OBJETIVO: Avaliar a acurácia e a reprodutibilidade das imagens da ressonância magnética (RM) nas lesões ligamentares traumáticas da coluna vertebral. MÉTODOS: Estudo retrospectivo para avaliação das imagens da RM de 32 pacientes com lesões traumáticas da coluna vertebral submetidos a tratamento cirúrgico. A avaliação das lesões do complexo ligamentar posterior (CLP) por imagens da RM foi realizada independentemente por dois radiologistas. Um grupo de pacientes tinha exames com inclusão de sequência com supressão de gordura sensível ao líquido (STIR ou SPAIR T2), além das imagens de rotina e no outro grupo os exames de RM não incluíam imagens com a supressão da gordura. As avaliações dos exames de imagem foram comparadas com as observações obtidas durante o ato cirúrgico. Foi realizada a analise de reprodutibilidade intra e interobservador entre os dois radiologistas pelo cálculo do coeficiente Kappa. RESULTADOS: Foi observada lesão do CLP em 21/32 pacientes. De forma geral a sensibilidade da RM para lesões do CLP variou entre 88,9% a 100% e a especificidade entre 0 e 50%. A concordância entre a avaliação dos radiologistas foi maior nos exames de ressonância magnética sem a supressão da gordura (Κ=0,6) do que nos exames que incluíram sequências com supressão (Κ=0,34). CONCLUSÃO: A avaliação de lesões dos ligamentos posteriores por meio da RM pelos radiologistas resultou em boa sensibilidade em relação aos achados cirúrgicos, porém com baixa especificidade, reproduzindo resultados da literatura. Na casuística atual a utilização do sinal de edema presente nas sequências com supressão de gordura diminuiu a concordância interobservador.

The HAM/TSP caused by HTLV-1 infection usually affects patients to disabling states, and sometimes can lead them to paraplegia presenting symptoms of depression and anxiety, impacting on quality of life. Objective: The purpose of this study was to evaluate the frequency of depression and anxiety and its impact on quality of life in HTLV-1-infected TSP/HAM patients. Material and Methods: This was a cross-sectional study including 67 asymptomatic (control group) and 63 with TSP/HAM subjects. The instruments used were a demographic questionnaire, scales for anxiety and depression diagnosis (BDI and BAI), questionnaire for the assessment of Quality of Life of the World Health Organization (WHOQOL-Brief) and neurological scale to measure the disability level (Osame’s Disability Status Scale). All patients had HTLV-I diagnosis by serological and molecular approaches, monitored at Instituto de Infectologia Emílio Ribas from May 2008 to July 2009. Data were analyzed statistically by frequencies, the Mann-Whitney test and the Spearman correlation test. Data among groups were analyzed and correlated with functional and severity aspects. Results: The results showed that patients with HAM/TSP compared to asymptomatic carriers had higher rates of depression (p < 0.001) and anxiety (p < 0.001), and impairment on quality of life in the areas of: dissatisfaction with health (p < 0.001), physical (p < 0.001) and the environment (p = 0.003). The main factors that correlated with levels of depression and anxiety and the domains of the WHOQOL-brief were: education, family income and social class. Conclusion: A well conducted evaluation and counseling may help in treatment, for a better quality of life of these patients.

Background Human T-cell lymphotropic virus type 1 (HTLV-1) infection can increase the risk of developing skin disorders. This study evaluated the correlation between HTLV-1 proviral load and CD4+ and CD8+ T cells count among HTLV-1 infected individuals, with or without skin disorders (SD) associated with HTLV-1 infection [SD-HTLV-1: xerosis/ichthyosis, seborrheic dermatitis or infective dermatitis associated to HTLV-1 (IDH)]. Methods A total of 193 HTLV-1-infected subjects underwent an interview, dermatological examination, initial HTLV-1 proviral load assay, CD4+ and CD8+ T cells count, and lymphproliferation assay (LPA). Results A total of 147 patients had an abnormal skin condition; 116 (79%) of them also had SD-HTLV-1 and 21% had other dermatological diagnoses. The most prevalent SD-HTLV-1 was xerosis/acquired ichthyosis (48%), followed by seborrheic dermatitis (28%). Patients with SD-HTLV-1 were older (51 vs. 47 years), had a higher prevalence of myelopathy/tropical spastic paraparesis (HAM/TSP) (75%), and had an increased first HTLV-1 proviral load and basal LPA compared with patients without SD-HTLV-1. When excluding HAM/TSP patients, the first HTLV-1 proviral load of SD-HTLV-1 individuals remains higher than no SD-HTLV-1 patients. Conclusions There was a high prevalence of skin disorders (76%) among HTLV-1-infected individuals, regardless of clinical status, and 60% of these diseases are considered skin disease associated with HTLV-1 infection.

Background: Despite advances in HIV treatment, bacterial pneumonia continues to cause considerable morbidity and mortality in patients with HIV infection. Studies of biomarker associations with bacterial pneumonia risk in treated HIVinfected patients do not currently exist. Methods: We performed a nested, matched, case-control study among participants randomized to continuous combination antiretroviral therapy (cART) in the Strategies for Management of Antiretroviral Therapy trial. Patients who developed bacterial pneumonia (cases) and patients without bacterial pneumonia (controls) were matched 1:1 on clinical center, smoking status, age, and baseline cART use. Baseline levels of Club Cell Secretory Protein 16 (CC16), Surfactant Protein D (SP-D), C-reactive protein (hsCRP), interleukin-6 (IL-6), and d-dimer were compared between cases and controls. Results: Cases (n = 72) and controls (n = 72) were 25.7% female, 51.4% black, 65.3% current smokers, 9.7% diabetic, 36.1% co-infected with Hepatitis B/C, and 75.0% were on cART at baseline. Median (IQR) age was 45 (41, 51) years with CD4+ count of 553 (436, 690) cells/mm3. Baseline CC16 and SP-D were similar between cases and controls, but hsCRP was significantly higher in cases than controls (2.94 mg/mL in cases vs. 1.93 mg/mL in controls; p = 0.02). IL-6 and d-dimer levels were also higher in cases compared to controls, though differences were not statistically significant (p-value 0.06 and 0.10, respectively). Conclusions: In patients with cART-treated HIV infection, higher levels of systemic inflammatory markers were associated with increased bacterial pneumonia risk, while two pulmonary-specific inflammatory biomarkers, CC16 and SP-D, were not associated with bacterial pneumonia risk.

The behavior of composed Web services depends on the results of the invoked services; unexpected behavior of one of the invoked services can threat the correct execution of an entire composition. This paper proposes an event-based approach to black-box testing of Web service compositions based on event sequence graphs, which are extended by facilities to deal not only with service behavior under regular circumstances (i.e., where cooperating services are working as expected) but also with their behavior in undesirable situations (i.e., where cooperating services are not working as expected). Furthermore, the approach can be used independently of artifacts (e.g., Business Process Execution Language) or type of composition (orchestration/choreography). A large case study, based on a commercial Web application, demonstrates the feasibility of the approach and analyzes its characteristics. Test generation and execution are supported by dedicated tools. Especially, the use of an enterprise service bus for test execution is noteworthy and differs from other approaches. The results of the case study encourage to suggest that the new approach has the power to detect faults systematically, performing properly even with complex and large compositions. En las sociedades actuales, marcadas por la movilidad, la migración, la desigualdad y una compleja red de relaciones sociales, el discurso cum- ple una función esencial como pieza fundamental en la constitución de prácticas sociales y en nuestra interacción diaria, en la que, entre mu- chas otras cosas, expresamos nuestra identidad o rechazo social. Justamente lo que me interesa en esta contribución es estudiar de qué manera las diferentes prácticas discursivas contribuyen a construir, reanalizar o subvertir estereotipos sociales y con qué funciones. Me val- dré para ello del estereotipo del naco y su representación en la sociedad mexicana actual. La figura del naco se presta perfectamente para estu- diar la importancia del estereotipo en la sociedad porque es controver- sial y ambivalente, pues suscita tanto rechazo como identificación. Es objeto de parodia y de risa, actuando a su vez como símbolo del genui- nísimo proletariado a la mexicana.
