This work proposes a novel approach for a suitable orientation of antibodies (Ab) on an immunosensing platform, applied here to the determination of 8-hydroxy-2′-deoxyguanosine (8OHdG), a biomarker of oxidative stress that has been associated to chronic diseases, such as cancer. The anti-8OHdG was bound to an amine modified gold support through its Fc region after activation of its carboxylic functions. Non-oriented approaches of Ab binding to the platform were tested in parallel, in order to show that the presented methodology favored Ab/Ag affinity and immunodetection of the antigen. The immunosensor design was evaluated by quartz-crystal microbalance with dissipation, atomic force microscopy, electrochemical impedance spectroscopy (EIS) and square-wave voltammetry. EIS was also a suitable technique to follow the analytical behavior of the device against 8OHdG. The affinity binding between 8OHdG and the antibody immobilized in the gold modified platform increased the charge transfer resistance across the electrochemical set-up. The observed behavior was linear from 0.02 to 7.0 ng/mL of 8OHdG concentrations. The interference from glucose, urea and creatinine was found negligible. An attempt of application to synthetic samples was also successfully conducted. Overall, the presented approach enabled the production of suitably oriented Abs over a gold platform by means of a much simpler process than other oriented-Ab binding approaches described in the literature, as far as we know, and was successful in terms of analytical features and sample application.

The autonomic nervous system (ANS) is known to be an important modulator in the pathogenesis of paroxysmal atrial fibrillation (PAF). Changes in ANS control of heart rate variability (HRV) occur during orthostatism to maintain cardiovascular homeostasis. Wavelet transform has emerged as a useful tool that provides time-frequency decomposition of the signal under investigation, enabling intermittent components of transient phenomena to be analyzed. AIM: To study HRV during head-up tilt (HUT) with wavelet transform analysis in PAF patients and healthy individuals (normals). METHODS: Twenty-one patients with PAF (8 men; age 58 +/- 14 yrs) were examined and compared with 21 normals (7 men, age 48 +/- 12 yrs). After a supine resting period, all subjects underwent passive HUT (60 degrees) while in sinus rhythm. Continuous monitoring of ECG and blood pressure was carried out (Task Force Monitor, CNSystems). Acute changes in RR-intervals were assessed by wavelet analysis and low-frequency power (LF: 0.04-0.15 Hz), high-frequency power (HF: 0.15-0.60 Hz) and LF/HF (sympathovagal) were calculated for 1) the last 2 min of the supine period; 2) the 15 sec of tilting movement (TM); and 3) the 1st (TT1) and 2nd (TT2) min of HUT. Data are expressed as means +/- SEM. RESULTS: Baseline and HUT RR-intervals were similar for the two groups. Supine basal blood pressure was also similar for the two groups, with a sustained increase in PAF patients, and a decrease followed by an increase and then recovery in normals. Basal LF, HF and LF/ HF values in PAF patients were 632 +/- 162 ms2, 534 +/- 231 ms2 and 1.95 +/- 0.39 respectively, and 1058 +/- 223 ms2, 789 +/- 244 ms2 and 2.4 +/- 0.36 respectively in normals (p = NS). During TM, LF, HF and LF/HF values for PAF patients were 747 +/- 277 ms2, 387 +/- 94 ms2 and 2.9 +/- 0.6 respectively, and 1316 +/- 315 ms2, 698 +/- 148 ms2 and 2.8 +/- 0.6 respectively in normals (p < 0.05 for LF and HF). During TF1, LF, HF and LF/ HF values for PAF patients were 1243 +/- 432 ms2, 302 +/- 88 ms2 and 7.7 +/- 2.4 respectively, and 1992 +/- 398 ms2, 333 +/- 76 ms2 and 7.8 +/- 0.98 respectively for normals (p < 0.05 for LF). During TF2, LF, HF and LF/HF values for PAF patients were 871 +/- 256 ms2, 242 +/- 51 ms2 and 4.7 +/- 0.9 respectively, and 1263 +/- 335 ms2, 317 +/- 108 ms2 and 8.6 +/- 0.68 respectively for normals (p < 0.05 for LF/HF). The dynamic profile of HRV showed that LF and HF values in PAF patients did not change significantly during TM or TT2, and LF/HF did not change during TM but increased in TT1 and TT2. CONCLUSION: Patients with PAF present alterations in HRV during orthostatism, with decreased LF and HF power during TM, without significant variations during the first minutes of HUT. These findings suggest that wavelet transform analysis may provide new insights when assessing autonomic heart regulation and highlight the presence of ANS disturbances in PAF.

To improve surgical safety, and to reduce the mortality and surgical complications incidence, the World Health Organization (WHO) developed the Surgical Safety Checklist (SSC). The SSC is a support of information that aids health professionals to reduce the number of complications, induction of anaesthesia, period before skin incision and period before leaving the operating room (OR). The SSC was tested in several countries of the world and their results shown that after introduction of the SSC the incidence of patient complication lowered from 11.0% to 7.0% (P<0.001), the rate of death declined from 1.5% to 0.8% (P = 0.003) and the nurses recognized that patients identity was more often con rmed (81.6% to 94.2%, P<0.01) in many institutions. Recently the SSC was also implemented in Portuguese hospitals, which led us to its study in the real clinical environment. An observational study was performed: several health professionals were observed and interviewed, to understand the functioning of the SSC in an OR, during the clinical routine. The objective of this study was to understand the current use of the SSC, and how it may be improved in terms of usability, taking advantage of the technological advancements such as mobile applications. During two days were observed 14 surgeries, only 2 surgeries met the requirements for the three phases of the SSC, as de ned by the WHO. Of the remaining 12 observed surgeries, 9 surgeries completed the last phase at the correct time. It was also observed that only in 2 surgeries all the phases of the SSC were read aloud to the team and that, in 7 surgeries, several items were read aloud and answered but no one was checking the SSC, only after the end of the phase. The observational study results disclose that several health professionals do not meet with rules of the WHO manual. This study demonstrates that it is urgent to change the mindset of health professionals, and that di erent features in the SSC may be useful to make it more easy to use. With the results of the observational study, a SSC application proposal was developed with new functionalities to improve and aid the health professional in its use. In this application the user can chose between a SSC already created to a speci c surgery or to create a new SSC, adding and adapting some questions from the WHO standard. To create a new SSC, the application is connected to an online questionnaire builder (JotForm). The choice for this online questionnaire builder went through three essential characteristics: number of types of questions, mainly checkbox, radio button and text; the possibility of to create sections inside sections and the API. In addition, in this proposal the improvements are focused in forcing the user to focus in the work ow of the SSC and to save the input timestamps and any actions made by them. Therefore, the following features was implemented to achieve that goal: display one item of the SSC at a time; display the stage where the SSC is; do not allow going back to the previous step; do not allow going forward to the next item if the current is not lled; do not allow going forward to the next item if the time it took to ll the item was too short and log any action made by the user.

The purpose of this work was to develop a reliable alternative method for the determination of the dithiocarbamate pesticide mancozeb (MCZ) in formulations. Furthermore, a method for the analysis of MCZ's major degradation product, ethylenethiourea (ETU), was also proposed. Cyclic voltammetry was used to characterize the electrochemical behavior of MCZ and ETU, and square-wave adsorptive stripping voltammetry (SWAdSV) was employed for MCZ quantification in commercial formulations. It was found that both MCZ and ETU are irreversibly reduced (− 0.6 V and − 0.5 V vs Ag/AgCl, respectively) at the surface of a glassy carbon electrode in a mainly diffusion-controlled process, presenting maximum peak current intensities at pH 7.0 (in phosphate buffered saline electrolyte). Several parameters of the SWAdSV technique were optimized and linear relationships between concentration and peak current intensity were established between 10–90 μmol L− 1 and 10–110 μmol L− 1 for MCZ and ETU, respectively. The limits of detection were 7.0 μmol L− 1 for MCZ and 7.8 μmol L− 1 for ETU. The optimized method for MCZ was successfully applied to the quantification of this pesticide in two commercial formulations. The developed procedures provided accurate and precise results and could be interesting alternatives to the established methods for quality control of the studied products, as well as for analysis of MCZ and ETU in environmental samples.

We study the peculiar dynamical features of a fractional derivative of complex-order network. The network is composed of two unidirectional rings of cells, coupled through a "buffer" cell. The network has a Z3 × Z5 cyclic symmetry group. The complex derivative Dα±jβ, with α, β ∈ R+ is a generalization of the concept of integer order derivative, where α = 1, β = 0. Each cell is modeled by the Chen oscillator. Numerical simulations of the coupled cell system associated with the network expose patterns such as equilibria, periodic orbits, relaxation oscillations, quasiperiodic motion, and chaos, in one or in two rings of cells. In addition, fixing β = 0.8, we perceive differences in the qualitative behavior of the system, as the parameter c ∈ [13, 24] of the Chen oscillator and/or the real part of the fractional derivative, α ∈ {0.5, 0.6, 0.7, 0.8, 0.9, 1.0}, are varied. Some patterns produced by the coupled system are constrained by the network architecture, but other features are only understood in the light of the internal dynamics of each cell, in this case, the Chen oscillator. What is more important, architecture and/or internal dynamics?

Doente do sexo feminino, 18 anos, raça negra, natural da Guiné-Bissau, com quadro clÃnico com 8 anos de evolução, de sinais inflamatórios ao nÃvel de ambos os ângulos da mandÃbula, com nódulos palpáveis e drenagem espontânea de material aquoso não purulento, após exodontia de molar mandibular. Para diagnóstico definitivo foi colocada indicação para biópsia mandibular sob anestesia geral. Na avaliação pré-operatória destacava-se uma via aérea previsivelmente muito difÃcil: mallampati IV, micrognátia com abertura da boca muito limitada (1 cm), distância tiromentoniana < 6 cm e mobilidade cervical limitada, pelo que se programou intubação endotraqueal guiada por fibroscopia. Procedeu-se à intubação nasotraqueal guiada por fibroscopia com tubo 7,0 com cuff, sob sedação endovenosa com midazolam e fentanil, sem intercorrências. A apresentação deste caso tem como objectivo realçar as dificuldades inerentes ao diagnóstico de actinomicose com frequente necessidade de procedimentos invasivos, sob anestesia geral, o que condiciona um risco anestésico importante associado à dificuldade na abordagem da via aérea.

A malária é considerada como a doença parasitária mais séria em humanos, infectando cerca de 5 a 10% da população mundial, estimando-se entre 300 a 600 milhões de casos e mais de dois milhões de mortes, anualmente. Apesar da severidade da doença, a compreensão da variabilidade da resposta do hospedeiro à infecção (traduzida desde a infecção silenciosa até formas crónicas da doença, passando por quadros clÃnicos potencialmente fatais), continua a ser um dos grandes desafios da investigação médica. Vários factores genéticos parasitários ou do hospedeiro, estado imune e nÃveis de exposição, contribuem para esta variabilidade, mas a sua importância relativa para a carga total da doença tem sido pouco estudada. Entre outros, é possÃvel salientar como fontes importantes de variabilidade na susceptibilidade à malária e gravidade da doença, factores intrÃnsecos ao hospedeiro tais como polimorfismos genéticos relacionados com as células sanguÃneas e factores parasitários como a composição da(s) população(ões) parasitária(s) presentes na infecção. Factores do hospedeiro humano relacionados com as células sanguÃneas (drepanocitose e deficiência na glucose-6-fosfato desidrogenase - G6PD) têm sido tradicionalmente estudados e relacionados com a gravidade da malária causada por Plasmodium falciparum. Relativamente à s populações parasitárias, das cinco espécies que infectam humanos, P. falciparum continua a ser responsável pela malária grave, apesar de muito recentemente alguma gravidade ser atribuÃda a Plasmodium vivax. Sabe-se que nas mesmas áreas outras espécies coexistem em muito maior prevalência, contrariando o que se pensava há algum tempo. Apesar de poucos estudos terem focado o tema das infecções mistas, existem alguns relatos de que eventuais interacções entre as diferentes espécies presentes simultaneamente no mesmo hospedeiro poderem afectar a susceptibilidade à doença. Com o objectivo geral de avaliar o efeito e a contribuição destes factores na susceptibilidade e gravidade da malária, analisando e comparando três grupos (estudo de caso-controlo) doentes com malária grave (MG), doentes com malária não-complicada (Mnc) e indivÃduos infectados assintomáticos (IA), realizámos em Angola de 2007 a 2010, um estudo em sete das 18 provÃncias com distintos nÃveis de endemicidade. Foram obtidas 1.416 amostras de sangue periférico de 1.198 indivÃduos assintomáticos e 218 doentes. O DNA obtido a partir destes isolados foi utilizado para detecção da presença de variantes genéticos relacionados com os eritrócitos (drepanocitose – análise do gene HBB, deficiência G6PD – análise do gene G6PD, antigénio Duffy-análise do gene DARC) e identificação das espécies de Plasmodium presentes, através de nested-PCR mediante a amplificação dos genes que codificam a subunidade menor do RNA ribossomal. Os resultados demonstraram prevalências superiores à s anteriormente descritas em relação à s seguintes espécies parasitárias: P. falciparum 98,2% vs 92,0% e Plasmodium malariae 10,7% vs 1,0% e inferior em relação a P. vivax 2,5% vs 7,0%. Foi reconfirmada a presença de Plasmodium ovale (descrita anteriormente), mas não publicada em documentos oficiais e relatada pela primeira vez em Angola a presença de infecções mistas (duplas e triplas) (15,7%) e de infecção por P. vivax em indivÃduos Duffy-negativos (dados publicados). Relativamente aos polimorfismos relacionados com os genes HBB e G6PD e provável associação com a protecção à malária, os nossos resultados confirmam a associação do traço falciforme (heterozigotia HbS), com a protecção à doença (OR = 0,30; IC 95% 0,18-0,49; p<0,001). Contudo, não foi encontrada, quer nos indivÃduos hemizigóticos, quer nos heterozigóticos para o alelo G6PD (A-) nenhuma evidência para a protecção a malária (MG e Mnc) (OR = 1,69; IC 95% 0,91-3,13; p=0,096). Os resultados desta investigação requerem um estudo mais aprofundado, com uma dimensão amostral maior, necessário à confirmação da nossa observação.

Meios seletivos para Fusobacterium foram, comparados no aspecto de sua eficiência no isolamento dos microrganismos fusiformes e quanto à inibição da flora concomitante. Material não calcificado do sulco gengival foi homogeneizado e diluÃdo em tampão de fosfato (0,067 M - pH 7,2); 0,1 ml foi semeado em agar sangue, no meio de Omata & Disraely, meio de Omata & Disraely sem sôro, meio de McMarthy & Snyder, agar sangue com vancomicina, meio de Onisi, meio de Onisi com sôro de cavalo e meio de Onisi com pH 7,0. Em todos os casos a incubação foi realizada em anaerobiose, em jarra tipo Brewer, com 95% nitrogênio e 5% gás carbônico, durante 4 dias. O meio de McCarthy & Snyder mostrou-se tão eficiente quanto o agar sangue em relação ao isolamento de Fusobacterium, ainda inibindo a flora concomitante, o meio de Omata & Disraely, com cristal violeta e estreptomicina, foi de certo modo inibidor também para Fusobacterium. O meio de Onisi, sugerido, sem sôro, possibilitou o isolamento de pequeno número de fusobactérias, provà velmente as amostras não exigentes de proteÃnas naturais.
