Training is a crucial tool for building the capacity necessary for prevention and control of cardiovascular diseases (CVDs) in developing countries. This paper summarizes some features of a 2-week workshop aimed at enabling local health professionals to initiate a comprehensive CVD prevention and control program in a context of limited resources. The workshops have been organized in the regions where CVD prevention programs are being contemplated, in cooperation with health authorities of the concerned regions. The workshop's content includes a broad variety of issues related to CVD prevention and control, and to program development. Strong emphasis is placed on "learning by doing," and groups of 5-6 participants conduct a small-scale epidemiological study during the first week; during the second week, they draft a virtual program of CVD prevention and control adapted to the local situation. This practice-oriented workshop focuses on building expertise among anticipated key players, strengthening networks among relevant health professionals, and advocating the urgent need to tackle the emerging CVD epidemic in developing countries.

Background: Exposure to fine particulate matter air pollutants (PM2.5) affects heart rate variability parameters, and levels of serum proteins associated with inflammation, hemostasis and thrombosis. This study investigated sources potentially responsible for cardiovascular and hematological effects in highway patrol troopers. Results: Nine healthy young non-smoking male troopers working from 3 PM to midnight were studied on four consecutive days during their shift and the following night. Sources of in-vehicle PM2.5 were identified with variance-maximizing rotational principal factor analysis of PM2.5-components and associated pollutants. Two source models were calculated. Sources of in-vehicle PM2.5 identified were 1) crustal material, 2) wear of steel automotive components, 3) gasoline combustion, 4) speed-changing traffic with engine emissions and brake wear. In one model, sources 1 and 2 collapsed to a single source. Source factors scores were compared to cardiac and blood parameters measured ten and fifteen hours, respectively, after each shift. The "speed-change" factor was significantly associated with mean heart cycle length (MCL, +7% per standard deviation increase in the factor score), heart rate variability (+16%), supraventricular ectopic beats (+39%), % neutrophils (+7%), % lymphocytes (-10%), red blood cell volume MCV (+1%), von Willebrand Factor (+9%), blood urea nitrogen (+7%), and protein C (-11%). The "crustal" factor (but not the "collapsed" source) was associated with MCL (+3%) and serum uric acid concentrations (+5%). Controlling for potential confounders had little influence on the effect estimates. Conclusion: PM2.5 originating from speed-changing traffic modulates the autonomic control of the heart rhythm, increases the frequency of premature supraventricular beats and elicits proinflammatory and pro-thrombotic responses in healthy young men. La hipertensión es la enfermedad cardiovascular más frecuente. En la consulta dental puede detectarse primariamente y referirla para su correcto manejo. La monitorización rutinaria de la presión sanguínea y la valoración de los factores de riesgo con una completa anamnesis puede ayudar a prevenir las emergencias médicas durante el tratamiento dental en pacientes insuficientemente o no tratados y la mortalidad y morbilidad de alteraciones como la insuficiencia renal o el fallo cardíaco, también pueden ser reducidas a través de la detección temprana de dicha alteración.

Background The global mortality caused by cardiovascular disease increases with weight. The Framingham study showed that obesity is a cardiovascular risk factor independent of other risks such as type 2 diabetes mellitus, dyslipidemia and smoking. Moreover, the main problem in the management of weight-loss is its maintenance, if it is achieved. We have designed a study to determine whether a group motivational intervention, together with current clinical practice, is more efficient than the latter alone in the treatment of overweight and obesity, for initial weight loss and essentially to achieve maintenance of the weight achieved; and, secondly, to know if this intervention is more effective for reducing cardiovascular risk factors associated with overweight and obesity. Methods This 26-month follow up multi-centre trial, will include 1200 overweight/obese patients. Random assignment of the intervention by Basic Health Areas (BHA): two geographically separate groups have been created, one of which receives group motivational intervention (group intervention), delivered by a nurse trained by an expert phsychologist, in 32 group sessions, 1 to 12 fortnightly, and 13 to 32, monthly, on top of their standard program of diet, exercise, and the other (control group), receiving the usual follow up, with regular visits every 3 months. Discussion By addressing currently unanswered questions regarding the maintenance in weight loss in obesity/overweight, upon the expected completion of participant follow-up in 2012, the IMOAP trial should document, for the first time, the benefits of a motivational intervention as a treatment tool of weight loss in a primary care setting.

BACKGROUND: We assessed the impact of a multicomponent worksite health promotion program for0 reducing cardiovascular risk factors (CVRF) with short intervention, adjusting for regression towards the mean (RTM) affecting such nonexperimental study without control group. METHODS: A cohort of 4,198 workers (aged 42 +/- 10 years, range 16-76 years, 27% women) were analyzed at 3.7-year interval and stratified by each CVRF risk category (low/medium/high blood pressure [BP], total cholesterol [TC], body mass index [BMI], and smoking) with RTM and secular trend adjustments. Intervention consisted of 15 min CVRF screening and individualized counseling by health professionals to medium- and high-risk individuals, with eventual physician referral. RESULTS: High-risk groups participants improved diastolic BP (-3.4 mm Hg [95%CI: -5.1, -1.7]) in 190 hypertensive patients, TC (-0.58 mmol/l [-0.71, -0.44]) in 693 hypercholesterolemic patients, and smoking (-3.1 cig/day [-3.9, -2.3]) in 808 smokers, while systolic BP changes reflected RTM. Low-risk individuals without counseling deteriorated TC and BMI. Body weight increased uniformly in all risk groups (+0.35 kg/year). CONCLUSIONS: In real-world conditions, short intervention program participants in high-risk groups for diastolic BP, TC, and smoking improved their CVRF, whereas low-risk TC and BMI groups deteriorated. Future programs may include specific advises to low-risk groups to maintain a favorable CVRF profile.

Objective: Tachycardia is associated with hypertension and is a predictor of cardiovascular events. The predictive effect of tachycardia might reflect its connection with hypertension. In this analysis of 15,245 VALUE study patients we explore whether tachycardia predicts cardiovascular endpoints in high risk hypertension and whether the in-trial blood pressure lowering modified the tachycardia - related risk. Methods: Heart rate from ECG readings at baseline and annually throughout the trial. Results: In the Cox Regression analysis the primary endpoint hazard ratio for a 10 beats per minute increment of baseline heart rate was 1.16 (1.12-1.2) p < 0.0001, 1.17 (1.13-1.22) p < 0.0001 and 1.22 (1.18-1.27) p < 0.0001 unadjusted, adjusted for baseline blood pressure and for blood pressure plus risk factors, respectively. Primary endpoints strikingly increased in the highest quintile of baseline heart rate (=/>79 beats). Primary endpoints in the highest heart rate quintile were 30 % higher in first, 55 % in second, 55 % in third, 52 % in fourth and 46 % in the fifth year of the study. The in-trial heart rate was also a potent predictor. The primary endpoint hazard ratios of highest heart rate quintile versus pooled lower 4 quintiles was (1.34-1.66) p < 0.0001 unadjusted, 1.52 (1.36-1.69) p <0.0001 adjusted for baseline blood pressure and risk factors and 1.52 (1.36-1.69) p < 0.0001 further adjusted for in trial pressure. The increase of primary events in the upper quintile of in-trial heart rate was 68% in the group with good and 63% in the group with inadequate blood pressure control (both p < 0.0001 by log rank test). Conclusions: 1./ Tachycardia is a short term marker and a long term predictor of adverse event in high risk hypertension. 2./ Tachycardia contributes to the residual cardiovascular risk regardless of the degree of BP control. We hypothesize heart rate lowering with appropriate drugs may further decrease the cardiovascular risk in patients with high risk hypertension and tachycardia.

Una gran parte de las empresas de todo el mundo, y especialmente las PYMES occidentales, están perdiendo mucho dinero, y como consecuencia de ello gran parte de su competitividad potencial, debido a que los costes totales de calidad en que incurren, son inaceptablemente elevados. En la mayoría de los casos no existe modelo coherente alguno para gestionar dichos costes, y en otros, los modelos aplicados se utilizan de forma parcial, rutinaria y con escaso convencimiento. El resultado de ello, como cabría esperar, es obviamente decepcionante. Evidentemente hay también empresas, generalmente las de gran tamaño (líderes del mercado y multinacionales), conocedoras de la gran importancia que para la disminución de sus costes supone disponer de un sistema adecuado de gestión de los costes de calidad. Invierten sistemáticamente en ello, obteniendo resultados satisfactorios, tanto en sus cuentas de resultados como en el nivel de motivación de sus empleados. Éstos, conscientes del interés de la Dirección por mejorar la calidad de sus productos y servicios, se sienten de alguna manera partícipes de ese proyecto, entendiendo que, en definitiva, el éxito de la empresa es también el suyo propio. Hemos analizado las causas de esta situación, utilizando las experiencias plasmadas en numerosos estudios llevados a cabo por expertos internacionales. Una vez localizados, acotados y definidos los aspectos débiles de los procedimientos aplicados en la actualidad, hemos diseñado un par de nuevos modelos de gestión de los costes totales de calidad, que tienen la virtud de haber eliminado aquellas etapas que, en la práctica, se han mostrado ineficaces, e incorporando otras, que han resultado ser muy útiles en diversos campos de la gestión de la calidad. El primero de los modelos, el más simple, puede ser utilizado para gestionar presupuestos de costes de calidad sencillos, mientras que el segundo contempla la posibilidad de lograr una mayor exactitud en las cifras de las previsiones, al tiempo que introduce alguna situación de incertidumbre.

La hipertensión arterial (HTA) constituye uno de los problemas de salud pública más importantes por su elevada prevalencia, sus complicaciones, alta mortalidad y morbilidad y el coste que determina su control y tratamiento. Es un factor de riesgo importante para la enfermedad cardiovascular y cerebro vascular, ya que favorece la formación de placas ateroscleróticas. La HTA está presente en ambos sexos y a cualquier edad provocando una disminución en la expectativa de vida. El hábito tabáquico, la hipertensión arterial, los niveles de colesterol, la obesidad y la inactividad física, el estrés, alcohol y consumo de sal, son considerados factores de riesgo modificables. El control de la hipertensión arterial, junto con los demás factores de riesgo que provocan alteraciones cardiovasculares, es probablemente uno de los mayores problemas de salud pública en el mundo. El objetivo de este trabajo es concienciar a los pacientes hipertensos que acuden al centro de salud de Fraga sobre la importancia de adoptar hábitos de vida saludable y de evitar factores de riesgo que empeoran su enfermedad, a través de la creación de un programa de educación sanitaria.

A les línies de producció de l’empresa Laboratorios Hipra, s’ha plantejat imprimir els codis de les caixes mitjançant nous làsers amb la tècnica de delineat làser, amb aquests nous models de làser es vol imprimir codis datamatrix. Per tal que això sigui possible, és molt important poder tenir un control de les impressions realitzades pel làser, ja que en cas contrari, podrien empaquetar-se caixes amb un marcatge erroni. S’ha dissenyatuna aplicació capaç de verificar i controlar les impressions realitzades pels làsers. Aquesta aplicació ha de ser capaç de verificar i controlar les caixes marcades tant amb codis de barres com amb datamatrix, a més a més de mostrar totes les dades dels marcatges per una pantalla SCADA. L’aplicació també incorpora un sistema d’expulsió de les caixes amb un marcatge d’inferior qualitat a la desitjada en cada moment

The Swiss Federal Office of Public Health's (FOPH) updated its recommendations on fat consumption in 2013. The report recommends that maximum 10% of the daily caloric intake should come from saturated fatty acids, and the total fat intake should account for 20-35%. There is no limitation to dietary cholesterol consumption. Recent studies have shown that replacing consumption of saturated fatty acids by unsaturated fatty acids is more beneficial in terms of cardiovascular prevention than a low fat diet. The purpose of this article is to review the FOPH recommendations on dietary fat as well as the current evidence about their cardiovascular benefit, and to provide a translation of these scientific recommendations into clinical advice.

BACKGROUND: Ischemic stroke is the leading cause of mortality worldwide and a major contributor to neurological disability and dementia. Terutroban is a specific TP receptor antagonist with antithrombotic, antivasoconstrictive, and antiatherosclerotic properties, which may be of interest for the secondary prevention of ischemic stroke. This article describes the rationale and design of the Prevention of cerebrovascular and cardiovascular Events of ischemic origin with teRutroban in patients with a history oF ischemic strOke or tRansient ischeMic Attack (PERFORM) Study, which aims to demonstrate the superiority of the efficacy of terutroban versus aspirin in secondary prevention of cerebrovascular and cardiovascular events. METHODS AND RESULTS: The PERFORM Study is a multicenter, randomized, double-blind, parallel-group study being carried out in 802 centers in 46 countries. The study population includes patients aged > or =55 years, having suffered an ischemic stroke (< or =3 months) or a transient ischemic attack (< or =8 days). Participants are randomly allocated to terutroban (30 mg/day) or aspirin (100 mg/day). The primary efficacy endpoint is a composite of ischemic stroke (fatal or nonfatal), myocardial infarction (fatal or nonfatal), or other vascular death (excluding hemorrhagic death of any origin). Safety is being evaluated by assessing hemorrhagic events. Follow-up is expected to last for 2-4 years. Assuming a relative risk reduction of 13%, the expected number of primary events is 2,340. To obtain statistical power of 90%, this requires inclusion of at least 18,000 patients in this event-driven trial. The first patient was randomized in February 2006. CONCLUSIONS: The PERFORM Study will explore the benefits and safety of terutroban in secondary cardiovascular prevention after a cerebral ischemic event.

L’ús generalitzat dels implants coclears ha possibilitat l’accés al món sonor a moltes persones amb sordesa pregona, facilitant la seva competència comunicativa en llenguatge oral. Objectiu: Amb aquest article pretenem realitzar una revisió de la descripció que tradicionalment ha caracteritzat la veu dels infants amb sordesa. Posteriorment, analitzem l’evolució de la qualitat vocal obtinguda amb una mostra de 12 infants (8 nenes i 4 nens) amb sordesa pregona congènita i una mitjana d’edat de 6 anys i 1 mes, usuaris d’Implant Coclear (IC), als quals vam realitzar una avaluació inicial de la seva qualitat vocal i una avaluació final, dos anys més tard. Els resultats es comparen amb els obtinguts per un grup control d’infants oients. Resultats i conclusions: Els resultats mostren com els perfils obtinguts pels infants sords usuaris d’IC evolucionen molt favorablement, essent els valors obtinguts més similars als del grup control en l’avaluació final que en la inicial. Finalment, ens qüestionem la validesa d’utilitzar alguns paràmetres acústics com a indicadors de la qualitat vocal dels infants amb pèrdua auditiva i sense problemes laringis.
