The farmer's lung disease : occupational medicine input

Background: Although there has been an abundant literature in recent years about farmer's lung disease, few addressed occupational measures able to maintain the farmer in his work. Nevertheless we know now that most of the farmers can be kept at the workplace by the way of occupational preventive measures. Methods: This matter is discussed from a case report. A farmer affected by the farmer's lung disease was sent to us by his pneumologist, in order to estimate the possibility of maintaining him in his job and to determine relevant changes at his workplace to minimize risk of exposure to dust antigen. This approach required a visit to the workplace by occupational physician and hygienist. Results: The visit of the workplace pointed out different habits and architectural particularities which were potential sources of exposure. The two main proposed measures to reduce the risk, were to wear respiratory masks while working inside the barn, such as preparing hay, feeding the cattle or sweeping the floor, and to build a direct access from the bathroom (shower and toilet) to the outside, allowing to go out of the barn after taking a shower and changing, without risk of being contaminated again. Although upgrading the shower-toilet is not yet completed to date, the already performed modifications led currently to significant clinical improvements, despite the risk of exposure was high since the animals were in the barn for more than two months. Conclusion: The treatment of the farmer's lung disease must be multidisciplinary involving general practitioner, pneumologist, occupational hygienist and occupational physician.

Le scaphandre et le papillon et l'adaptation filmique du "je" littéraire: l'oeil qui écrit

Lorsque le cinéaste Julian Schnabel propose, dix ans après la sortie du livre "Le scaphandre et le papillon" de Jean-Dominique Bauby (1997), une "adaptation" de cette autobiographie, il prend pour sujet sa création même, en plaçant au premier plan les éléments qui, dans le péritexte, thématisaient les conditions de l'écriture. Bauby a en effet écrit cette autobiographie alors qu'il souffrait de locked-in syndrome, étant entièrement paralysé à l'exception de son oeil gauche. C'est avec l'oeil qu'il écrivait, dictant chaque lettre d'un battement de cils tandis qu'une assistante récitait à voix haute un code alphabétique. L'acte d'écriture devenait ainsi une performance audiovisuelle, saisissable cinématographiquement. Or, cet article montre comment Schnabel complexifie ce dispositif en faisant "plonger" le spectateur dans l'univers intérieur de Bauby à l'aide de procédés filmiques (caméra subjective, voix over à la première personne, flashbacks, visions) qui permettent de transposer toute la poésie et la force du texte. Narrant au "je" la vie, la pensée et les sens qui vibrent derrière les battements d'"aile" de cet oeil-papillon, cette autobiographie traduit de fait un univers visionnaire que Bauby lui-même appelle son "cinéma personnel". Schnabel actualise cette métaphore avec son film qui repousse les limites de la focalisation interne et met en abyme les pouvoirs du cinéma.

Potential detrimental effects of a phytoestrogen-rich diet on male fertility in mice.

Soy and soy-based products are widely consumed by infants and adult individuals. There has been speculation that the presence of isoflavone phytoestrogens in soybean cause adverse effects on the development and function of the male reproductive system. The purpose of this study was to examine the influence of dietary soy and phytoestrogens on testicular and reproductive functions. Male mice were fed from conception to adulthood with either a high soy-containing diet or a soy-free diet. Although adult mice fed a soy-rich diet exhibited normal male behaviour and were fertile, we observed a reduced proportion of haploid germ cells in testes correlating with a 25% decrease in epididymal sperm counts and a 21% reduction in litter size. LH and androgens levels were not affected but transcripts coding for androgen-response genes in Sertoli cells and Gapd-s, a germ cell-specific gene involved in sperm glycolysis and mobility were significantly reduced. In addition, we found that dietary soy decreased the size of the seminal vesicle but without affecting its proteolytic activity. Taken together, these studies show that long-term exposure to dietary soy and phytoestrogens may affect male reproductive function resulting in a small decrease in sperm count and fertility.

The nine-year sustained cost-containment impact of swiss pilot physicians-pharmacists quality circles.

BACKGROUND: Six pioneer physicians-pharmacists quality circles (PPQCs) located in the Swiss canton of Fribourg (administratively corresponding to a state in the US) were under the responsibility of 6 trained community pharmacists moderating the prescribing process of 24 general practitioners (GPs). PPQCs are based on a multifaceted collaborative process mediated by community pharmacists for improving compliance with clinical guidelines within GPs' prescribing practices. OBJECTIVE: To assess, over a 9-year period (1999-2007), the cost-containment impact of the PPQCs. METHODS: The key elements of PPQCs are a structured continuous quality improvement and education process; local networking; feedback of comparative and detailed data regarding costs, drug choice, and frequency of prescribed drugs; and structured independent literature review for interdisciplinary continuing education. The data are issued from the community pharmacy invoices to the health insurance companies. The study analyzed the cost-containment impact of the PPQCs in comparison with GPs working in similar conditions of care without particular collaboration with pharmacists, the percentage of generic prescriptions for specific cardiovascular drug classes, and the percentage of drug costs or units prescribed for specific cardiovascular drugs. RESULTS: For the 9-year period, there was a 42% decrease in the drug costs in the PPQC group as compared to the control group, representing a $225,000 (USD) savings per GP only in 2007. These results are explained by better compliance with clinical and pharmacovigilance guidelines, larger distribution of generic drugs, a more balanced attitude toward marketing strategies, and interdisciplinary continuing education on the rational use of drugs. CONCLUSIONS: The PPQC work process has yielded sustainable results, such as significant cost savings, higher penetration of generics and reflection on patient safety, and the place of "new" drugs in therapy. The PPQCs may also constitute a solid basis for implementing more comprehensive collaborative programs, such as medication reviews, adherence-enhancing interventions, or disease management approaches.

Dépister ou diagnostiquer les risques psychosociaux : quels outils?

Le recours aux échelles de mesure ou à des questionnaires est fréquent et souvent perçu comme la panacée pour évaluer des risques professionnels qui touchent à la sphère psychosociale. Néanmoins, ces outils doivent être réservés à des étapes très spécifiques d'une démarche de prévention des risques psychosociaux et leur choix parmi les échelles disponibles doit reposer sur des critères précis. Avant d'engager une démarche de prévention il sera possible d'utiliser certaines données existantes dans l'entreprise à l'occasion de l'étape prédiagnostique. L'analyse de ces données permettra de décider d'engager ou non une démarche de prévention qui nécessitera en premier lieu un diagnostic approfondi. Celui-ci devra évaluer d'une part les contraintes vécues au travail, et, d'autre part, l'état de santé des salariés. Le diagnostic des contraintes perçues peut s'appuyer sur des entretiens ou sur des échelles existantes telles que celles de Karasek et de Siegrist. Pour évaluer le niveau de stress chronique, on dispose également de questionnaires validés qui cotent les symptômes physiologiques et/ou psychologiques et, si la situation est déjà détériorée, on peut explorer les atteintes à la santé à travers des échelles de santé mentale.
