INTRODUCTION: The risk that hip preserving surgery may negatively influence the performance and outcome of subsequent total hip replacement (THR) remains a concern. The aim of this study was to identify any negative impact of previous hip arthroscopy on THR. METHODS: Out of 1271 consecutive patients who underwent primary THR between 2005 and 2009, 18 had previously undergone ipsilateral hip arthroscopy. This study group (STG) was compared with two control groups (CG, same approach, identical implants; MCG, paired group matched for age, BMI and Charnley categories). Operative time, blood loss, evidence of heterotopic bone and implant loosening at follow-up were compared between the STG and the MCG. Follow-up WOMAC were compared between the three groups. RESULTS: Blood loss was not found to be significantly different between the STG and MCG. The operative time was significantly less (p < 0.001) in the STG. There was no significant difference in follow-up WOMAC between the groups. No implant related complications were noted in follow-up radiographs. Two minor complications were documented for the STG and three for the MCG. CONCLUSION: We have found no evidence that previous hip arthroscopy negatively influences the performance or short-term clinical outcome of THR.

Uncontrolled complement activation is central to the occurrence of atypical hemolytic uremic syndrome (aHUS) and can result in thrombotic microangiopathies (TMAs).These terms encompass a group of heterogenic inherited or acquired diseases that recent research suggests may be triggered by the complement cascade. Pathogenetic triggers of complement activation include immunologic disorders, genetics, infections, systemic diseases, pregnancy, drug administration, metabolic diseases, transplantation, or triggers of mixed cause. Hallmarks of aHUS and other TMAs include increased vascular endothelium thromboresistance, leukocyte adhesion to damaged endothelium, complement consumption, coagulation abnormalities, and vascular shear stress, whereas common end points of these mechanisms include hemolytic anemia, thrombocytopenia with microvascular infarction, and predisposition for decreased kidney function and other organ involvement. The central role of the complement cascade as a disease trigger suggests a possible therapeutic target. Eculizumab, a first-in-class humanized monoclonal anti-C5 antibody that has been successful in the treatment of paroxysmal nocturnal hemoglobinuria, a disorder of complement-induced hemolytic anemia, received approval for the treatment of aHUS in the United States and Europe in late 2011. We review the treatment of aHUS and other TMAs, focusing on the role of eculizumab, including its pharmacology, mechanism of action, and approved dosing recommendations and health economic considerations. Finally, the potential for future indications for eculizumab use in other complement-driven diseases is discussed.

El present treball presenta els resultats d’un estudi sobre la utilització i la disponibilitat d’informació científica entre els investigadors de les universitats del CBUC. Per a la seva realització s’han emprat dues tècniques de recollida de dades. En primer lloc es va distribuir una enquesta als 2.299 investigadors afiliats a les universitats catalanes que havien publicat almenys un article a alguna revista indexada al ISI Web of Science durant 2008. Es van obtenir 910 respostes (40%). A continuació, es va analitzar la disponibilitat al CBUC de les revistes referenciades als articles d’aquests mateixos investigadors publicats durant 2008 a revistes indexades al ISI Web of Science.

The paper develops a stability theory for the optimal value and the optimal set mapping of optimization problems posed in a Banach space. The problems considered in this paper have an arbitrary number of inequality constraints involving lower semicontinuous (not necessarily convex) functions and one closed abstract constraint set. The considered perturbations lead to problems of the same type as the nominal one (with the same space of variables and the same number of constraints), where the abstract constraint set can also be perturbed. The spaces of functions involved in the problems (objective and constraints) are equipped with the metric of the uniform convergence on the bounded sets, meanwhile in the space of closed sets we consider, coherently, the Attouch-Wets topology. The paper examines, in a unified way, the lower and upper semicontinuity of the optimal value function, and the closedness, lower and upper semicontinuity (in the sense of Berge) of the optimal set mapping. This paper can be seen as a second part of the stability theory presented in [17], where we studied the stability of the feasible set mapping (completed here with the analysis of the Lipschitz-like property).

En muqueuse malpighienne lisse, le Bleu de Toluidine présente un intérêt diagnostic considérable, non seulement lors de l'endoscopie, mais lors de la chirurgie d'exérèse des carcinomes intra-épithéliaux et micro-invasifs de la voie digestive supérieure (bouche, pharynx, oesophage). L'utilisation systématique de ce colorant vital nous a permis le diagnostic et l'étude morphologique endoscopique de 18 carcinomes « précoces » de l'oesophage (9 in situ, 3 dysplasies sévères, 1 microinvasif, 5 limités à la sous-muqueuse). La multicentricité et la multifocalité de ces lésions est fréquente (80 à 90 % des cas), ce qui impose leur exérèse chirurgicale sous Bleu de Toluidine, afin de détecter les foyers de carcinome in situ à distance de la lésion primaire. La connaissance exacte des faux positifs et négatifs permet de limiter les erreurs d'interprétation lors de la coloration.

La mayoría de las democracias latinoamericanas se caracterizaban por un esquema de competencia partidaria bipartidista. Se observó, no obstante, en los años ochenta-noventa un re-ordenamiento concomitante a toda la región de los sistemas de partidos, cuya principal consecuencia fue la deconstrucción de los bipartidismos. Esta situación nos lleva a preguntarnos si es posible identificar causas comunes que den cuenta de este fenómeno. Demostramos en este trabajo, a través del estudio de Colombia, la Argentina y Uruguay, que aquello consistió más bien en un realineamiento de los patrones de representación dentro de los sistemas de partido. Los procesos de cada uno de estos países, cuyos bipartidismos suelen considerarse como “tradicionales”, se caracterizaron por dinámicas específicas y diferenciadas no extrapolables a toda la región. Presentaremos asimismo las lógicas de realineamiento partidario y destacaremos los “eventos críticos” más propensos a la transformación política en la región.

Le "Sūtra des contemplations de Vie-Infinie" a joué un rôle déterminant dans l'histoire de la Terre pure, cette tradition qui occupe jusqu'à nos jours une place majeure dans le bouddhisme de l'Extrême-Orient. Typique de l'idéal du Grand Véhicule, ce texte fournit une méthode complexe pour visualiser le Buddha Amida et son royaume en cette vie-ci, ainsi qu'une pratique plus accessible : la récitation du nom de ce buddha (nembutsu) pour aller naître auprès de lui dès la vie prochaine. Inconnu des sources indiennes mais traduit en chinois au début du Ve siècle de notre ère, ce sūtra est depuis une cinquantaine d'annés l'objet d'un débat sur son origine véritable. Cependant, il occupe une position clef entre les traductions chinoises archaïques du "Grand Sūtra de l'agencement de la Sukhāvatī" (IIIe s.) et la floraison de l'école de la Terre pure à l'ère des Tang (VIIe s.). En outre, le "Sūtra des contemplations" se distingue comme l'une des sources importantes de l'art bouddhique sino-japonais, depuis l'Asie centrale jusqu'au Japon médiéval. Ses représentations sur les grandes fresques et bannières peintes des VIIIe et IXe siècles, découvertes dans les grottes de Dunhuang, témoignent d'un long développement iconographique, dont les étapes antérieures peuvent être retracées par l'archéologie. Le double intérêt philologique et iconographique de ce texte en fait donc un jalon déterminant dans l'enquête tentant de cerner le processus de l'acclimatation en sol chinois de cette tradition venue de l'Inde via la Route de la soie. Pour la première fois, la présente étude traite de ces deux dimensions conjointement, en fournissant une introduction détaillée et une traduction commentée du texte, doublée d'une analyse fouillée de son évolution artistique en Chine jusqu'à sa représentation classique au Japon, le « Mandala de Taima ».

Fifty-eight infants and children with a severe subglottic stenosis underwent a partial cricotracheal resection with primary thyrotracheal anastomosis. There were 2 grade II, 40 grade III, and 16 grade IV stenoses according to the Myer-Cotton classification. A 100% subglottic lumen was formed in 34 cases and a better than 80% lumen in 23 cases. Fifty-four of the 58 (93%) patients are presently decannulated; one patient sustained a complete restenosis and three patients with a better than 80% subglottic airway still await decannulation for the following reasons: severe tracheomalacia, bilateral cricoarytenoïd joint fixation and laryngeal malformation with fusion of the vocal cords in each case respectively. Forty-four patients have no exercise intolerance, 8 live fully normally but present a slight exertional dyspnea, one patient with a laryngeal malformation is decannulated but suffers from a severe exertional dyspnea, and 4 patients are still not decannulated. The voice is normal in 20 cases, a slight dysphonia is present in 17, a moderate to severe dysphonia in another 17 and 4 patients are still not decannulated.

There has been an ardent interest in herbivore saliva due to its roles in inducing plant defenses and its impact on herbivore fitness. Two techniques are described that inhibit the secretion of labial saliva from the caterpillar, Helicoverpa zea, during feeding. The methods rely on cauterizing the caterpillar's spinneret, the principal secretory structure of the labial glands, or surgically removing the labial salivary gland. Both methods successfully inhibit secretion of saliva and the principal salivary enzyme glucose oxidase. Caterpillars with inhibited saliva production feed at similar rates as the untreated caterpillars, pupate, and emerge as adults. Glucose oxidase has been suggested to increase the caterpillar's survival through the suppression of inducible anti-herbivore defenses in plants. Tobacco (Nicotiana tabacum) leaves fed on by caterpillars with ablated salivary glands had significantly higher levels of nicotine, an inducible anti-herbivore defense compound of tobacco, than leaves fed upon by caterpillars with intact labial salivary glands. Tomato (Lycopersicon esculentum) leaves fed upon by caterpillars with suppressed salivary secretions showed greatly reduced evidence of hydrogen peroxide formation compared to leaves fed upon by intact caterpillars. These two methods are useful techniques for determining the role that saliva plays in manipulating plant anti-herbivore defenses.

This is the third national Survey of Lifestyle, Attitudes and Nutrition (SLÃÂÅN) in Ireland, conducted in 2007 using face-to-face interviews with adults aged 18 years or over, interviewed at home addresses. SLÃÂÅN 2007 follows on from two previous surveys using postal questionnaires in 1998, involving 6,539 respondents with a 62% response rate, and in 2002, involving 5,992 respondents with a 53% response rate. In the case of such a very special building project, the crucial stake for sustainable development is the fact that space systems are extreme cases of environmental constraints. In- deed, they constitute an interesting model as an analogy can be made between Martian utmost conditions and some of the possible extreme one's that Earth might soon face. The didactic ob- jective of the project is to use the context of a building on Mars to teach an approach which raises the students awareness to design and plan all steps of a building in a sustainable way, i.e. build, with the available resources, living spaces that satisfy human needs and leave as intact as possible the external environment. The paper presents the approach and the feedback of this student project, more specifically ENAC Learning Unit", which involved 17 students from envi- ronmental, civil engineering and architecture sections from EPFL. All the same, it involved pro- fessors from all three domains, as well as aerospace and Mars specialists, which gave seminars during the course of the semester. The students were separated in groups, and the project con- sisted of two phases: 1) analysis of the context and resources, 2) project design and critic. Both organisational, technical and pedagogical aspects of the experience are presented. The outcome was very positive, with students experiencing for their first time multidisciplinary work and the iterative process of design under multiple constraints.

The PHA�coordinated Northern Ireland's participation in ECDC's National Prevalence Survey on�Hospital-Acquired Infections & on Antimicrobial Use. Hospitals in Northern Ireland participated in data collection between May and June 2012.This report provides a snapshot of the levels of hospital-acquired infections (HAI) and levels of antimicrobial use (AMU) in hospitals in Northern Ireland during 2012.There have been three previous HAI PPS surveys and the last survey was carried out in 2006. It is difficult to compare each survey as the data was collected in a different way. However, after making allowances, there was an overall drop in HAI prevalence of 18% from 2006 to 2012.The PPS data collection was undertaken by hospital teams between May and June 2012 (one hospital deferred data collection until September 2012 because of a move to a new hospital); 16 hospitals surveyed 3,992 eligible patients. The median age of all patients was 66 years. A total of 383 (10 per cent) children under 16 years of age were surveyed.�Key results from this year's survey:The prevalence of HAI was 4.2%. A total of 166 patients were diagnosed with an active HAI with 3 patients having more than one infection.When comparing ward specialties, HAI prevalence was highest for patients in adult intensive care units (ICUs) at 9.1 per cent, followed by care of the elderly wards at 5.7%.The most common types of HCAI were respiratory infections (including pneumonia and infections of the lower respiratory tract) (27.9 per cent of all infections), surgical site infections (18.9 per cent) and urinary tract infections (UTI) (11.8 per cent).Since the last PPS in 2006 there has been a reduction in MRSA infections - from 0.9 per cent �of the hospital population to less than 0.1 per cent in patients; and a five-fold reduction in C. difficile infections (from 1.1 per cent to 0.2 per cent).The prevalence of antimicrobial use was 29.5%.Most antibiotic use (60 per cent) in hospitals was in patients receiving treatment for infections which commenced in the community. Eleven percent of surgical prophylaxis was prescribed for greater than one day.��

En aquest TFC, un estudiant amb coneixements de Java, però sense experiència prèvia en aplicacions distribuïdes, dissenya i implementa un exemple típic d'una aplicació distribuïda que consisteix en una aplicació de comerç electrònic utilitzant la tecnologia J2EE i amb el seguiment de patrons de disseny.
