RESUMO - O envelhecimento demogrfico uma constatao e ao mesmo tempo uma oportunidade para viver de forma mais saudvel e autnoma o mais tempo possvel. A principal razo para a admisso de pessoas idosas em instituies e para o uso desproporcionado de servios de sade o declnio funcional que acompanha o envelhecimento. Os Cuidados de Sade Primrios apresentamse como contexto ideal para promover cuidados antecipatrios. A avaliao da autonomia funcional multidimensional permite a deteco de perturbaes incipientes. Os programas de rastreios facilitam o desencadear de recursos para a deteco precoce e tratamento. Os mdicos sentem diversas barreiras na aplicao dos rastreios, como a falta de familiaridade com os testes, bem como a falta de tempo ou recursos. O estudo aqui proposto visa conhecer, a ttulo exploratrio, as atitudes dos mdicos de famlia face aos rastreios de autonomia funcional multidimensional em pessoas idosas; avaliar o grau de conhecimento sobre esta abordagem multidimensional, no que diz respeito s reas e instrumentos a avaliar; caracterizar as prticas na prestao de cuidados preventivos s pessoas idosas e caracterizar as condies genricas necessrias para a aplicao destes rastreios no contexto dos cuidados de sade primrios. Para tal, ir basearse num questionrio autoadministrado aos mdicos de famlia dos 22 Agrupamentos de Centros de Sade, na Regio de Sade de Lisboa e Vale do Tejo. As suas concluses podero contribuir para uma uniformizao do conceito de autonomia funcional multidimensional e para a realizao de um levantamento de necessidades de formao sobre a abordagem multidimensional e interdisciplinar.

RESUMO: Os mdicos de famlia devem participar na deteco precoce dos factores de risco que favorecem o declnio funcional nas pessoas idosas. A avaliao estruturada das suas necessidades poder contribuir para identificar os problemas de sade que traduzam deteriorao funcional. O objectivo deste trabalho foi avaliar a exequibilidade da implementao de um processo de avaliao de necessidades de cuidados em sade das pessoas idosas na consulta de MGF.Seleccionou-se uma amostra no aleatria de pacientes com 65 ou mais anos de idade, na consulta mdica de uma Unidade de Sade Familiar da regio de Lisboa. A avaliao de necessidades foi realizada, pelos mdicos, com cinco itens da entrevista Camberwell Assessment of Need for the Elderly (CANE-5). Avaliou-se a percepo dos mdicos e dos pacientes sobre este processo de avaliao de necessidades mediante um questionrio escrito e entrevistas individuais, respectivamente. Identificaram-se necessidades em 38 (75%) dos 51 pacientes avaliados. Do total das 83 necessidades identificadas, 17 no estavam cobertas. O sofrimento psicolgico foi a necessidade no coberta mais frequente. A comparao das avaliaes do mdico com as do paciente mostrou concordncia razovel ou boa nas cinco reas avaliadas. Esta avaliao foi bem aceite e considerada til na perspectiva dos mdicos e dos pacientes. A principal dificuldade identificada pelos mdicos foi o tempo despendido na avaliao face durao da consulta. Este estudo aponta para a exequibilidade da utilizao da entrevista CANE-5 na prtica clnica de MGF. No entanto, ser importante alargar este estudo a amostras de maior dimenso e avaliar a utilidade da entrevista em intervenes sobre a funcionalidade dos pacientes idosos.----------ABSTRACT: General practitioners (GP) should participate in the early detection of risk factors for fuctional disability in elderly people. Structured needs assessments may contribute to a better identification of health problems that are linked to functional decline. The aim of this study was to assess the feasibility of a structured assessment of needs in the elderly, in the context of opportunistic screening in primary care. A convenience sample was selected of patients aged 65 years and over with scheduled appointments in one general practice in the Lisbon region. The assessment of needs was done by their GPs, using five items of the Camberwell Assessment of Need for the Elderly (CANE-5). Perceptions of GPs and patients about this process of needs assessment were ev+aluated by means of a written questionnaire and individual interviews, respectively. Needs were identified in 38 (75%) of 51 patients. Seventeen unmet needs were identified, out of a total number of 83 needs. Psychological distress was the most frequent unmet need. GPs and patients assessments showed moderate or good agreement in all five areas. This needs assessment process was well accepted and considered useful by both GPs and patients. The main difficulty, according to the views of GPs, was the time consumed in this process, given the length of consultation. This study suggests the feasibility of using the CANE-5 interview in clinical practice in primary care settings. However it is important to replicate this study in larger samples and to evaluate the usefulness of the interview regarding interventions related to functionality in elderly patients.

The theme of this dissertation is the finite element method applied to mechanical structures. A new finite element program is developed that, besides executing different types of structural analysis, also allows the calculation of the derivatives of structural performances using the continuum method of design sensitivities analysis, with the purpose of allowing, in combination with the mathematical programming algorithms found in the commercial software MATLAB, to solve structural optimization problems. The program is called EFFECT Efficient Finite Element Code. The object-oriented programming paradigm and specifically the C ++ programming language are used for program development. The main objective of this dissertation is to design EFFECT so that it can constitute, in this stage of development, the foundation for a program with analysis capacities similar to other open source finite element programs. In this first stage, 6 elements are implemented for linear analysis: 2-dimensional truss (Truss2D), 3-dimensional truss (Truss3D), 2-dimensional beam (Beam2D), 3-dimensional beam (Beam3D), triangular shell element (Shell3Node) and quadrilateral shell element (Shell4Node). The shell elements combine two distinct elements, one for simulating the membrane behavior and the other to simulate the plate bending behavior. The non-linear analysis capability is also developed, combining the corotational formulation with the Newton-Raphson iterative method, but at this stage is only avaiable to solve problems modeled with Beam2D elements subject to large displacements and rotations, called nonlinear geometric problems. The design sensitivity analysis capability is implemented in two elements, Truss2D and Beam2D, where are included the procedures and the analytic expressions for calculating derivatives of displacements, stress and volume performances with respect to 5 different design variables types. Finally, a set of test examples were created to validate the accuracy and consistency of the result obtained from EFFECT, by comparing them with results published in the literature or obtained with the ANSYS commercial finite element code.

In this work we present the thermal characterization of the full scope of polyhydroxyalcanoate and poly(lactic acid) blends obtain by injection molding. Blends of polyhydroxyalcanoate and poly(lactic acid) (PHA/PLA) were prepared in different compositions ranging from 0100% in steps of 10%. The blends were injection molded and then characterized by differential scanning calorimetry (DSC), scanning electron microscopy (SEM) and wide angle X-ray diffraction (WAXD). The increment of PHA fraction increased the degree of crystallinity of the blend and the miscibility of the base polymers as veried by the Fox model. The WAXD analysis indicates that the presence of PHA hindered the PLA crystallization. The crystallization evolution trough PHA weight fraction (wf) shows a phase inversion around 50-60%. SEM analyses conrmed that the miscibility of PHA/PLA blends increased with the incorporation of PHA and became total for values of PHA higher that 50%.

Le Programme cantonal diabte (PCD) du canton de Vaud a pour objectifs de rduire durablement l'impact du diabte sur la population vaudoise. Il a t labor l'initiative du Service de la sant publique du canton de Vaud et la stratgie du programme a t construite sur la base des propositions de plusieurs groupes de travail. Ce programme inclut divers projets complmentaires qui ont t progressivement mis sur pied partir de 2010. 2012 marque la fin de la premire phase du programme et une valuation-bilan du programme est demande. Cette valuation-bilan fait suite une premire phase de travaux qui ont consist en une valuation de l'valuabilit du programme. Les rsultats de cette premire phase ont t dlivrs en dcembre 2011 au groupe de pilotage et consigns dans un rapport en fvrier 2012. L'IUMSP propose donc pour 2012 des travaux en continuit de ce qui a t fait en 2011 et qui vont dans le sens d'un bilan critique et de propositions pour un systme de monitoring long terme du programme, comportant des indicateurs d'activit ( harmoniser lorsque c'est possible dans des projets similaires), des indicateurs de couverture et de rsultats. Despite clinical experience that suggests a high burden of care among relatives of individuals with a primary malignant brain tumor (PMBT), little is known about their actual needs. In this study, the caregivers' personal experiences, quality of life, burden of care, and psychological well-being were examined. Fifty-nine percent did not receive any financial aid for home care, 33% had increased risk for psychosomatic problems, 45% had anxiety, and 33% increased depression levels. The caregiver's quality of life was most strongly affected by the burden of care (p < .001) and the patient's mental state (p < .03). To improve the situation, empathetic professionals and an early implementation of palliative care and social work are required.

The PhunkyFoods Programme (PFP) aids schools in meeting healthy lifestyles curriculum requirements by delivering key, consistent health messages in-line with current Government recommendations. The aim is to help schools deliver a whole-school approach to healthy lifestyles and to engage with all pupils, and their families, in promoting tangible health behaviour changes in a fun, lively and positive manner. The short-term objectives (6-months) of the PFP are; 1. To improve school staff competence, confidence and in delivering healthy lifestyles messaging in school. 2. To improve the health-related knowledge of pupils taking part in the PFP. The medium-term objectives (12-18 months) of the PFP are; 3. To improve the dietary behaviours of children taking part in the PFP (increase fruit and veg intake, increase breakfast consumption, decrease HFSS foods, decrease fizzy drink consumption). 4. To increase the levels of habitual phsyical activity undertaken by the children participating in the PFP. The longer-term objectives (+18=months) of the PFP are; 5.
