The high penetration of distributed energy resources (DER) in distribution networks and the competitiveenvironment of electricity markets impose the use of new approaches in several domains. The networkcost allocation, traditionally used in transmission networks, should be adapted and used in the distribu-tion networks considering the specifications of the connected resources. The main goal is to develop afairer methodology trying to distribute the distribution network use costs to all players which are usingthe network in each period. In this paper, a model considering different type of costs (fixed, losses, andcongestion costs) is proposed comprising the use of a large set of DER, namely distributed generation(DG), demand response (DR) of direct load control type, energy storage systems (ESS), and electric vehi-cles with capability of discharging energy to the network, which is known as vehicle-to-grid (V2G). Theproposed model includes three distinct phases of operation. The first phase of the model consists in aneconomic dispatch based on an AC optimal power flow (AC-OPF); in the second phase Kirschen’s andBialek’s tracing algorithms are used and compared to evaluate the impact of each resource in the net-work. Finally, the MW-mile method is used in the third phase of the proposed model. A distributionnetwork of 33 buses with large penetration of DER is used to illustrate the application of the proposedmodel.

RESUMO: Objectivo: O exercício tem sido identificado como um factor de promoção da qualidade de vida em mulheres submetidas a cirurgia por cancro de mama. Mas os níveis de actividade física tendem a reduzir após o diagnóstico de cancro de mama e a manterem-se baixos após fim dos tratamentos. O objectivo deste estudo é verificar se um programa de exercício físico supervisionado, associado a estratégias motivacionais em mulheres submetidas a cirurgia por cancro da mama e que mantêm a intervenção usual praticada, é mais efectivo no aumento dos níveis de actividade física, na redução do índice de massa corporal e na melhoria da qualidade de vida, do que o tratamento usualmente praticado. Metodologia: Trata-se de um estudo piloto experimental, aleatorizado e controlado. Cumpriram os critérios de inclusão no estudo 37 utentes submetidas a cirurgia por cancro de mama no Hospital Fernando Fonseca, Amadora, e submetidos à intervenção usual da fisioterapia. Foram aleatoriamente distribuídas pelos grupos experimental (n=19) e de controlo (n=18). O contacto telefónico foi feito por um elemento externo, cego em relação à distribuição dos sujeitos. Alguns elementos desistiram ou não puderam participar no estudo. O número de sujeitos final foi de 11 para o grupo experimental e de 10 para o grupo de controlo, idênticos na média de idades. Todos os sujeitos mantiveram a intervenção usual da fisioterapia. Os sujeitos do grupo experimental foram ainda submetidos ao programa de exercício e promoção da actividade física. Todo o grupo foi avaliado no início do programa, no final do mesmo (12 semanas) e após 6 meses de follow-up, nos seguintes indicadores: nível de actividade física, pelo International Physical Activity Questionnaire (IPAQ), índice de massa corporal e qualidade de vida, pelos questionários: European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) e Supplementary Questionnaire Breast Cancer Module (QLQ-BR23). Resultados: O programa implementado demonstrou-se efectivo no aumento dos níveis de actividade física (p=0,017) e na qualidade de vida associada ao estado funcional (p=0,016) e ao estado de saúde global auto-percepcionado (p=0,030) no final do programa. Foi ainda possível identificar resultados positivos noutras variáveis estudadas, como por exemplo a fadiga auto-reportada, mas que não se demonstraram estatisticamente significativos, facto que pode atribuir-se à reduzida dimensão da amostra. Conclusões: Apesar de algumas limitações, este estudo é de grande a importância para a comunidade científica preocupada com crescente problemática do cancro de mama. Mais uma vez se demonstra que o exercício físico é uma estratégia importante para a melhoria do estado de saúde de pacientes confrontados com doença. Esta comprovação é de grande interesse para os profissionais de saúde e particularmente para os fisioterapeutas dedicados à promoção da saúde nesta população.---------------------ABSTRACT: Purpose: Exercise has been identified as a quality of life promoting factor, amongst women after breast cancer surgery. But the physical activity levels reduce significantly after a diagnosis of breast cancer and remain low after treatment is completed. The aim of this study is to determine whether a supervised group exercise program allied to motivating strategies in women after breast cancer surgery in conjunction with standard treatment, is more effective in improving physical activity levels, reducing body mass index and promoting quality of life, than standard treatment on its own. Methods: This is a pilot randomized controlled trial. Thirty-seven women, submitted to breast cancer surgery in the Hospital Fernando Fonseca, Amadora, and submitted to standard physiotherapy intervention, completed the inclusion criteria. They were randomly allocated into intervention group (n=19) and control group (n=18). The telephone contact was done by an external element, blind to the subjects’ allocation. Some elements declined or could not participate. The final intervention group was n=11 and the control n=10, similar in age. All subjects maintained the standard physiotherapy intervention. The experimental group was submitted to the exercise and health promotion program. The complete group was evaluated in the beginning of the program, at the final (after 12 weeks) and after 6 months follow-up, using as outcomes: physical activity index with the international Physical Activity Questionnaire (IPAQ), the body mass index and quality of life using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and Supplementary Questionnaire Breast Cancer Module (QLQBR23). Results: The studied program demonstrated to be effective in improving the physical activity index (p=0,017) and quality of live related to physical functioning (p=0,016) and to global health status (p=0,030) at the end of the program. It was possible to find positive results in some other outcomes, such as fatigue, although the differences were not statistically significant. We believe that these results can be attributed to the small sample size. Conclusions: Although we can identify some methodological constrains, this is a very important study for scientific community working on the breast cancer subject. Once more, exercise was identified as an important strategy to improve global heath status in breast cancer patients. This represents an important contribution to the health professionals and mostly for physiotherapists working on health promotion subject.

ABSTRACT – Background: Primary Health Care (PHC) is usually the first contact with the health system, and health professionals are key mediators for enabling citizens to take care of their health. In Portugal, great improvements have been achieved in the biometric indicators of maternal and child health during the last decades. Nevertheless, scant attention has been paid to the mental health dimension, in spite of the recognition of its importance, being pregnancy and early childhood crucial opportunities in the lifecycle for mental health promotion, especially in the early years of life, with a strong impact in the health of the child. The impact of early attachment between mother and baby on maternal and child health has long been recognized. This attachment can be influenced by some factors, as the mother’s emotional adjustment. Attention to these factors may facilitate implementation of both positive conditions and preventative measures. Family support during the transition to parenthood has been highlighted as an effective measure and PHC professionals are in a privileged position as information sources as well as mediators. Aims: The project we present describes an action-research process developed together among academic researchers and health professionals to embrace these issues. We intend to enable health professionals to support families in the transition to parenthood thereby promoting children’s mental health. Approach: The project is driven by a participatory approach intended to lead to reorganization of health care during pregnancy and early childhood. Effective change happens when those involved are interested and motivated, what makes their participation so important. Reflection about current practices and needs, and knowledge about evidence-based interventions have been guiding the selection of changes to introduce in clinical practice for family support and development of parenthood skills and self-confidence. Development: We summarize the main steps in development: the initial assessment and the picture taken from the community under study; the decision making process; the training programme of PHC professionals in action; the review of the protocols of maternal consultation, home visits and antenatal education; the implementation planning; the plan for evaluation the effectiveness of the changes introduced in the delivery of maternal and child health care units. The already developed work has shown that motivation, leadership and organizational issues are decisive for process development.-------------------------- RESUMO - Os Cuidados de Saúde Primários são habitualmente o primeiro contacto com o sistema de saúde e os profissionais de saúde são mediadores chave na capacitação dos cidadãos para cuidarem da sua saúde. Em Portugal, nas últimas décadas, têm-se alcançado grandes melhorias nos indicadores biométricos de saúde materno-infantil. Contudo, tem-se dedicado pouca atenção à dimensão de saúde mental, apesar do reconhecimento da sua importância. A gravidez e primeira infância têm sido apontadas como uma oportunidade crucial no ciclo de vida para a promoção da saúde mental. É dado especial enfoque aos primeiros tempos de vida, dado o forte impacto na saúde da criança. O impacte da vinculação precoce entre a mãe e o bebé na saúde da mãe e da criança há muito que é reconhecido. Esta vinculação pode ser influenciada por vários factores, nomeadamente pelo ajustamento emocional da mãe. A focalização nestes aspectos pode facilitar a criação de condições favoráveis e a implementação de medidas preventivas. O suporte familiar durante o período de transição para a parentalidade tem sido enfatizado como uma medida eficaz e os Cuidados de Saúde Primários estão numa posição privilegiada como fontes de informação e como mediadores. O projecto que apresentamos descreve um processo de investigação- acção desenvolvido em parceria entre investigadores académicos e profissionais de saúde para abordar os aspectos referidos. Pretende-se capacitar os profissionais de saúde para apoiarem as famílias na transição para a parentalidade, promovendo assim a saúde mental das crianças. O projecto baseia-se numa abordagem participativa, direccionada para a reorganização dos cuidados durante a gravidez e primeiros tempos de vida. A mudança efectiva acontece quando os envolvidos estão interessados e motivados, o que torna a sua participação tão importante. A reflexão acerca das práticas e necessidades actuais e o conhecimento acerca de intervenções baseadas na evidência têm guiado a selecção das alterações a introduzir na prática clínica, no sentido de promover o suporte familiar e o desenvolvimento de competências parentais e auto-confiança. Neste artigo, apresentamos as etapas principais do desenvolvimento do projecto: avaliação inicial da comunidade em estudo; processo de tomada de decisão; programa de formação dos profissionais dos Cuidados de Saúde Primários; revisão dos protocolos da consulta de saúde materna, visita domiciliária e educação pré-natal; planeamento da implementação; plano de avaliação da efectividade das alterações introduzidas na prestação de cuidados. Distributed real-time systems such as automotive applications are becoming larger and more complex, thus, requiring the use of more powerful hardware and software architectures. Furthermore, those distributed applications commonly have stringent real-time constraints. This implies that such applications would gain in flexibility if they were parallelized and distributed over the system. In this paper, we consider the problem of allocating fixed-priority fork-join Parallel/Distributed real-time tasks onto distributed multi-core nodes connected through a Flexible Time Triggered Switched Ethernet network. We analyze the system requirements and present a set of formulations based on a constraint programming approach. Constraint programming allows us to express the relations between variables in the form of constraints. Our approach is guaranteed to find a feasible solution, if one exists, in contrast to other approaches based on heuristics. Furthermore, approaches based on constraint programming have shown to obtain solutions for these type of formulations in reasonable time.

Cystic echinococcosis (CE) is very common in the southern part of Rio Grande do Sul State where sheep and cattle raising is the most important economical activity. Prevalence in intermediate hosts is well known due to reports in slaughterhouses while human reports are of discussed value. This is due to underreporting of cases and few epidemiological studies make it difficult to assess the situation of echinococcosis in the population. Whereas cattle infection rate is more or less stable around 12% of the slaughter animals, in sheep there has been an increasing number in the last five years. Some efforts have been done to control the zoonosis but no effective results were obtained so far. Probably educational efforts to change human practices (feeding dogs with raw viscera), periodic treatment of dogs with praziquantel and joined actions with slaughterhouses could bring more attention to improve some control measures.

Stroke is one of the most common conditions requiring rehabilitation, and its motor impairments are a major cause of permanent disability. Hemiparesis is observed by 80% of the patients after acute stroke. Neuroimaging studies showed that real and imagined movements have similarities regarding brain activation, supplying evidence that those similarities are based on the same process. Within this context, the combination of mental practice (MP) with physical and occupational therapy appears to be a natural complement based on neurorehabilitation concepts. Our study seeks to investigate if MP for stroke rehabilitation of upper limbs is an effective adjunct therapy. PubMed (Medline), ISI knowledge (Institute for Scientific Information) and SciELO (Scientific Electronic Library) were terminated on 20 February 2015. Data were collected on variables as follows: sample size, type of supervision, configuration of mental practice, setting the physical practice (intensity, number of sets and repetitions, duration of contractions, rest interval between sets, weekly and total duration), measures of sensorimotor deficits used in the main studies and significant results. Random effects models were used that take into account the variance within and between studies. Seven articles were selected. As there was no statistically significant difference between the two groups (MP vs control), showed a - 0.6 (95% CI: -1.27 to 0.04), for upper limb motor restoration after stroke. The present meta-analysis concluded that MP is not effective as adjunct therapeutic strategy for upper limb motor restoration after stroke.

Mental practice is an internal reproduction of a motor act (whose intention is to promote learning and improving motor skills). Some studies have shown that other cognitive strategies also increase the strength and muscular resistance in healthy people by the enhancement of the performance during dynamic tasks. Mental training sessions may be primordial to improving muscle strength in different subjects. The aim of this study was to systematically review and meta-analiyze studies that assessed whether mental practice is effective in improving muscular strength. We conducted an electronic-computed search in Pub-Med/Medline and ISI Web of Knowledge, Scielo and manual searchs, searching papers written in English between 1991 and 2014. There were 44 studies in Pub-Med/Medline, 631 in ISI Web of Knowledge, 11 in Scielo and 3 in manual searchs databases. After exclusion of studies for duplicate, unrelated to the topic by title and summary, different samples and methodologies, a meta-analysis of 4 studies was carried out to identify the dose-response relationship. We did not find evidence that mental practice is effective in increasing strength in healthy individuals. There is no evidence that mental practice alone can be effective to induce strength gains or to optimize the training effects.

The external forces applied in swimming starts have been often studied, but using direct analysis and simple interpretation data processes. This study aimed to develop a tool for vertical and horizontal force assessment based on the swimmers' propulsive and structural forces (passive forces due to dead weight) applied during the block phase. Four methodological pathways were followed: the experimented fall of a rigid body, the swimmers' inertia effect, the development of a mathematical model to describe the outcome of the rigid body fall and its generalization to include the effects of the inertia, and the experimental swimmers' starting protocol analysed with the inclusion of the developed mathematical tool. The first three methodological steps resulted in the description and computation of the passive force components. At the fourth step, six well-trained swimmers performed three 15 m maximal grab start trials and three-dimensional (3D) kinetic data were obtained using a six degrees of freedom force plate. The passive force contribution to the start performance obtained from the model was subtracted from the experimental force due to the swimmers resulting in the swimmers' active forces. As expected, the swimmers' vertical and horizontal active forces accounted for the maximum variability contribution of the experimental forces. It was found that the active force profile for the vertical and horizontal components resembled one another. These findings should be considered in clarifying the active swimmers' force variability and the respective geometrical profile as indicators to redefine steering strategies.

O maior desafio da escola atual é conjugar a escolaridade obrigatória e universal com a ânsia do sucesso educativo. Uma escola justa e democrática não pode limitar-se a selecionar os que têm mais mérito, tem, obrigatoriamente, de se preocupar com aqueles que têm mais dificuldades em ter sucesso e que se vão autoexcluindo. Um dos dispositivos clássicos para potenciar os resultados académicos são os Apoios Educativos, porém no Agrupamento de Escolas X, um Território Educativo de Intervenção Prioritária (TEIP), os resultados obtidos mantêm-se abaixo dos padrões nacionais. É este o ponto de partida do nosso estudo, pois, apesar do investimento considerável no aumento de apoios educativos neste Agrupamento, muitos alunos continuam a registar insucesso educativo.Partindo desta realidade percecionada com a análise documental sobre os Apoios Educativos neste Agrupamento, e após uma ação de acompanhamento da Inspeção Geral da Educação e Ciência, surgiu a necessidade de orientar o nosso olhar para uma análise de todo o processo dos apoios de modo a perceber como se pode otimizar o impacto dos apoios no sucesso dos alunos e, consequentemente, ter o reconhecimento exterior. Tendo por base a análise de conteúdo das entrevistas a docentes de Português e Matemática, envolvidos no processo dos apoios educativos e do focus group realizado com alunos que beneficiaram destes apoios educativos, e suportada por um enquadramento teórico e legal, delineou-se um plano de ação, que visa a melhoria do sucesso educativo de todos os alunos e envolve toda a organização escolar. Perceciona-se a preponderância da coordenação e da monitorização num processo que se pretende efetivo e não de simples receituário, mas acima de tudo, toma-se consciência que para funcionar é necessário que seja devidamente clarificado, sentido e apropriado por todos os atores envolvidos.

A globalização, nas suas diversas dimensões, está a tornar o mundo cada vez mais pequeno, alterando as nossas vidas e transformando a forma como as empresas fazem negócios. Estamos cada vez mais expostos aos traços e práticas das outras culturas à medida que a tecnologia diminui os custos da comunicação global e das viagens. A globalização está a obrigar as empresas a serem mais competitivas ao mesmo tempo que os países reduzem as barreiras ao comércio internacional e ao investimento estrangeiro. A competitividade das empresas obriga-as a serem mais eficientes e eficazes, logo a pensarem os seus processos internos de forma a procurarem a sua otimização e melhorarem a sua produtividade, com a notória consequência na satisfação do cliente. Para a satisfação do cliente é necessário que as operações dentro do sistema de distribuição, nomeadamente as realizadas no armazém, sejam otimizadas. Esta otimização é atingida quando as encomendas dos clientes estiverem completas e prontas a serem despachadas em tempo útil e todos os processos necessários para o efeito tenham ocorrido no menor espaço de tempo possível e com o mínimo de recursos. O planeamento das operações no armazém tem de determinar o tempo e a ordem das operações bem como os recursos necessários de forma a não acontecerem congestionamentos. A Caetano Parts, uma empresa de revenda de peças de substituição automóvel, identificou um congestionamento na localização de peças que chegavam pela primeira vez ao seu armazém, o que suscitou a necessidade do desenvolvimento de uma ferramenta que permitisse proceder à localização destas peças de uma forma mais expedita, permitindo ganhos significativos de tempo na arrumação das peças e consequentemente no tempo gasto na sua procura/separação e posterior envio para os clientes. A ferramenta desenvolvida consiste num programa em Visual Basic for Applications (VBA) cuja conceção, desenvolvimento e descrição das funcionalidades serão abordados ao longo do trabalho. Os testes realizados ao longo do desenvolvimento validaram o correto funcionamento da ferramenta.

In Colombia, pentavalent antimonials and miltefosine are the drugs of choice for the treatment of cutaneous leishmaniasis; however, their toxicity, treatment duration, (treatment adherence problems), cost, and decreased parasite sensitivity make the search for alternative treatments of American cutaneous leishmaniasis necessary. Based on the results found in a controlled, open, randomized, phase III clinical trial, the efficacy and safety of miltefosine was compared to that of thermotherapy for the treatment of cutaneous leishmaniasis in Colombia. Adult patients from the Colombian army participated in the study; they received either 50 mg of miltefosine three times per day for 28 days by the oral route (n = 145) or a thermotherapy (Thermomed®) application of 50 °C for 30 seconds over the lesion and surrounding area (n = 149). Both groups were comparable with respect to their sociodemographic, clinical, and parasitological characteristics. The efficacy of miltefosine by protocol and by intention to treat was 70% (85/122 patients) and 69% (85/145 patients), respectively. The adverse effects were primarily gastrointestinal for miltefosine and pain at the lesion site after treatment for thermotherapy. No statistically significant difference was found in the efficacy analysis (intention to treat and protocol) between the two treatments. ClinicalTrials.gov: NCT00471705.
