As the glycoprotein GPIIb/IIIa receptor is the final common pathway in platelet aggregation, antagonists of this receptor cause a profound inhibition of aggregation induced by any agonist. The short-term efficacy and safety of GPIIb/IIIa antagonists in patients undergoing coronary angioplasty was demonstrated with murine 7E3 Fab, but this antibody was immunogenic. Abciximab is a chimeric human-mouse monoclonal antibody that is less immunogenic. The first major trial with a GPIIb/IIIa antagonist was the EPIC trial with abciximab, which showed that abciximab reduced the ischemic complications of coronary balloon angioplasty and atherectomy in high-risk patients, but increased the risk of bleeding. Subsequent studies showed that using less concurrent heparin reduced bleeding. Abciximab also reduced the rate of revascularization. Further studies have shown that the benefits of abciximab extended to all patients undergoing angioplasty (EPILOG), including patients with unstable angina (CAPTURE) and acute myocardial infarction (RAPPORT). Clinical trials with eptifibatide and tirofiban have failed to demonstrate benefit, at the doses used, in angioplasty. Abciximab and eptifibatide, but not oral xemilofiban, improve the safety of the coronary stenting procedure. Shortterm intravenous treatment with lamifiban, eptifibatide or tirofiban is beneficial in acute coronary syndromes (unstable angina, non-Q wave myocardial infarction). Orally active GPIIb/IIIa antagonists are being developed for use in acute coronary syndromes and myocardial infarction. However, no benefit has been shown with lefradafiban in acute coronary syndromes and sibrafiban and orbofiban are harmful. Eptifibatide, lamifiban and abciximab improve coronary patency in myocardial infarction, and long-term trials of GPIIb/IIIa antagonists are being conducted in acute myocardial infarction. Abciximab can cause thrombocytopenia, and all the GPIIb/IIIa antagonists increase the incidence of bleeding, but there is no excess of intracranial hemorrhage. (C) 2001 Prous Science. Aortic thromboembolism is one of the most serious and difficult-to-manage complications. of feline cardiac disease. Most, but not all, cats presenting with signs of aortic thromboembolism are found to have underlying cardiac disease at the time of presentation. In most cases no underlying disease has been diagnosed prior to presentation with paresis/paralysis and profound anxiety. This article will review commonly used treatments for thromboembolism and agents proposed for prophylaxis. Many of the proposed treatments are themselves associated with a high morbidity rate and long term clinical trials are required to make comparative risk-to-benefit ratio assessments of these different options. In cats which do survive the initial treatment, clinicians are still faced with the perplexing problem of long term thrombus prevention, as a majority of cats have been shown to re-embolise despite prophylaxis.

SUMMARY: In differentiated healthcare, the promotion of health and health education activities are indispensable for personal empowerment, preparing the individual to prevent complications of his own disease, exercising a greater control over his own health, resulting in global health gains. This investigation was intended to analyse the satisfaction of liver transplant patients about the nursing care given, concerning health education, as well as to try and identify aspects that influenced the patient’s perception and experience when dealing with the preparation of returning home. The study was descriptive, with a mixed approach of exploratory and mixed inquiry, with qualitative and quantitative orientation, in a deductive-inductive direction. The population integrated a convenience sample of 75 liver transplant patients admitted at the Curry Cabral Hospital. The results were analysed according to the objectives, based on the methodological movement structure, level of expectations, experience and perception of the patient’s satisfaction. This study draws attention to the fact that demographic factors such as age and hospitalisation time influenced expectations, as well as previous experiences influenced the manner in which these patients received information during their hospital stay. The patients submitted to liver transplant, questioned in this study, affirmed, in a global manner, that nursing care given during admission, concerning health education for returning home were "Good/Very Good" and "Reasonable". Expectations, previous experiences, experience with and perceptions about nursing care as well as hospital discharge preparation were in this context, recognized as influences on the satisfaction of nursing care given. It is indispensable, therefore, that activities related to health education autonomously developed by nurses, contribute to emphasize the professional role of nursing care, highlighting the nurse’s intervention with the patient and/or family when returning home.

Three distinct syndromes caused by schistosomiasis have been described: cercarial dermatitis or swimmer's itch, acute schistosomiasis or Katayama fever, and chronic schistosomiasis. Complications of acute schistosomiasis have also been reported. The absence of a serological marker for the acute stage has hindered early diagnosis and treatment. Recently, an ELISA test using KLH (keyhole limpet haemocyanin) as antigen, has proved useful in differentiating acute from chronic schistosomiasis mansoni. Clinical and experimental evidence indicate that steroids act synergistically with schistosomicides in the treatment of Katayama syndrome. In this paper, clinical, diagnostic and therapeutic features of acute schistosomiasis are updated.

The increase in human longevity is a fact prevalent nowadays, but to require great efforts on health promotion and disease prevention and is also in the elderly high risk of developing illnesses as they age. It is therefore important contribution of the individuals themselves towards mitigating these risks and delaying the development and exacerbations of disease once manifested. Thus, the general purpose of this study is to describe how the knowledge of diabetes, may be related to complications of the disease. We conducted an exploratory study in the Hospital Nossa Senhora do Rosario EPE, in elderly people living in the county of Barry, who attended the hospital Diabetes Day, after the medical and nursing care. The method used for the sampling was not probabilistic by convenience sampling (n = 50). The instrument used for data collection was a questionnaire applied in the form of survey to collect socio-demographic data, general health, mental state, data on diabetes and comorbidities. The data were processed by computer, using the SPSS and content analysis. The results showed that participants had knowledge about their disease. We found that respondents who have had complications due to diabetes responded correctly to questions about the medication is more important than diet and exercise to control diabetes than those who never had complications due to diabetes, who answered incorrectly or did not know the answer the question.

Para tal investigação reconhece-se a pertinência da aplica-ção da escala de avaliação do sentido de coerência – Questionário Orientação para Viver (Nunes, 1999), da escala de avaliação do autocuidado com a diabetes – Escala de Avaliação das Activida-des de Autocuidado com a Diabetes (Bastos, Severo e Lopes, 2007) e do Formulário de Controlo Metabólico na Diabetes (European Society of Cardiology e European Association for the Study of Diabetes cit. por Direcção-Geral da Saúde, 2007), numa amostra constituída por 391 pessoas com diabetes de tipo 2, seleccionadas por amostragem aleatória estratificada proporcional, de entre os utentes inscritos nas 33 instituições de Cuidados de Saúde Primários, do distrito de Viana do Castelo --------------------------- ABSTRACT - Diabetes mellitus is considered a global scourge, not only for reaching millions of people, but also because there has been an increase in its prevalence and, consequently, its chronic complications, which translate into increased spending on health and especially in the suffering of people with diabetes, their families and community. The prevention of chronic complications of diabetes includes adoption healthy behaviors that on the day to day, translate into competence in self-care. Therefore it is important to know how far the sense of coherence – understood as influential personal characteristic of the strategies used by the events originators of stress, affecting the mental health and physical health – may determine the choice of health behaviors and, as such, can be considered as an aspect about which health pro-fessionals will invest in strengthening their offspring and thus ensure the facilitation of the process of adopting healthy behaviors. According to the proposed retrospective exploratory cross-sectional study aims to discover the relationship between the level of sense of coherence and level of self-care and metabolic control on people with type 2 diabetes. For this research recognizes the appropriateness of applying the rating scale of sense of coherence – Questionário Orientação para Viver (Nunes, 1999), the rating scale of self-care in diabetes – Escala de Avaliação das Actividades de Autocuidado com a Di-abetes (Bastos, Severo and Lopes, 2007) and the Formulário de Controlo Metabólico na Diabetes (European Society of Cardiology and European Association for the Study of Diabetes cit. por Di-recção-Geral da Saúde, 2007), a sample of 391 people with type 2 diabetes, selected by propor-tional stratified random sampling from among registered users in 33 institutions of Primary Health Care, on district of Viana do Castelo.

Cerebral phaeohyphomycosis ("chromoblastomycosis") is a rare intracranial lesion. We report the first human culture-proven case of brain abscesses due to Fonsecaea pedrosoi in Brazil. The patient, a 28 year-old immunocompetent white male, had ocular manifestations and a hypertensive intracranial syndrome. Magnetic resonance imaging (MRI) of the brain revealed a main tumoral mass involving the right temporo-occipital area and another smaller apparently healed lesion at the left occipital lobe. A cerebral biopsy was performed and the pathological report was cerebral chromoblastomycosis. The main lesion was enucleated surgically and culture of the necrotic and suppurative mass grew a fungus identified as Fonsecaea pedrosoi. The patient had received a knife wound sixteen years prior to his hospitalization and, more recently, manifested a pulmonary granulomatous lesion in the right lung with a single non-pigmented form of a fungus present. It was speculated that the fungus might have gained entrance to the host through the skin lesion, although a primary respiratory lesion was not excluded. The patient was discharged from the hospital still with ocular manifestations and on antimycotic therapy and was followed for eight months without disease recurrence. Few months after he had complications of the previous neuro-surgery and died. A complete autopsy was performed and no residual fungal disease was found.

Inhalation injuries are currently the factor most responsible for mortality in thermally injured patients. Inhalation injuries may occur independently, but generally occur together with skin burn. Smoke inhalation affects all levels of the respiratory system and the extent of the inhalation injury depends on the duration, exposure, amount and toxicity of the fume temperature, concentration and solubility of toxic gases, the occurrence of the accident in a closed space and pre-existing diseases. Smoke inhalation also induces changes in the systemic organs with the need for more fluid for resuscitation. Systemic vasoconstriction, with an elevation in systemic vascular resistance, a fall in myocardial contractility and a great increase in lymphatic flow in soft tissue are the most important changes in systemic organs. On admission of a burn patient there is a high suspicion of inhalation injury when there are signs and symptoms such as hoarseness, strides, dyspnea, carbonaceous sputum, anxiety or disorientation, with or without face burns. The patient with these findings has partial airway obstruction and there is substantial risk complete airway obstruction occurring of secondary to the edema. Patients with suspected inhalation injury should be intubated so as to maintain airway patency and avoid a total obstruction. This group of patients frequently develop respiratory failure with the need for mechanical ventilatory support. Nosocomial infections, sepsis and multiple organ system failure may occur. Late complications of inhalation injury are tracheitis, tracheal stenosis or tracheomalacia and chronic airway disease, which is relatively rare. Early diagnosis of inhalation injury and treatment in a Burn Unit by a group of highly motivated clinicians and a good team of nurses is essential in order to decrease the morbidity and mortality related to inhalation injury.

Objectives: Literature review of classification, epidemiology, pathophysiology, microbiology, clinical presentation, diagnosis, complications, treatment and prevention of urinary tract infections (UTI) in pregnancy. Data Sources and Review Methods: Bibliographic research in Medline, through PubMed and Medscape, of systematic reviews, observational studies, clinical guidelines, meta-analyses and randomized controlled trials published between January 1992 and December 2010. Results: Asymptomatic bacteriuria occurs in 2 to 10% of pregnant women, 30% of which will develop acute pyelonephritis if left untreated. Treatment of asymptomatic bacteriuria has been shown to reduce the risk of pyelonephritis in pregnancy. Therefore, screening and treatment of this form of UTI has become a standard of obstetrical care, although the ideal duration of the treatment is still controverse. Acute pyelonephritis is one of the most common medical complications of pregnancy and may be associated with maternal, obstetric and perinatal complications. There is no clear consensus in the literature regarding antibiotic choice or duration of therapy for symptomatic UTI. Increasing antibiotic resistance complicates the choice of empirical regimens and local resistance rates need to be taken into account when deciding the therapy. Considering the high rate of recurrence of UTI during pregnancy, prophylactic measures need to be taken in pregnant women who have already experienced UTI during their current pregnancy. Conclusions: Although UTI is a generally benign condition in non-pregnant women, it may be a potentially serious complication during pregnancy. Early diagnosis and treatment of UTI during pregnancy are mandatory and can prevent severe maternal and perinatal complications.

Schistosomiasis haematobia or urinary schistosomiasis is one of the main public health problems in Africa and the Middle East. A single dose of 40 mg praziquantel per kg body weight continues to be the treatment of choice for this infection. The aims of this follow-up were to study the post-treatment course of a patient infected with S. haematobium and not submitted to re-exposure, and to identify complications of the disease and/or therapeutic failure after praziquantel treatment by histopathological analysis. Treatments were repeated under medical supervision to ensure the correct use of the drug. In view of the suspicion of lesions in cystoscopy, the patient was submitted to bladder biopsy. The histopathological characteristics observed in biopsies obtained, after each treatment, indicated viability of parasite eggs and activity of granulomas.

Perinatal mortality rate is an important mark to evaluate women and perinatal health care. It is of utmost importance to know causes and the evolution of its two components aiming to improve health care in different fields – sanitary conditions, diagnosis and treatment of infectious disease, immunisations, diagnosing and caring for medical diseases induced by pregnancy or directly related to it, providing skilled birth attendance, preventing birth asphyxia, preventing preterm birth complications and infections. In high-income countries the epidemiology varies mainly with social and economic conditions; in low-income countries, paired with poverty, undernutrition, superstition, lack of medical care, deficient basic sanitary conditions are also found. Also, in rich countries, responsible for 1% of deaths, data are published and improvements evaluated, while in low-income countries responsible for 99% of deaths numbers and causes are unknown, making difficult to implement cost effective interventions, a reason why “stillbirth rates in low-income countries are now where they were in high-income countries 50 to 100 years ago”. Knowledge on causes of death are very important as often what is needed are “simple” measures as improvement of sanitary conditions and immunisation programmes rather than high technologies. About four million babies dye each year in the first 28 days of life and another 3 million dye before birth in the third-trimester, with 98% occurring in low-income and middle income countries and more than 1 million occurring during labour and delivery. Classically stillbirths are the major component of perinatal mortality rate. Causes of death are even more difficult to know. In low-income countries a great proportion of women give birth at home. Worldwide the main causes of stillbirth are asphyxia due to obstructed labour, eclampsia, abruption placenta and umbilical cord complications - making valid the assumption that skilled birth attendance would decrease stillbirth; and infection - chorioamnioitis, syphilis and malaria. In high-income countries placental pathology and infection, congenital anomalies, complications of preterm birth and post term delivery, are the most common. If in low-income countries famine and lack of provisions and health care are common, in high-income countries, advanced maternal age and diabetes, obesity, hypertension, smoking, are frequent findings.

Introduction: Hepatitis C virus (HCV) infection in patients with hereditary bleeding disorders (HBDs), as a consequence of treatment with transfusion of human bloodderived components between the late 1970s and 1980s, represents a major health concern. Objectives: Assessment and evaluation of the burden of HCV infection, its complications, and treatment in a population of patients with HBDs. Methods: Analysis of a series of 161 patients with HBDs treated in the Immunohemotherapy Service of the Centro Hospitalar de Lisboa Central (Lisboa, Portugal), consultation and systematic review of the patients clinical processes, elaboration of a database comprising the information gathered; and statistical study of its variables: age, gender, degree of severity of the bleeding disorder, treatment modality, and major and minor complications of HCV infection. Results: Sixty-five (40%) of the 161 patients have HCV infection. Among the patients with hemophilia A, 36% are severe and 62% of those have HCV infection; 9% moderate with 57%; 25% mild with 20%. In the hemophilia B group, 8% are severe with 23% infected and 6% moderate or mild with 10%. Concerning the patients with von Willebrand disease, 12% have type 2 with 16% infected and 4% have type 3 with 86%. Conclusions: HCV infection represents a very significant complication of the treatment employed in the past in the studied population. Considering that most of these patients were infected in the late 1970s and early 1980s, and the natural evolution of HCV infection in patients without bleeding disorders, it is expected that the prevalence of major complications will rise significantly in the coming years. Prophylactic measures should be implemented to enhance the follow-up protocols and prevent further development of liver damage in these patients.

The development of antibodies to factor VIII is one of the most serious complications of haemophilia treatment. Approximately 30% of patients with severe haemophilia develop neutralizing inhibitors to replacement FVIII. Although most patients with inhibitors do not bleed more frequently than patients without inhibitors, bleeding is more difficult to control and this patients suffer more severe bleeding and have greater morbidity and mortality. Patients with persistent high-titer inhibitor who are not candidates or fail ITI, pose a great challenge to haemophilia management. The efficacy and safety of prophylaxis with bypassing agents in reducing bleeding tendency, has been described in numerous studies. Patients and methods: We report tree adult severe haemophilia A patients, two with persistent high-titre inhibitors and one who failed ITI, on prophylactic treatment after several significant musculoskeletal and life-threatening haemorrhagic episodes (intrabdominal/intramuscular) and pseudotumor haemorrhage. Treatment regimens consisted of APCC (Feiba®) in doses of 60-70UKg-1, 2-3 times per week, according underlying bleeding phenotype. Breakthrough bleeds were treated with either APCC (Feiba®) or rFVIIa (NovoSeven®). Results and Conclusion: There was reduction in total bleeding episodes in two patients (43% to 80%) and one patient remained stable, while receiving prophylaxis. Absence of severe and life threatening bleeding episodes, as well as inpatient stays, contributing to a better quality of life in those patients, was observed. APCC (Feiba®) was well tolerated and no thrombotic events were observed.
