Objectives. We compared the mental health risk to unpaid caregivers bereaved of a care recipient with the risk to persons otherwise bereaved and to nonbereaved caregivers.

Methods. We linked prescription records for antidepressant and anxiolytic drugs to characteristics and life-event data of members of the Northern Ireland Longitudinal Study (n = 317 264). Using a case-control design, we fitted logistic regression models, stratified by age, to model relative likelihood of mental health problems, using the proxy measures of mental health–related prescription.

Results. Both caregivers and bereaved individuals were estimated to be at between 20% and 50% greater risk for mental health problems than noncaregivers in similar circumstances (for bereaved working-age caregivers, odds ratio = 1.41; 95% confidence interval = 1.27, 1.56). For older people, there was no evidence of additional risk to bereaved caregivers, though there was for working-age people. Older people appeared to recover more quickly from caregiver bereavement.

Conclusions. Caregivers were at risk for mental ill health while providing care and after the death of the care recipient. Targeted caregiver support needs to extend beyond the life of the care recipient.

The introduction of parallel processing architectures allowed the real time impelemtation of more sophisticated control algorithms with tighter specifications in terms of sampling time. However, to take advantage of the processing power of these architectures the control engeneer, due to the lack of appropriate tools, must spend a considerable amount of time in the parallelizaton of the control algorithm.

This papers describes an extantion of previous works on the subject of neural network proportional, integral and derivative (PID) autotuning. Basically, neural networks are employed to supply the three PID parameters, according to the integral of time multiplied by the absolute error (ITAE) criterion, to a standard PID controller.

Evidence suggests that children with developmental coordination disorder (DCD) have lower levels of cardiorespiratory fitness (CRF) compared to children without the condition. However, these studies were restricted to field-based methods in order to predict V02 peak in the determination of CRF. Such field tests have been criticised for their ability to provide a valid prediction of V02 peak and vulnerability to psychological aspects in children with DCD, such as low perceived adequacy toward physical activity. Moreover, the contribution of physical activity to the variance in V02 peak between the two groups is unknown. The purpose of our study was to determine the mediating role of physical activity and perceived adequacy towards physical activity on V02 peak in children with significant motor impairments. This prospective case-control design involved 122 (age 12-13 years) children with significant motor impairments (n=61) and healthy matched controls (n=61) based on age, gender and school location. Participants had been previously assessed for motor proficiency and classified as a probable DCD (p-DCD) or healthy control using the movement ABC test. V02 peak was measured by a progressive exercise test on a cycle ergometer. Perceived adequacy was measured using a 7 -item subscale from Children's Selfperception of Adequacy and Predilection for Physical Activity scale. Physical activity was monitored for seven days with the Actical® accelerometer. Children with p-DCD had significantly lower V02 peak (48.76±7.2 ml/ffm/min; p:50.05) compared to controls (53.12±8.2 ml/ffm/min), even after correcting for fat free mass. Regression analysis demonstrated that perceived adequacy and physical activity were significant mediators in the relationship between p-DCD and V02 peak. In conclusion, using a stringent laboratory assessment, the results of the current study verify the findings of earlier studies, adding low CRF to the list of health consequences associated with DCD. It seems that when testing for CRF in this population, there is a need to consider the psychological barriers associated with their condition. Moreover, strategies to increase physical activity in children with DCD may result in improvement in their CRF.

Introduction: Developmental coordination disorder (DCD) is a prevalent condition characterized by poor motor proficiency that interferes with a child‟s activities of daily living. Children with DCD often experience compromised health-related fitness components such as cardiorespiratory fitness (CRF). Purpose: To better understand the physical activity and fitness characteristics of children with probable DCD (pDCD), with a particular focus on CRF. Specifically: (1) to present a synopsis of current literature; (2) to determine the longitudinal trajectories of CRF; (3) to compare the submaximal CRF of children with and without pDCD. Methods: A comprehensive, systematic literature review was conducted of the recent available data on fitness and physical activity and pDCD (Chapter 2). This review provided the background for the other two studies included in this thesis. In Chapter 3, a prospective cohort design was used to assess how CRF in children with pDCD changes over time (56 months) relative to a group of typically developing controls. Using a nested-case control design, 63 subjects with pDCD and 63 matched controls from the larger sample were recruited to participate in the lab-based component of the study (Chapter 4). In this investigation CRF was examined using the oxygen cost of work (VO2) during an incremental test on a cycle ergometer. Results: The literature review showed that fitness parameters, including CRF and physical activity levels, were consistently reduced in children with pDCD. Chapter 3 demonstrated that the difference in CRF between children with pDCD and typically developing children is substantial, and that it tends to increase over time. Results from VO2 assessments showed that children with pDCD utilized more oxygen to sustain the same submaximal workloads compared to typically developing children. Conclusions: Findings from this thesis have made several important contributions to our understanding of children with pDCD. Since differences in CRF between children with and without pDCD tend to worsen over time, this adds to the argument that interventions intended to improve CRF may be appropriate for children with motor difficulties. This thesis also presented the first evidence suggesting that DCD involves higher energy expenditure, and could help explain why children with pDCD perform poorly on tasks requiring CRF.

I present a novel design methodology for the synthesis of automatic controllers, together with a computational environment---the Control Engineer's Workbench---integrating a suite of programs that automatically analyze and design controllers for high-performance, global control of nonlinear systems. This work demonstrates that difficult control synthesis tasks can be automated, using programs that actively exploit and efficiently represent knowledge of nonlinear dynamics and phase space and effectively use the representation to guide and perform the control design. The Control Engineer's Workbench combines powerful numerical and symbolic computations with artificial intelligence reasoning techniques. As a demonstration, the Workbench automatically designed a high-quality maglev controller that outperforms a previous linear design by a factor of 20.

Objetivo: Evaluar el impacto de los factores maternos y perinatales: peso previo al embarazo, presencia de hipertensión, diabetes y tabaquismo, peso al nacer y tiempo de lactancia con el desarrollo de sobrepeso y obesidad en los niños de 5 a 10 años en una población bogotana. Materiales y métodos: se realizó un diseño de casos y controles no pareado, incluyendo 528 niños con sus respectivas madres con una relación 1:2, en quienes se identificaron los descritos, y también factores postnatales relevantes: dieta, ejercicio y tiempo de pantalla. Se analizó con un modelo de regresión logística binomial la importancia de cada uno para la patología Resultados:Se encontró una mayor prevalencia de casos en los estratos 1 y 2 comparados con los demás estratos (p= 0,034). También se encontraron diferencias en la prevalencia de sobrepeso y obesidad en las madres de los casos (27%; p =0,01), mayor ingesta calórica en el grupo de los controles (42,9% vs 30,9%; p= 0,04). En el modelo de regresión logística se encontró que los factores que en conjunto mostraron significancia estadística fueron, un peso al nacer mayor de 2500 gr OR 2,31(IC 95%:1,13-4,78 p=0,021), un IMC materno antes de la gestación > de 25 OR 3,42(IC 95%:1,42-8,29 p=0,06), peso al año mayor de 7,5 kg OR 2,77 (IC 95%:1,11-2,78 p=0,028) y una ingesta calórica entre 600 a 1900 calorías/día OR 1.58 (IC 95%:1,06-2,35 p=0,02).Conclusiones: son factores de riesgo para el desarrollo de la obesidad infantil, la progresión hacia un percentil superior en peso, antes el embarazo, nacimiento y primer año de vida.

La Tuberculosis es la segunda enfermedad infecciosa que más muertes ocasiona en adultos luego del VIH. La mayoría de casos pueden ser tratados adecuadamente con los antibióticos disponibles, sin embargo, la prevalencia de multidrogorresistencia está aumentando, generando más fallas en el tratamiento, mayores costos en el sistema de salud y mayor mortalidad. Objetivo: Determinar los factores de riesgo asociados al desarrollo de tuberculosis multidrogorresistente en pacientes de 18 años o más, afiliados a 3 Empresas Administradoras de Planes de Beneficios en Colombia, durante los años 2008 a 2011. Métodos: Se realizó un estudio de casos y controles emparejado, 1 a 4, 45 casos y 180 controles, de pacientes de la base de datos del programa de tuberculosis de 3 Empresas Administradoras de Planes de Beneficios, evaluando variables demográficas, socioeconómicas y clínicas. Resultados: La mediana de edad de los casos fue de 43 años y la de los controles de 39.5 años, en los casos predominó el sexo masculino con 73.3%, mientras que en los controles fue mayor el sexo femenino con 51.1%. Se encontró asociación estadísticamente significativa entre la tuberculosis multidrogorresistente y el sexo masculino (OR ajustado 4.47 IC 95% [1.01; 19.75]), seguridad social (OR ajustado 57.6 IC 95% [4.6; 712.8]) y tratamiento previo (OR ajustado 56.2 IC 95% [10.03; 314.79]. Conclusiones: Ser hombre y tener tratamiento previo para tuberculosis son factores de riesgo para el desarrollo de multidrogorresistencia. Es necesario realizar más estudios con el sistema de salud colombiano para profundizar en los hallazgos con respecto al régimen subsidiado y el desarrollo de Tuberculosis multidrogorresistente.

Introducción: el delirium es el trastorno neuropsiquiátrico más frecuente entre pacientes hospitalizados, se asocia con aumento en el tiempo de hospitalización, complicaciones nosocomiales e incluso se le atribuye aumento de la mortalidad de forma independiente. Existen estudios acerca de factores de riesgo para la condición, enfocados en las patologías, medicaciones y antecedentes de los pacientes; sin embargo son escasos los que buscan identificar los factores propios de los pacientes y del proceso de hospitalización, el cual es el objetivo del presente. Metodología: se realizó un estudio con diseño de casos y controles, incluyendo pacientes de la Fundación Cardioinfantil - Instituto de Cardiología con diagnóstico de delirium, hospitalizados entre marzo de 2011 y marzo de 2013. Con una muestra calculada de 201 pacientes, 67 casos y 134 controles (2:1), se equiparó los grupos para edad, piso de hospitalización, tipo de habitación hospitalaria y la fecha de hospitalización, obteniendo las variables de interés. Resultados: se encontró distintas variables relacionadas con delirium, algunas de estas clásicamente asociadas con la condición, posterior al análisis de los datos, las variables más fuertemente relacionadas con la condición fueron el tipo de ingreso, escala visual análoga del dolor, índice de Barthel y el tiempo de estancia en urgencias. Discusión: en nuestra revisión, el presente estudio es el primero de nuestro medio que busca identificar los factores de riesgo dados por la condición basal de los pacientes en su ingreso al hospital, estos factores fácilmente identificables son de utilidad para la prevención de la condición.

Introducción: El programa de Tutores Pares-TP es una iniciativa creada en la EMCS de la Universidad del Rosario que brinda acompañamiento académico a través de estudiantes-tutores a pares menos avanzados. Éste entrega a sus Tutores sistemáticamente, herramientas para desempeñarse armónicamente en el ejercicio de guía y provee habilidades para el manejo del saber. Este estudio busca explorar posibles ´impactos´ generados tras la participación de estudiantes de medicina como TPs dentro de un programa estructurado. Materiales y métodos: Estudio cualitativo que involucró la construcción y aplicación de encuestas a grupos focales –TPs, Docentes y Familiares- creadas a partir de seis ejes/categorías que enmarcan al médico ideal. Las respuestas obtenidas de preguntas cerradas –en escala valorativa- y de naturaleza abierta fueron sometidas a análisis descriptivo –modas- y triangulación. Resultados: 41 tutores, agrupados en 4 grupos de análisis, evidenciaron un impacto general positivo con predominio en habilidades interpersonales (60%,65%,66%,45%, respectivamente), funciones/actividades basadas en la práctica y mejoramiento (57%,67%,60%,45%) y la forma como se emplean los conocimientos (47%,70%,67%,48%). Ocho docentes encuestados consideraron relevante el impacto del programa en habilidades interpersonales-(49%), conocimientos-(42%) y la interacción con colegas-(38%). En los padres de familia hay consenso en el cambio en habilidades interpersonales, funciones basadas en la práctica y mejoramiento y en actitudes-valores ético/morales. Dichos resultados están en paralelo con las observaciones plasmadas en las preguntas abiertas. Conclusiones: Se evidenció un impacto general positivo en la formación y desempeño profesional tras la participación como TPs dentro del programa; hallazgo que soporta aquellos publicados de experiencias académicas similares.

This paper specifically examines the implantation of a microelectrode array into the median nerve of the left arm of a healthy male volunteer. The objective was to establish a bi-directional link between the human nervous system and a computer, via a unique interface module. This is the first time that such a device has been used with a healthy human. The aim of the study was to assess the efficacy, compatibility, and long term operability of the neural implant in allowing the subject to perceive feedback stimulation and for neural activity to be detected and processed such that the subject could interact with remote technologies. A case study demonstrating real-time control of an instrumented prosthetic hand by means of the bi-directional link is given. The implantation did not result in infection, and scanning electron microscope images of the implant post extraction have not indicated significant rejection of the implant by the body. No perceivable loss of hand sensation or motion control was experienced by the subject while the implant was in place, and further testing of the subject following the removal of the implant has not indicated any measurable long term defects. In this paper, a new model-based proportional–integral–derivative (PID) tuning and controller approach is introduced for Hammerstein systems that are identified on the basis of the observational input/output data. The nonlinear static function in the Hammerstein system is modelled using a B-spline neural network. The control signal is composed of a PID controller, together with a correction term. Both the parameters in the PID controller and the correction term are optimized on the basis of minimizing the multistep ahead prediction errors. In order to update the control signal, the multistep ahead predictions of the Hammerstein system based on B-spline neural networks and the associated Jacobian matrix are calculated using the de Boor algorithms, including both the functional and derivative recursions. Numerical examples are utilized to demonstrate the efficacy of the proposed approaches.

In this thesis, it is developed the robustness and stability analysis of a variable structure model reference adaptive controller considering the presence of disturbances and unmodeled dynamics. The controller is applied to uncertain, monovariable, linear time-invariant plants with relative degree one, and its development is based on the indirect adaptive control. In the direct approach, well known in the literature, the switching laws are designed for the controller parameters. In the indirect one, they are designed for the plant parameters and, thus, the selection of the relays upper bounds becomes more intuitive, whereas they are related to physical parameters, which present uncertainties that can be known easier, such as resistances, capacitances, inertia moments and friction coefficients. Two versions for the controller algorithm with the stability analysis are presented. The global asymptotic stability with respect to a compact set is guaranteed for both cases. Simulation results under adverse operation conditions in order to verify the theoretical results and to show the performance and robustness of the proposed controller are showed. Moreover, for practical purposes, some simplifications on the original algorithm are developed
