A nossa sociedade está, visivelmente, em busca de padrões éticos, de padrões de melhoria social, de padrões de melhoria económica. Acredita-se, pois que a ética e a transparência podem se constituir em elementos essenciais no sentido de assegurar que qualquer governo atinja os seus objectivos com eficiência, efectividade e dentro dos preceitos legais da ordem democrática. Tendo o Governo de Cabo Verde eleito a ética e a transparência na gestão da coisa pública como fundamentos de uma “boa governação”, é imprescindível criar mecanismos de controlo, a fim de verificar se tal determinação está sendo efectivada. Para tanto, é fundamental que haja, uma análise criteriosa, considerando não só os aspectos da administração, mas principalmente, os aspectos políticos e sociais envolvidos. Isto se justifica, ademais, pelo facto de haver mais eficiência governativa quando as instituições funcionam com regras, com transparência, com ética, com comprometimento ao serviço público, com profissionalismo e quando estão sujeitas ao controlo social e aos mecanismos de "check and balances". Em Cabo Verde existe uma assimetria entre o governo, a oposição, e a sociedade no que diz respeito à ética e à transparência na Administração Pública. Esta constatação nos motivou a efectuar uma reflexão sobre o assunto, não no intuito de julgar mas, sim, de pesquisar os actos e as regras praticadas na Administração Pública Cabo-verdiana, ou seja, pelo Poder Executivo Cabo-Verdiano, posto que tais actos, em última instância, visam assegurar o alcance do bem comum no âmbito referido. Assim sendo, o objectivo deste trabalho é reflectir sobre a questão da ética e da transparência na Administração Pública Cabo-verdiana, desenvolvendo um pensamento crítico e fundamentado a respeito do tema. O que se busca, neste caso, é assinalar propostas práticas face ao modelo actual de Reforma Administrativa em Cabo Verde, contribuindo para o seu aperfeiçoamento. Em termos de contexto, realça-se que o estudo abrangerá a vertente da Administração Central. Para alcançar o objectivo deste trabalho, fez-se uma pesquisa bibliográfica sobre a ética e Transparência na Administração Pública e sobre diversas teorias relacionadas com a ética nas organizações. Utilizou-se o método inquisitivo baseado em 5 questionário com perguntas fechadas e abertas para se obter a percepção dos dirigentes, funcionários públicos e utentes da Administração Pública, bem como dos políticos que hoje actuam neste âmbito. Os dados recolhidos proporcionam indicações evidentes que é preciso mudar de paradigma no que diz respeito à Ética e à Transparência na Administração Pública Cabo-verdiana.

No intuito verificar a dinâmica da ocorrência do paludismo no concelho de Santa Cruz, Santiago, Cabo Verde foi realizada uma análise retrospectiva de dados reportados para o Concelho no período de 2001 a 2006. Para efeitos de análise foram também registados os fatores sexo, idade, local de residência, origem da infecção e número de óbitos. Os dados registados foram descritos através de medidas de tendência central e de variabilidade e posteriormente comparados. A análise dos resultados demonstrou a presença de 18 casos no período em estudo e uma diminuição da ocorrência a partir de 2004. Óbitos não foram registados. A ocorrência foi maior nas zonas de Ribeira Seca, Órgãos, e Laranjeira, seguidas de Caiumbra e Santa Cruz. A maioria dos casos foi autóctone, com ocorrência em indivíduos tanto do sexo feminino quanto do masculino com 19±13,4 anos de idade. Nas áreas visitadas pode se constatar a falta de retretes, reduzido número de habitações com lajes, problemas de escolaridade, falta de contentores e de qualquer tipo de tratamento do lixo. A falta de conhecimento sobre a doença também ficou patente. Estes resultados apontam para a necessidade da realização de estudos que identifiquem a dinâmica desses factores quando relacionados à doença.

To determine the role of Chlamydia trachomatis in miscarriage, we prospectively collected serum, cervicovaginal swab specimens, and placental samples from 386 women with and without miscarriage. Prevalence of immunoglobulin G against C. trachomatis was higher in the miscarriage group than in the control group (15.2% vs. 7.3%; p = 0.018). Association between C. trachomatis-positive serologic results and miscarriage remained significant after adjustment for age, origin, education, and number of sex partners (odds ratio 2.3, 95% confidence interval 1.1-4.9). C. trachomatis DNA was more frequently amplified from products of conception or placenta from women who had a miscarriage (4%) than from controls (0.7%; p = 0.026). Immunohistochemical analysis confirmed C. trachomatis in placenta from 5 of 7 patients with positive PCR results, whereas results of immunohistochemical analysis were negative in placenta samples from all 8 negative controls tested. Associations between miscarriage and serologic/molecular evidence of C. trachomatis infection support its role in miscarriage.

Systematics, phylogeny and geographical distribution of the South American species of Centris (Paracentris) Cameron, 1903, and Centris (Penthemisia) Moure, 1950, including a phylogenetic analysis of the "Centris group" sensu Ayala, 1998 (Hymenoptera, Apoidea, Centridini). A cladistic analysis with the objective of testing the hypothesis of monophily of Centris (Paracentris) Cameron, 1903, and of studying its phylogenetic relationships with the other subgenera that belong to the Centris group, sensu Ayala, 1998, and the relationships among the species that occur in South America, is presented. Centris (Paracentris) is a group of New World bees of amphitropical distribution, especially diversified in the Andes and in the xeric areas of South and North America. Thirty-one species were included in the analysis, four considered as outgroup, and 49 characters, all from external morphology and genitalia of adult specimens. Parsimony analyses with equal weights for the characters and successive weighting were performed with the programs NONA and PAUP, and analyses of implied weighting with the program PeeWee. The strict consensus among the trees obtained in all the analyses indicates that C. (Paracentris), as previously recognized, is a paraphyletic group. In order to eliminate that condition, the subgenera C. (Acritocentris), C. (Exallocentris) and C. (Xerocentris), all described by SNELLING (1974) are synonymized under C. (Paracentris). The subgenus C. (Penthemisia) Moure, 1950, previously considered a synonym of C. (Paracentris), is reinstated, but in a more restricted sense than originally proposed and with the following species: Centris brethesi Schrottky, 1902; C. buchholzi Herbst, 1918; C. chilensis (Spinola, 1851), C. mixta mixta Friese, 1904, and C. mixta tamarugalis Toro & Chiappa, 1989. Centris mixta, previously recognized as the only South American species of the subgenus C. (Xerocentris), a group supposedly amphitropical, came out as the sister-species of C. buchholzi. The following South American species were recognized under Centris (Paracentris): Centris burgdorfi Friese, 1901; C. caelebs Friese, 1900; C. cordillerana Roig-Alsina, 2000; C. euphenax Cockerell, 1913; C. flavohirta Friese, 1900; C. garleppi (Schrottky, 1913); C. klugii Friese, 1900; C. lyngbyei Jensen-Haarup, 1908; C. mourei Roig-Alsina, 2000; C. neffi Moure, 2000; C. nigerrima (Spinola, 1851); C. toroi sp. nov.; C. tricolor Friese, 1900; C. unifasciata (Schrottky, 1913), and C. vogeli Roig-Alsina, 2000. The relationships among the subgenera of the "Centris group" were: (Xanthemisia (Penthemisia (Centris s. str. - Paracentris))). Centris xanthomelaena Moure & Castro 2001, an endemic species of the Caatinga and previously considered a C. (Paracentris), came out as the sister group of C. (Centris) s. str. A new species of C. (Paracentris) from Chile is described: Centris toroi sp. nov. Lectotypus designations and redescriptions are presented for Centris burgdorfi, C. caelebs, C. lyngbyei, C. tricolor, C. autrani Vachal, 1904 and C. smithii Friese, 1900. New synonyms proposed: C. buchholzi Herbst, 1918 = Centris wilmattae Cockerell, 1926 syn. nov.; C. caelebs Friese, 1900 = Paracentris fulvohirta Cameron, 1903. The female of C. vogeli Roig-Alsina, 2000 and the male of C. xanthomelaena are described.

The genus Anthidium Fabricius in the South America: key for the species, descriptive notes, and geographical distribution (Hymenoptera, Megachilidae, Anthidiini). The Anthidiini, in South America, is represented by a single genus Anthidium Fabricius, 1804 (type-species: Apis manicata Linnaeus, 1758). Thirty nine species are treated in this paper, as follows: Anthidium alsinai Urban, 2001; A. andinum Joergensen, 1912; A. anurospilum Moure, 1957 nom. reval. Some taxonomic comments are made for each species, and new data on geographic distribution are also given. The females of A. andinum, A. igori, A. rozeni and the male of A. anurospilum are described for the first time. Identification keys (for males and females), as well as illustrations for almost all species, are provided.

BACKGROUND: Data on the association between vitamin D and upper digestive tract neoplasms are limited. METHODS: In two case-control studies in Italy, we examined the relation between dietary vitamin D intake and squamous cell carcinoma of the esophagus (SCCE; 304 cases) and oral/pharyngeal cancer (804 cases). Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated by multiple logistic regression. RESULTS: Adjusted ORs for SCCE and oral/pharyngeal cancer were 0.58 (95% CI 0.39-0.86) and 0.76 (95% CI 0.60-0.94), respectively, for the highest tertile of vitamin D intake. Using a reference group of those in the highest tertile of vitamin D who were never/former smokers, ORs were 8.7 (95% CI 4.1-18.7) for SCCE and 10.4 (95% CI 6.9-15.5) for oral/pharyngeal cancer among heavy smokers in the lowest vitamin D tertile; similarly, compared with those in the highest tertile of vitamin D who drank <3 alcoholic drinks/day, corresponding ORs were 41.9 (95% CI 13.7-128.6) for SCCE and 8.5 (95% CI 5.7-12.5) for oral/pharyngeal cancer, among heavy alcohol drinkers in the lowest vitamin D tertile. CONCLUSION: We observed inverse associations between dietary vitamin D intake and risk of SCCE and, perhaps, oral/pharyngeal cancer, which were most pronounced among heavy current smokers and heavy consumers of alcohol.

Background Folate deficiency leads to DNA damage and inadequate repair, caused by a decreased synthesis of thymidylate and purines. We analyzed the relationship between dietary folate intake and the risk of several cancers. Patients and methods The study is based on a network of case-control studies conducted in Italy and Switzerland in 1991-2009. The odds ratios (ORs) for dietary folate intake were estimated by multiple logistic regression models, adjusted for major identified confounding factors. Results For a few cancer sites, we found a significant inverse relation, with ORs for an increment of 100 μg/day of dietary folate of 0.65 for oropharyngeal (1467 cases), 0.58 for esophageal (505 cases), 0.83 for colorectal (2390 cases), 0.72 for pancreatic (326 cases), 0.67 for laryngeal (851 cases) and 0.87 for breast (3034 cases) cancers. The risk estimates were below unity, although not significantly, for cancers of the endometrium (OR = 0.87, 454 cases), ovary (OR = 0.86, 1031 cases), prostate (OR = 0.91, 1468 cases) and kidney (OR = 0.88, 767 cases), and was 1.00 for stomach cancer (230 cases). No material heterogeneity was found in strata of sex, age, smoking and alcohol drinking. Conclusions Our data support a real inverse association of dietary folate intake with the risk of several common cancers.

We investigate the determinants of regional development using a newly constructed database of 1569 sub-national regions from 110 countries covering 74 percent of the world s surface and 97 percent of its GDP. We combine the cross-regional analysis of geographic, institutional, cultural, and human capital determinants of regional development with an examination of productivity in several thousand establishments located in these regions. To organize the discussion, we present a new model of regional development that introduces into a standard migration framework elements of both the Lucas (1978) model of the allocation of talent between entrepreneurship and work, and the Lucas (1988) model of human capital externalities. The evidence points to the paramount importance of human capital in accounting for regional differences in development, but also suggests from model estimation and calibration that entrepreneurial inputs and possibly human capital externalities help understand the data.

To complement the existing treatment guidelines for all tumour types, ESMO organises consensus conferences to focus on specific issues in each type of tumour. The Second ESMO Consensus Conference on Lung Cancer was held on 11-12 May 2013 in Lugano. A total of 35 experts met to address several questions on management of patients with non-small-cell lung cancer (NSCLC) in each of four areas: pathology and molecular biomarkers, early stage disease, locally advanced disease and advanced (metastatic) disease. For each question, recommendations were made including reference to the grade of recommendation and level of evidence. This consensus paper focuses on recommendations for pathology and molecular biomarkers in relation to the diagnosis of lung cancer, primarily non-small-cell carcinomas.

BACKGROUND: This study assessed whether breast cancer (BC) patients express similar levels of needs for equivalent severity of symptoms, functioning difficulties, or degrees of satisfaction with care aspects. BC patients who did (or not) report needs in spite of similar difficulties were identified among their sociodemographic or clinical characteristics. PATIENTS AND METHODS: Three hundred and eighty-four (73% response rate) BC patients recruited in ambulatory or surgery hospital services completed the European Organisation for Research and Treatment of Cancer Quality of Life questionnaire (EORTC QLQ)-C30 quality of life [health-related quality of life (HRQOL)], the EORTC IN-PATSAT32 (in-patient) or OUT-PATSAT35 (out-patient) satisfaction with care, and the supportive care needs survey short form 34-item (SCNS-SF34) measures. RESULTS: HRQOL or satisfaction with care scale scores explained 41%, 45%, 40% and 22% of variance in, respectively, psychological, physical/daily living needs, information/health system, and care/support needs (P < 0.001). BC patients' education level, having children, hospital service attendance, and anxiety/depression levels significantly predicted differences in psychological needs relative to corresponding difficulties (adjusted R(2) = 0.11). Medical history and anxiety/depression levels significantly predicted differences in information/health system needs relative to degrees of satisfaction with doctors, nurses, or radiotherapy technicians and general satisfaction (adjusted R(2) = 0.12). Unmet needs were most prevalent in the psychological domains across hospital services. CONCLUSIONS: Assessment of needs, HRQOL, and satisfaction with care highlights the subgroups of BC patients requiring better supportive care targeting.

Revisão do gênero Antiteuchus Dallas (Heteroptera, Pentatomidae, Discocephalinae). O gênero Antiteuchus Dallas, 1851 com 46 espécies descritas é revisado; as espécies são redescritas com base na morfologia externa da genitália do macho e em algumas características da morfologia geral do corpo. Neodine Kirkaldy, 1909 é considerado sinônimo junior de Antiteuchus. As espécies foram subdivididas em seis grupos aqui propostos: A. amplus, A. supinatus, A. peruensis, A. tesselatus, A. marmoratus e A. mixtus. Oito novas espécies são descritas: A. amapensis sp. nov., A. beckerae sp. nov., A. doesburgi sp. nov., A. exiguus sp. nov., A. ledeburi sp. nov., A. marini sp. nov., A. melanicus sp. nov. and A. similis sp. nov. Nove espécies e uma subespécie são sinonimizadas. Antiteuchus varians Ruckes, 1964 é considerada sinônimo júnior de A. pallescens Stål, 1868; A. englemani Rolston, 1993 de A. amplus (Walker, 1867); A. tripterus limbativentris Ruckes, 1964 e A. minor Engleman, 1983 de A. tripterus (Fabricius, 1787); A. fuscus (Ruckes, 1959), A. piceus (Palisot de Beauvois, 1805), A. subgibbus Engleman, 1983, A. subimpunctatus Ruckes, 1964, A. unicolor (Westwood, 1837) e A. variolosus (Westwood, 1837) de A. mixtus (Fabricius, 1787). Antiteuchus tatei (Ruckes, 1958) é considerada species inquirenda. Empicoris marmoreus Spinola, 1837 é colocada em incertae sedis e tratada como species inquirenda. O macho de A. pictus, até agora desconhecido, é descrito. Chaves de identificação para os machos das espécies de Antiteuchus e para os grupos de espécies são apresentadas.

BACKGROUND: Venous thromboembolism (VTE) prophylaxis remains underutilized, particularly in cancer patients. We explored clinical predictors of prophylaxis in hospitalized cancer patients before the onset of acute VTE. METHODS: In the SWiss Venous ThromboEmbolism Registry, 257 cancer patients (61 +/- 15 years) with acute VTE and prior hospitalization for acute medical illness or surgery within 30 days (91% were at high risk with Geneva VTE risk score > or =3) were enrolled. RESULTS: Overall, 153 (60%) patients received prophylaxis (49% pharmacological and 21% mechanical) before the onset of acute VTE. Outpatient status at the time of VTE diagnosis [odds ratio (OR) 0.31, 95% confidence interval (CI) 0.18-0.53], ongoing chemotherapy (OR 0.51, 95% CI 0.31-0.85), and recent chemotherapy (OR 0.53, 95% CI 0.32-0.88) were univariately associated with the absence of VTE prophylaxis. In multivariate analysis, intensive care unit admission within 30 days (OR 7.02, 95% CI 2.38-20.64), prior deep vein thrombosis (OR 3.48, 95% CI 2.14-5.64), surgery within 30 days (OR 2.43, 95% CI 1.19-4.99), bed rest >3 days (OR 2.02, 95% CI 1.08-3.78), and outpatient status (OR 0.38, 95% CI 0.19-0.76) remained the only independent predictors of thromboprophylaxis. CONCLUSIONS: Although most hospitalized cancer patients were at high risk, 40% did not receive any prophylaxis before the onset of acute VTE. There is a need to improve thromboprophylaxis in cancer patients, particularly in the presence of recent or ongoing chemotherapy.

Comunidade de abelhas (Hymenoptera, Apoidea) em ecossistema de dunas na Praia de Panaquatira, São José de Ribamar, Maranhão, Brasil. Foi analisada a estrutura da comunidade de Apoidea de uma área restrita de dunas primárias em São José de Ribamar, Maranhão, Brasil. Amostragens foram realizadas quinzenalmente durante um ano com metodologia padronizada totalizando 24 coletas. As coletas ocorreram no período das 12:00 às 18:00 h no primeiro dia e das 6:00 às 12:00 h no segundo, realizadas por dois coletores. Um total de 3305 indivíduos de 31 espécies pertencentes a quatro famílias (Apidae>Halictidae>Megachilidae>Andrenidae em número de indivíduos) foram coletadas nas flores. Centris com 14 espécies e 890 indivíduos foi o gênero mais rico e abundante. O padrão de abundância e riqueza foi bastante semelhante ao de outros habitats de dunas no nordeste brasileiro. Das espécies amostradas, 61% foram representadas por menos de 36 indivíduos e apenas 5 espécies foram muito abundantes com mais de 177 indivíduos: Apis mellifera Linnaeus, Centris (Centris) leprieuri Spinola, Eulaema (Apeulema) nigrita Lepeletier, Eufriesea surinamensis Linnaeus e Xylocopa (Neoxylocopa) cearensis Ducke. As abelhas estiveram presentes durante todo o ano, apresentando picos de abundância no período de maior precipitação. A atividade diária foi maior entre 06:00 e 11:00 h, quando a temperatura aumentava e a umidade relativa decrescia.
