The goal of this study is to characterize the clinical outline of patients in postoperative of bariatric surgery attended in an Intensive Care Unit and identify the nursing diagnosis presented by these patients in accordance to Taxonomy II of North American Nursing Diagnosis Association (NANDA). This is a transversal, descriptive and prospective study that was desenvolved at the Intensive Care Unit of the Clinical Hospital of Botucatu Medical School – UNESP. The population was composed by all the patients who were submitted to the bariatric surgery and were attended at the Intensive Care Unit in the period between June and August of 2010, totalizing 13 patients. The data were collected based on the Nursing History that is composed by the Anamnesis, Physical Examination and Complementary Exams. The results were worked out: a) among the 13 patients studied, 10 are women (76,9%) and 3 men (23,1%); the majority is between 30 and 40 years old; 11 people of 13 are morbid obese, which means that they have Body Mass Index higher than 40 kg/m2; b) It was identified 22 nursing diagnosis and 14 of these were found in 100% of the patients. They belong to the dominions of Activity/Rest, Comfort, Elimination and Changing, Security and Protection, Functions and Relationships. Therefore, in relation to the factors, it was conclude that nursing diagnosis help nurses to understand the unsettled human answers or the risks to disturbance in an individual way, which contributes to plan specific interventions to these patients. It was possible to verify that the stage of the nursing diagnosis that is part of the nursing process should be executed by the nurses systematically, because it has been realized by the nurses’ clinical idea, but not in a systematical way

O objetivo do estudo foi analisar o efeito da tarefa dupla na variabilidade do andar livre e adaptativo em pacientes com demência de Alzheimer. Foram avaliados 30 indivíduos de ambos os sexos. Os participantes realizaram 5 tentativas para cada condição de andar (livre e adaptativo, com tarefa dupla e sem tarefa dupla), totalizando 20 tentativas.As variáveis espaço-temporais foram coletadas através de um sistema optoeletrônico. Para o andar livre foram considerados os cinco passos centrais da passarela. Para o andar adaptativo foram considerados os três passos anteriores ao obstáculo, o passo de ultrapassagem e o primeiro passo após a ultrapassagem. Os valores do desvio-padrão de cada variável de cada indivíduo nas 5 tentativas para cada condição de andar foram utilizados para calcular a variabilidade dos seguintes parâmetros: comprimento, largura, duração da fase de suporte simples e de duplo suporte, e velocidade de cada passo. Para verificar as diferenças entre as condições experimentais, foram empregadas ANOVAs, com medidas repetidas para a condição experimental. Os resultados indicaram que a tarefa dupla teve pouco efeito na variabilidade do andar livre de pacientes com DA. As variações ocorreram com maior frequência quando a tarefa dupla foi exigida concomitante com o obstáculo, deixando claro que os efeitos da doença influenciam no planejamento da ação. Conclui-se que a tarefa executiva de contagem regressiva não exerceu influência nas variáveis da marcha livre e que o andar adaptativo com contagem apresentou resultados com maior variabilidade, sendo mais desafiador e exigindo maiores ajustes dos pacientes

The hardness has an important role in quality control, in research studies and metallurgical and mechanical specification, selection and comparison of various materials. This property is of extreme importance in the oil industry because it is a determining factor to ascertain the safety of the material used in pressure vessels and pipelines. Due to the inability to stop the equipment while checking the hardness, the hardness testers are widely used portable method UCI, its great advantage is the fact that an essay fast, simple realization and not be considered a non-destructive testing with a good relationship money. The objective is to determine if there is significant difference in hardness measurements between 80 and 1200 sandpaper using a portable hardness tester UCI method, the material applied in gas storage spheres composition ASTM 516 Gr 70. After determining the number of homogeneity, we performed the hardness profile to isolate the major factors influencing the hardness part: cold rolling and segregation of impurities. Factors Cooling and sanding were analyzed using the method of design of experiments (DOE), in which it was demonstrated that neither variables nor their interactions, has significant influence on the hardness measurements by portable MIC 10. This fact will lead to reduction in time and cost for surface preparation

The society continues demonstrating its need regarding the products and services, even though it becomes increasingly value the protection of the environment. In this context, environmental management is defined as a management method that emphasizes to continuously improve results and promote sustainable development. Moreover it has as a tool the laws which provide the benchmarks and mechanisms for companies to adapt their activities in such a way that does not alter the quality of the environment. In this sense, in order to propose improvements to a dairy aiming their environmental suitability, a present study was prepared by following the production activities of the enterprise, the raising of standards and applicable legal requirements and environmental assessment for the preparation of an array of aspects and impacts with the task of identifying the most significant according to MOREIRA (2006). The results indicated that some of the impacts of high relevance are related to the generation of wastewater and solid waste, which do not have treatment and / or proper disposal and indeed require intervention for their suitability. Through, these results were drawn up proposals for actions conformation. In addition, they were proposed meeting the requirement for health and safety at work, in accordance with the standard regulations. This study has the function, therefore, propose to the company in question, an environmental management plan that ensures the protection of the environment and health of employees and consequently the improvement of production processes and products

A atorvastatina, amplamente utilizada no tratamento de dislipidemias, é fármaco inibidor da enzima 3-hidroxi-3-metilglutaril coenzima A redutase (HMG-CoA) redutase que atua na etapa limitante da síntese do colesterol nas células hepáticas. O transporte hepatocelular da atorvastatina é mediado principalmente pelo polipeptídeo transportador de ânions orgânicos (OATP) 1B1, o qual é altamente polimórfico geneticamente e está sujeito a interação com outros fármacos usados na clínica. O presente estudo teve como objetivo revisar a literatura científica baseada em estudos clínicos, in vitro e experimentais a fim de avaliar o impacto de polimorfismos genéticos de OATP1B1 na disposição cinética, eficácia e toxicidade da atorvastatina. Os estudos clínicos demonstram que o polimorfismo c.521T>C, presente nos alelos SLCO1B1 *5 e *15, resulta em alteração funcional da atividade transportadora, sendo que indivíduos homozigotos para o alelo raro C, apresentam maiores concentrações plasmáticas de atorvastatina e seus metabólitos ativos 2-hidroxiatorvastatina e 4- hidroxiatorvastatina. Entretanto, o aumento na concentração plasmática da atorvastatina e seus metabólitos ativos em indivíduos c.521 CC parece não ser acompanhado de redução na concentração dos mesmos no sítio alvo, tendo em vista que não foram encontradas diferenças significativas na redução da LDL-C ou colesterol total quando comparados aos indivíduos homozigotos para o alelo selvagem. O alelo SLCO1B1*15 mostrou-se significativamente associado a miopatia induzida pela atorvastatina, apesar do pequeno número de pacientes investigados. Estudos in vitro em células expressando OATP1B1 são necessários a fim de investigar o impacto do polimorfismo c.521T>C na concentração de atorvastatina nos hepatócitos. Introduction: The study of graft donor sites, whether from the anatomical, physiological or morphological point of view, has become a topic of current interest, due to the increasing number of patients needing facial bone reconstruction for various reasons. Purpose: In view of the need to constantly improve surgical techniques for autogenous bone graft harvesting, still considered the best choice for facial bone reconstruction, this paper describes an anatomical study on dry skulls in order to evaluate the average thickness of the parietal bone. Material and Methods: Measurements of this bone were taken with a goniometer, at four previously defined points, in the region that is often used as a donor site, in 49 dry skulls (98 parietal bones). The results were evaluated using the T test. Results: Thickness was measured at four predetermined points. The mean values (Point A = 4898mm, B = 4517mm, C = 6185mm, D = 4280mm) show that the bone can be even thinner than previously reported in the literature in other studies of the same nature. The largest bone thickness is in the medial and posterior region. Conclusion: A knowledge of these anatomical characteristics is helpful in preventing possible surgical complications, as well as making it safer for the surgeon to remove this graft and providing more information on whether or not to indicate this region as a bone graft donor site.

To calculate the estimated and actual height and weight measures in cancer patients, evaluate the correlation and concordance of these measures and to identify the malnutrition patients. 62 cancer patients, treated at the Amaral Carvalho Hospital (Jaú- SP). For classifi cation of nutritional status, the Patient-Generated Subjective Global Assessment, the Body Mass Index and the relationship with usual weight were used. For the estimated weight was used the algorithm Chumlea et al.7 and estimated height, the equation proposed by Chumlea et al.8 and Kwok & Whitelaw24. The Pearson Correlation Coeffi cient and the Intraclass Correlation Coeffi cient was calculated and simple linear regression was conducted. To compare the malnutrition detected by different methods, the Cochran’s Q test were performed. There was a strong and signifi cant correlation between the estimated and actual weight (r=0,95; CI95%=0,92-0,97; p<0,001) and excellent concordance (ρ=0,95). For height measurements, a signifi cant correlation was observed between the actual and estimated measurement the method Chumlea et al.8 (r=0,78; CI95%=0,66-0,86; p<0,001) and Kwok & Whitelaw24 (r=0,85; CI95%: 0,75-0,90; p<0,001). Concordance was “Good” (ρ=0,72) and “Moderate” (ρ=0,67) between the actual height and estimated by Chumlea et al.8 and Kwok & Whitelaw,24 respectively. There was a strong correlation between the actual and estimated weight and height measures. The reproducibility of the equation proposed by Chumlea et al.8 to calculate height was better. The nutritional status, the patients classifi ed as malnutrition was greater when using the Patient-Generated Subjective Global Assessment and the relationship with usual weight.

Introduction: the assessment of the activity of rheumatoid arthritis and juvenile idiopathic arthritis is made by means of different tools, respectively DAS-28 and JADAS.Objective: To compare DAS-28 and JADAS with scores of 71, 27 and 10 joint counts in juvenile idiopathic arthritis.Method: A secondary analysis of a phase III placebo-controlled trial, testing safety and efficacy of abatacept was conducted in 8 patients with 178 assessment visits. Joint count scores for active and limited joints, physician's and parents'global assessment by 0-10 cm Visual Analog Scale, and erythrocyte sedimentation rate normalized to 0-10 scale, in all visits. The comparison among the activity indices in different observations was made through Anova or adjusted gamma model. The paired observations between DAS-28 and JADAS 71, 27 and 10, respectively, were analyzed by linear regression.Results: There were significant differences among individual measures, except for ESR, in the first four months of biological treatment, when five of the eight patients reached ACR-Pedi 30, with improvement. The indices of DAS-28, JADAS 71, 27 and 10 also showed significant difference during follow-up. Linear regression adjusted model between DAS-28 and JADAS resulted in mathematical formulas for conversion: [DAS-28 = 0.0709 (JADAS 71) + 1.267] (R-2 = 0.49); [DAS-28 = 0.084 (JADAS 27) + 1.7404] (R-2 = 0.47) and [DAS-28 = 0.1129 (JADAS-10) + 1.5748] (R-2 = 0.50).Conclusion: The conversion of scores of DAS-28 and JADAS 71, 27 and 10 for this mathematical model would allow equivalent application of both in adolescents with arthritis.
