Because an enriched environment (EE) enhances T-cell activity and T-lymphocytes contribute to immunopathogenesis during heterologous dengue virus (DENV) infections, we hypothesised that an EE increases dengue severity. To compare single serotype (SS) and antibody-enhanced disease (AED) infections regimens, serial intraperitoneal were performed with DENV3 (genotype III) infected brain homogenate or anti-DENV2 hyperimmune serum followed 24 h later by DENV3 (genotype III) infected brain homogenate. Compared AED for which significant differences were detected between the EE and impoverished environmental (IE) groups (Kaplan-Meyer log-rank test, p = 0.0025), no significant differences were detected between the SS experimental groups (Kaplan-Meyer log-rank test, p = 0.089). Survival curves from EE and IE animals infected with the AED regimen were extended after corticoid injection and this effect was greater in the EE than in the IE group (Kaplan-Meyer log-rank test, p = 0.0162). Under the AED regimen the EE group showed more intense clinical signs than the IE group. Dyspnoea, tremor, hunched posture, ruffled fur, immobility, pre-terminal paralysis, shock and death were associated with dominant T-lymphocytic hyperplasia and presence of viral antigens in the liver and lungs. We propose that the increased expansion of these memory T-cells and serotype cross-reactive antibodies facilitates the infection of these cells by DENV and that these events correlate with disease severity in an EE.

Analisa-se diferentes aspectos da epidemiologia da transmissão da dengue através de modelos matemáticos. Supo˜e-se que o controle vetorial atua como uma taxa de mortalidade adicional em cada fase do ciclo de vida do mosquito Ae. aegypti e estuda-se a eficiência dos diferentes mecanismos utilizados pela Superintendência de Controle de Endemias (SUCEN) no estado de São Paulo para o controle da doença. A modelagem usual de equações diferencias ordinárias (EDO) permite a obtenção de valores limiares para a taxa de oviposição do vetor e para a transmissão da doença, as quais depende das suposições biológicas que geram o modelo, dos parâmetros entomológicos do mosquito, do período de incubação do vírus, e da imunidade e dinâmica vital do homem. Estes limiares dividem o espaço de solução em ausência do vetor, presença do vetor e ausência de transmissão da dengue, e presença do vetor com transmissão da dengue. Estes valores fornecem uma estimativa do esforço de controle necessário para diminuir ou parar a transmissão da dengue. A suposição de população vetorial grande com distribuição espacial homogênea (modelos de equações diferencias ordinárias discutidos anteriormente) é verificada através da modelagem discreta de autômatos celulares (AC). Mostra-se que a heterogeneidade espacial gerada pela distribuição de criadouros facilita a colonização e dispersão do inseto. Como os parâmetros entomológicos do vetor dependem da temperatura, observa-se que no verão o processo de dispersão e colonização do mosquito é facilitado, e que o controle adulticida deve ser aplicado nesta época. Neste livro, o autor discute as recorrentes epidemias de dengue no Brasil (e no mundo) sob a ótica da geografia da saúde. Segundo ele, a doença - que havia sido erradicada em vários países, incluindo o Brasil, entre as décadas de 1950 e 1970 - teve seu caráter modificado com a expansão e consolidação desigual dos espaços urbanos. No território brasileiro tal expansão foi acompanhada do aumento exponencial do número e do tamanho das cidades, do crescimento do fluxo de pessoas e materiais e, aliada a esses fatores, da degradação da saúde pública. O mosquito Aedes aegypti, vetor da dengue, adaptou-se facilmente ao novo contexto e passou a se reproduzir e contaminar pessoas até com mais facilidade, inclusive disseminando em grande escala a versão mais letal da moléstia, a dengue hemorrágica. Para o pesquisador, só é possível compreender essa mudança qualitativa e quantitativa da doença em todo o mundo e, sobretudo, compreender sua dinâmica particularmente no Brasil, levando-se em conta as mudanças que ocorreram na sociedade e no espaço geográfico nesse período. Isto é, aliás, segundo ele, condição sine qua non para a erradicação definitiva da dengue no país.

The increasing population of Aedes aegypti mosquitoes on Madeira Island (Portugal) resulted in the first autochthonous dengue outbreak, which occurred in October 2012. Our study establishes the first genetic evaluation based on the mitochondrial DNA (mtDNA) genes [cytochrome oxidase subunit I (COI) and NADH dehydrogenase subunit 4 (ND4)] and knockdown resistance ( kdr ) mutations exploring the colonisation history and the genetic diversity of this insular vector population. We included mosquito populations from Brazil and Venezuela in the analysis as putative geographic sources. The Ae. aegyptipopulation from Madeira showed extremely low mtDNA genetic variability, with a single haplotype for COI and ND4. We also detected the presence of two important kdr mutations and the quasi-fixation of one of these mutations (F1534C). These results are consistent with a unique recent founder event that occurred on the island of Ae. aegyptimosquitoes that carry kdr mutations associated with insecticide resistance. Finally, we also report the presence of the F1534C kdr mutation in the Brazil and Venezuela populations. To our knowledge, this is the first time this mutation has been found in South American Ae. aegypti mosquitoes. Given the present risk of Ae. aegypti re-invading continental Europe from Madeira and the recent dengue outbreaks on the island, this information is important to plan surveillance and control measures.

This study aimed to analyze the spatial distribution of dengue risk and its association with socio-environmental conditions. This was an ecological study of the counts of autochthonous dengue cases in the municipality of Campinas, São Paulo State, Brazil, in the year 2007, aggregated according to 47 coverage areas of municipal health centers. Spatial models for mapping diseases were constructed with Bayesian hierarchical models, based on Integrated Nested Laplace Approximation (INLA). The analyses were stratified according to two age groups, 0 to 14 years and above 14 years. The results indicate that the spatial distribution of dengue risk is not associated with socio-environmental conditions in the 0 to 14 year age group. In the age group older than 14 years, the relative risk of dengue increases significantly as the level of socio-environmental deprivation increases. Mapping of socio-environmental deprivation and dengue cases proved to be a useful tool for data analysis in dengue surveillance systems.

In this study, a mathematical model with temporal dependence for dengue transmission was developed, considering coupling between human population and the vector mosquito, and a sorotype circulating on population. This model was analysed with the goal to explain disease's periodicity. Finally, a genetic algorithm was set up to study model's sensibility.

OBJECTIVE: To describe the investigation of a sylvatic yellow fever outbreak in the state of Sao Paulo and the main control measures undertaken.METHODS: This is a descriptive study of a sylvatic yellow fever outbreak in the Southwestern region of the state from February to April 2009. Suspected and confirmed cases in humans and in non-human primates were evaluated. Entomological investigation in sylvatic environment involved capture at ground level and in the tree canopy to identify species and detect natural infections. Control measures were performed in urban areas to control Aedes aegypti. Vaccination was directed at residents living in areas with confirmed viral circulation and also at nearby cities according to national recommendation.RESULTS: Twenty-eight human cases were confirmed (39.3% case fatality rate) in rural areas of Sarutaia, Piraju, Tejupa, Avare, and Buri. The deaths of 56 non-human primates were also reported, 91.4% were Allouatta sp. Epizootics was confirmed in two non-human primates in the cities of Itapetininga and Buri. A total of 1,782 mosquitoes were collected, including Haemagogus leucocelaenus, Hg. janthinomys/capricornii, and Sabethes chloropterus, Sa. purpureus and Sa. undosus. Yellow fever virus was isolated from a group of Hg. Leucocelaenus from Buri. Vaccination was carried out in 49 cities, with a total of 1,018,705 doses. Nine serious post-vaccination adverse events were reported.CONCLUSIONS: The cases occurred between February and April 2009 in areas with no recorded yellow fever virus circulation in over 60 years. The outbreak region occurred outside the original recommended vaccination area with a high percentage of susceptible population. The fast adoption of control measures interrupted the human transmission within a month and the confirmation of viral circulation in humans, monkeys and mosquitoes. The results allowed the identification of new areas of viral circulation but further studies are required to clarify the dynamics of the spread of this disease.
