Liver cancer accounts for nearly 10% of all cancers in the US. Intrahepatic Arterial Radiomicrosphere Therapy (RMT), also known as Selective Internal Radiation Treatment (SIRT), is one of the evolving treatment modalities. Successful patient clinical outcomes require suitable treatment planning followed by delivery of the microspheres for therapy. The production and in vitro evaluation of various polymers (PGCD, CHS and CHSg) microspheres for a RMT and RMT planning are described. Microparticles with a 30±10 µm size distribution were prepared by emulsion method. The in vitro half-life of the particles was determined in PBS buffer and porcine plasma and their potential application (treatment or treatment planning) established. Further, the fast degrading microspheres (≤ 48 hours in vitro half-life) were labeled with 68Ga and/or 99mTc as they are suitable for the imaging component of treatment planning, which is the primary emphasis of this dissertation. Labeling kinetics demonstrated that 68Ga-PGCD, 68Ga-CHSg and 68Ga-NOTA-CHSg can be labeled with more than 95% yield in 15 minutes; 99mTc-PGCD and 99mTc-CHSg can also be labeled with high yield within 15-30 minutes. In vitro stability after four hours was more than 90% in saline and PBS buffer for all of them. Experiments in reconstituted hemoglobin lysate were also performed. Two successful imaging (RMT planning) agents were found: 99mTc-CHSg and 68Ga-NOTA-CHSg. For the 99mTc-PGCD a successful perfusion image was obtained after 10 minutes, however the in vivo degradation was very fast (half-life), releasing the 99mTc from the lungs. Slow degrading CHS microparticles (> 21 days half-life) were modified with p-SCN-b-DOTA and labeled with 90 Y for production of 90Y-DOTA-CHS. Radiochemical purity was evaluated in vitro and in vivo showing more than 90% stability after 72 and 24 hours respectively. All agents were compared to their respective gold standards (99mTc-MAA for 68Ga-NOTA-CHSg and 99m Tc-CHSg; 90Y-SirTEX for 90Y-DOTA-CHS) showing superior in vivo stability. RMT and RMT planning agents (Therapy, PET and SPECT imaging) were designed and successfully evaluated in vitro and in vivo.

Le ultime ricerche in campo oncologico sulle cure antitumorali sono indirizzate verso una categoria definita target therapy. In particolare tra le più promettenti, le terapie antiangiogenetiche, il cui scopo primario è quello di limitare l’apporto di sangue al tumore. In questo contesto la Tomografia Computerizzata (TC) perfusionale rappresenta un’importante tecnica di imaging radiologico in grado, teoricamente, di fornire misure quantitative, che permettano una valutazione, anche precoce, della risposta alle terapie antiangiogenetiche. I principali problemi aperti in questo campo riguardano la standardizzazione dei protocolli di acquisizione e di elaborazione delle sequenze perfusionali, che determinano la scarsa riproducibilità dei risultati intra- ed inter-paziente, non consentendone l’uso nella pratica clinica. In letteratura sono presenti diversi studi riguardanti la perfusione dei tumori polmonari, ma vi sono pochi studi sull’affidabilità dei parametri perfusionali calcolati. Questa Tesi si propone di analizzare, quantificare e confrontare gli errori e l’affidabilità dei parametri perfusionali calcolati attraverso la TC perfusionale. In particolare, vengono generate delle mappe di errore ed effettuati dei confronti di diverse regioni del polmone sano. I risultati ottenuti dall’analisi dei casi reali sono discussi al fine di poter definire dei livelli di affidabilità dei parametri perfusionali e di quantificare gli errori che si commettono nella valutazione dei parametri stessi. Questo studio preliminare consentirà, quindi, un’analisi di riproducibilità, permettendo, inoltre, una normalizzazione dei valori perfusionali calcolati nella lesione, al fine di effettuare analisi intra- ed inter-paziente.

PURPOSE: To evaluate if the ileum resection changes the functioning liver cell mass, the hepatic metabolism and the biodistribution of radiopharmaceutical in rats. METHODS: Twelve Wistar rats weighing 285g±34g were randomly divided into the ileum resection group (n = 6) and sham group rats (n = 6). After 30 days, they were anesthetized and 0.1mL of 99m-Tc-phytate(0.66MBq) was injected via femoral vein. After 30 minutes, blood samples were collected for red blood cells radioactive labeling and serum ALT, AST and gammaGT. Liver samples were used for 99m-Tc-phytatepercentage of radioactivity/gram of tissue and histopathology. Student’s t test was used with significance 0.05. RESULTS: There was a higher uptake of 99m-Tc-phytate in the liver of sham rats, compared to the ileum resection group (p<0.05). GammaGT, ALT and AST were increased in ileum resection rats compared to sham (p<0.05). The he patocytes count was significantly lower in ileum resection group than in sham (p<0.05). Liver: body mass ratio was lower in experimental animals than in sham group (p<0.05). CONCLUSION: These data support that the ileum has important role in liver function and liver mass regulation, and they have potential clinical implications regarding the pathogenesis of liver injury following lower bowel resection.

Study with the purpose to examine the effects of duodenal switch (DS), regularly performed in morbidly obese patients, on biodistribution of sodium pertechnetate in several organs of rats. There was no early or late mortality in either rats groups. The values of percent radioactivity per gram of tissue (%ATI/g), showed no significant difference in liver, stomach, small bowel, duodenum, kidney, heart, bladder, bone and brain, when compared the DS rats with sham and controls rats. A postoperative significant increase (p<0.05) in mean %ATI/g levels was observed in spleen, pancreas and muscle in group DS rats, as compared to group S and C rats. In the lung there was an increase and in thyroid a decrease in mean %ATI/g of DS rats, when compared to sham rats (p<0.05). In conclusion, the biliopancreatic diversion with duodenal switch in rats modified the biodistribution of sodium pertechnetate in thyroid, lung, pancreas, spleen and muscle

This study aimed to evaluate if the splenectomy alters the biodistribution of 99mTc-DMSA and renal function in Wistar rats. The animals were separated in the groups: splenectomy (n = 6) and control (n = 6). After splenectomy (15 days), the administration of 0.1ml of 99mTc-DMSA IV (0.48 MBq) was carried out. Thirty minutes later, kidney, heart, lung, thyroid, stomach, bladder and femur and samples of blood were isolated. The organs were weighed, counted and the percentage of radioactivity -g (%ATI-g) determined. Serum urea and creatinine, hematocrit, leukocytes and platelets were measured. Statistics by t test (p<0.05) was done. There was a significant reduction in %ATI-g in kidney and blood (p<0.05) of splenectomized animals, a significant increase (p<0.05) of urea (88.8 ± 18.6 mg-dL) and creatinine (0.56 ± 0.08 mg-dL), compared to the controls (51.5±1.6, 0.37±0.02mg-dL, respectively), as well as increase in platelets and leucocytes, and hematocrit reduction. The analysis of the results indicates that in rats, splenectomy seems to alter the renal function and the uptake of 99mTc-DMSA

La caratterizzazione del parenchima polmonare è un aspetto cruciale per l’identificazione dell’enfisema e dell’air trapping in pazienti affetti da broncopneumopatia cronica ostruttiva (BPCO). L’innovazione presente in questo lavoro di tesi è quella di utilizzare a questo scopo l’analisi tessiturale mediante il metodo delle matrici di co-occorrenza su immagini di tomografia computerizzata (TC) acquisite in inspirio ed espirio co-registrate. La co-registrazione che ha portato le immagini acquisite in espirio sullo stesso sistema di riferimento di quelle acquisite in inspirio è avvenuta utilizzando la trasformazione diffeomorfa B-Spline SyN, implementata nel software ANTs, ed è stata applicata sia alle immagini TC che alle features estratte da esse. Dalle matrici di co-occorrenza è stata calcolata la feature Energia, che misura l’uniformità dei livelli di grigio contenuti nell’immagine, e quindi la sua omogeneità. Partendo dal fatto che le aree parenchimali affette da enfisema e air trapping hanno alti livelli di omogeneità dovuti alla presenza dell’aria intrappolata al loro interno, l’idea alla base di questo lavoro è stata quella di identificare queste aree attraverso gli alti valori che l’Energia assume in loro corrispondenza. Sono state quindi stabilite sperimentalmente alcune soglie basate sui valori assunti dall’Energia in inspirio e in espirio. Sulla base di queste il parenchima polmonare è stato clusterizzato in diverse aree identificanti i tessuti sani, quelli affetti da enfisema e quelli affetti dall’air trapping. La clusterizzazione ottenuta è risultata coerente con la reale distribuzione dell’enfisema e dell’air trapping nei pazienti analizzati, dimostrando la validità della tecnica utilizzata e aprendo nuovi scenari futuri alla caratterizzazione del parenchima polmonare in questa patologia.
