In this 'Summary Guidance for Daily Practice', we describe the basic principles of prevention and management of foot problems in persons with diabetes. This summary is based on the International Working Group on the Diabetic Foot (IWGDF) Guidance 2015. There are five key elements that underpin prevention of foot problems: (1) identification of the at-risk foot; (2) regular inspection and examination of the at-risk foot; (3) education of patient, family and healthcare providers; (4) routine wearing of appropriate footwear, and; (5) treatment of pre-ulcerative signs. Healthcare providers should follow a standardized and consistent strategy for evaluating a foot wound, as this will guide further evaluation and therapy. The following items must be addressed: type, cause, site and depth, and signs of infection. There are seven key elements that underpin ulcer treatment: (1) relief of pressure and protection of the ulcer; (2) restoration of skin perfusion; (3) treatment of infection; (4) metabolic control and treatment of co-morbidity; (5) local wound care; (6) education for patient and relatives, and; (7) prevention of recurrence. Finally, successful efforts to prevent and manage foot problems in diabetes depend upon a well-organized team, using a holistic approach in which the ulcer is seen as a sign of multi-organ disease, and integrating the various disciplines involved.

Recommendations - 1 To identify a person with diabetes at risk for foot ulceration, examine the feet annually to seek evidence for signs or symptoms of peripheral neuropathy and peripheral artery disease. (GRADE strength of recommendation: strong; Quality of evidence: low) - 2 In a person with diabetes who has peripheral neuropathy, screen for a history of foot ulceration or lower-extremity amputation, peripheral artery disease, foot deformity, pre-ulcerative signs on the foot, poor foot hygiene and ill-fitting or inadequate footwear. (Strong; Low) - 3 Treat any pre-ulcerative sign on the foot of a patient with diabetes. This includes removing callus, protecting blisters and draining when necessary, treating ingrown or thickened toe nails, treating haemorrhage when necessary and prescribing antifungal treatment for fungal infections. (Strong; Low) - 4 To protect their feet, instruct an at-risk patient with diabetes not to walk barefoot, in socks only, or in thin-soled standard slippers, whether at home or when outside. (Strong; Low) - 5 Instruct an at-risk patient with diabetes to daily inspect their feet and the inside of their shoes, daily wash their feet (with careful drying particularly between the toes), avoid using chemical agents or plasters to remove callus or corns, use emollients to lubricate dry skin and cut toe nails straight across. (Weak; Low) - 6 Instruct an at-risk patient with diabetes to wear properly fitting footwear to prevent a first foot ulcer, either plantar or non-plantar, or a recurrent non-plantar foot ulcer. When a foot deformity or a pre-ulcerative sign is present, consider prescribing therapeutic shoes, custom-made insoles or toe orthosis. (Strong; Low) - 7 To prevent a recurrent plantar foot ulcer in an at-risk patient with diabetes, prescribe therapeutic footwear that has a demonstrated plantar pressure-relieving effect during walking (i.e. 30% relief compared with plantar pressure in standard of care therapeutic footwear) and encourage the patient to wear this footwear. (Strong; Moderate) - 8 To prevent a first foot ulcer in an at-risk patient with diabetes, provide education aimed at improving foot care knowledge and behaviour, as well as encouraging the patient to adhere to this foot care advice. (Weak; Low) - 9 To prevent a recurrent foot ulcer in an at-risk patient with diabetes, provide integrated foot care, which includes professional foot treatment, adequate footwear and education. This should be repeated or re-evaluated once every 1 to 3 months as necessary. (Strong; Low) - 10 Instruct a high-risk patient with diabetes to monitor foot skin temperature at home to prevent a first or recurrent plantar foot ulcer. This aims at identifying the early signs of inflammation, followed by action taken by the patient and care provider to resolve the cause of inflammation. (Weak; Moderate) - 11 Consider digital flexor tenotomy to prevent a toe ulcer when conservative treatment fails in a high-risk patient with diabetes, hammertoes and either a pre-ulcerative sign or an ulcer on the distal toe. (Weak; Low) - 12 Consider Achilles tendon lengthening, joint arthroplasty, single or pan metatarsal head resection, or osteotomy to prevent a recurrent foot ulcer when conservative treatment fails in a high-risk patient with diabetes and a plantar forefoot ulcer. (Weak; Low) - 13 Do not use a nerve decompression procedure in an effort to prevent a foot ulcer in an at-risk patient with diabetes, in preference to accepted standards of good quality care. Autoimmune regulator (AIRE) is the gene mutated in the human polyglandular autoimmune disease called Autoimmune polyendocrinopathy, candidiasis, and ectodermal dystrophy (APECED) that belongs to the Finnish disease heritage. Murine Aire has been shown to be important in the generation of the T cell central tolerance in the thymus by promoting the expression of ectopic tissue-specific antigens in the thymic medulla. Aire is also involved in the thymus tissue organization during organogenesis. In addition to the thymus, AIRE/Aire is expressed in the secondary lymphoid organs. Accordingly, a role for AIRE/Aire in the maintenance of peripheral tolerance has been suggested. Peripheral tolerance involves mechanisms that suppress immune responses in secondary lymphoid organs. Regulatory T cells (Tregs) are an important suppressive T cell population mediating the peripheral tolerance. Tregs are generated in the thymus but also in the peripheral immune system T cells can acquire the Treg-phenotype. The aim of this study was to characterize Tregs in APECED patients and in the APECED mouse model (Aire-deficient mice). In the mouse model, it was possible to separate Aire expression in the thymus and in the secondary lymphoid organs. The relative importance of thymic and peripheral Aire expression in the maintenance of immunological tolerance was studied in an experimental model that was strongly biased towards autoimmunity, i.e. lymphopenia-induced proliferation (LIP) of lymphocytes. This experimental model was also utilised to study the behaviour of T cells with dual-specific T cell receptors (TCR) during the proliferation. The Treg phenotype was studied by flow cytometry and relative gene expression with real-time polymerase chain reaction. TCR repertoires of the Tregs isolated from APECED patients and healthy controls were also compared. The dual-specific TCRs were studied with the TCR repertoire analysis that was followed with sequencing of the chosen TCR genes in order to estimate changes in the dual-specific TCR diversity. The Treg function was tested with an in vitro suppression assay. The APECED patients had normal numbers of Tregs but the phenotype and suppressive functions of the Tregs were impaired. In order to separate Aire functions in the thymus from its yet unknown role in the secondary lymphoid organs, the phenomenon of LIP was utilised. In this setting, the lymphocytes that are adoptively transferred to a lymphopenic recipient proliferate to stimuli from self-originating antigens. This proliferation can result in autoimmunity if peripheral tolerance is not fully functional. When lymphocytes that had matured without Aire in the thymus were transferred to lymphopenic Aire-sufficient recipients, no clinical autoimmunity followed. The Aire-deficient donor-originating lymphocytes hyperproliferated, and other signs of immune dysregulation were also found in the recipients. Overt autoimmunity, however, was prevented by the Aire-deficient donor-originating Tregs that hyperproliferated in the recipients. Aire-deficient lymphopenic mice were used to study whether peripheral loss of Aire had an impact on the maintenance of peripheral tolerance. When normal lymphocytes were transferred to these Aire-deficient lymphopenic recipients, the majority of recipients developed a clinically symptomatic colitis. The colitis was confirmed also by histological analysis of the colon tissue sections. In the Aire-deficient lymphopenic recipients Tregs were proliferating significantly less than in the control group s recipients that had normal Aire expression in their secondary lymphoid organs. This study shows that Aire is not only important in the central tolerance but is also has a significant role in the maintenance of the peripheral tolerance both in mice and men. Aire expressed in the secondary lymphoid organs is involved in the functions of Tregs during an immune response. This peripheral expression appears to be relatively more important in some situations since only those lymphopenic recipients that had lost peripheral expression of Aire developed a symptomatic autoimmune disease. This AIRE-related Treg defect could be clinically important in understanding the pathogenesis of APECED.

Entamoeba histolytica-specific serum IgG, IgA, IgM and IgE antibodies were assayed in cases of amoebiasis in an endemic area. Patient groups consisted of amoebic liver abscess (n=18), pre-abscess hepatic amoebiasis (n=22) and amoebic colitis (n=30). Control subjects comprised 26 asymptomatic cyst passers, 13 giardiasis cases, 20 typhoid patients and 24 non-amoebic individuals. Serum IgG was assayed by ELISA, using a monoclonal anti IgG β- galactosidase (IgG β-gal) conjugate, a polyclonal avidin biotin horse radish peroxidase (AB-HRP), and a polyclonal anti IgG horse radish peroxidase (IgG HRP) conjugate. IgA and IgM were assayed by the β-gal ELISA and IgE by AB-HRP. Diagnostically significant IgG and IgA while lower IgM and IgE antibody levels were seen in extraintestinal cases. About 40% of suspected pre-abscess hepatic amoebiasis cases were confirmed by antibody estimation. All isotype levels in most dysentery cases were in the range of the controls.

Background: Genetic variants of NOD2 are linked to inflammatory bowel disease (IBD) etiology. Results: DSS model of colitis in wild-type and inducible nitric-oxide synthase (iNOS) null mice revealed that NOD2-iNOS/NO-responsive microRNA-146a targets NUMB gene facilitating Sonic hedgehog (SHH) signaling. Conclusion: miR-146a-mediated NOD2-SHH signaling regulates gut inflammation. Significance: Identification of novel regulators of IBD provides new insights into pathophysiology and development of new therapy concepts. Inflammatory bowel disease (IBD) is a debilitating chronic inflammatory disorder of the intestine. The interactions between enteric bacteria and genetic susceptibilities are major contributors of IBD etiology. Although genetic variants with loss or gain of NOD2 functions have been linked to IBD susceptibility, the mechanisms coordinating NOD2 downstream signaling, especially in macrophages, during IBD pathogenesis are not precisely identified. Here, studies utilizing the murine dextran sodium sulfate model of colitis revealed the crucial roles for inducible nitric-oxide synthase (iNOS) in regulating pathophysiology of IBDs. Importantly, stimulation of NOD2 failed to activate Sonic hedgehog (SHH) signaling in iNOS null macrophages, implicating NO mediated cross-talk between NOD2 and SHH signaling. NOD2 signaling up-regulated the expression of a NO-responsive microRNA, miR-146a, that targeted NUMB gene and alleviated the suppression of SHH signaling. In vivo and ex vivo studies confirmed the important roles for miR-146a in amplifying inflammatory responses. Collectively, we have identified new roles for miR-146a that established novel cross-talk between NOD2-SHH signaling during gut inflammation. Potential implications of these observations in therapeutics could increase the possibility of defining and developing better regimes to treat IBD pathophysiology.

The River Lune was at one time one of the best salmon fisheries in England and Wales with very high catches to both rods and netsmen. During the 1960's, the stock was decimated by the disease Ulcerative Dermal Necrosis which caused a dramatic reduction in catches. Catches have not recovered to pre-disease levels. The target egg deposition rate for the River Lune system which should produce the maximum number of surplus fish returning to the system has been determined at 13.8 million eggs. This will require 4,779 adult salmon to survive to successfully spawn. Under current rates of exploitation, the target egg deposition will occur at a declared rod catch of 1,974 salmon and a net catch of 2,627 salmon. Two automatic fish counters are in operation on the river system. Data from these suggest that, within the measures of compliance, the river has met it's target egg deposition for the last 3 years.

A radioterapia é amplamente utilizada no tratamento de tumores pélvicos, incluindo os de bexiga, intestino e reto. Ela apresenta efeitos danosos, notadamente em tecidos que apresentam intensa renovação celular, sendo a mucosa intestinal altamente susceptível. Nesse contexto, a suplementação dietética com aminoácidos tem se mostrado uma terapia promissora para minimizar este dano. O objetivo desse estudo foi determinar o efeito da suplementação dietética com os aminoácidos L-arginina e glicina na estrutura da parede do cólon de ratos submetidos a irradiação abdominal. Quarenta ratos Wistar machos adultos foram distribuídos aleatoriamente em quatro grupos, cada um com dez animais: I controle não irradiado e sem suplementação de aminoácidos; II controle irradiado e sem suplementação de aminoácidos; III irradiado e suplementado com L-arginina; IV irradiado e suplementado com glicina. O período de suplementação dietética foi de 14 dias, com a irradiação ocorrendo no 8. dia do experimento. A análise estereológica mostrou que a irradiação provocou diminuição do volume total da parede colônica dos animais dos grupos II e III em relação aos animais saudáveis, mas não dos ratos que receberam suplementação de glicina. A camada mucosa dos animais irradiados de todos os grupos diminuiu quando comparada com os ratos saudáveis não irradiados. Os animais irradiados que não receberam suplementação de aminoácido apresentaram diminuição da camada muscular da mucosa, quando comparados com os grupos I e IV, e o grupo de ratos suplementados com glicina apresentou aumento significativo da camada submucosa em relação aos grupos I e II. Os animais do grupo III mostraram diminuição da camada muscular própria em comparação aos grupos I e IV. A suplementação com L-arginina foi eficaz na manutenção do volume parcial do epitélio da camada mucosa. Nossos resultados sugerem que a suplementação de glicina apresentou efeitos superiores ao da suplementação com L-arginina na estrutura da parede colônica, haja vista que foi capaz de manter a espessura da parede e a superfície epitelial da mucosa, enquanto a Larginina foi capaz de manter o volume parcial do epitélio e a superfície epitelial, mas não o volume total da parede intestinal.

Reconhecida como agente de doença humana em 1982, E.coli enterohemorrágica (EHEC) pode causar diarréia sanguinolenta, colite hemorrágica e síndrome hemolítica urêmica (SHU). EHEC constitui um subgrupo especialmente virulento das E.coli produtoras de toxina de Shiga (Stx). O fator crítico da sua virulência é a toxina Shiga, capaz de interromper a síntese proteica da célula eucariótica. São conhecidos dois subgrupos de Stx, Stx1 e Stx2. Stx1 possui duas variantes Stx1c e Stx1d. Stx2 possui muitas variantes. Estudos epidemiológicos sugerem que cepas com os perfis toxigênicos Stx2 ou Stx2/Stx2c seriam mais frequentemente associadas a pacientes com SHU. Além da expressão de Stx, EHEC do sorotipo O157:H7 colonizam a mucosa intestinal induzindo a formação de lesões denominadas attaching/effacing (A/E). Para a produção da lesão A/E, é necessária a presença de uma ilha de patogenicidade cromossômica denominada LEE, composta por cinco operons, LEE 1 a LEE5. Em LEE 5 são codificadas a adesina intimina e o seu receptor Tir, o qual é translocado por um sistema de secreção tipo III (SSTT) e em LEE 4 são codificadas as proteínas secretadas EspA,B e D. Em EHEC O157:H7 são descritos muitos fatores de virulência, codificados em ilhas de patogenicidade, no cromossomo e no megaplasmídio pO157. Bovinos são o principal reservatório deste patógeno e alimentos de origem bovina e produtos contaminados com fezes de bovinos são causadores de surtos epidêmicos. Em nosso país EHEC O157:H7 é isolada do reservatório animal mas é muito rara a sua ocorrência em doença humana. Notamos que nas cepas bovinas predomina Stx2c, enquanto nas cepas humanas predomina o perfil toxigenico Stx2/Stx2c. Quanto a interação com enterocitos humanos cultivados in vitro (linhagem Caco-2), verificamos que tanto cepas bovinas quanto humanas mostram idêntica capacidade de invadir e persistir no compartimento intracelular das células Caco-2. No entanto, em comparação com as cepas humanas, as cepas bovinas mostram uma reduzida capacidade de produzir lesões A/E. Empregamos qPCR para aferir a transcrição de três diferentes locus (eae, espA e tir) situados nos operons LEE4 e LEE5 de cepas bovinas e humanas, durante a infecção de células Caco-2. Verificamos diferenças na expressão dos genes, especialmente espA, entre cepas bovinas e humanas com maior expressão para estas ultimas, em linha com os achados dos testes FAS. Através de clonagem e expressão de proteínas recombinantes, purificamos as proteínas Eae, EspA e Tir e obtivemos anticorpos específicos, empregados para acompanhar a sua expressão ao longo da infecção de células Caco-2, por imunofluorescencia. Verificamos que as três proteínas são detectadas tanto em cepas bovinas quanto humanas, mas nestas ultimas, a marcação é precoce e torna-se mais intensa com o avanço da infecção. Nossos resultados indicam que cepas EHEC O157:H7 isoladas do reservatório bovino em nosso país apresentam diferenças importantes em relação ao perfil toxigenico e a capacidade de indução de lesões A/E, características apontadas na literatura como relevantes para a virulência do micro-organismo. Por outro lado, nossos achados quanto a capacidade de invadir e multiplicar-se no interior de enterócitos pode explicar a persistência do patógeno no reservatório animal e a sua capacidade de transmissão horizontal.

Marine mammals, such as dolphins, can serve as key indicator species in coastal areas by reflecting the effects of natural and anthropogenic stressors. As such they are often considered sentinels of environmental and ecosystem health (Bossart 2006; Wells et al. 2004; Fair and Becker 2000). The bottlenose dolphin is an apex predator and a key component of many estuarine environments in the southeastern United States (Woodward-Clyde Consultants 1994; SCDNR 2005). Health assessments of dolphins are especially critical in areas where populations are depleted, show signs of epidemic disease and/or high mortality and/or where habitat is being altered or impacted by human activities. Recent assessments of environmental conditions in the Indian River Lagoon, Florida (IRL) and the estuarine waters surrounding Charleston, South Carolina (CHS) highlight the need for studies of the health of local bottlenose dolphins. While the condition of southeastern estuaries was rated as fair in the National Coastal Condition Report (U.S. EPA 2001), it was noted that the IRL was characterized by poorer than expected benthic communities, significant sediment toxicity and increased nutrient concentrations. Similarly, portions of the CHS estuary have sediment concentrations of aliphatic aromatic hydrocarbons, select inorganic metals, and some persistent pesticides far in excess of reported bioeffect levels (Hyland et al. 1998). Long-term trends in water quality monitoring and recent scientific research suggest that waste load assimilation, non-point source runoff impacts, contaminated sediments, and toxic pollutants are key issues in the CHS estuary system. Several ‘hot spots’ with high levels of heavy metals and organic compounds have been identified (Van Dolah et al. 2004). High concentrations of anthropogenic trace metals, polychlorinated biphenyls (PCB’s) and pesticides have been found in the sediments of Charleston Harbor, as well as the Ashley and Cooper Rivers (Long et al. 1998). Two superfund sites are located within the CHS estuary and the key contaminants of concern associated with these sites are: polycyclic aromatic hydrocarbons (PAH), lead, chromium, copper, arsenic, zinc and dioxin. Concerns related to the overall health of IRL dolphins and dermatologic disease observed in many dolphins in the area (Bossart et al. 2003) initiated an investigation of potential factors which may have impacted dolphin health. From May-August 2001, 35 bottlenose dolphins died in the IRL during an unusual mortality event (MMC 2003). Many of these dolphins were diagnosed with a variety of skin lesions including proliferative ulcerative dermatitis due to protozoa and fungi, dolphin pox and a vesicular dermatopathy of unknown etiology (Bossart et al. 2003). Multiple species from fish to dolphins in the IRL system have exhibited skin lesions of various known and unknown etiologies (Kane et al. 2000; Bossart et al. 2003; Reif et al. 2006). On-going photo-identification (photo-ID) studies have documented skin diseases in IRL dolphins (Mazzoil et al. 2005). In addition, up to 70% of green sea turtles in the IRL exhibit fibropapillomas, with the highest rates of occurrence being seen in turtles from the southern IRL (Hirama 2001).

Investigations conducted since July 1988 on ulcerative fish epidemics in Assam, India, indicated that mainly four species of fishes belonging to the genera Puntius, Channa, Macrognathus and Mystus were widely affected by the disease. Results indicated that outbreak of the disease may not be due to organic pollution of water or radioactive and heavy-metallic contamination. Bacterial culture revealed colonies of Escherichia coli and Pseudomonas aeruginosa in the surface muscular lesions and gill tissues while preliminary electron microscopic studies indicated the presence of viruses in the muscles and gills of diseased fishes.

Health status of juvenile silver carp, Hypophthalmichthys molitrix and silver barb, Barbodes gonionotus were investigated in three fish farms following different farming conditions through clinical and histopathological examinations for a period of nine months. Here the fishes and water quality parameters were sampled on monthly basis. Among the water quality parameters, water temperature has a distinct effect on fish health observed during the winter season. Different clinical signs like scale loss, dermal lesion, fin erosion were observed, while histopathologically necrosis, pyknosis, inflammation, haemorrhage, hypertrophy, vacuoles, missing of gill lamellae and clubbing were evidenced in the investigated fishes. The study showed that pathological symptoms were mainly increased during the winter season and H. molitrix exhibited severe pathological symptoms in compare to B. gonionotus during the investigation. It was also found that fishes of BAU farm was comparatively in the best condition, while, the fishes of other farms were severely affected during the experimental observations. In addition, disease like Epizootic Ulcerative Syndrome (EUS), protozoan disease and suspected bacterial colonies were clearly evidenced in the fishes of Government and NGO fish farms.

An investigation was carried out to observe histopathological changes in liver and kidney of suspected epizootic ulcerative syndrome (EUS)-affected shing fish, Heteropneustes fossilis (Bloch) collected from the "Agro-3 fish farm" situated at Boilor, Trishal, Mymensingh. Focal necrosis, haemorrhages and atrophy of the sinusoidal region were observed in the liver tissue. Fungal granulomas were found both in liver and kidney. In some cases fatty depositions were observed in all over the hepatic tissue. Degeneration and necrosis of renal tubular epithelial cells were also occurred. Missing of glomerulus and necrosis surrounding the Bowman's capsule in the kidney tissue were found.

An experiment was carried out for a period of six months during October 2008 to March 2009 to investigate the health status of a snakehead, Channa punctatus through clinical and histopathological technique. Fish were collected from two fish markets of Mymensingh district. Clinically and histopathologically, it was observed that fishes from both the markets were healthy in October and March but moderately affected in November and February. In the months of December and January, 7.5- 8% of the fishes were affected clinically and showing various clinical signs like, discolouration, deep ulcer, ill health, scale loss and rough skin. Histopathologically, in the month of December and January, major observed pathologies of skin and muscle were necrosis, vacuums, fungal granuloma and loss of dermis. Gills were affected having parasitic cysts, monogenetic trematode, clubbing, loss of primary and secondary gill lamellae, hemorrhage, necrosis and hypertrophy. Vacuoles, pyknosis, hepatic necrosis, hemorrhages and fungal granuloma were observed in liver. Renal pathology included necrosis and pyknosis of kidney tubules, hemorrhages, presence of bacterial colony and vacuoles. From present findings, it was found that, fishes from urban market were more affected with diseases than pre-urban market especially in the months of December and January when compared with other months. From overall observation, C. punctatus were severely affected by epizootic ulcerative syndrome (EUS), dactylogyrosis, protozoan and bacterial diseases during colder months of the year.

Ethnopharmacological relevance: A common plant used to treat several gastric disorders is Buddleja scordioides Kunth,commonly known as salvilla. Aim of thes tudy: To detect inflammatory markers,in order to evaluate the gastroprotective potential of salvilla infusions,as this could have beneficial impact on the population exposed to gastric ulcers and colitis. Materials and methods: The present work attempted infusions were prepared with B. scordioides (1% w/w) lyophilized and stored.Total phenolic content and GC–MS analysis were performed. Wistar rats were divided into five groups a negative vehicle control,an indomethacin group,and three experimental groups,named preventive,curative,and suppressive. All rats were sacrificed under deep ether anesthesia(6h)after the last oral administration of indomethacin/infusion.The rat stomachs were promptly excised,weighed,and chilled in ice-cold and 0.9%NaCl.Histological analysis,nitrites quantification and immunodetection assays were done. Results: B.scordioides infusions markedly reduced the visible hemorrhagic lesions induced byindomethacin in rat stomachs,also showed down-regulation of COX2, IL-8 and TNFα and up-regulation of COX-1with a moderate down-regulation of NFkB and lower amount of nitrites.However,this behavior was dependent on the treatment,showing most down-regulation of COX-2,TNFα and IL-8 in the curative treatment;more down-regulation of NF-kB in the preventive treatment;and more up-regulation of COX-1 for the suppressor and preventive treatments. Conclusion: The anti-inflammatory potential of B. scordioides infusions could be related with the presence of polyphenols as quercetin in the infusion and how this one is consumed.

Clostridium difficile is mainly a nosocomial pathogen and is a significant cause of antibioticassociated diarrhea. It is also implicated in the majority of cases of pseudomembranous colitis. The main etiological agent of C. difficile-associated diarrhea (CDAD) is perturbations to the gut microbiota by broad-spectrum antibiotics. Recently, thuricin CD, a two-peptide narrow spectrum sactibiotic bacteriocin with potent activity against C. difficile has been discovered. It is produced by Bacillus thuringiensis DPC6431. The efficacy of thuricin CD against a range of C. difficile clinical isolates has been determined in the form of minimum inhibitory concentration (MIC) values and compared to metronidazole, vancomycin, ramoplanin and actagardine in this thesis. Furthermore, by assessing paired combinations of the above-mentioned antimicrobials, it was determined that ramoplanin and actagardine function in a synergistic manner against the majority of C. difficile isolates. The functions of the genes in the thuricin CD gene cluster have also been elucidated by cloning the cluster and expressing thuricin CD in a heterologous Bacillus subtilis host and are described herein. In addition, the immunity mechanisms employed by the B. thuringiensis DPC6431 producer to protect itself from the antimicrobial actions of thuricin CD have also been elucidated. It has been shown that a small immunity peptide, TrnI, is involved in thuricin CD immunity, most likely by intercepting the thuricin CD peptides and/or blocking their access to the thuricin CD receptor. This immunity peptide and also the ABC-transporter system TrnFG serve to protect the B. thuringiensis host against thuricin CD.
