Mycobacterium tuberculosis strains resistant to streptomycin (SM), isoniazid (INH), and/or rifampin (RIF) as determined by the conventional Löwenstein-Jensen proportion method (LJPM) were compared with the E test, a minimum inhibitory concentration susceptibility method. Discrepant isolates were further evaluated by BACTEC and by DNA sequence analyses for mutations in genes most often associated with resistance to these drugs (rpsL, katG, inhA, and rpoB). Preliminary discordant E test results were seen in 75% of isolates resistant to SM and in 11% to INH. Discordance improved for these two drugs (63%) for SM and none for INH when isolates were re-tested but worsened for RIF (30%). Despite good agreement between phenotypic results and sequencing analyses, wild type profiles were detected on resistant strains mainly for SM and INH. It should be aware that susceptible isolates according to molecular methods might contain other mechanisms of resistance. Although reproducibility of the LJPM susceptibility method has been established, variable E test results for some M. tuberculosis isolates poses questions regarding its reproducibility particularly the impact of E test performance which may vary among laboratories despite adherence to recommended protocols. Further studies must be done to enlarge the evaluated samples and looked possible mutations outside of the hot spot sequenced gene among discrepant strains.

A report published in 2002, Monitoring the State of the East Midlands. Sustainable Development Objectives and Targets for the East Midlands. Health Indicators, proposed a set of seven high-level health indicators for monitoring health status and health inequalities in the Region. The report also proposed a number of health improvement and health inequality reduction targets drawn from key national and regional strategy documents including Saving Lives: Our Healthier Nation and The East Midlands Integrated Regional Strategy. These relate to: - Life expectancy at birth. - Teenage pregnancy rate. - Mortality rate from circulatory disease in people aged under 75. - Mortality rate from cancer in people aged under 75. - Mortality rate from accidents in people of all ages. - Suicide rate in people of all ages. - Prevalence of cigarette smoking in people aged 16 and over. Progress towards these targets will indicate that the twin aims of the regional public health strategy Investment for Health - to improve health and to reduce health inequalities - are being achieved. This report updates these indicators with the latest available data. At the time of writing, data were available for years up to and including 2003 for most indicators. Please note that the latest data are provisional at this stage.

These synthetic estimates were produced as part of a research project to test and produce area-level estimates of healthy lifestyle behaviours, which was carried out at the National Centre for Social Research. The estimates were produced in response to the twin requirements to develop small area estimates for Neighbourhood Statistics and to meet local public health information needs. Synthetic estimates with 95% Confidence Intervals (1) have been prepared using 2000-2002 data from the Health Survey for England, the 2001 Census and other information, at the 2003 Census Area Statistics (CAS) ward and Primary Care Organisation (PCO)(2) geographic level for the following variables: Prevalence of current smoking (at the time the data was collected); Obesity of adults; Binge drinking for adults; Consumption of 5 or more portions of fruit and vegetables a day (adults); Consumption of 3 or more portions of fruit and vegetables day (children).

There are no validated criteria for the diagnosis of sensory neuronopathy (SNN) yet. In a preliminary monocenter study a set of criteria relying on clinical and electrophysiological data showed good sensitivity and specificity for a diagnosis of probable SNN. The aim of this study was to test these criteria on a French multicenter study. 210 patients with sensory neuropathies from 15 francophone reference centers for neuromuscular diseases were included in the study with an expert diagnosis of non-SNN, SNN or suspected SNN according to the investigations performed in these centers. Diagnosis was obtained independently from the set of criteria to be tested. The expert diagnosis was taken as the reference against which the proposed SNN criteria were tested. The set relied on clinical and electrophysiological data easily obtainable with routine investigations. 9/61 (16.4 %) of non-SNN patients, 23/36 (63.9 %) of suspected SNN, and 102/113 (90.3 %) of SNN patients according to the expert diagnosis were classified as SNN by the criteria. The SNN criteria tested against the expert diagnosis in the SNN and non-SNN groups had 90.3 % (102/113) sensitivity, 85.2 % (52/61) specificity, 91.9 % (102/111) positive predictive value, and 82.5 % (52/63) negative predictive value. Discordance between the expert diagnosis and the SNN criteria occurred in 20 cases. After analysis of these cases, 11 could be reallocated to a correct diagnosis in accordance with the SNN criteria. The proposed criteria may be useful for the diagnosis of probable SNN in patients with sensory neuropathy. They can be reached with simple clinical and paraclinical investigations.

Discordances exist in epidemiological studies regarding the association between the intake of nutrients and death and disease. We evaluated the social and health profile of persons who consumed olive oil in a prospective population cohort investigation (Pizarra study) with a 6-year follow-up. A food frequency questionnaire and a 7 d quantitative questionnaire were administered to 538 persons. The type of oil used in food preparation was determined by direct measurement of the fatty acids in samples obtained from the kitchens of the participants at baseline and after follow-up for 6 years. The fatty acid composition of the serum phospholipids was used as an endogenous marker of the type of oil consumed. Total fat intake accounted for a mean 40 % of the energy (at baseline and after follow-up). The concordance in intake of MUFA over the study period was high. The fatty acid composition of the serum phospholipids was significantly associated with the type of oil consumed and with fish intake. The concentration of polar compounds and polymers, indicative of degradation, was greater in oils from the kitchens where sunflower oil or refined olive oil was used, in oils used for deep frying and in oils that had been reused for frying five times or more. Consumption of olive oil was directly associated with educational level. Part of the discordance found in epidemiological studies between diet and health may be due to the handling of oils during food preparation. The intake of olive oil is associated with other healthy habits.

Although it has long been known that genetic factors play a major role in shaping the electroencephalogram (EEG), progress on identifying the underlying genes has, until recently, been limited. Using quantitative trait loci (QTL) analyses several genomic loci affecting the sleep EEG could be mapped in the mouse. For three of these QTLs the responsible genes were identified leading to the implication of novel signaling pathways affecting EEG traits. Moreover, the insight that in the mouse the sleep-wake dependent dynamics in the expression of EEG slow waves during sleep is under strong genetic control has paved the way for candidate gene studies in humans investigating the contribution of specific polymorphism to the trait-like inter-individual differences in the susceptibility to sleep loss. Candidate gene studies in the mouse were also instrumental in establishing an alternative, noncircadian function for clock genes in the homeostatic regulation of sleep and modulating rhythmic EEG activity of thalamocortical origin. Future efforts should combine system genetics approaches in the mouse and genome-wide association studies in humans to facilitate uncovering the molecular pathways that shape brain activity.

The main result of this work is a parametric description of the spectral surfaces of a class of periodic 5-diagonal matrices, related to the strong moment problem. This class is a self-adjoint twin of the class of CMV matrices. Jointly they form the simplest possible classes of 5-diagonal matrices.

La Malformació de Chiari tipus I (MCI) ha estat definida tradicionalment com la herniació de les amígdales cerebel•loses d’almenys 5mm, a través del forat mange. En general, els símptomes es posen de manifest durant la segona o tercera dècada de vida, tot i que s’han descrit casos pediàtrics. Donada la complexitat del quadre clínic, per realitzar un diagnòstic adient es requereix avaluació clínica i estudi de neuroimatge. La tècnica de preferència és la ressonància magnètica d’imatge, considerant-se actualment com a pacients de MCI aquells que presenten un descens de les amígdales superior a 3mm per sota del forat magne. L'existència de casos asimptomàtics dificulta establir una prevalença concreta, però s’ha estimat que podria estar entre 1/1000 a 1/5000 sent major en dones que en homes (2:1 aproximadament). Fins el moment, es desconeix l’etiologia de la malaltia però la hipòtesi més acceptada és que MCI és deguda al desenvolupament insuficient del mesoderm paraxial. Diferents estudis realitzats fins el moment evidencien que almenys, un subgrup de pacients amb MCI són deguts a contribució genètica: 1) casos d’agregació familiar amb afectes en tres generacions; 2) estudis de bessons 3) associació amb síndromes genètics coneguts amb herència mendeliana produïts per anomalies óssies que donen suport a la hipòtesi de la insuficiència del mesoderm com a causa de MCI. Davant l’evidència clara d’un component genètic com a principal causant de l’etiologia de MCI, l’objectiu del projecte va ser la identificació de les bases genètiques de la MCI, tant en gens responsables de les formes mendelianes com en gens responsables de les formes complexes de MCI mitjançant dues estratègies: 1-Identificació de variants genètiques de susceptibilitat en pacients amb MCI mitjançant estudis d’associació de tipus cas-control. 2-Anàlisi genètic de formes monogèniques mitjançant l’anàlisi de lligament a marcardors polimòrfics i la seqüenciació del DNA a gran escala.

One third of the population is affected by a sleep disorder with a major social, medical, and economic impact. Although very little is known about the genetics of normal sleep, familial and twin studies indicate an important influence of genetic factors. Most sleep disorders run in families and in several of them the contribution of genetic factors is increasingly recognised. With recent advances in the genetics of narcolepsy and the role of the hypocretin/orexin system, the possibility that other gene defects may contribute to the pathophysiology of major sleep disorders is worth indepth investigation.

Most forms of myopathy may involve the respiratory muscles and progress to respiratory failure. However, the diagnosis of myopathy is seldom considered in an adult patient with no history of muscle disease and presenting with respiratory failure. Nemaline myopathy (NM) is a rare disorder characterized by symmetrical diffuse muscle weakness and rod-like nemaline bodies in muscle fibers. Respiratory muscle involvement is a major determinant of mortality in congenital NM, but is rare in late onset NM. Here, we report that acute or chronic respiratory failure may be caused by NM in subjects with no known history of muscle disease. Adult-onset NM was diagnosed in a 67-year-old woman with chronic respiratory insufficiency. Late onset childhood NM was revealed by respiratory failure in twin sisters aged 31. The diagnosis was established by muscle biopsy and electron microscopy (and mutations in the nebulin gene in the two sisters). Long-term clinical improvement was obtained with non-invasive ventilation (NIV) in the three patients. In conclusion, respiratory failure in an adult patient with no known history may correspond to NM with diaphragm involvement. Long-term outcome may be favorable with NIV.

The Voxel Imaging PET (VIP) Path nder project got the 4 year European Research Council FP7 grant in 2010 to prove the feasibility of using CdTe detectors in a novel conceptual design of PET scanner. The work presented in this thesis is a part of the VIP project and consists of, on the one hand, the characterization of a CdTe detector in terms of energy resolution and coincidence time resolution and, on the other hand, the simulation of the setup with the single detector in order to extend the results to the full PET scanner. An energy resolution of 0.98% at 511 keV with a bias voltage of 1000 V/mm has been measured at low temperature T=-8 ºC. The coincidence time distribution of two twin detectors has been found to be as low as 6 ns FWHM for events with energies above 500 keV under the same temperature and bias conditions. The measured energy and time resolution values are compatible with similar ndings available in the literature and prove the excellent potential of CdTe for PET applications. This results have been presented in form of a poster contribution at the IEEE NSS/MIC & RTSD 2011 conference in October 2011 in Valencia and at the iWoRID 2012 conference in July 2012 in Coimbra, Portugal. They have been also submitted for publication to "Journal of Instrumentation (JINST)" in September 2012.

INTRODUCTION: Infertility treatments are a major source of the increase in multiple pregnancies (MPs). AIMS: The aims of the present study were (1.) to investigate the origin and maternal/neonatal outcomes of MP and (2.) to review the different measures that can be adopted to reduce these serious complications. METHODS: The study included all women with multiple births between 1 January 1995 and 31 December 2006 at the University Hospital of Bern, Switzerland. The outcomes associated with the various origins of MP (natural conception, ovarian stimulation [OS] ‒ in-vitro fertilisation [IVF-ICSI]) were analysed using a multinomial logistic regression model. An analysis of the Swiss law on reproductive medicine and its current proposed revision, as well as a literature review using Pubmed, was carried out. RESULTS: A total of 592 MP were registered, 91% (n = 537) resulted in live births. There was significantly more neonatal/maternal morbidity in MP after OS compared with natural conception and even with the IVF-ICSI group. With a policy of elective single embryo transfer (eSET), twin rates after IVF-ICSI can be reduced to <5% and triplets to <1%. CONCLUSIONS: After OS, more triplets are found and the outcome of MP is worse. MP is known to be associated with morbidity, mortality, and economic and social risks. To counteract these complications (1.) better training for physicians performing OS should be encouraged and (2.) the Swiss law on reproductive medicine needs to be changed, with the introduction of eSET policies. This would lead to a dramatic decrease in neonatal and maternal morbidity/mortality as well as significant cost reductions for the Swiss healthcare system.

This study analyses the stratigraphy, structure and kinematics of the northern part of the Adula nappe of the Central Alps. The Adula nappe is one of the highest basement nappes in the Lower Penninic nappe stack of the Lepontine Dome. This structural position makes possible the investigation of the transition between the Helvetic and North Penninic paleogeographic domains. The Adula nappe is principally composed of crystalline basement rocks. The investigation of the pre-Triassic basement shows that it contains several Palaeozoic detrital metasedimentary formations dated from the Cambrian to the Ordovician. These formations contain also some volcanic or intrusive magmatic rocks. Ordovician metagranites dated at ~450 Ma are also a common rock-type of the Adula basement. These formations underwent Alpine and Variscan deformation and metamorphism. Permian granites (Zervreila orthogneiss, dated at ~290 Ma) have intruded this pre-structured basement in a post-orogenic geodynamic context. Due to their age, the Zervreila orthogneiss are good markers for alpine deformation. The stratigraphy of the Mesozoic and Paleogene sedimentary cover of the Adula nappe is essential to unraveling its pre- orogenic history. The autochthonous cover is assigned to a North Penninic Triassic series that testifies for a transition between the Helvetic and Briançonnais Triassic domains. The Adula domain goes through an emersion during the Middle Jurassic, and is part of a topographic high during the first phase of the Alpine rift. The sediments of the late Middle Jurassic show a drowning phase associated with a tectonic activity and a breccia formation. In the neighbouring domains, coeval with the drowning phase in the Adula domain, a strong extensional crustal delamination and a scattered magmatic activity is associated with the main opening of the North Penninic domain. The Upper Jurassic of the Adula nappe is characterized by a carbonate formation comparable with those in the Helvetic or Subbriaçonnais domains. Flysch s.l. deposition starts probably at the end of the Cretaceous. These sediments are deposited on a large unconformity testifying for a Cretaceous sedimentary gap. The Adula nappe exhibits a very complex structure. This structure is formed by several deformation phases. Two ductile deformations are responsible for the nappe emplacement. The first deformation phase is associated with a folding compatible with a top-to-south movement at the top of the nappe. The second phase is dominant and pervasive throughout the whole nappe. It goes with a strong north vergent folding and the main nappe emplacement. These two phases cause the exhumation and emplacement of a coherent, although pre-structured, piece of continental crust. Two further deformation phases postdate the nappe emplacement. - Ce travail concerne l'étude géologique de la partie nord de la nappe de l'Adula dans les Alpes centrales. La nappe de l'Adula est l'une des nappes cristallines la plus élevée dans la pile des nappes du Pennique inférieur des Alpes lepontines. Cette position particulière permet d'étudier la transition entre les nappes des domaines helvétique et pennique inférieur. La nappe de l'Adula est principalement composée de socle cristallin : l'étude de l'histoire géologique du socle est donc l'un des thèmes de cette recherche. Ce socle contient plusieurs formations métasédimentaires paléozoïques du Cambrien à I'Ordovicien. Ces métasédiments sont issus de formations clastiques comprenant souvent des roches magmatiques volcaniques et intrusives. Ces métasédiments ont subi les cycles orogéniques varisque et alpin. La nappe de l'Adula contient plusieurs corps magmatiques granitiques métamorphisés. Les premiers métagranites sont Ordovicien et témoignent d'un environnement de marge active. Ces granites sont aussi polymétamorphiques. Les deuxièmes métagranites sont représentés par les orthogneiss de type Zervreila. Ce métagranite est d'âge permien (-290 Ma). Il est mis en place dans un contexte tectonique post-orogénique. Ce granite est un maqueur de la déformation alpine car il n'est pas affecté par les orogenèses précédentes, flippy Le contenu stratigraphique des roches mésozoïques et cénozoiques de la couverture sédimentaire de la nappe de l'Adula est'important pour en étudier son histoire pré-alpine. La couverture autochtone est composée d'une série d'âge triasique d'affinité nord-pennique, un faciès qui marque la transition entre les domaines helvétiques et briançonnais au Trias. Le domaine paléogéographique représenté dans la nappe de l'Adula connaît une émersion pendant le Jurassique moyen. Cette émersion marque le commencement du rift dans le domaine alpin. La sédimentation de la fin du Jurassique moyen est marquée par une transgression marine accompagnée par des mouvements tectoniques et la formation d'une brèche. Cette transgression est contemporaine des importants mouvements tectoniques et des manifestations magmatiques dans les unités voisines qui marquent la phase principale d'ouverture du bassin nord-pennique. Le Jurassique supérieur est caractérisé par l'instauration d'une sédimentation carbonatée comparable à celle du domaine helvétique ou subbriançonnais. Une sédimentation flyschoïde, probablement du Crétacé à Tertiaire, est déposée sur une importante discordance qui témoigne d'une lacune au Crétacé. La structure complexe de la nappe de l'Adula témoigne de nombreuses phases de déformation. Ces phases de déformation sont en partie issues de la mise en place de la nappe et de déformations plus tardives. La mise en place de la nappe produit deux phases de déformation ductile : la première produit un plissement compatible avec un cisaillement top-vers-le sud dans la partie supérieure de la nappe; la deuxième produit un intense plissement qui accompagne la mise en place de la nappe vers le nord. Ces deux phases de déformation témoignent d'un mécanisme d'exhumation par déformation ductile d'un bloc cohérent.

Introduction: Pulmonary fat embolism (PFE) can be a cause of death in cases with trauma, during orthopedic surgery and also in non-traumatic conditions, such as burns, pancreatitis, fatty liver or sickle cell disease. As PMA becomes more widespread, it is important to determine how it affects the diagnosis of PFE. Aims: The aim of this study was to determine if the oily contrast liquid used in PMA induces artefactual PFE, if such artefacts differ from original PFE and if PFE can be detected and graded before PMA. Material and methods: Cases of adults without signs of postmortem change and for which an autopsy with angiography was performed were selected for this study. Pulmonary biopsies of each lung were taken before and after the angiography as were fragments of each lung with a twin-edged knife during the autopsy. The samples were examined under the microscope without fixation or staining and after an Oil-Red O staining. PFE was graded according to Falci et al. Results: Non-artefactual (original) PFE was diagnosed in 4 cases on pre-PMA biopsies. As expected, structures with the aspect of PFE were present in all cases after angiography. The microscopical aspect of original and PMA induced PFE was identical. Grading of the PFE according to Falci et al. was depending on the quality of the biopsies. Conclusions: PMA with oily contrast induces artefactual PFE that cannot be visually differentiated from original PFE. Original PFE can however be diagnosed with pre-angiography biopsies. In order to assure the diagnosis and correct grading of PFE, the quality of the biopsy should be checked before PMA with oily contrast.

Children occupy centre-stage in any new welfare equilibrium. Failure to support families may produce either of two undesirable scenarios. We shall see a society without children if motherhood remains incompatible with work. A new family policy needs to recognize that children are a collective asset and that the cost of having children is rising. The double challenge is to eliminate the constraints on having children in the first place, and to ensure that the children we have are ensured optimal opportunities. The simple reason why a new social contract is called for is that fertility and child quality combine both private utility and societal gains. And like no other epoch in the past, the societal gains are mounting all-the-while that families’ ability to produce these social gains is weakening.In the following 1 analyze the twin challenges of fertility and child development. I then examine which kind of policy mix will ensure both the socially desired level of fertility and investment in our children? The task is to identify a Paretian optimum that will maximize efficiency gains and social equity simultaneously.
