the χ2association test. The Hardy-Weinberg Equilibrium was tested using the Haploview Program.RESULTS: The results showed no association between genotypes and preeclampsia development (χ2=2.0; p=0.4). When the AG and GG genotypes were grouped according to allele G presence or absence (genotype AA), the data showed that the presence of allele G was not significantly different between cases (women with preeclampsia) and controls (χ2=0.0; p=1.0). The LT-α gene polymorphism, position +252 (rs909253), seems not to be an important candidate for the development of preeclampsia. Other inflammatory genes should be researched, and studies involving gene-environment interactions should be performed, in order to reach a better understanding of the etiology of the preeclampsia.

Abstract:Lawsonia intracellularis infection on a horse farm in the Midwest region of Brazil is described. Thirty-nine foals a few days to months old from a herd with 300 horses, experienced diarrhea with variable characteristics and intensities, weight loss, hyperemic mucous membranes and dehydration. In foals 3 to 6 months of age, hypoproteinemia associated with submandibular edema were also common. Intestinal fragments of a 7-month-old foal were sent to an animal disease laboratory for diagnosis. The observed macroscopic lesions were hyperemic serosa, thickening of the intestinal wall with a corrugation, thickening of the mucosa folds and reduction of intestinal lumen. Histological analysis of the small and large intestine revealed enterocyte hyperplasia of the crypts associated with diffuse marked decrease in the number of goblet cells and positive L. intracellularis antigen labeling by immunohistochemistry. Three out of 11 animals of the same property were seropositive for L. intracellularis, demonstrating the circulation of the agent throughout the farm, but none were PCR positive in fecal samples. Based on clinical signs and pathological findings, the diagnosis of equine proliferative enteropathy was confirmed.

Kasvuolosuhteet vaihtelevat tavallisesti hyvinkin paljon peltolohkoittain, ja samalla tavoin myös sadoissa voi olla suurta lohkokohtaista vaihtelua. Satotason lohkokohtainen määrittäminen ei kuitenkaan ole itsestään selvä käytäntö kaikilla tiloilla. Tiedon avulla olisi mahdollista tarkentaa ravinnetaselaskentaa ja säätää lannoitusta vastaamaan paremmin kasvin tarpeita. Myös parhaiten soveltuva viljelytekniikka voi vaihdella lohkolta toiselle. Mittaamalla ja tiedot ylös kirjaamalla voi seurata toimenpiteiden vaikutuksia pidemmällä aikavälillä. Raportissa tarkastellaan sadon määrittämisen keinoja, niiden käytännön edellytyksiä, rajoituksia sekä virhemahdollisuuksia. Pääpaino on tilatason viljelyn satoarvioinnissa, minkä lisäksi raportissa käsitellään myös hanketoiminnassa sovellettavia toimintamalleja. Raportti on toteutettu osana Tehoa maatalouden vesiensuojeluun (TEHO) -hanketta.

Tehoa maatalouden vesiensuojeluun (TEHO) -hankkeen tavoitteena on edistää maatalouden vesiensuojelua. Tähän julkaisuun on koottu tuloksia TEHO-hankkeen aikana tehdyistä kokeiluista ja seurannoista. Julkaisun ensimmäinen osa käsittelee fosforin kerrostumista suorakylvölohkoilla ja suojavyöhykkeillä. Fosforin kerrostuminen maan pintakerroksiin pellon muokkauksen loputtua on ongelmallista varsinkin liukoisen fosforin osalta, koska se on tällöin alttiina huuhtoutumiselle pintavalunnan mukana. Toisessa osassa käsitellään TEHO-hankkeen ja Sokerijuurikkaan tutkimuskeskuksen yhteistyössä toteuttamaa kokeilua, jossa lietelantaa levitettiin sokerijuurikkaalle kasvukaudella. Näin voidaan vähentää maan tiivistymistä ja saada ravinteet paremmin juurikkaiden käyttöön. Kolmannessa osassa kuvataan sian lietelannan ja biokaasulaitoksen maanparannusmädätteen vaikutuksia ohrasatoon eri lannanlevitysmenetelmillä. Neljänteen osaan on kerätty kokemuksia lietelannan separointikokeilusta kahdeksalla TEHO-tilalla. Viides osa käsittelee vermikuliittia (GeoTrap), joka on Turun yliopistossa kehitetty materiaali ammoniumtypen poistoon. Sitä voitaisiin hyödyntää lannan typpivalumien ehkäisyyn ja typpiravinteiden talteenottoon esimerkiksi jaloittelutarhojen valumavesistä. Julkaisu on toteutettu osana Tehoa maatalouden vesiensuojeluun (TEHO) -hanketta.

Leaf CO2 assimilation (A) as a function of photosynthetic photon flux density (Q) or intercellular CO2 concentration (Ci) and chlorophyll fluorescence measurements were carried out on four tropical woody species growing in forest gap and understorey (Bauhinia forficata Link. and Guazuma ulmifolia Lam. as pioneers, and Hymenaea courbaril L. and Esenbeckia leiocarpa Engl. as non-pioneers). Chlorophyll fluorescence indicated similar acclimation capacities of photochemical apparatus to contrasting light environments irrespective to plant species. Maximum CO2 assimilation and quantum yield derived from A/Q curves indicated higher photosynthetic capacity in pioneer than in non-pioneer species in forest gap. However, the differences among species did not show a straightforward relation with their successional status regarding data derived from A/Q curves under understorey conditions. Both successional groups are able to sustain positive carbon balance under contrasting natural light availabilities, modifying photochemical and biochemical photosynthetic traits with similar phenotypic plasticity capacity.

Polymerase chain reaction (PCR) has been widely investigated for the diagnosis of tuberculosis. However, before this technique is applied on clinical samples, it needs to be well standardized. We describe the use of McFarland nephelometer, a very simple approach to determine microorganism concentration in solution, for PCR standardization and DNA quantitation, using Mycobacterium tuberculosis as a model. Tuberculosis is an extremely important disease for the public health system in developing countries and, with the advent of AIDS, it has also become an important public health problem in developed countries. Using Mycobacterium tuberculosis as a research model, we were able to detect 3 M. tuberculosis genomes using the McFarland nephelometer to assess micobacterial concentration. We have shown here that McFarland nephelometer is an easy and reliable procedure to determine PCR sensitivity at lower costs.

The emergence of multidrug-resistant strains of Mycobacterium tuberculosis has increased the need for rapid drug susceptibility tests, which are needed for adequate patient treatment. The objective of the present study was to evaluate the mycobacteria growth indicator tube (MGIT) system to detect multidrug-resistant M. tuberculosis strains. The MGIT system was compared with two standard methods (proportion and resistance ratio methods). One hundred clinical M. tuberculosis isolates [25 susceptible to isoniazid (INH) and rifampicin (RIF), 20 resistant to INH, 30 resistant to INH-RIF, and 25 resistant to INH-RIF and other drugs] obtained in the State of São Paulo were tested for INH and RIF susceptibility. Full agreement among the tests was found for all sensitive and all INH-resistant strains. For RIF-resistant strains results among the tests agreed for 53 (96.4%) of 55 isolates. Results were obtained within 6 days (range, 5 to 8 days), 28 days and 12 days when using MGIT, the proportion method and the resistance ratio methods, respectively. The MGIT system presented an overall agreement of 96% when compared with two standard methods. These data show that the MGIT system is rapid, sensitive and efficient for the early detection of multidrug-resistant M. tuberculosis.

Human herpesvirus 8 (HHV-8) is a newly described herpesvirus that is etiologically associated with all forms of Kaposi's sarcoma (KS). Seroepidemiological studies have shown high prevalence rates of HHV-8 antibodies among men who have sex with men (MSM) and AIDS patients, African children, Brazilian Amerindians, and elderly individuals in certain regions of Europe. The aim of the present study was to determine the prevalence of HHV-8 antibodies in healthy children and young adults from different cities in São Paulo State, and in a population at high risk for HHV-8 infection: HIV-negative MSM, and AIDS patients with and without KS. Antibodies to HHV-8 latency-associated nuclear antigen and lytic-phase antigens were detected by immunofluorescence assays. In 643 healthy children and young adults from the general population attending a vaccination program for yellow fever in ten different cities in São Paulo State, the prevalence of HHV-8 antibodies detected by the presence of latent or lytic antigens ranged from 1.0 to 4.1% in the different age groups (mean = 2.5%). In the MSM group, the prevalence was 31/95 (32.6%). In the group of patients with AIDS, the prevalence was 39.2% (51/130) for non-KS patients and 98.7% (77/78) for AIDS patients with the diagnosis of KS confirmed by histopathological examination. We conclude that HHV-8 has a restricted circulation among healthy children and young adults in the general population of São Paulo State and a high prevalence among MSM and AIDS patients.

We studied the effect of oral and portal vein administration of alloantigens on mouse skin allograft survival. Graft receptor BALB/c mice received spleen cells (30, 90, 150 or 375 x 10(6)) from donor C57BL/6 mice intragastrically on three successive days, starting seven days before the skin graft. Allograft survival was significantly increased with the feeding of 150 x 10(6) allogeneic spleen cells by one gavage (median survival of 12 vs 14 days, P <= 0.005) or when 300 x 10(6) cells were given in six gavage (12 vs 14 days, P < 0.04). A similar effect was observed when 150 x 10(6) spleen cells were injected into the portal vein (12 vs 14 days, P <= 0.03). Furthermore, prolonged allograft survival was observed with subcutaneous (12 vs 16 days, P <= 0.002) or systemic (12 vs 15 days, P <= 0.016) application of murine interleukin-4 (IL-4), alone or in combination with spleen cell injection into the portal vein (12 vs 18 days, P <= 0.0018). Taken together, these results showed that tolerance induction with spleen cells expressing fully incompatible antigens by oral administration or intraportal injection partially down-modulates skin allograft rejection. Furthermore, these findings demonstrated for the first time the effect of subcutaneous or systemic IL-4 application on allograft skin survival suggesting its use as a beneficial support therapy in combination with a tolerance induction protocol.

Induced oral tolerance to mucosal-exposed antigens in immunized animals is of particular interest for the development of immunotherapeutic approaches to human allergic diseases. This is a unique feature of mucosal surfaces which represent the main contact interface with the external environment. However, the influence of oral tolerance on specific and natural polyreactive IgA antibodies, the major defense mechanism of the mucosa, is unknown. We have shown that oral administration of an extract of the dust mite Dermatophagoides pteronyssinus (Dp) to primed mice caused down-regulation of IgE responses and an increase in tumor growth factor-ß secretion. In the present study, we observed that primed inbred female A/Sn mice (8 to 10 weeks old) fed by gavage a total weight of 1.0-mg Dp extract on the 6th, 7th and 8th days post-immunization presented normal secretion of IL-4 and IL-10 in gut-associated lymphoid tissue and a decreased production of interferon gamma induced by Dp in the draining lymph nodes (13,340 ± 3,519 vs 29,280 ± 2,971 pg/ml). Mice fed the Dp extract also showed higher levels of serum anti-Dp IgA antibodies and an increase of IgA-secreting cells in mesenteric lymph nodes (N = 10), reflecting an increase in total fecal IgA antibodies (N = 10). The levels of secretory anti-Dp IgA antibodies increased after re-immunization regardless of Dp extract feeding. Oral tolerance did not interfere with serum or secretory IgA antibody reactivity related to self and non-self antigens. These results suggest that induction of oral tolerance to a Dp extract in sensitized mice triggered different regulatory mechanisms which inhibited the IgE response and stimulated systemic and secretory IgA responses, preserving the natural polyreactive IgA antibody production.

Three recombinant antigens of Treponema pallidum Nichols strain were fused with GST, cloned and expressed in Escherichia coli, resulting in high levels of GST-rTp47 and GST-rTp17 expression, and supplementation with arginine tRNA for the AGR codon was needed to obtain GST-rTp15 overexpression. Purified fusion protein yields were 1.9, 1.7 and 5.3 mg/l of cell culture for GST-rTp47, GST-rTp17 and GST-rTp15, respectively. The identities of the antigens obtained were confirmed by automated DNA sequencing using ABI Prism 310 and peptide mapping by Finningan LC/MS. These recombinant antigens were evaluated by immuno-slot blot techniques applied to 137 serum samples from patients with a clinical and laboratory diagnosis of syphilis (61 samples), from healthy blood donors (50 samples), individuals with sexually transmitted disease other than syphilis (3 samples), and from individuals with other spirochetal diseases such as Lyme disease (20 samples) and leptospirosis (3 samples). The assay had sensitivity of 95.1% (95% CI, 86.1 to 98.7%) and a specificity of 94.7% (95% CI, 87.0 to 98.7%); a stronger reactivity was observed with fraction rTp17. The immunoreactivity results showed that fusion recombinant antigens based-immuno-slot blot techniques are suitable for use in diagnostic assays for syphilis.
