First published April 6, 2011; doi:10.1152/jn.00070.2011.-Hypertension elicited by chronic intermittent hypoxia (CIH) is associated with elevated activity of the thoracic sympathetic nerve (tSN) that exhibits an enhanced respiratory modulation reflecting a strengthened interaction between respiratory and sympathetic networks within the brain stem. Expiration is a passive process except for special metabolic conditions such as hypercapnia, when it becomes active through phasic excitation of abdominal motor nerves (AbN) in late expiration. An increase in CO(2) evokes late-expiratory (late-E) discharges phase-locked to phrenic bursts with the frequency increasing quantally as hypercapnia increases. In rats exposed to CIH, the late-E discharges synchronized in AbN and tSN emerge in normocapnia. To elucidate the possible neural mechanisms underlying these phenomena, we extended our computational model of the brain stem respiratory network by incorporating a population of presympathetic neurons in the rostral ventrolateral medulla that received inputs from the pons, medullary respiratory compartments, and retrotrapezoid nucleus/parafacial respiratory group (RTN/pFRG). Our simulations proposed that CIH conditioning increases the CO(2) sensitivity of RTN/pFRG neurons, causing a reduction in both the CO(2) threshold for emerging the late-E activity in AbN and tSN and the hypocapnic threshold for apnea. Using the in situ rat preparation, we have confirmed that CIH-conditioned rats under normal conditions exhibit synchronized late-E discharges in AbN and tSN similar to those observed in control rats during hypercapnia. Moreover, the hypocapnic threshold for apnea was significantly lowered in CIH-conditioned rats relative to that in control rats. We conclude that CIH may sensitize central chemoreception and that this significantly contributes to the neural impetus for generation of sympathetic activity and hypertension.

The hypothesis that growth hormone (GH) up-regulates the expression of enzymes, matrix proteins, and differentiation markers involved in mineralization of tooth and bone matrices was tested by the treatment of Lewis dwarf rats with GH over 5 days, The molar teeth and associated alveolar bone were processed for immunohistochemical demonstration of bone morphogenetic proteins 2 and 4 (BMP-2 and -4), bone morphogenetic protein type IA receptor (BMPR-IA), bone alkaline phosphatase (ALP), osteocalcin (OC), osteopontin (OPN), bone sialoprotein (BSP), and E11 protein (E11), The cementoblasts, osteoblasts, and periodontal ligament (PDL) cells responded to GH by expressing BMP-2 and -4, BMPR-IA, ALP, OC, and OPN and increasing the numbers of these cells. No changes were found in patterns of expression of the late differentiation markers BSP and E11 in response to GH, Thus, GH evokes expression of bone markers of early differentiation in cementoblasts, PDL cells, and osteoblasts of the periodontium. We propose that the induction of BMP-2 and -4 and their receptor by GH compliments the role of GH-induced insulin-like growth factor 1 (IGF-1) in promoting bone and tooth root formation.

The radiation chemistry of two TFE/PMVE copolymers with TFE mole fractions of 0.66 and 0.81 has been studied under vacuum using Co-60 gamma -radiation over absorbed dose ranges up to 4.2 MGy. The radiolysis temperature was 313 K for both TFE/PMVE copolymers. New structure formation in the copolymers was identified by solid-state F-19 NMR and the G-values for new chain ends of 2.1 and 0.5 and for branching sites of 0.9 and 0.2 have been obtained for the TFE/PMVE with TFE mole fractions of 0.66 and 0.81, respectively. The relative yields of-O-CF3 and -CF2-CF3 chain ends were found to be proportional to the copolymer composition, but the yields of the -CF2-CF3 chain ends and -CF- branch points mere not linearly related ia the composition. rather they wets correlated with the radical yields measured at 77 K. Modulations in the excitability of spinal reflex pathways during passive rhythmic movements of the lower limb have been demonstrated by a number of previous studies [4]. Less emphasis has been placed on the role of supraspinal pathways during passive movement, and on tasks involving the upper limb. In the present study, transcranial magnetic stimulation (TMS) was delivered to subjects while undergoing passive flexion-extension movements of the contralateral wrist. Motor evoked potentials (MEPs) of flexor carpi radialis (FCR) and abductor pollicus brevis (APB) muscles were recorded. Stimuli were delivered in eight phases of the movement cycle during three different frequencies of movement. Evidence of marked modulations in pathway excitability was found in the MEP amplitudes of the FCR muscle, with responses inhibited and facilitated from static values in the extension and flexion phases, respectively. The results indicated that at higher frequencies of movement there was greater modulation in pathway excitability. Paired-pulse TMS (sub-threshold conditioning) at short interstimulus intervals revealed modulations in the extent of inhibition in MEP amplitude at high movement frequencies. In the APE muscle, there was some evidence of phasic modulations of response amplitude, although the effects were less marked than those observed in FCR. It is speculated that these modulatory effects are mediated via Ia afferent pathways and arise as a consequence of the induced forearm muscle shortening and lengthening. Although the level at which this input influences the corticomotoneuronal pathway is difficult to discern, a contribution from cortical regions is suggested. Dysfunction of the articulatory subsystem (i.c.. the lips, tongue, and jaw) has bccn identified as a major contributor to the reduction in speech intelligibility experienced by a high proportion of people with multiple sclerosis (MS). In particular. consonant imprecision has been reported to be the articulatory deficit that contributes most to variations in overall intelligibility of MS speakers. Electropalatography(EPG) IS an instrurncntal technique that visually documents the location and timing of tongue-topalatc contacts during speech. Although such a technique would be valuablc in objectively assessing the articulatory disturbances exhibited by individuals with dysarthria ia motor speech disorder) associated with MS, to-date no such study ha< been reported. The aim of the present study was to use EPG to assess tongue-to-palate contact patterns and articulatory timing in patients with dysarthria associated with MS. A dysarthric participant with a diagnosis of definite MS was fitted with an acrylic EPG palate (Reading EPG.?) and asked to read aloud a list of single syllable words which contained lingual consonants in the word-initial position and in consonant clusters. Each mord was repeated five times. The EPG palate was specifically moulded to tit the participant's hard palate and contained 62 electrodes that detected the tongue contacts. A non-neurologically impaired participant matched for age and sex servcd as a control. The results of the study revealed that the tongue-to-palate contacts produced by the participant with MS varied from those produced by the control in a number of ways in regard to spatial configurations and timing characteristics exhibited. The rcsults arc discussed in relation to the neuropathophysiological effects of MS on speech production. The potcntial use of EPG in programs for treating speech disorders associated with MS will be highlightcd.

Background: Measurement and improvement of quality of care is a priority issue in health care. Patients hospitalized with acute coronary syndromes (ACS) constitute a high-risk population whose care, if shown to be suboptimal on the basis of available research evidence, may benefit from quality improvement interventions. Aim: To evaluate the quality of in-hospital care for patients with ACS, using explicit quality indicators. Methods: Retrospective case note review was undertaken of 397 patients admitted to three teaching hospitals in Brisbane, Queensland, Australia, between 1 October 2000 and 17 April 2001. The main out-come measures were 12 process-of-care quality indicators, calculated as either: (i) the proportion of all patients who received specific interventions or (ii) the proportion of ideal patients who received -specific interventions (i.e. patients with clear indi-cations and lacking contraindications). Results: Quality indicators with values above 80% included: (i) patient selection for thrombolysis (100%) and discharge prescription of beta-blockers (84%), (ii) antiplatelet agents (94%) and (iii) lipid-lowering agents (82%). Indicators with values between 50% and 80% included: (i) timely per-formance of electrocardiogram (ECG) on admission (61%), (ii) early coronary angiography (75%), (iii) measurement of serum lipids (71%) and (iv) discharge prescription of angiotensin-converting-enzyme (ACE) inhibitors (73%). Indicators with values <50% included: (i) timely administration of thrombolysis (35%), (ii) non-invasive risk assessment (23%) and (ii) formal in-hospital and post-hospital cardiac rehabilitation (47% and 7%, respectively). Conclusion: There were delays in performing ECG and administering thrombolysis to patients who presented to emergency departments with ACS. Improvement is warranted in use of non-invasive procedures for identifying high-risk patients who may benefit from coronary revascularization as well as use of serum lipid measurements, ACE inhibitors and cardiac rehabilitation.

The effect of dietary vitamin E on immunoglobulin A (IgA) antibody production, which acts as the first line of defence at the intestinal mucosa, has not been evaluated in chickens. In the present study the impact of the inclusion of supplementary levels of vitamin E to the diet, on total and antigen-specific IgA antibody titres, T-cell subsets and Ia+ cells, was assessed. From hatching, chickens received a maize-based diet which was supplemented with either 25, 250, 2500 or 5000 mg dl-alpha-tocopherol acetate/kg. Primary immunisation with tetanus toxoid (T. toxoid) emulsified in a vegetable oil-in-water adjuvant was administered by the intraperitoneal route at 21 d of age. At 35 d of age all birds received an oral booster vaccination of T. toxoid. Significantly higher total IgA antibody titres were present in the day 42 intestinal scrapings of birds receiving the 5000 mg/kg vitamin E-supplemented diet (VESD) (P=0.05) and a notable increase was observed in birds receiving the 250 mg/kg VESD (P=0.06). Background: Congestive heart failure (CHF) is an increasingly prevalent poor-prognosis condition for which effective interventions are available. It is -therefore important to determine the extent to which patients with CHF receive appropriate care in Australian hospitals and identify ways for improving suboptimal care, if it exists. Aim: To evaluate the quality of in-hospital acute care of patients with CHF using explicit quality indicators based on published guidelines. Methods: A retrospective case note review was -performed, involving 216 patients admitted to three teaching hospitals in Brisbane, Queensland, Australia, between October 2000 and April 2001. Outcome measures were process-of-care quality -indicators calculated as proportions of all, or strongly -eligible (ideal), patients who received -specific interventions. Results: Assessment of underlying causes and acute precipitating factors was undertaken in 86% and 76% of patients, respectively, and objective evaluation of left ventricular function was performed in 62% of patients. Prophylaxis for deep venous thrombosis (DVT) was used in only 29% of ideal patients. Proportions of ideal patients receiving pharmacological treatments at discharge were: (i) angiotensin--converting enzyme inhibitors (ACEi) (82%), (ii) target doses of ACEi (61%), (iii) alternative vasodilators in patients ineligible for ACEi (20%), (iv) beta-blockers (40%) and (v) warfarin (46%). Conclusions: Opportunities exist for improving quality of in-hospital care of patients with CHF, -particularly for optimal prescribing of: (i) DVT prophylaxis, (ii) ACEi, (iii) second-line vasodilators, (iv) beta-blockers and (v) warfarin. More research is needed to identify methods for improving quality of in-hospital care.

??rea de Prote????o Ambiental (APA) ?? uma Unidade de Conserva????o de uso direto, onde a estrat??gia de gerenciamento visa compatibilizar as atividades humanas com a preserva????o da vida silvestre, a prote????o dos recursos naturais, e a estabilidade e a melhoria da qualidade de vida das popula????es envolvidas. A APA Federal Canan??ia/Iguape/Peru??be, na Regi??o do Vale do Ribeira, sudeste do Estado de S??o Paulo possui uma extens??o de 234.000 ha, abrangendo 7 munic??pios e 6 ilhas oce??nicas, com sede administrativa em Iguape. Foi criada com o objetivo de proteger o Complexo Estuarino-Lagunar de Iguape-Canan??ia-Paranagu??, um dos principais ecossistemas do planeta, por tratar-se de viveiro natural de organismos aqu??ticos. Este complexo ?? constitu??do pelo ecossistema Mata Atl??ntica, restingas e mangues, cost??es rochosos e lagunas. Se destaca a Bacia Hidrogr??fica do rio Ribeira de Iguape que nasce no Estado do Paran?? e desagua no Oceano Atl??ntico (Iguape, SP). A ??rea apresenta caracter??sticas de um reduzido desenvolvimento econ??mico regional. Nos munic??pios do litoral, o turismo e a pesca s??o relevantes. Em rela????o ?? pesca, o principal recurso ?? a manjuba (Anchoviella lepidentostole), esp??cie an??droma que migra do oceano para o rio, no ver??o, para a desova. Esta pesca ?? realizada por popula????es ribeirinhas e pescadores artesanais, constituindo-se na atividade econ??mica de maior rentabilidade. Devido ?? sobrepesca e explora????o na ??rea da foz do rio, conjugadas com problemas ambientais, o rendimento do recurso declinou nas d??cadas de 80 e 90, resultando em grandes preju??zos e gerando muitos conflitos sociais. At?? o ano de 1990, a pesca da manjuba, que ocorre em ??poca de Piracema, era autorizada pelo IBAMA durante todo o per??odo de safra (outubro a mar??o) atrav??s de portarias regionais. Foi institu??do o primeiro per??odo de defeso (dezembro-janeiro), para a safra 1990/1991; isto resultou em manifesta????es veementes de desagravo pela comunidade local, pelos industriais da pesca e pelo setor pol??tico. A atitude de rebeldia ?? medida tomada pelo ??rg??o ambiental era uma constante, resultando em confrontos entre os pescadores e os fiscais e um conseq??ente desgaste da institui????o. Al??m disto, a quest??o da pesca predat??ria na foz do rio tinha que ser, tamb??m, levada em considera????o

O Programa de Interc??mbio cria uma oportunidade para maior intera????o do CADE com a sociedade, pela promo????o de oportunidade de experi??ncia pr??tica na ??rea de defesa da concorr??ncia, para jovens em fase de forma????o universit??ria, com o aproveitamento, pelo CADE, da capacidade desses jovens, preparando-os para o trabalho na comunidade e no setor privado. O Programa est?? totalmente desvinculado de uma id??ia de "est??gio". As diversas institui????es de ensino universit??rio e entidades sem fins lucrativos, nacionais e internacionais, manifestam sua inten????o numa coopera????o t??cnica com o CADE, atrav??s de Conv??nios e Protocolos. A partir da assinatura do termo, podem enviar um determinado n??mero de estudantes, inclusive p??s-graduandos, durante seu per??odo de f??rias, com interesse em desenvolver trabalhos nas ??reas da defesa da concorr??ncia. O Programa pretende cumprir um papel educativo ao difundir a cultura da concorr??ncia e ao mesmo tempo ajudar a formar profissionais para atuar na defesa da concorr??ncia. Nesse sentido, tem tido um enorme sucesso, com quase nenhuma propaganda, pois o interesse dos alunos ?? demonstrado perante a institui????o, que procura o CADE para integrar o Programa. Os pedidos t??m chegado de norte a sul do Pa??s e, caso fosse alardeada a exist??ncia desse Programa, o CADE n??o teria condi????es de atender a t??o grande demanda (s??o, em m??dia, 20 estudantes por per??odo), pois ?? um ??rg??o com um quadro muito reduzido de pessoal e h?? a necessidade de um acompanhamento t??cnico junto a esses estudantes, que fazem um trabalho pr??tico de an??lise e aplica????o da legisla????o vigente nas mat??rias em tramita????o no Conselho

A id??ia desse projeto surgiu a partir de uma experi??ncia junto ?? Comiss??o Permanente de Licita????o ??? CPL da ENAP, onde se constatava freq??entemente a baixa qualidade dos produtos adquiridos com pre??os elevados, um processo de compras muito demorado e um reduzido n??mero de fornecedores. A CPL prop??s mudan??as, recriou os processos e redefiniu as responsabilidades entre as unidades gestoras da institui????o. Com cerca de um ano e meio de experi??ncia implementada as mudan??as j?? foram observadas, quais sejam: queda significativa dos pre??os dos produtos e servi??os oferecidos pelo mercado, aumento do n??mero de fornecedores, melhoria na qualidade dos produtos e servi??os adquiridos, transpar??ncia e maior brevidade no processo de compras
