Introduction La stratégie de reperfusion coronarienne par voie percutanée (PCI: percutaneous coronary intervention) est considérée comme étant la méthode de choix dans la prise en charge urgente des STEMI(H). Actuellement, les deux accès artériels principaux pour les PCI sont l'artère fémorale et l'artère radiale. La voie radiale est préconisée en première intention par les guidelines actuelles car elle serait associée à moins de complications hémorragiques. Objectif L'objectif de cette étude était de comparer la voie d'abord radiale à la voie fémorale, chez les patients admis pour un STEMI, en analysant le succès de la procédure de revascularisation, l'évolution clinique et les complications. Méthode Il s'agit d'une étude observationnelle, comprenant 268 patients admis au CHUV entre le 1er janvier et le 31 décembre 2013, avec le diagnostic de STEMI. Le choix de la voie d'accès artériel (fémorale ou radiale) était laissé au cardiologue interventionnel, sans randomisation. Les patients ont été séparés en 2 groupes, selon la voie d'abord vasculaire choisie au début de la procédure de revascularisation (intention to treat). Les endpoints primaires étaient les saignements majeurs (≥ 3 selon BARC)(A), et le door to balloon time(B). Les endpoints secondaires étaient les MACE(C), les saignements mineurs(A), le taux succès des procédures(D), le temps de fluoroscopie, la quantité de produit de contraste, et le taux de crossover(E) Résultats 268 patients en STEMI ont été inclus dans cette étude, pour un geste de revascularisation en urgence. La moyenne d'âge était de 64.3 ans, avec 73.1% d'hommes. 3 cas de saignements majeurs(A) ont lieu avec la voie radiale (3.4%), et 10 avec la voie fémorale (5.6%), p=0.44. Le door to balloon time ne diffère pas de manière significative selon la voie d'accès employée : 42 min (34-57) pour le groupe radial, et 48 min (31-61) pour le groupe fémoral, p=0.09. Les taux de MACE étaient de 8.0% avec la voie radiale, et de 6.7% avec la voie fémorale, p=0.7. Le taux de crossover était de 4.5% avec la voie radiale, et 0.6% avec la voie fémorale, p=0.02. Le temps de fluoroscopie était de 7min 28sec (5min 9 sec - 12min 25) pour la voie fémorale, contre 12min 22sec (9min 30 sec - 16min 19sec) pour la voie radiale, p < 0.05. La quantité de produit de contraste nécessaire était de 120 ml (100-160) pour le groupe fémoral, et de 170 ml (140-210) pour le groupe radial, p < 0.05. Le taux de succès était comparable entres les 2 groupes : 97.7% pour le groupe radial et 96.0% pour le groupe fémoral, p=0.47. 5.7% des patients du groupe radial sont décédés, contre 5.7% du groupe fémoral, p=0.97. Conclusion Notre étude démontre que la prise en charge d'un STEMI par voie radiale est possible, puisqu'elle n'allonge pas le « door to balloon time », qu'elle n'augmente pas le taux de MACE ni les saignements majeurs, que soit au point de ponction ou non. Par contre la voie radiale augmente la quantité de produit de contraste nécessaire, et allonge le temps de fluoroscopie de manière non négligeable. De futures études permettront peut-être de mieux définir les groupes de patients qui pourraient bénéficier le plus d'une approche radiale.

We report an experiment where participants observed an attack on their virtual body as experienced in an immersive virtual reality (IVR) system. Participants sat by a table with their right hand resting upon it. In IVR, they saw a virtual table that was registered with the real one, and they had a virtual body that substituted their real body seen from a first person perspective. The virtual right hand was collocated with their real right hand. Event-related brain potentials were recorded in two conditions, one where the participant"s virtual hand was attacked with a knife and a control condition where the knife only struck the virtual table. Significantly greater P450 potentials were obtained in the attack condition confirming our expectations that participants had a strong illusion of the virtual hand being their own, which was also strongly supported by questionnaire responses. Higher levels of subjective virtual hand ownership correlated with larger P450 amplitudes. Mu-rhythm event-related desynchronization in the motor cortex and readiness potential (C3-C4) negativity were clearly observed when the virtual hand was threatened as would be expected, if the real hand was threatened and the participant tried to avoid harm. Our results support the idea that event-related potentials may provide a promising non-subjective measure of virtual embodiment. They also support previous experiments on pain observation and are placed into context of similar experiments and studies of body perception and body ownership within cognitive neuroscience.

Contexte   L'hyperaldostéronisme primaire (HAP) est actuellement considéré comme la première cause d'hypertension artérielle secondaire. Son diagnostic est capital car il permet d'identifier des patients avec une cause potentiellement réversible d'hypertension. Cependant, le diagnostic et l'attitude thérapeutique (traitement médical vs. chirurgical) ne sont pas toujours évidents. Le cathétérisme sélectif des veines surrénaliennes (CVS) permet la détermination d'une latéralisation de la sécrétion d'aldostérone avec moins de risques de faux négatifs et de faux positifs que l'imagerie par scanner ou par IRM. Contrairement à d'autres pays européens, asiatiques ou américains, il n'existe pour le moment aucune donnée publiée sur le cathétérisme des veines surrénaliennes en Suisse.  Objectifs Déterminer le taux de succès du CVS ainsi que ses déterminants cliniques au cours des 5 dernières années au CHUV, ainsi que démontrer l'utilité d'un cathétérisme des veines surrénaliennes lorsqu'une surrénalectomie est envisagée dans le cadre d'un hyperaldostéronisme primaire  Méthodes Cette étude est rétrospective et monocentrique. La récolte de données s'est faite à partir de dossiers de 68 patients atteints d'hypertension artérielle (HTA) secondaire sur HAP biochimiquement confirmé et ayant eu un CVS au cours des 5 dernières années.  Résultats Au CHUV, le taux de succès du CVS (cathétérisme bilatéral des veines surrénaliennes) s'élève à 60% (41/68). Dans 53% des cas, il y a discordance entre les résultats de l'imagerie standard et ceux du CVS. Une attitude thérapeutique basée sur l'imagerie standard aurait entrainé des traitements inappropriés : 43% des patients auraient subi une chirurgie inadéquate, et 10% n'auraient pas été candidats à une sanction chirurgicale alors qu'elle aurait été indiquée sur la base du CVS.  Conclusions Le CVS doit être considéré comme l'étalon-or dans la prise en charge diagnostique de l'HAP lorsqu'une intervention chirurgicale est possible. En se basant uniquement sur l'imagerie, il en résulterait un taux élevé de traitement inapproprié. Aux vues des difficultés techniques associées à cet examen, de nouvelles procédures doivent être mises en place pour mener à une amélioration de son taux de succès et à l'interprétation des résultats.

Introduction Selon les recommandations internationales, l'angioplastie primaire est le traitement de choix de l'infarctus transmural aigu du myocarde (STEMI). L'évidence de ce constat reste peu claire en ce qui concerne certaines sous-­‐populations. C'est le cas, par exemple, des personnes d'âge avancé. La problématique du traitement de l'infarctus du myocarde chez le sujet âgé semble essentielle à une époque de vieillissement démographique mondial, et ce car le patient type présentant un STEMI évolue parallèlement à ce phénomène. L'objectif de la présente étude était d'observer la survie à long terme des octogénaires résidant en Suisse après traitement d'un infarctus de type STEMI selon les recommandations internationales. Méthodologie Inclusion rétrospective de tous les patients âgés de ≥ 80 ans victimes d'un STEMI entre janvier 2008 et novembre 2011. Informations démographiques et pathologiques récoltées à partir des archives et dossiers informatisés du Centre hospitalier universitaire vaudois (CHUV). Critère d'évaluation principal : mortalité à 3 ans, toutes causes confondues. Données de mortalité obtenues par le dossier informatisé (si décès intra-­‐hospitalier) ou par contact avec les autorités administratives de la Suisse romande. Critères d'évaluation secondaire : réussite de l'intervention et événements intra-­‐hospitaliers (informations obtenues grâce au dossier informatisé). Ce\e recherche clinique a reçu l'aval de la Commission d'éthique cantonale vaudoise. Résultats 108 patients correspondaient à nos critères dans la période sélectionnée ; 5 ont été exclus car non domiciliés en Suisse. Collectif final (103 patients) âgé en moyenne de 84.3 ans. Une majorité de femmes (51.4%) et une plus faible proportion de tabagiques étaient les divergences par rapport à la population STEMI générale. À 3 ans, une mortalité supérieure à 32% fut enregistrée dont plus du tiers des décès survenant dans le premier mois suivant l'infarctus. Discussion et conclusion La mortalité objectivée chez l'octogénaire traité par angioplastie pour un STEMI est particulièrement élevée, inattendue et semble plus importante que celle d'un octogénaire non coronarien. Ce constat devra être confirmé par d'autres auteurs car les études concernant cette problématique sont encore rares.

Total lack of visual experience [dark rearing (DR)] is known to prolong the critical period and delay development of sensory functions in mammalian visual cortex. Recent results show that neurotrophins (NTs) counteract the effects of DR on functional properties of visual cortical cells and exert a strong control on critical period duration. NTs are known to modulate the development and synaptic efficacy of neurotransmitter systems that are affected by DR. However, it is still unknown whether the actions of NTs in dark-reared animals involve interaction with neurotransmitter systems. We have studied the effects of DR on the expression of key molecules in the glutamatergic and GABAergic systems in control and NT-treated animals. We have found that DR reduced the expression of the NMDA receptor 2A subunit and its associated protein PSD-95 (postsynaptic density-95), of GRIP (AMPA glutamate receptor interacting protein), and of the biosynthetic enzyme GAD (glutamic acid decarboxylase). Returning dark-reared animals to light for 2 hr restored normal expression of the above-mentioned proteins almost completely. NT treatment specifically counteracts DR effects; NGF acts primarily on the NMDA system, whereas BDNF acts primarily on the GABAergic system. Finally, the action of NT4 seems to involve both excitatory and inhibitory systems. These data demonstrate that different NTs counteract DR effects by modulating the expression of key molecules of the excitatory and inhibitory neurotransmitter systems

The aim of this study was to estimate the diagnostic value of renal cortex copper (Cu) concentration in clinical cases of acute copper poisoning (ACP). A total of 97 calves that died due to subcutaneous copper administration were compiled in eleven farms. At least, one necropsy was conducted on each farm and samples for complementary analysis were taken. The degree of autolysis in each necropsy was evaluated. The cases appeared on extensive grazing calf breeding and intensive feedlot farms, in calves of 60 to 200 kg body weight. Mortality varied from 0.86 to 6.96 %, on the farms studied. The first succumbed calf was found on the farms between 6 and 72 hours after the susbcutaneous Cu administration. As discrepancies regarding the reference value arose, the local value (19.9 parts per million) was used, confirming the diagnosis of acute copper poisoning in 93% of the analyzed kidney samples. These results confirm the value of analysis of the cortical kidney Cu concentration for the diagnosis of acute copper poisoning.

A total of 182 young adult male Wistar rats were bilaterally implanted with cannulae into the CA1 region of the dorsal hippocampus and into the amygdaloid nucleus, the entorhinal cortex, and the posterior parietal cortex. After recovery, the animals were trained in a step-down inhibitory avoidance task. At various times after training (0, 30, 60 or 90 min) the animals received a 0.5-µl microinfusion of vehicle (saline) or 0.5 µg of muscimol dissolved in the vehicle. A retention test was carried out 24 h after training. Retention test performance was hindered by muscimol administered into both the hippocampus and amygdala at 0 but not at 30 min posttraining. The drug was amnestic when given into the entorhinal cortex 30, 60 or 90 min after training, or into the parietal cortex 60 or 90 min after training, but not before. These findings suggest a sequential entry operation, during the posttraining period, of the hippocampus and amygdala, the entorhinal cortex, and the posterior parietal cortex in memory processing

The fundamental role of N-methyl-D-aspartate (NMDA) receptors in many cortical functions has been firmly defined, as has its involvement in a number of neurological and psychiatric diseases. However, until recently very little was known about the anatomical localization of NMDA receptors in the cerebral cortex of mammals. The recent application of molecular biological techniques to the study of NMDA receptors has provided specific tools which have greatly expanded our understanding of the localization of NMDA receptors in the cerebral cortex. In particular, immunocytochemical studies on the distribution of cortical NMDA receptors have shown that NMDA receptors are preferentially localized on dendritic spines, have disclosed an unknown fraction of presynaptic NMDA receptors on both excitatory and inhibitory axon terminals, and demonstrated that cortical astrocytes do express NMDA receptors. These studies suggest that the effects induced by the activation of NMDA receptors are not due solely to the opening of NMDA channels on neuronal postsynaptic membranes, as previously assumed, but that the activation of presynaptic and glial NMDA receptors may mediate part of these effects

Rapid eye movement (REM) sleep deprivation induces several behavioral changes. Among these, a decrease in yawning behavior produced by low doses of cholinergic agonists is observed which indicates a change in brain cholinergic neurotransmission after REM sleep deprivation. Acetylcholinesterase (Achase) controls acetylcholine (Ach) availability in the synaptic cleft. Therefore, altered Achase activity may lead to a change in Ach availability at the receptor level which, in turn, may result in modification of cholinergic neurotransmission. To determine if REM sleep deprivation would change the activity of Achase, male Wistar rats, 3 months old, weighing 250-300 g, were deprived of REM sleep for 96 h by the flower-pot technique (N = 12). Two additional groups, a home-cage control (N = 6) and a large platform control (N = 6), were also used. Achase was measured in the frontal cortex using two different methods to obtain the enzyme activity. One method consisted of the obtention of total (900 g supernatant), membrane-bound (100,000 g pellet) and soluble (100,000 g supernatant) Achase, and the other method consisted of the obtention of a fraction (40,000 g pellet) enriched in synaptic membrane-bound enzyme. In both preparations, REM sleep deprivation induced a significant decrease in rat frontal cortex Achase activity when compared to both home-cage and large platform controls. REM sleep deprivation induced a significant decrease of 16% in the membrane-bound Achase activity (nmol thiocholine formed min-1 mg protein-1) in the 100,000 g pellet enzyme preparation (home-cage group 152.1 ± 5.7, large platform group 152.7 ± 24.9 and REM sleep-deprived group 127.9 ± 13.8). There was no difference in the soluble enzyme activity. REM sleep deprivation also induced a significant decrease of 20% in the enriched synaptic membrane-bound Achase activity (home-cage group 126.4 ± 21.5, large platform group 127.8 ± 20.4, REM sleep-deprived group 102.8 ± 14.2). Our results suggest that REM sleep deprivation changes Ach availability at the level of its receptors through a decrease in Achase activity

Lesions of the entorhinal cortex produce retrograde memory impairment in both animals and humans. Here we report the effects of bilateral entorhinal cortex lesions caused by the stereotaxic infusion of N-methyl-D-aspartate (NMDA) in rats at two different moments, before or after the training session, on memory of different tasks: two-way shuttle avoidance, inhibitory avoidance and habituation to an open field. Pre- or post-training entorhinal cortex lesions caused an impairment of performance in the shuttle avoidance task, which agrees with the previously described role of this area in the processing of memories acquired in successive sessions. In the inhibitory avoidance task, only the post-training lesions had an effect (amnesia). No effect was observed on the open field task. The findings suggest that the role of the entorhinal cortex in memory processing is task-dependent, perhaps related to the complexity of each task

We studied the distribution of NADPH-diaphorase activity in the visual cortex of normal adult New World monkeys (Saimiri sciureus) using the malic enzyme "indirect" method. NADPH-diaphorase neuropil activity had a heterogeneous distribution. In coronal sections, it had a clear laminar pattern that was coincident with Nissl-stained layers. In tangential sections, we observed blobs in supragranular layers of V1 and stripes throughout the entire V2. We quantified and compared the tangential distribution of NADPH-diaphorase and cytochrome oxidase blobs in adjacent sections of the supragranular layers of V1. Although their spatial distributions were rather similar, the two enzymes did not always overlap. The histochemical reaction also revealed two different types of stained cells: a slightly stained subpopulation and a subgroup of deeply stained neurons resembling a Golgi impregnation. These neurons were sparsely spined non-pyramidal cells. Their dendritic arbors were very well stained but their axons were not always evident. In the gray matter, heavily stained neurons showed different dendritic arbor morphologies. However, most of the strongly reactive cells lay in the subjacent white matter, where they presented a more homogenous morphology. Our results demonstrate that the pattern of NADPH-diaphorase activity is similar to that previously described in Old World monkeys

The effects of methylmercury (MeHg) on histochemical demonstration of the NADPH-diaphorase (NADPH-d) activity in the striate cortex were studied in 4 adult cats. Two animals were used as control. The contaminated animals received 50 ml milk containing 0.42 µg MeHg and 100 g fish containing 0.03 µg MeHg daily for 2 months. The level of MeHg in area 17 of intoxicated animals was 3.2 µg/g wet weight brain tissue. Two cats were perfused 24 h after the last dose (group 1) and the other animals were perfused 6 months later (group 2). After microtomy, sections were processed for NADPHd histochemistry procedures using the malic enzyme method. Dendritic branch counts were performed from camera lucida drawings for control and intoxicated animals (N = 80). Average, standard deviation and Student t-test were calculated for each data group. The concentrations of mercury (Hg) in milk, fish and brain tissue were measured by acid digestion of samples, followed by reduction of total Hg in the digested sample to metallic Hg using stannous chloride followed by atomic fluorescence analysis. Only group 2 revealed a reduction of the neuropil enzyme activity and morphometric analysis showed a reduction in dendritic field area and in the number of distal dendrite branches of the NADPHd neurons in the white matter (P<0.05). These results suggest that NADPHd neurons in the white matter are more vulnerable to the long-term effects of MeHg than NADPHd neurons in the gray matter.

The neuroendocrine system regulates several organic functions such as reproduction, metabolism and adaptation to the environment. This system shows seasonal changes linked to the environment. The experimental model used in the present study was Lagostomus maximus maximus (viscacha). The reproduction of males of this species is photoperiod dependent. Twenty-four adult male viscachas were captured in their habitat at different times during one year. The adrenal glands were processed for light microscopy. Serial cuts were stained with hematoxylin-eosin for the morphometric study, and 100 nuclei of each zone of the adrenal cortex were counted per animal. Data were analyzed statistically by ANOVA and the Tukey test. The cells of the glomerulosa zone are arranged in a tube-shaped structure. The fasciculata zone has large cells with central nuclei and clearly visible nucleoli and with a vacuolar cytoplasm. In the reticularis zone there are two of types of cells, one with a nucleus of fine chromatin and a clearly visible nucleolus and the other with nuclear pycnosis. Morphometric analysis showed maximum nuclear volumes during the February-March period with values of 133 ± 7.3 µm3 for the glomerulosa, 286.4 ± 14.72 µm3 for the fasciculata, and 126.3 ± 9.49 µm3 for the reticularis. Minimum nuclear volumes were observed in August with values of 88.24 ± 9.9 µm3 for the glomerulosa, 163.7 ± 7.78 µm3 for the fasciculata and 64.58 ± 4.53 µm3 for the reticularis. The short winter photoperiod to which viscacha is subjected could inhibit the adrenal cortex through a melatonin increase which reduces the nuclear volume as well as the cellular activity.
