FUNDAMENTO: Em Síndrome Coronariana Aguda (SCA) sem Supradesnivelamento do segmento ST (SST) é importante estimar a probabilidade de eventos adversos. Para esse fim, as diretrizes recomendam modelos de estratificação de risco. O escore de risco Dante Pazzanese (escore DANTE) é um modelo simples de estratificação de risco, composto das variáveis: aumento da idade (0 a 9 pontos); antecedente de diabete melito (2 pontos) ou acidente vascular encefálico (4 pontos); não uso de inibidor da enzima conversora da angiotensina (1 ponto); elevação da creatinina (0 a 10 pontos); combinação de elevação da troponina e depressão do segmento ST (0 a 4 pontos). OBJETIVO: Validar o escore DANTE em pacientes com SCA sem SST. MÉTODOS: Estudo prospectivo, observacional, com inclusão de 457 pacientes, de setembro de 2009 a outubro de 2010. Os pacientes foram agrupados em: muito baixo, baixo, intermediário e alto risco de acordo com a pontuação do modelo original. A habilidade preditiva do escore foi avaliada pela estatística-C. RESULTADOS: Foram 291 (63,7%) homens e a média da idade 62,1 anos (11,04). Dezessete pacientes (3,7%) apresentaram o evento de morte ou (re)infarto em 30 dias. Ocorreu aumento progressivo na proporção do evento, com aumento da pontuação: muito baixo risco = 0,0%; baixo risco = 3,9%; risco intermediário = 10,9%; alto risco = 60,0%; p < 0,0001. A estatística-C foi de 0,87 (IC 95% 0,81-0,94; p < 0,0001). CONCLUSÃO: O escore DANTE apresentou excelente habilidade preditiva para ocorrência dos eventos específicos e pode ser incorporado na avaliação prognóstica de pacientes com SCA sem SST.

Fundamento: Os modelos prognósticos disponíveis para Síndrome Coronariana Aguda (SCA) podem ter limitações de performance, por terem sido elaborados há vários anos, ou problemas de aplicabilidade. Objetivos: Elaborar escores para predição de eventos desfavoráveis em 30 dias e 6 meses, em pacientes com SCA, com ou sem Supradesnivelamento de ST (SST), atendida em hospital privado terciário. Métodos: Coorte prospectiva de pacientes consecutivos com SCA admitidos entre agosto/2009 a junho/2012. O desfecho primário composto foi a ocorrência de óbito, infarto ou reinfarto, Acidente Vascular Cerebral (AVC), parada cardiorrespiratória e sangramento maior. As variáveis preditoras foram selecionadas de dados clínicos, laboratoriais, eletrocardiográficos e da terapêutica. O modelo final foi obtido por meio de regressão logística e submetido a validação interna, utilizando-se bootstraping. Resultados: Incluímos 760 pacientes, 132 com SCA com SST e 628 sem SST. A idade média foi 63,2 ± 11,7 anos, sendo 583 homens (76,7%). O modelo final para eventos em 30 dias contém cinco preditores: idade ≥ 70 anos, antecedente de neoplasia, Fração de Ejeção do Ventrículo Esquerdo (FEVE) < 40%, troponinaI > 12,4 ng/mL e trombólise. Na validação interna, o modelo mostrou ter boa performance com área sob a curva de 0,71.Os preditores do modelo para 6 meses são: antecedente de neoplasia, FEVE < 40%, trombólise, troponina I > 14,3 ng/mL, creatinina > 1,2 mg/dL, antecedente de doença pulmonar obstrutiva crônica e hemoglobina < 13,5 g/dL. Na validação interna, o modelo apresentou boa performance com área sob a curva de 0,69. Conclusões: Desenvolvemos escores de fácil utilização e boa performance para predição de eventos adversos em 30 dias e 6 meses em pacientes com SCA.

S’han utilitzat uns proxies nous per a estimar les temperatures superficials de l’aigua i de l’aire dels darrers 2000 anys de la zona de l’estret del Skagerrak per tal d’estudiar l’evolució del clima en aquesta zona i comparar-la amb la tendència del pal d’hoquei observada en diversos registres de l’hemisferi Nord. Els proxies utilitzats estan basats el els GDGTS i són el TEX86, que permet estimar la SST, el MAAT que permet estimar la temperatura mitjana anual de l’aire, el BIT que permet estimar la proporció de material terrestre en sediments marins o lacustres i el CBT que permet estimar el pH del sòl. Aquestes estimacions s’han comparat amb altres registres de la mateixa zona d’estudi. S’observa que el senyal de temperatura estimat pel TEX86 prové d’una zona en la columna d’aigua situada entre la zona fòtica i el fons marí. L’estimació del MAAT permet observar un òptim climàtic medieval, una petita edat del gel i un increment accentuat de les temperatures en els darrers 200 anys en la zona d’estudi. D’altra banda sembla que el MAAT estima la temperatura anual de l’aire a nivell regional, no local. L’estimació del BIT permet observar que la major part del testimoni sedimentari analitzat prové de material autòcton. S’observen dos pics al voltant de 445 DC i 1462 DC que podrien indicar episodis de pluges torrencials i/o canvis en la dinàmica de les corrents marines al Skagerrak. Finalment el pH estimat mitjançant el CBT sembla ser no representatiu dels sòls de Noruega, per tant aquesta forma d’estimar el pH podria ser no vàlida per als sediments marins.

En aquest projecte s’ha estudiat la relació entre els canvis en les temperatures superficials de l’Oceà Atlàntic i els canvis en la circulació atmosfèrica en el segle XX. Concretament s’han analitzat dos períodes de estudi: el primer des del 1940 al 1960 i el segon des del 1980 fins al 2000. S’ha posat especial interès en les anomalies en les temperatures superficials del mar en la regió tropical de l’Oceà Atlàntic i la possible interconnexió amb els canvis climàtics observats i predits. Per a la realització de l’estudi s’han dut a terme una sèrie d’experiments utilitzant el model climàtic elaborat a la universitat d’UCLA (UCLA‐AGCM model). Els resultats obtinguts han estat analitzats en forma de mapes i figures per a cada variable d’estudi. També s’ha fet una comparació entre els resultats obtinguts i altres trobats en altres treballs publicats sobre el mateix tema de recerca. Els resultats obtinguts són molt amplis i poden tenir diverses interpretacions. Tot i així algunes de les conclusions a les quals s’ha arribat són: les diferències més significatives per a les variables estudiades i trobades a partir dels resultats obtinguts del model per als dos períodes d’estudi són en els mesos d’hivern i a la zona dels tròpics; concretament a parts del nord de sud Amèrica i a parts del nord d’Àfrica. S’han trobat també canvis significatius en els patrons de precipitació sobre aquestes mateixes zones. També s’ha observant un moviment cap al nord de la zona d’interconvergència tropical i pot ser degut a l’anòmal gradient trobat a la zona equatorial en les temperatures superficial de l’Oceà. Tot i així per a una definitiva discussió i conclusions sobre els resultats dels experiments, seria necessari un estudi més ampli i profund.

OBJECTIVE: The hyperglycemic hyperinsulinemic clamp technique using intraduodenally infused glucose is an attractive tool for studying postprandial glucose metabolism under strictly controlled conditions. Because it requires the use of somatostatin (SST), we examined, in this study, the effect of SST on intestinal glucose absorption. CONTEXT: Twenty-six normal volunteers were given a constant 3-h intraduodenal infusion of glucose (6 mg.kg(-1).min(-1)) labeled with [2-(3)H]glucose for glucose absorption measurement. During glucose infusion, 19 subjects received iv SST at doses of 10-100 ng.kg(-1).min(-1) plus insulin and glucagon, and seven subjects were studied under control conditions. In the controls, glucose was absorbed at a rate that, after a 20-min lag period, equaled the infusion rate. RESULTS: With all the doses of SST tested, absorption was considerably delayed but equaled the rate of infusion after 3 h. At that time, only 5 +/- 2% of the total amount of infused glucose was unabsorbed in the control subjects vs. 36 +/- 2% (P < 0.001) in the SST-infused subjects. In the latter, the intraluminal residue was almost totally absorbed within 40 min of the cessation of SST infusion. At the lowest dose of SST tested (10 ng.kg(-1).min(-1)), suppression of insulin secretion was incomplete. CONCLUSION: These properties of SST hamper the use of intraduodenal hyperglycemic hyperinsulinemic clamps as a tool for exploring postprandial glucose metabolism.

Existeixen tres tipus d'infeccions greus causades per Streptococcus del grup A (GAS): la fascitis necrotitzant (FN), la miositis i la síndrome de xoc tòxic estreptocòccica (SST). Es tracta d'un estudi retrospectiu en el qual es van revisar les històries clíniques de cinc casos de FN per Streptococcus pyogenes. Es descriuen dades epidemiològiques, característiques clíniques del pacient i de la infecció, paràmetres de laboratori, mètodes diagnòstics, tractaments rebuts i curs evolutiu dels pacients. Es va estimar la taxa de mortalitat de la cohort de pacients. El retard en el diagnòstic i tractament d'aquest tipus d'infeccions són les causes principals de mortalitat.

Tropical cyclones are affected by a large number of climatic factors, which translates into complex patterns of occurrence. The variability of annual metrics of tropical-cyclone activity has been intensively studied, in particular since the sudden activation of the North Atlantic in the mid 1990’s. We provide first a swift overview on previous work by diverse authors about these annual metrics for the North-Atlantic basin, where the natural variability of the phenomenon, the existence of trends, the drawbacks of the records, and the influence of global warming have been the subject of interesting debates. Next, we present an alternative approach that does not focus on seasonal features but on the characteristics of single events [Corral et al., Nature Phys. 6, 693 (2010)]. It is argued that the individual-storm power dissipation index (PDI) constitutes a natural way to describe each event, and further, that the PDI statistics yields a robust law for the occurrence of tropical cyclones in terms of a power law. In this context, methods of fitting these distributions are discussed. As an important extension to this work we introduce a distribution function that models the whole range of the PDI density (excluding incompleteness effects at the smallest values), the gamma distribution, consisting in a powerlaw with an exponential decay at the tail. The characteristic scale of this decay, represented by the cutoff parameter, provides very valuable information on the finiteness size of the basin, via the largest values of the PDIs that the basin can sustain. We use the gamma fit to evaluate the influence of sea surface temperature (SST) on the occurrence of extreme PDI values, for which we find an increase around 50 % in the values of these basin-wide events for a 0.49 C SST average difference. Similar findings are observed for the effects of the positive phase of the Atlantic multidecadal oscillation and the number of hurricanes in a season on the PDI distribution. In the case of the El Niño Southern oscillation (ENSO), positive and negative values of the multivariate ENSO index do not have a significant effect on the PDI distribution; however, when only extreme values of the index are used, it is found that the presence of El Niño decreases the PDI of the most extreme hurricanes.

Named entity recognizers are unable to distinguish if a term is a general concept as "scientist" or an individual as "Einstein". In this paper we explore the possibility to reach this goal combining two basic approaches: (i) Super Sense Tagging (SST) and (ii) YAGO. Thanks to these two powerful tools we could automatically create a corpus set in order to train the SuperSense Tagger. The general F1 is over 76% and the model is publicly available.

The quantitative estimation of Sea Surface Temperatures from fossils assemblages is afundamental issue in palaeoclimatic and paleooceanographic investigations. TheModern Analogue Technique, a widely adopted method based on direct comparison offossil assemblages with modern coretop samples, was revised with the aim ofconforming it to compositional data analysis. The new CODAMAT method wasdeveloped by adopting the Aitchison metric as distance measure. Modern coretopdatasets are characterised by a large amount of zeros. The zero replacement was carriedout by adopting a Bayesian approach to the zero replacement, based on a posteriorestimation of the parameter of the multinomial distribution. The number of modernanalogues from which reconstructing the SST was determined by means of a multipleapproach by considering the Proxies correlation matrix, Standardized Residual Sum ofSquares and Mean Squared Distance. Silver Code (SilC) was originally discovered in [1–4] for 2×2 multiple-input multiple-output (MIMO) transmission. It has non-vanishing minimum determinant 1/7, slightly lower than Golden code, but is fast-decodable, i.e., it allows reduced-complexity maximum likelihood decoding [5–7]. In this paper, we present a multidimensional trellis-coded modulation scheme for MIMO systems [11] based on set partitioning of the Silver Code, named Silver Space-Time Trellis Coded Modulation (SST-TCM). This lattice set partitioning is designed specifically to increase the minimum determinant. The branches of the outer trellis code are labeled with these partitions. Viterbi algorithm is applied for trellis decoding, while the branch metrics are computed by using a sphere-decoding algorithm. It is shown that the proposed SST-TCM performs very closely to the Golden Space-Time Trellis Coded Modulation (GST-TCM) scheme, yetwith a much reduced decoding complexity thanks to its fast-decoding property.

Prepreg-materiaalit ovat olleet käytössä veneiden ja lentokoneiden valmistuksessa vuosia. Vasta muutamia vuosia sitten prepreg-materiaalia alettiin soveltaa apuvälinealaan. Koska prepreg-ortoosien valmistaminen on vain yksi alaluokka ortoosien valmistamisessa, opetusmateriaalia opiskelijoille ei ole paljon tarjolla. Tulevaisuudessa prepreg-materiaalien käyttö ortoosien valmistuksessa tulee pysymään samana tai sen käyttö tulee jopa kasvamaan. Siksi tietoutta prepreg-aineiden käytöstä ja koko valmistusprosessin kulusta oli tärkeää saada apuvälinetekniikan opiskelijoille. Päämääräni oli tehdä keräämistäni tiedoista prepreg-materiaalien käyttöopas. Ennen oppaan valmistamista, keräsin materiaalia oppaaseen erilaisista lähteistä. Haastattelin prepreg- ortoosien asiantuntijaa, luin prepreg-materiaalien valmistajien komposiittitietoja ja muita prepreg- tuotteista olevia teknisiä tietoja. Materiaalin halusin koota helppoon ja kompaktiin tilaan, joten ideana oli tuottaa CD-ROM, jolta tieto olisi helposti poimittavissa. Jotta CD-ROM täyttäisi vaatimustasoni, opiskelin myös oppaan suunnitteluun liittyvää kirjallisuutta. Prepreg-materiaalien käyttöoppaasta tuli loppujen lopuksi 33 sivua pitkä. Opas koostuu 7 pääotsikosta aina materiaaleista prepreg-ortoosin valmistusprosessiin. Pääotsikoiden alla on monia alaotsikoita, jotka kertovat ominaisuuksia ja teknisiä tietoja prepreg-materiaaleista. Opas valmistettiin PowerPoint- ohjelmalla, mikä teki siitä helppokäyttöisen. Jokaiseen osioon pääsee hyperlinkkien kautta painamalla haluamaansa otsikkoa sisällysluettelossa. Moniin kappaleisiin on lisätty kuvia, jotka tuovat lisää informaatiota tekstiin. Oppaan ulkoasu ja värimaailma on myös suunniteltu aiheen mukaan. Oppaasta tulevat hyötymään monet opiskelijat ja jopa työssä olevat, jotka haluavat tietoja prepreg- materiaaleista. Tiedon kokoaminen yhteen helpottaa tiedon hakemista ja säästää paljon aikaa. CD-ROM-opas säilyy myös paremmin kuin paperinen.

BACKGROUND: Anatomical total shoulder arthroplasty (TSA) for glenohumeral osteoarthritis (OA) and severe posterior glenoid wear may entail early postoperative complications (recurrence of posterior subluxation, glenoid loosening). To avoid these mechanical problems, reverse shoulder arthroplasty (RSA) has recently been proposed, mainly for its intrinsic stability. Our purpose was to present the results of TSA and RSA in glenohumeral OA with posterior glenoid wear of at least 20°. HYPOTHESIS: By virtue of its constrained design, RSA could prevent recurrence of posterior subluxation and limit the occurrence of mechanical complications. MATERIALS AND METHODS: A consecutive series of 23 patients (27 shoulders) were treated for glenohumeral OA with total shoulder prostheses: 19 TSAs and 8 RSAs. Mean age was 70years (range, 47-85years), mean retroversion angle 28° (20°-50°) and mean subluxation index 74% (57-89%). Constant Score, Subjective Shoulder Value (SSV), QuickDASH and Simple Shoulder Test (SST) were measured, and radiological examinations were performed at a mean follow-up of 52months (24-95months). RESULTS: TSA and RSA patients respectively displayed Constant Scores of 65 and 65, SSV of 79% and 74%, QuickDASH of 16 and 27, and SST of 88 and 78. Two patients underwent surgical revision of TSA because of glenoid loosening; 52% of TSA patients presented complete radiolucent lines and 11% recurrence of posterior subluxation. CONCLUSION: Complications are frequently observed after shoulder arthroplasty for OA with severe glenoid retroversion. RSA could be an alternative to TSA for selected patients, independently of rotator cuff status. Studies on RSA in this specific indication with longer follow-up are now needed. Variation in temperature affects the biology of sea turtles at a range of scales. To elucidate the drivers of seasonality of nesting and duration of season, databases across four species of sea turtles (Caretta caretta n=37, Chelonia mydas n=64, Dermochelys coriacea n=44 and Eretmochelys imbricata n=36) at a global scale were created. By using remotely sensed sea surface temperature data, thermal profiles across the nesting season were generated. Duration of nesting season was correlated with latitude in all species but was more tightly coupled with temperature; seasons were significantly longer with increased mean SST. In general, nesting seasonality occurred at warmest time of the year. SST for the month before, month after and the month of peak nesting significantly affected the month of peak nesting.

The late Early Triassic sedimentary-facies evolution and carbonate carbon-isotope marine record (delta(13)C(carb)) of ammonoid-rich, outer platform settings show striking similarities between the South ChinaBlock (SCB) and the widely distant Northern Indian Margin (NIM). The studied sections are located within the Triassic Tethys Himalayan belt (Losar section, Himachal Pradesh, India) and the Nanpanjiang Basin in the South China Block (Jinya section, Guangxi Province), respectively. Carbon isotopes from the studied sections confirm the previously observed carbon cycle perturbations at a time of major paleoceanographic changes in the wake of the end-Permian biotic crisis. This study documents the coincidence between a sharp increase in the carbon isotope composition and the worldwide ammonoid evolutionary turnover (extinction followed by a radiation) occurring around the Smithian-Spathian boundary. Based on recent modeling studies on ammonoid paleobiogeography and taxonomic diversity, we demonstrate that the late Early Triassic (Smithian and Spathian) was a time of a major climate change. More precisely, the end Smithian climate can be characterized by a warm and equable climate underlined by a flat, pole-to-equator, sea surface temperature (SST) gradient, while the steep Spathian SST gradient suggests latitudinally differentiated climatic conditions. Moreover, sedimentary evidence suggests a transition from a humid and hot climate during the Smithian to a dryer climate from the Spathian onwards. By analogy with comparable carbon isotope perturbations in the Late Devonian, Jurassic and Cretaceous we propose that high atmospheric CO(2) levels could have been responsible for the observed carbon cycle disturbance at the Smithian-Spathian boundary. We suggest that the end Smithian ammonoid extinction has been essentially caused by a warm and equable climate related to an increased CO(2) flux possibly originating from a short eruptive event of the Siberian igneous province. This increase in atmospheric CO(2) concentrations could have additionally reduced the marine calcium carbonate oversaturation and weakened the calcification potential of marine organisms, including ammonoids, in late Smithian oceans. Introduction: Quantification of daily upper-limb activity is determinant in the evaluation of shoulder surgery. For a number of shoulder diseases, roblems in performing daily activities have been expressed in terms of upper-limb usage. Althought many instruments measure upper-limb movements, there is no accepted standard or widely used objective measure and no device to differenciate left or right shoulder usage. We present an objective method to measure the mobility and quantify the usage of dominant and healthy or painfull shoulder movement during daily life. Methods: 12 patients with unilateral pathological shoulder (rotator cuff disease) are compared to 18 control subjects (10 right and 8 left handed). Both SST and DASH questionnaires were completed by each one. Three inertial miniature modules including triaxial gyroscopes and accelerometers were fixed on the dorsal side of both humerus, and on the thorax. An ambulatory datalogger have recorded the signals during one day. Results: We observed that right handed healthy subjects used 18% and 26% more their dominant shoulder during respectively stand and sit postures while left handed subjects used 8% and 18% more their left side. In walking periods, shoulder mobility was quite alike for both sides. Patients affected on their dominant arm (PD group) mostly used their non-dominant side (respectively 5% and 9% during stand and sit). For the patients affected on their non-dominant shoulder (PND group), this difference is respectively 28% and 26%. Moreover, we can note that, during walking periods, a difference can be observed (on the contrary to controls). Patients used 13% and 15% more their nonpathologic side respectively for PD and PND groups. Conclusion: Inertial sensors, during long-term ambulatory monitoring of upper limbs, can quantify the difference between dominant and nondominant sides. Patients used more their non affected shoulder during daily life. For the PD group, the difference with control can be shown during walking. These results are very encouraging for future evaluation of patients with shoulder injuries since it can provide an objective evaluation of the shoulder mobility and of the treatment outcome during daily life.
