In the city of Sao Paulo, where about 11 million people live, landslides and flooding occur frequently, especially during the summer. These landslides cause the destruction of houses and urban equipment, economic damage, and the loss of lives. The number of areas threatened by landslides has been increasing each year. The objective of this article is to analyze the probability of risk and susceptibility to shallow landslides in the Limoeiro River basin, which is located at the head of the Aricanduva River basin, one of the main hydrographic basins in the city of Sao Paulo. To map areas of risk, we created a cadastral survey form to evaluate landslide risk in the field. Risk was categorized into four levels based on natural and anthropogenic factors: R1 (low risk), R2 (average risk), R3 (high risk), and R4 (very high risk). To analyze susceptibility to shallow landslides, we used the SHALSTAB (Shallow Landsliding Stability) mathematical model and calculated the Distribution Frequency (DF) of the susceptibility classes for the entire basin. Finally, we performed a joint analysis of the average Risk Concentration (RC) and Risk Potential (RP). We mapped 14 risk sectors containing approximately 685 at-risk homes, more than half of which presented a high (R3) or very high (R4) probability of risk to the population. In the susceptibility map, 41% of the area was classified as stable and 20% as unconditionally unstable. Although the latter category accounted a smaller proportion of the total area, it contained a concentration (RC) of 41% of the mapped risk areas with a risk potential (RP) of 12%. We found that the locations of areas predicted to be unstable by the model coincided with the risk areas mapped in the field. This combination of methods can be applied to evaluate the risk of shallow landslides in densely populated areas and can assist public managers in defining areas that are unstable and inappropriate for occupation. The aim of this study was to validate the use of fingernail fluoride concentrations at ages 2-7 years as predictors of the risk for developing dental fluorosis in the permanent dentition. Fifty-six children of both genders (10-15 years of age) had their incisors and premolars examined for dental fluorosis using the Thylstrup-Fejerskov index. Fingernail fluoride concentrations were obtained from previous studies when children were 2-7 years of age. Data were analyzed by unpaired t test, ANOVA, and Fisher's exact test when the fingernail fluoride concentrations were dichotomized (<= 2 or > 2 mu g/g). Children with dental fluorosis had significantly higher fingernail fluoride concentrations than those without the condition, and the concentrations tended to increase with the severity of fluorosis (r(2) = 0.47, p < 0.0001). Using a fingernail fluoride concentration of 2 mu g/g at ages 2-7 years as a threshold, this biomarker had high sensitivity (0.84) and moderate specificity (0.53) as a predictor for dental fluorosis. The high positive predictive value indicates that fingernail fluoride concentrations should be useful in public health research, since it has the potential to identify around 80% of children at risk of developing dental fluorosis. Objective: To evaluate the degree of perception of laypersons, dental professionals, and dental students regarding dental esthetics in cases with mandibular central incisor extraction. Materials and Methods: Using a smile photograph of a person with normal occlusion and all teeth, modifications were made to reflect the extraction of a mandibular incisor of various compositions and sizes. For this purpose a program specifically for image manipulation (Adobe Photoshop CS3, Adobe Systems Inc) was used. After manipulation the images were printed on photographic paper, attached to a questionnaire and distributed to laypersons, dental professionals, and dental students (n = 90) to evaluate the degree of perception and esthetic using a scale of attractiveness, where 0 = hardly attractive, 5 = attractive, and 10 = very attractive. The differences between examiners were checked by the Mann-Whitney test. All the statistics were performed with a confidence level of 95%. Results: The results demonstrated the skill of the dental professionals and dental students in perceiving the difference between cases of normal occlusion and cases where an incisor was lacking (P < .05). The photograph in which the lateral incisors were shown to be larger than the central incisor was the one that obtained the highest value among the cases of extraction in all groups of evaluators. Conclusions: It can be concluded that dental professionals and dental students are more skillful at identifying deviation from normality. In addition, central incisor extraction should always be discarded when there are other treatment options available. (Angle Orthod. 2012;82:732-738.)

The availability and uptake of Cd by lettuce (Lactuca sativa L.) in two common tropical soils (before and after liming) were studied in order to derive human health-based risk soil concentration. Cadmium concentrations ranging from 1 to 12 mg kg(-1) were added to samples from a clayey Oxisol and a sandy-loam Ultisol under glasshouse conditions. After incubation, a soil sample was taken from each pot, the concentration of Cd in the soil was determined, lettuce was grown during 36 d, and the edible parts were harvested and analyzed for Cd. A positive linear correlation was observed between total soil Cd and the Cd concentration in lettuce. The amount of Cd absorbed by lettuce grown in the Ultisol was about twice the amount absorbed in the Oxisol. Liming increased the soil pH and slightly reduced Cd availability and uptake. CaCl2 extraction was better than DTPA to reflect differences in binding strength of Cd between limed and unlimed soils. Risk Cd concentrations in the Ultisol were lower than in the Oxisol, reflecting the greater degree of uptake from the Ultisol. The derived risk Cd values were dependent on soil type and the exposure scenario.

Objective: To assess the risk factors for delayed diagnosis of uterine cervical lesions. Materials and Methods: This is a case-control study that recruited 178 women at 2 Brazilian hospitals. The cases (n = 74) were composed of women with a late diagnosis of a lesion in the uterine cervix (invasive carcinoma in any stage). The controls (n = 104) were composed of women with cervical lesions diagnosed early on (low-or high-grade intraepithelial lesions). The analysis was performed by means of logistic regression model using a hierarchical model. The socioeconomic and demographic variables were included at level I (distal). Level II (intermediate) included the personal and family antecedents and knowledge about the Papanicolaou test and human papillomavirus. Level III (proximal) encompassed the variables relating to individuals' care for their own health, gynecologic symptoms, and variables relating to access to the health care system. Results: The risk factors for late diagnosis of uterine cervical lesions were age older than 40 years (odds ratio [OR] = 10.4; 95% confidence interval [CI], 2.3-48.4), not knowing the difference between the Papanicolaou test and gynecological pelvic examinations (OR, = 2.5; 95% CI, 1.3-4.9), not thinking that the Papanicolaou test was important (odds ratio [OR], 4.2; 95% CI, 1.3-13.4), and abnormal vaginal bleeding (OR, 15.0; 95% CI, 6.5-35.0). Previous treatment for sexually transmissible disease was a protective factor (OR, 0.3; 95% CI, 0.1-0.8) for delayed diagnosis. Conclusions: Deficiencies in cervical cancer prevention programs in developing countries are not simply a matter of better provision and coverage of Papanicolaou tests. The misconception about the Papanicolaou test is a serious educational problem, as demonstrated by the present study.

Background: Angola is one of the African countries with the highest morbidity and mortality rates and a devastating lack of human resources for health, including nursing. The World Health Organization stimulates and takes technical cooperation initiatives for human resource education and training in health and education, with a view to the development of countries in the region. The aim in this study was to identify how nurses affiliated with nursing education institutions perceive the challenges nursing education is facing in Angola. Methods: After consulting the National Directory of Human Resources in Angola, the nurse leaders affiliated with professional nursing education institutions in Angola were invited to participate in the study by email. Data were collected in February 2009 through the focus group technique. The group of participants was focused on the central question: what are the challenges faced for nursing education in your country? To register and understand the information, besides the use of a recorder, the reporters elaborated an interpretative report. Data were coded using content analysis. Results: Fourteen nurses participated in the meeting, most of whom were affiliated with technical nursing education institutions. It was verified that the nurse leaders at technical and higher nursing education institutions in Angola face many challenges, mainly related to the lack of infrastructure, absence of trained human resources, bureaucratic problems to regularize the schools and lack of material resources. On the opposite, the solutions they present are predominantly centered on the valuation of nursing professionals, which implies cultural and attitude changes. Conclusions: Public health education policies need to be established in Angola, including action guidelines that permit effective nursing activities. Professional education institutions need further regularizations and nurses need to be acknowledged as key elements for the qualitative enhancement of health services in the country.

CONTEXT: Esophageal dysphagia is the sensation that the ingested material has a slow transit or blockage in its normal passage to the stomach. It is not always associated with motility or transit alterations. OBJECTIVES: To evaluate in normal volunteers the possibility of perception of bolus transit through the esophagus after swallows of liquid and solid boluses, the differences in esophageal contraction and transit with these boluses, and the association of transit perception with alteration of esophageal contraction and/or transit. METHODS: The investigation included 11 asymptomatic volunteers, 4 men and 7 women aged 19-58 years. The subjects were evaluated in the sitting position. They performed swallows of the same volume of liquid (isotonic drink) and solid (macaroni) boluses in a random order and in duplicate. After each swallow they were asked about the sensation of bolus passage through the esophagus. Contractions and transit were evaluated simultaneously by solid state manometry and impedance. RESULTS: Perception of bolus transit occurred only with the solid bolus. The amplitude and area under the curve of contractions were higher with swallows of the solid bolus than with swallows of the liquid bolus. The difference was more evident in swallows with no perception of transit (n = 12) than in swallows with perception (n = 10). The total bolus transit time was longer for the solid bolus than for the liquid bolus only with swallows followed by no perception of transit. CONCLUSION: The results suggest that the perception of esophageal transit may be the consequence of inadequate adaptation of esophageal transit and contraction to the characteristics of the swallowed bolus.

OBJECTIVE: To evaluate the perception of social support and the relationship of sociodemographic, clinical and metabolic control variables in individuals with diabetes mellitus and foot ulcers in an outpatient unit. METHODS: A quantitative cross-sectional approach was carried out using a social support network inventory. RESULTS: Participants had a high perception of social support; family and health professionals were identified as the main support sources. Fasting plasma glucose values were directly related with social support. CONCLUSION: Family members were identified as the main support source, which emphasizes their importance in the health care process.

CONTEXT AND OBJECTIVE: Children and adolescents who live in situations of social vulnerability present a series of health problems. Nonetheless, affirmations that sensory and cognitive abnormalities are present are a matter of controversy. The aim of this study was to investigate aspects to auditory processing, through applying the brainstem auditory evoked potential (BAEP) and behavioral auditory processing tests to children living on the streets, and comparison with a control group. DESIGN AND SETTING: Cross-sectional study in the Laboratory of Auditory Processing, School of Medicine, Universidade de São Paulo. METHODS: The auditory processing tests were applied to a group of 27 individuals, subdivided into 11 children (7 to 10 years old) and 16 adolescents (11 to 16 years old), of both sexes, in situations of social vulnerability, compared with an age-matched control group of 10 children and 11 adolescents without complaints. The BAEP test was also applied to investigate the integrity of the auditory pathway. RESULTS: For both children and adolescents, there were significant differences between the study and control groups in most of the tests applied, with significantly worse performance in the study group, except in the pediatric speech intelligibility test. Only one child had an abnormal result in the BAEP test. CONCLUSIONS: The results showed that the study group (children and adolescents) presented poor performance in the behavioral auditory processing tests, despite their unaltered auditory brainstem pathways, as shown by their normal results in the BAEP test.

[EN]This study analysed the differences in self-perception, goal orientation and participation in physical activity (PA) in girls (N= 244) attending military schools. Girls had moderately higher levels of perceived competence, and there were no significant differences between age-groups. The figure for task-orientation was higher than ego-orientation. Girls expressed a positive attitude toward school and PE. Most girls did not practiced PA outside school, but 63,9% were involved in school sports. It seems that the military educational institutions are being successful in helping students to adopt physically active lifestyles. The development of perception of competence, task-orientation, and favourable attitudes seem to be important factors to enhance the levels of PA among students.

La distorsione della percezione della distanza tra due stimoli puntuali applicati sulla superfice della pelle di diverse regioni corporee è conosciuta come Illusione di Weber. Questa illusione è stata osservata, e verificata, in molti esperimenti in cui ai soggetti era chiesto di giudicare la distanza tra due stimoli applicati sulla superficie della pelle di differenti parti corporee. Da tali esperimenti si è dedotto che una stessa distanza tra gli stimoli è giudicata differentemente per diverse regioni corporee. Il concetto secondo cui la distanza sulla pelle è spesso percepita in maniera alterata è ampiamente condiviso, ma i meccanismi neurali che manovrano questa illusione sono, allo stesso tempo, ancora ampiamente sconosciuti. In particolare, non è ancora chiaro come sia interpretata la distanza tra due stimoli puntuali simultanei, e quali aree celebrali siano coinvolte in questa elaborazione. L’illusione di Weber può essere spiegata, in parte, considerando la differenza in termini di densità meccano-recettoriale delle differenti regioni corporee, e l’immagine distorta del nostro corpo che risiede nella Corteccia Primaria Somato-Sensoriale (homunculus). Tuttavia, questi meccanismi sembrano non sufficienti a spiegare il fenomeno osservato: infatti, secondo i risultati derivanti da 100 anni di sperimentazioni, le distorsioni effettive nel giudizio delle distanze sono molto più piccole rispetto alle distorsioni che la Corteccia Primaria suggerisce. In altre parole, l’illusione osservata negli esperimenti tattili è molto più piccola rispetto all’effetto prodotto dalla differente densità recettoriale che affligge le diverse parti del corpo, o dall’estensione corticale. Ciò, ha portato a ipotizzare che la percezione della distanza tattile richieda la presenza di un’ulteriore area celebrale, e di ulteriori meccanismi che operino allo scopo di ridimensionare – almeno parzialmente – le informazioni derivanti dalla corteccia primaria, in modo da mantenere una certa costanza nella percezione della distanza tattile lungo la superfice corporea. E’ stata così proposta la presenza di una sorta di “processo di ridimensionamento”, chiamato “Rescaling Process” che opera per ridurre questa illusione verso una percezione più verosimile. Il verificarsi di questo processo è sostenuto da molti ricercatori in ambito neuro scientifico; in particolare, dal Dr. Matthew Longo, neuro scienziato del Department of Psychological Sciences (Birkbeck University of London), le cui ricerche sulla percezione della distanza tattile e sulla rappresentazione corporea sembrano confermare questa ipotesi. Tuttavia, i meccanismi neurali, e i circuiti che stanno alla base di questo potenziale “Rescaling Process” sono ancora ampiamente sconosciuti. Lo scopo di questa tesi è stato quello di chiarire la possibile organizzazione della rete, e i meccanismi neurali che scatenano l’illusione di Weber e il “Rescaling Process”, usando un modello di rete neurale. La maggior parte del lavoro è stata svolta nel Dipartimento di Scienze Psicologiche della Birkbeck University of London, sotto la supervisione del Dott. M. Longo, il quale ha contribuito principalmente all’interpretazione dei risultati del modello, dando suggerimenti sull’elaborazione dei risultati in modo da ottenere un’informazione più chiara; inoltre egli ha fornito utili direttive per la validazione dei risultati durante l’implementazione di test statistici. Per replicare l’illusione di Weber ed il “Rescaling Proess”, la rete neurale è stata organizzata con due strati principali di neuroni corrispondenti a due differenti aree funzionali corticali: • Primo strato di neuroni (il quale dà il via ad una prima elaborazione degli stimoli esterni): questo strato può essere pensato come parte della Corteccia Primaria Somato-Sensoriale affetta da Magnificazione Corticale (homunculus). • Secondo strato di neuroni (successiva elaborazione delle informazioni provenienti dal primo strato): questo strato può rappresentare un’Area Corticale più elevata coinvolta nell’implementazione del “Rescaling Process”. Le reti neurali sono state costruite includendo connessioni sinaptiche all’interno di ogni strato (Sinapsi Laterali), e connessioni sinaptiche tra i due strati neurali (Sinapsi Feed-Forward), assumendo inoltre che l’attività di ogni neurone dipenda dal suo input attraverso una relazione sigmoidale statica, cosi come da una dinamica del primo ordine. In particolare, usando la struttura appena descritta, sono state implementate due differenti reti neurali, per due differenti regioni corporee (per esempio, Mano e Braccio), caratterizzate da differente risoluzione tattile e differente Magnificazione Corticale, in modo da replicare l’Illusione di Weber ed il “Rescaling Process”. Questi modelli possono aiutare a comprendere il meccanismo dell’illusione di Weber e dare così una possibile spiegazione al “Rescaling Process”. Inoltre, le reti neurali implementate forniscono un valido contributo per la comprensione della strategia adottata dal cervello nell’interpretazione della distanza sulla superficie della pelle. Oltre allo scopo di comprensione, tali modelli potrebbero essere impiegati altresì per formulare predizioni che potranno poi essere verificate in seguito, in vivo, su soggetti reali attraverso esperimenti di percezione tattile. E’ importante sottolineare che i modelli implementati sono da considerarsi prettamente come modelli funzionali e non intendono replicare dettagli fisiologici ed anatomici. I principali risultati ottenuti tramite questi modelli sono la riproduzione del fenomeno della “Weber’s Illusion” per due differenti regioni corporee, Mano e Braccio, come riportato nei tanti articoli riguardanti le illusioni tattili (per esempio “The perception of distance and location for dual tactile pressures” di Barry G. Green). L’illusione di Weber è stata registrata attraverso l’output delle reti neurali, e poi rappresentata graficamente, cercando di spiegare le ragioni di tali risultati.

L’interazione che abbiamo con l’ambiente che ci circonda dipende sia da diverse tipologie di stimoli esterni che percepiamo (tattili, visivi, acustici, ecc.) sia dalla loro elaborazione per opera del nostro sistema nervoso. A volte però, l’integrazione e l’elaborazione di tali input possono causare effetti d’illusione. Ciò si presenta, ad esempio, nella percezione tattile. Infatti, la percezione di distanze tattili varia al variare della regione corporea considerata. Il concetto che distanze sulla cute siano frequentemente erroneamente percepite, è stato scoperto circa un secolo fa da Weber. In particolare, una determinata distanza fisica, è percepita maggiore su parti del corpo che presentano una più alta densità di meccanocettori rispetto a distanze applicate su parti del corpo con inferiore densità. Oltre a questa illusione, un importante fenomeno osservato in vivo è rappresentato dal fatto che la percezione della distanza tattile dipende dall’orientazione degli stimoli applicati sulla cute. In sostanza, la distanza percepita su una regione cutanea varia al variare dell’orientazione degli stimoli applicati. Recentemente, Longo e Haggard (Longo & Haggard, J.Exp.Psychol. Hum Percept Perform 37: 720-726, 2011), allo scopo di investigare come sia rappresentato il nostro corpo all’interno del nostro cervello, hanno messo a confronto distanze tattili a diverse orientazioni sulla mano deducendo che la distanza fra due stimoli puntuali è percepita maggiore se applicata trasversalmente sulla mano anziché longitudinalmente. Tale illusione è nota con il nome di Illusione Tattile Orientazione-Dipendente e diversi risultati riportati in letteratura dimostrano che tale illusione dipende dalla distanza che intercorre fra i due stimoli puntuali sulla cute. Infatti, Green riporta in un suo articolo (Green, Percpept Pshycophys 31, 315-323, 1982) il fatto che maggiore sia la distanza applicata e maggiore risulterà l’effetto illusivo che si presenta. L’illusione di Weber e l’illusione tattile orientazione-dipendente sono spiegate in letteratura considerando differenze riguardanti la densità di recettori, gli effetti di magnificazione corticale a livello della corteccia primaria somatosensoriale (regioni della corteccia somatosensoriale, di dimensioni differenti, sono adibite a diverse regioni corporee) e differenze nella dimensione e forma dei campi recettivi. Tuttavia tali effetti di illusione risultano molto meno rilevanti rispetto a quelli che ci si aspetta semplicemente considerando i meccanismi fisiologici, elencati in precedenza, che li causano. Ciò suggerisce che l’informazione tattile elaborata a livello della corteccia primaria somatosensoriale, riceva successivi step di elaborazione in aree corticali di più alto livello. Esse agiscono allo scopo di ridurre il divario fra distanza percepita trasversalmente e distanza percepita longitudinalmente, rendendole più simili tra loro. Tale processo assume il nome di “Rescaling Process”. I meccanismi neurali che operano nel cervello allo scopo di garantire Rescaling Process restano ancora largamente sconosciuti. Perciò, lo scopo del mio progetto di tesi è stato quello di realizzare un modello di rete neurale che simulasse gli aspetti riguardanti la percezione tattile, l’illusione orientazione-dipendente e il processo di rescaling avanzando possibili ipotesi circa i meccanismi neurali che concorrono alla loro realizzazione. Il modello computazionale si compone di due diversi layers neurali che processano l’informazione tattile. Uno di questi rappresenta un’area corticale di più basso livello (chiamata Area1) nella quale una prima e distorta rappresentazione tattile è realizzata. Per questo, tale layer potrebbe rappresentare un’area della corteccia primaria somatosensoriale, dove la rappresentazione della distanza tattile è significativamente distorta a causa dell’anisotropia dei campi recettivi e della magnificazione corticale. Il secondo layer (chiamato Area2) rappresenta un’area di più alto livello che riceve le informazioni tattili dal primo e ne riduce la loro distorsione mediante Rescaling Process. Questo layer potrebbe rappresentare aree corticali superiori (ad esempio la corteccia parietale o quella temporale) adibite anch’esse alla percezione di distanze tattili ed implicate nel Rescaling Process. Nel modello, i neuroni in Area1 ricevono informazioni dagli stimoli esterni (applicati sulla cute) inviando quindi informazioni ai neuroni in Area2 mediante sinapsi Feed-forward eccitatorie. Di fatto, neuroni appartenenti ad uno stesso layer comunicano fra loro attraverso sinapsi laterali aventi una forma a cappello Messicano. E’ importante affermare che la rete neurale implementata è principalmente un modello concettuale che non si preme di fornire un’accurata riproduzione delle strutture fisiologiche ed anatomiche. Per questo occorre considerare un livello astratto di implementazione senza specificare un’esatta corrispondenza tra layers nel modello e regioni anatomiche presenti nel cervello. Tuttavia, i meccanismi inclusi nel modello sono biologicamente plausibili. Dunque la rete neurale può essere utile per una migliore comprensione dei molteplici meccanismi agenti nel nostro cervello, allo scopo di elaborare diversi input tattili. Infatti, il modello è in grado di riprodurre diversi risultati riportati negli articoli di Green e Longo & Haggard.

Proper hazard identification has become progressively more difficult to achieve, as witnessed by several major accidents that took place in Europe, such as the Ammonium Nitrate explosion at Toulouse (2001) and the vapour cloud explosion at Buncefield (2005), whose accident scenarios were not considered by their site safety case. Furthermore, the rapid renewal in the industrial technology has brought about the need to upgrade hazard identification methodologies. Accident scenarios of emerging technologies, which are not still properly identified, may remain unidentified until they take place for the first time. The consideration of atypical scenarios deviating from normal expectations of unwanted events or worst case reference scenarios is thus extremely challenging. A specific method named Dynamic Procedure for Atypical Scenarios Identification (DyPASI) was developed as a complementary tool to bow-tie identification techniques. The main aim of the methodology is to provide an easier but comprehensive hazard identification of the industrial process analysed, by systematizing information from early signals of risk related to past events, near misses and inherent studies. DyPASI was validated on the two examples of new and emerging technologies: Liquefied Natural Gas regasification and Carbon Capture and Storage. The study broadened the knowledge on the related emerging risks and, at the same time, demonstrated that DyPASI is a valuable tool to obtain a complete and updated overview of potential hazards. Moreover, in order to tackle underlying accident causes of atypical events, three methods for the development of early warning indicators were assessed: the Resilience-based Early Warning Indicator (REWI) method, the Dual Assurance method and the Emerging Risk Key Performance Indicator method. REWI was found to be the most complementary and effective of the three, demonstrating that its synergy with DyPASI would be an adequate strategy to improve hazard identification methodologies towards the capture of atypical accident scenarios.

The present dissertation aims at analyzing the construction of American adolescent culture through teen-targeted television series and the shift in perception that occurs as a consequence of the translation process. In light of the recent changes in television production and consumption modes, largely caused by new technologies, this project explores the evolution of Italian audiences, focusing on fansubbing (freely distributed amateur subtitles made by fans for fan consumption) and social viewing (the re-aggregation of television consumption based on social networks and dedicated platforms, rather than on physical presence). These phenomena are symptoms of a sort of ‘viewership 2.0’ and of a new type of active viewing, which calls for a revision of traditional AVT strategies. Using a framework that combines television studies, new media studies, and fandom studies with an approach to AVT based on Descriptive Translation Studies (Toury 1995), this dissertation analyzes the non-Anglophone audience’s growing need to participation in the global dialogue and appropriation process based on US scheduling and informed by the new paradigm of convergence culture, transmedia storytelling, and affective economics (Jenkins 2006 and 2007), as well as the constraints intrinsic to multimodal translation and the different types of linguistic and cultural adaptation performed through dubbing (which tends to be more domesticating; Venuti 1995) and fansubbing (typically more foreignizing). The study analyzes a selection of episodes from six of the most popular teen television series between 1990 and 2013, which has been divided into three ages based on the different modes of television consumption: top-down, pre-Internet consumption (Beverly Hills, 90210, 1990 – 2000), emergence of audience participation (Buffy the Vampire Slayer, 1997 – 2003; Dawson’s Creek, 1998 – 2003), age of convergence and Viewership 2.0 (Gossip Girl, 2007 – 2012; Glee, 2009 – present; The Big Bang Theory, 2007 - present).

Although it is clear that regional analgesia in association with general anaesthesia substantially reduces postoperative pain, the benefits in terms of overall perioperative outcome are less evident. The aim of this nonsystematic review was to evaluate the effect on middle and long-term postoperative outcomes of adding regional perioperative analgesia to general anaesthesia. This study is based mostly on systematic reviews, large epidemiological studies and large or high-quality randomized controlled trials that were selected and evaluated by the author. The endpoints that are discussed are perioperative morbidity, cancer recurrence, chronic postoperative pain, postoperative rehabilitation and risk of neurologic damage. Epidural analgesia may have a favourable but very small effect on perioperative morbidity. The influence of other regional anaesthetic techniques on perioperative morbidity is unclear. Preliminary data suggest that regional analgesia might reduce the incidence of cancer recurrence. However, adequately powered randomized controlled trials are lacking. The sparse literature available suggests that regional analgesia may prevent the development of chronic postoperative pain. Rehabilitation in the immediate postoperative period is possibly improved, but the advantages in the long term remain unclear. Permanent neurological damage is extremely rare. In conclusion, while the risk of permanent neurologic damage remains extremely low, evidence suggests that regional analgesia may improve relevant outcomes in the long term. The effect size is mostly small or the number-needed-to-treat is high. However, considering the importance of the outcomes of interest, even minor improvement probably has substantial clinical relevance.
