A remodelação óssea é um processo contínuo, que possibilita a substituição de osso envelhecido e danificado por tecido novo. A reparação ao redor de implantes osseointegráveis e enxertos ósseos em situações desfavoráveis depende do estabelecimento de um adequado contato entre tecido ósseo e implante por meio da estabilidade primária. Tendo em vista a importância da estabilidade primária do implante na biomecânica e conseqüente osseointegração, a associação de biomateriais reabsorvíveis ao sítio de instalação dos implantes se torna relevante. Destes biomateriais, destaca-se o copolímero de ácido polilático/ poliglicólico (PLA/PGA), que poderia contribuir com a estabilidade temporária dos implantes.

Bone reconstructions are traditionally conducted with autogenous grafts harvested from intra- or extra-oral donor sites to reestablish the lost bone volume for further implant-prosthetic rehabilitation. The calvarial bone has been studied as an excellent donor site in large atrophic situations, presenting low resorption rates, as well as complications and minimal morbidity. The hospitalization time is short, with low pain levels, short functional limitations, and invisible scars. The skull microarchitecture is predominantly cortical in the presence of growth factors that demonstrate their osteogenic, osteoinductive, and osteoconductive abilities resulting in low resorption rate and high predictability when compared to the iliac crest. Dural lacerations, extra and subdural bleeding, cerebrospinal fluid leakage, and brain damage have been minimized due to the development of surgical technique. The delimitation of diploe, preserving the internal skull cortex before osteotomy at the donor made it possible to reduce accidents and complications. The aim of this paper is to show a technical and to discuss aspects of the use of calvarial bone in the reconstruction of severely atrophic maxilla for oral rehabilitation with osseointegrated implants.

Bone is a specialized connective tissue, vascular and dynamic changes over the life of the organism. When injured, has a unique ability to regenerate and repair without the presence of scars, but in some situations due to the size of the defect, the bone tissue does not regenerate so completely, it is necessary to carry out bone grafting procedures. Considering there are various types of grafts and various donor sites. Thus, the aim of this study was to review the literature to some type of graft most commonly used in dentistry. Given the importance of bone reconstruction in oral and maxillofacial surgery, it is necessary to know the viability and influence of biomaterials, or not associated with autogenous grafts in bone repair. Even this, with many qualities, but further studies should be done to achieve each day, a synthetic material compatible with bone tissue lost in adequate amounts without requiring extra-oral surgeries that are considered to be devoted to higher morbidity.

Many patients seeking dental care wish to improve facial and smile aesthetics to be accepted in modern day society. In denture wearers, the physiological resorption causes atrophy mainly in the maxilla, being necessary to carry out reconstruction techniques and sometimes orthognathic surgery to improve occlusal stability and facial harmony. The aim of this study is to discuss the features related to the rehabilitation of edentulous patients with indication for reconstruction of the maxilla using bone grafts and orthognathic surgery by means of a clinical case. In the present case, after the prosthetic rehabilitation, the patient was full satisfied with obtained results and dismissed the initially proposed surgical protocol. Therefore, professionals should provide therapeutic options but the patient’s opinion should prevail provided its clinical feasibility.

Objectives: to evaluate implant survival immediately placed after tooth extraction considering different sites, prosthodontic modalities, and the need for biomaterials. Material and methods: dental records of 500 patients treated with dental implants between 2004 and 2011 were screened. Results: only 200 records (20%) corresponded to immediate implants. Reasons for tooth extraction included extensive caries, bone loss, and root fractures. From the 197 immediate dental implants, 86 were placed in the maxilla with a survival rate of 93.9% and 111 in the mandible (survival rate of 99.1%). The overall survival rate was 97.46%. Prosthodontic modalities identified were: Brånemark classic complete denture screwed prostheses (36%), overdentures (5.6%), fixed partial denture (31%), and single-tooth prostheses (27.4%). Also, it was observed that in 33% of cases there was a need for the use of grafts and/or biomaterials. Conclusion: it can be concluded that, when correctly indicated, immediate implants are an excellent choice. The anterior mandibular region, screwed and overdenture-type prostheses presented higher success rates when associated to immediate implant placement. The need for bone graft/biomaterial does not affect the clinical results.

Due to the expansion of orchards, problems with soil pathogens are frequent and cause injury and/or make passion fruit cultivation impossible in Brazil. Thus, studies with potential species for rootstocks become indispensable to assist in the technological development of the culture. It is necessary to take into account several aspects of grafting, for the adoption of appropriate techniques, among them we can highlight the anatomy, physiology and also biochemistry. This study aimed to review the existing papers related to grafting of passion fruit and also to understand the operation of grafting itself. It was observed that several publications are related to the topic of the present review and that grafting is a viable alternative to the cultivation of passion fruit, however additional studies about grafting and production are needed.

Macronutrients in grafted cucumber in areas with gall nematodes. Grafting is an often recommended technique for cucumber culture in areas infested with gall nematodes. This study aimed to evaluate the content of macronutrients in grafted cucumber in two rootstocks on soil infested with Meloidogyne incognita race 2 or Meloidogyne javanica, since they may decrease the effectiveness of water and nutrient absorption. One evaluated 9 treatments (factorial 3x3: nongrafted hybrid cucumber Tsuyataro, grafted on squash hybrid Shelper or grafted on squash hybrid Excitte Ikki x without inoculation, inoculation with M. incognita or M. javanica), in a randomized blocks design, with 4 replicates and 6 plants per plot. The characteristics evaluated were: macronutrient content at 72 days after transplantation and macronutrient content at the end of the cycle (99 days after transplantation). The nutrient contents evaluated didn’t differ according to the nematode species (with or without inoculation), however, there’re differences with regard to the rootstocks, with higher values of phosphorus and potassium in grafted plants and higher values of magnesium, calcium, and sulfur in nongrafted plants at the end of cycle.

Desde a antiguidade, enxertos e membranas são estudados para promoverem um reparo ósseo otimizado. O reparo ósseo consiste na reabsorção do tecido necrosado e de seu coágulo, juntamente com um processo inflamatório que libera fatores de crescimento que irão reparar o osso danificado. Em algumas situações, o osso lesionado não tem a capacidade de se auto reparar, portanto, são necessárias intervenções cirúrgicas para inserir um enxerto ósseo. Entretanto, há uma grande dificuldade em se encontrar um material que forneça os fatores necessários para o crescimento ósseo. Para isso, foram confeccionadas membranas à base de ácido polilático e poli-ε-caprolactona (PLC) (Purasorb: PLC 7015 - Purac, Holanda), com a incorporação de fosfato de lantânio (PLC/LaPO4) e oxiapatita dopada com 20% de lantânio (PLC/La20OAP) pelo Instituto de Química de Araraquara, UNESP. Assim, o objetivo do presente estudo foi avaliar a citotoxicidade desses materiais por meio dos testes XTT e sobrevivência clonogênica. Os eluatos foram preparados com as membranas citadas de acordo com a ISO 10993-12. O cloridrato de doxorrubicina foi utilizado como controle positivo para ambos os testes. Como Controle Negativo (CN) foram utilizadas somente as células CHO-K1 (sem a ação de qualquer tratamento) por 24 horas. Os eluatos foram mantidos em contato com células CHO-K1 por 24 horas. Como os dados apresentaram aderência à curva normal, foi aplicada a análise de variância (ANOVA) one-way, seguido dos testes de Tukey e Dunnett (p<0,05). Verificou-se que os materiais testados não demonstraram absorbância estatisticamente diferente em relação ao CN (p>0,05; Dunnett - XTT) e também não causaram comprometimento na capacidade proliferativa das células (p>0,05; Dunnett - Sobrevivência clonogênica). Assim, pode-se concluir que as amostras de PLC, PLC/LaPO4 e PLC/La20OAP não apresentaram citotoxicidade em células CHO-K1.

Objectives: Retrospective evaluation of cases of limb replantation after avulsion injuries. Evaluation of the techniques and tactics used, that contributed to success and good functional results. Methods: Forty-three patients' records were assessed. All the cases had been submitted to limb replantation after avulsion injuries. Results: The majority of the cases were young men. The most common injury was to the thumbs. The surgical techniques and tactics used were: nerve grafting, vein grafting, transposition of the digital vessels, limb shortening, and heterotopic replantation. The most commonly used technique was vein graft. The limb survival rate was high (93%), as was patient satisfaction. Conclusion: Replantation after avulsion injury depends on the correct diagnosis of the limb viability and the use of appropriate surgical techniques and tactics for each case. The experience of the team of surgeons and a good hospital structure are essential for good results. There are few articles in medical literature about the indications, techniques and results of limb replantation after avulsion injuries. We believe that this retrospective evaluation can bring new information and contributions to the correct management of this highly complex situation. Level of evidence IV, Case Series.
