Analisa-se a evolução da mortalidade por DIP em Salvador-BA e seus diferenciais intra-urbanos mediante estudo de série temporal e de agregados espaciais, nos anos noventa. O DATASUS e declarações de óbitos foram as fontes de dados. Na análise temporal, empregou-se mortalidade proporcional, taxa de mortalidade e razão de mortalidade padronizada (RMP). No estudo espacial, analisou-se as taxas de mortalidade por DIP segundo um índice de condições de vida (ICV). Entre 1991 e 1995, a mortalidade proporcional por DIP foi de 8,3% e o risco de morrer variou entre 55,9 e 34,0 por 100 mil habitantes. No período seguinte, a variação foi entre 52,8 e 41,1 por 100 mil habitantes. A razão de mortalidade padronizada por doenças infecciosas e parasitárias em 1998 foi de 1,3. As doenças infecciosas intestinais continuam sendo uma das principais causas de morte desse grupo. As áreas da cidade onde as condições de vida eram mais baixas concentravam as maiores taxas de mortalidade por DIP. A despeito do declínio, ainda existe um excesso de mortalidade por DIP em Salvador. O modelo de desenvolvimento do país e a reemergência de algumas doenças podem estar contribuindo para este padrão.

Entre 98 exames parasitológicos de fezes de pacientes do Centro de Saúde de Soledad, Anzoátegui, Venezuela, foram encontrados 46 (46,9%) positivos para Blastocystis hominis.

This paper describes an epidemiologic investigation carried out in Imbiribeira district, where a canine visceral leishmaniasis case was recorded. Despite the absence of seropositive dogs and sand flies, these findings are not sufficient to discard the occurrence of a zoonotic cycle of visceral leishmaniasis in Recife.

An HIV seroprevalence and molecular study was conducted among 935 subjects: 723 female commercial sex workers, 92 men who have sex with men and 120 HIV-positive volunteers. The reported injection drug use rates were 0.7% in female commercial sex workers and 3% in men who have sex with men. Sexually transmitted infections were reported in 265 (37%) of the female commercial sex workers and 38 (41%) of the men who have sex with men. A total of 20 (2.8%) female commercial sex workers and 12 (13%) men who have sex with men became HIV infected during the study period. A history of sexually transmitted infection increased the risk of subsequent HIV infection twofold (adjusted odds ratio of 2.5) among the female commercial sex workers, while cocaine use had an adjusted odds ratios of 6.61 among men who have sex with men. From 130 samples, and based on heteroduplex mobility assaying for the env gene, with sequencing of part of pol and/or full genomes, subtype B was the predominant subtype identified (66%); followed by subtype F (22%) and subtype C (4%). Recombinant CRF12-BF strains were identified in 6% and CRF17_BF was identified in 2%.

INTRODUÇÃO: A força da reemergência do vírus do dengue e a gravidade destas infecções colocaram esta doença na agenda de prioridades das instituições responsáveis pela proteção à saúde das populações. Aspecto importante para a compreensão da epidemiologia do dengue nos dias atuais refere-se ao conhecimento dos padrões da difusão espaço-temporal, entretanto são escassas as investigações que abordam esta questão. Este estudo descreve o processo de difusão do dengue no estado da Bahia, de 1994 a 2000. MÉTODOS: Trata-se de estudo ecológico espaço-temporal, tendo como unidades de análise município, semana epidemiológica, mês, trimestre e ano. Procedeu-se construção da curva de tendência e realizou-se mapeamento seqüencial da ocorrência de dengue por município para o período. RESULTADOS: Foram notificados 164.050 casos de dengue e a introdução desse vírus na Bahia, diferentemente de outros estados, se deu por um município de pequeno porte, constatando-se intermitência temporal e espacial nos registros de casos no início desta epidemia. Contudo, a partir de 1995 o processo de difusão da doença se deu de forma rápida e intensa. O vírus circulou em todas as zonas climáticas do estado o que revela o seu elevado poder de transmissão. A maior intensidade de detecção de casos e expansão territorial foi nas regiões úmidas e semi-úmidas do litoral, ideais para a sobrevivência e proliferação do vetor, e também, por serem mais densamente povoadas. CONCLUSÕES: Hipoteticamente, o padrão espaço-temporal intermitente de detecção de casos observado inicialmente, poderia permitir o controle da progressão da epidemia, caso houvesse ações de combate vetorial estruturadas.

INTRODUCTION: Visceral leishmaniasis presents urban behavior in some Brazilian cities, with domestic dogs as the main infection source. In Cuiabá, MT, canine visceral leishmaniasis was diagnosed and characterized as recommended by the Ministry of Health. METHODS: Biological samples from suspected canine carriers were analyzed by the isoenzyme electrophoresis technique. The 6PGDH enzyme and reference strain IOC/L0566 (MHOM/BR/1975/M2903) of Leishmania (Leishmania) infantum was used as one of the controls. RESULTS: Electrophoresis analysis revealed that the canine isolates belonged to the species L. (L.) infantum. CONCLUSIONS: The authors emphasize the importance of species characterization, particularly in areas of mixed infection like Cuiabá.

INTRODUCTION: A single nucleotide polymorphism (SNP) in the gene encoding gamma interferon influences its production and is associated with severity of infectious diseases. This study aimed to evaluate the association of IFNγ+874T/A SNP with duration of disease, morbidity, and development of retinochoroiditis in acute toxoplasmosis. METHODS: A case-control study was conducted among 30 patients and 90 controls. RESULTS: Although statistical associations were not confirmed, A-allele was more common among retinochoroiditis cases and prolonged illness, while T-allele was more frequent in severe disease. CONCLUSIONS: Despite few cases, the results could indicate a relation between IFNγ+874T/A single nucleotide polymorphism and clinical manifestations of toxoplasmosis.

INTRODUCTION: Some viruses of the Herpesviridae family are frequently the etiologic agents of oral lesions associated with HIV. The aim of this study was to identify the presence of herpes simplex virus types 1 and 2 (HSV-1, HSV-2), Varicella Zoster virus (VZV), Epstein-Barr virus (EBV), human cytomegalovirus (HCMV), human herpesvirus type 6, type 7 and type 8 (HHV-6, HHV-7 and HHV-8) in the oral cavity of HIV-infected children/adolescents and verify the association between viral subtypes and clinical factors. METHODS: The cells of oral mucosa were collected from 50 HIV infected children/adolescents, 3-13 years old (mean age 8.66). The majority (66%) of selected were girls, and they were all outpatients at the pediatric AIDS clinic of a public hospital in Rio de Janeiro. Nested-PCR was used to identify the viral types. RESULTS: Absence of immunosuppression was observed in 66% of the children. Highly active antiretroviral therapy (HAART) was used by 72.1% of selected and moderate viral load was observed in 56% of the children/adolescents. Viral types were found in 86% of the children and the subtypes were: HSV-1 (4%), HSV-2 (2%), VZV (4%), EBV (0%), HCMV (24%), HHV6 (18%), HHV-7 (68%), HHV8 (0%). CONCLUSIONS: The use of HAART has helped to reduce oral lesions, especially with herpes virus infections. The health professionals who work with these patients should be aware of such lesions because of their predictive value and the herpes virus can be found circulating in the oral cavity without causing lesions.

Introduction Chagasic megaesophagus (CM) is the most common digestive manifestation of Chagas disease in Brazil, and the State of Goiás is one of the most affected regions. In recent decades, the Hospital das Clínicas (HC)/Universidade Federal de Goiás (UFG) has been a reference center for the study and treatment of CM. The objective of this study was to characterize the current epidemiological profile of patients with CM observed at the HC of the UFG from 1998 to 2010. Methods In total, 939 patient records were analyzed, and age, gender, place of birth, serology, symptoms and radiological classification according to Rezende et al. were analyzed. Results The median patient age was 55 years. Male patients were more (54%) prevalent than female patients. The prevalence of younger patients (less than 31 years of age) was 4.2%, but 82.1% of the younger patients were from State of Bahia. Patients older than 40 years were the majority (85.5%). The radiological groups were distributed as follows: Group I (35.9%), Group II (32.9%), Group III (17%) and Group IV (14.2%). Conclusions Compared with previous studies by the same group in 1975, 1994 and 1995, the number of younger patients decreased, and the frequency curve has shifted to older patients.

Introduction CD4+CD25+ T lymphocytes have been implicated in the regulation of host inflammatory response against Trypanosoma cruzi, and may be involved in the clinical course of the disease. Methods Peripheral blood mononuclear cells from patients with chronic Chagas disease were cultured in the presence of T. cruzi recombinant antigens and assayed for lymphocytes at distinct time points. Results It was possible to differentiate clinical forms of chronic Chagas disease at days 3 and 5 according to presence of CD4+CD25+ T cells in cell cultures. Conclusions Longer periods of cell culture proved to be potentially valuable for prospective evaluations of CD4+CD25+ T lymphocytes in patients with chronic Chagas disease.

AbstractINTRODUCTION:Studies that generate information that may reduce the dengue death risk are essential. This study analyzed time trends and risk factors for dengue mortality and fatality in Brazil from 2001 to 2011.METHODS:Time trends for dengue mortality and fatality rates were analyzed using simple linear regression. Associations between the dengue mortality and the case fatality rates and socioeconomic, demographic, and health care indicators at the municipality level were analyzed using negative binomial regression.RESULTS:The dengue hemorrhagic fever case fatality rate increased in Brazil from 2001 to 2011 (β=0.67; p=0.036), in patients aged 0-14 years (β=0.48; p=0.030) and in those aged ≥15 years (β=1.1; p<0.01). Factors associated with the dengue case fatality rate were the average income per capita (MRR=0.99; p=0.038) and the number of basic health units per population (MRR=0.89; p<0.001). Mortality rates increased from 2001 to 2011 (β=0.350; p=0.002).Factors associated with mortality were inequality (RR=1.02; p=0.001) high income per capita (MRR=0.99; p=0.005), and higher proportions of populations living in urban areas (MRR=1.01; p<0.001).CONCLUSIONS:The increases in the dengue mortality and case fatality rates and the associated socioeconomic and health care factors, suggest the need for structural and intersectoral investments to improve living conditions and to sustainably reduce these outcomes.

Foram estudados a dieta, a composição corporal e o valor de energia bruta do conteúdo estomacal, filé e fígado, bem como alguns frutos e sementes que fazem parte da dieta do matrinxã, relacionando-se estes parâmetros com as flutuações do nível da água durante um ciclo anual. Observou-se que a espécie se alimenta de sementes, frutos, flores, restos de vegetais e insetos, apresentando hábito alimentar onívoro. Estômagos vazios foram frequentes (68,5%) nos diferentes períodos hidrológicos. As análises proximais e valor da energia bruta do filé e fígado dos peixes analisados não apresentaram diferenças significativas entre os sexos (P>0,05). No entanto, observou-se diferenças entre os períodos de águas baixas e altas. As médias das composições centesimais e os valores de energia bruta do conteúdo estomacal, expressos com base na matéria seca revelaram, na enchente, um baixo teor de proteína (8,9%) e cinza (1,3%), alto valor de lipídios (70,2%) e energia (775 Kcal EB/100g). Na seca, o teor de proteína (24,9%) e cinza (6,3%) foram maiores, porém os teores de lipídios (13,2%) e de energia (416,9 Kcal EB 100g) foram mais baixos que na cheia. Foi verificado abundante depósito de gordura cavitária o ano todo, diminuindo em janeiro, mês em que o matrinxã realiza a desova.

Objetivos Comparar a ocorrência de sintomas depressivos entre dois grupos homogêneos de pacientes em diálise, um em hemodiálise (HD) e outro em diálise peritoneal (DP), verificando o possível papel preditor do método, assim como avaliar a influência de variáveis sociodemográficas e clínicas no diagnóstico deste transtorno. Métodos Amostra envolveu dois grupos homogêneos de pacientes em TRS, 30 em HD e 30 em DP. Aplicou-se o Beck Depression Inventory (BDI), escala que avalia a presença e intensidade de sintomas depressivos. Resultados Os grupos apresentavam características sociodemográficas e clínicas semelhantes, exceto quanto à escolaridade. A média de pontos do BDI no grupo HD foi maior que no grupo DP: 12,53 versus 11,13 (p = 0,352). A presença de sintomas depressivos no grupo em HD ocorreu em 36,7% dos pacientes contra apenas 23,3% do grupo DP (OR 1,9 [IC 0,61 – 5,86]). Não houve diferenças quanto às variáveis, exceto escolaridade. Dos pacientes com ensino fundamental I, 52,9% apresentaram sintomas. Nos pacientes com ensino médio ou superior, a ocorrência de sintomas depressivos foi 20% (p = 0,051). Conclusão Houve tendência à ocorrência de sintomas depressivos em pacientes em HD quando comparados aos pacientes em DP. O risco dessa ocorrência foi quase duas vezes maior nos em HD. Menor escolaridade associou-se à ocorrência de sintomas. As médias da pontuação do BDI nos dois grupos não foram significantes.

OBJECTIVE: Detect of cardiac alterations in children with AIDS and compare their evolution with the administration of only one anti-retroviral and the recent cases who received drugs in combination. METHODS: We prospectively studied 47 children in 3 groups: group 1, 20 cases treated only with zidovudine; group 2, 10 patients treated initially with zidovudine and later with a combination of drugs and in group 3, 17 patients, who receiced two or three since the beginning. In all patients it was done chest X-ray, EKG and echocardiography every 6 months and after death complete pathological study. RESULTS: Among the 45 patients cases 26 (57%) were index cases. Malnutrition, diarrhea tachycardia, signs of congestive heart failure, pericardial effusion, abnormal ventricular repolarization and arrhythmias were more frequent in group 1. Echocardiographic abnormalities were present in 10 (50%) children of group 1. They were less frequent in the others two groups. In regard to the outcome in group 1, two patients had worsening of sings of cardiomyopaty and 4 died. Cardiac dysfunction in all cases of group 2 and 3 improved with the medication. CONCLUSION:- The children who received combination and their cardiac alterations had more favorable outcome than those who received only one drug.
