Promoting and protecting the health of Iowans is the mission of the Iowa Department of Public Health (IDPH). Surveillance of notifiable health conditions is essential in establishing what, how, and when events impact the public’s health. Multiple divisions and bureaus are dedicated to accomplishing the goals of surveillance. In 2012, in addition to 850 cases reported with no lab results, there were more than 79,000 laboratory results of infectious diseases and conditions submitted to IDPH disease surveillance programs. IDPH also investigates non-infectious conditions related to lead, occupational, and environmental hazards like carbon monoxide. Approximately 100,000 blood lead test results were reported to IDPH in 2012. UPDATE: An amended annual report is also attached with this document, the amended report was produced on October 29, 2014

BACKGROUND: Community-acquired respiratory viral infections (RVIs) are common in lung transplant patients and may be associated with acute rejection and bronchiolitis obliterans syndrome (BOS). The use of sensitive molecular methods that can simultaneously detect a large panel of respiratory viruses may help better define their effects. METHODS: Lung transplant recipients undergoing serial surveillance and diagnostic bronchoalveolar lavages (BALs) during a period of 3 years were enrolled. BAL samples underwent multiplex testing for a panel of 19 respiratory viral types/subtypes using the Luminex xTAG respiratory virus panel assay. RESULTS: Demographics, symptoms, and forced expiratory volume in 1 sec were prospectively collected for 93 lung transplant recipients enrolled. Mean number of BAL samples was 6.2+/-3.1 per patient. A respiratory virus was isolated in 48 of 93 (51.6%) patients on at least one BAL sample. Of 81 positive samples, the viruses isolated included rhinovirus (n=46), parainfluenza 1 to 4 (n=17), coronavirus (n=11), influenza (n=4), metapneumovirus (n=4), and respiratory syncytial virus (n=2). Biopsy-proven acute rejection (> or =grade 2) or decline in forced expiratory volume in 1 sec > or =20% occurred in 16 of 48 (33.3%) patients within 3 months of RVI when compared with 3 of 45 (6.7%) RVI-negative patients within a comparable time frame (P=0.001). No significant difference was seen in incidence of acute rejection between symptomatic and asymptomatic patients. Biopsy-proven obliterative bronchiolitis or BOS was diagnosed in 10 of 16 (62.5%) patients within 1 year of infection. CONCLUSION: Community-acquired RVIs are frequently detected in BAL samples from lung transplant patients. In a significant percentage of patients, symptomatic or asymptomatic viral infection is a trigger for acute rejection and obliterative bronchiolitis/BOS.

Après une étude des notions de justice, juridiction et cour constitutionnelle, la présente thèse aborde le contrôle des normes par les organes de la Confédération ; un accent est mis sur le recours auTribunal fédéral contre les actes normatifs cantonaux. La justice constitutionnelle par les organes cantonaux est ensuite étudiée de façon générale, en examinant successivement les exigences posées par le droit fédéral à cet égard et les différentes formes de contrôle abstrait dans les cantons dépourvus de cour constitutionnelle. Chaque cour constitutionnelle cantonale (Nidwald, Jura, Vaud, Grisons, Genève) fait ensuite l'objet d'une analyse approfondie, portant aussi bien sur les aspects historiques et procéduraux que sur la jurisprudence. Une dernière partie est consacrée à l'examen, théorique et empirique, des arguments entrant en ligne de compte dans le choix d'un canton de se doter d'une cour constitutionnelle ou non, suivie d'une analyse comparée des cours existantes. Enfin, les rapports entre ces dernières d'une part et leTribunal fédéral d'autre part sont examinés.

BACKGROUND: Surveillance of multiple congenital anomalies is considered to be more sensitive for the detection of new teratogens than surveillance of all or isolated congenital anomalies. Current literature proposes the manual review of all cases for classification into isolated or multiple congenital anomalies. METHODS: Multiple anomalies were defined as two or more major congenital anomalies, excluding sequences and syndromes. A computer algorithm for classification of major congenital anomaly cases in the EUROCAT database according to International Classification of Diseases (ICD)v10 codes was programmed, further developed, and implemented for 1 year's data (2004) from 25 registries. The group of cases classified with potential multiple congenital anomalies were manually reviewed by three geneticists to reach a final agreement of classification as "multiple congenital anomaly" cases. RESULTS: A total of 17,733 cases with major congenital anomalies were reported giving an overall prevalence of major congenital anomalies at 2.17%. The computer algorithm classified 10.5% of all cases as "potentially multiple congenital anomalies". After manual review of these cases, 7% were agreed to have true multiple congenital anomalies. Furthermore, the algorithm classified 15% of all cases as having chromosomal anomalies, 2% as monogenic syndromes, and 76% as isolated congenital anomalies. The proportion of multiple anomalies varies by congenital anomaly subgroup with up to 35% of cases with bilateral renal agenesis. CONCLUSIONS: The implementation of the EUROCAT computer algorithm is a feasible, efficient, and transparent way to improve classification of congenital anomalies for surveillance and research.

Registries are among the oldest methods used in public health for epidemiological surveillance and decision making in the area of communicable diseases. Although other sources of data are now available in many developed countries, registries still provide important information. This article reviews the main aims and characteristics of modern registries, providing several examples of current epidemiological problems. Practical advantages and disadvantages of registries are also discussed, as well as some developmental perspectives in this area.

A large variation in neonatal herpes incidence is observed in USA and Europe. Better knowledge of neonatal herpes epidemiology is important to inform local prevention strategies. Between 2002 and 2008, the Swiss Paediatric Surveillance Unit reported prospectively proven neonatal herpes simplex virus infections. During the study period seven cases were declared, for an incidence of 1.6/100,000 (95% CI 0.64-3.28/100,000) live births. This is one of the lowest incidences of neonatal herpes reported.

The overall aim of the project has been to assess the extent to which data on the frequency of occurence of STDs [Sexually transmitted diseases] might be useful in the monitoring and evaluation of AIDS prevention programmes. The objectives have been to answer the following questions: (a) Can measures of STD occurence be used as an outcome measure of AIDS/HIV preventive efforts ? In particular: -> which diseases might be useful ? -> in what ways could they be used ? (b) If measures of STD occurence can be used in this way, is existing surveillance data in Western Europe adequate for the purpose ? If not why not ? (c) What do data from existing STD surveillance systems tell us about the success or failure of AIDS prevention to date ? (d) What needs to be done in order taht STD surveillance data in the countries of Western Europe could be used for this purpose ? [Authors, p. 4]

Cet article se propose de réfléchir aux réformes de l'administration en étudiant ce qu'elles deviennent au niveau des agents publics de base ou 'street-level bureaucrats'. Si la littérature qui leur est consacrée a jusqu'ici mis en avant à la fois l'autonomie décisionnelle de ces acteurs ainsi que les stratégies de contrôle de celle-ci, nous proposons ici de revisiter la problématique de l'autonomie et du contrôle à ce niveau en rendant compte de la manière dont elle est affectée par les réformes de « nouvelle gestion publique » d'une part, et les « nouvelles technologies de l'information et de la communication » d'autre part. Pour ce faire, la partie empirique présente les résultats préliminaires d'une recherche de l'auteur portant sur un type d'agent public aux premières lignes de la mise en oeuvre de la politique suisse du chômage, à savoir les taxateurs d'une caisse cantonale d'indemnisation, laquelle fonctionne selon une logique « néo-managériale ». Les données analysées montrent que si les éléments du dispositif NGP/TIC conduisent globalement à un renforcement considérable du contrôle hiérarchique et organisationnel d'une autonomie qui reste néanmoins importante, ceci s'accompagne également de modes d'appropriation spécifiques et intéressants dans ce qu'ils ont d'inattendu, de créatif voire de subversif au niveau des agents sur le terrain.
