O espectro dos dermatófitos na região central do Rio Grande do Sul demonstrou importante variação na frequência das espécies no período 1988-1992. As espécies antropofilicas Trichophyton rubrum, T. mentagrophytes var. interdigitale e Epidermophyton floccosum diminuíram em frequência, enquanto as espécies zoofílicas Microsporum canis e T. mentagrophytes var. mentagrophytes apresentaram sensível aumento. São comentadas as alterações na relação parasita/hospedeiro em função das alterações na morfología dos agentes e a dificuldade do isolamento destes agentes em cultivo.

Realizou-se um inquérito clínico-epidemiológico em área endêmica para teníase-cisticercose. Foram examinados 1080 (32,2%) indivíduos da população total, encontrando-se 198 (18,3%) indivíduos referindo antecedente de teníase, e 103 (9,5%) apresentaram história anterior ou atual de convulsões. Destes últimos, 39 (37,8%) referiram início das crises na vida adulta, e 62 (60%) foram avaliados laboratorialmente. Em 21 (33,8%) casos, o resultado da tomografia mostrou calcificações intracranianas compatíveis com neurocisticercose, em número e localização variáveis, mas sem evidência de atividade da doença. Traçados eletroencefalográficos anormais foram lidos em 21 (33,8%) pacientes e alterações no exame do líquido cefalorraquiano (LCR) detectadas em 27 (43,5%), em 3 (4,8%) foi verificada presença de eosinófilos. Somente LCR de 26 (41,9%) pacientes foram submetidos a pesquisa de anticorpos para cisticerco, obtendo-se positividade em 6 (23%) deles, por ensaio imunoenzimático (ELISA) ou reação de imunofluorescência indireta. Outras alterações do LCR foram devidas a aumento variável das proteínas. Considerando-se os fatores epidemiológicos de risco para teníase-cisticercose na região estudada e sua correlação com as alterações laboratoriais mencionadas acima com as crises convulsivas, encontra-se uma provável prevalência de 1,9% para a neurocisticercose.

A parasitological survey was carried out on 222 inhabitants of five farms in Holambra, located 30 km north of Campinas, São Paulo, Brazil, on October 1992. Approximately 70% of the inhabitants were found to be infected with at least one species of intestinal parasite. The positive rates of 6 helminths and 7 protozoan species detected are as follows: 5.4% Ascaris lumbricoides; 8.6% Trichuris trichiura; 19.8% Necator americanus; 10.4% Strongyloides stercoralis; 14% Enterobius vermicularis; 0.9% Hymenolepis nana; 3.2% Entamoeba histolytica; 2.7% E. hartmanni; 9.9% E. coli; 14.0% Endolimax nana; 2.3% Iodamoeba butschlii; 10.4% Giardia lamblia; 37.8% Blastocystis hominis. The positive rates of helminth infection were generaly higher in the younger-group under 16 years-old than those in the elder group aged 16 or more, whereas the infection rates of protozoan species were higher in the elder group. The infection rate of Strongyloides was found to be 10.4% by a newly developed sensitive method (an agarplate culture methods).

In the present study we report the results of an analysis, based on serotyping, multilocus enzyme electrophoresis (MEE), and ribotyping of N. meningitidis serogroup C strains isolated from patients with meningococcal disease (MD) in Rio Grande do Sul (RS) and Santa Catarina (SC) States, Brazil, as the Center of Epidemiology Control of Ministry of Health detected an increasing of MD cases due to this serogroup in the last two years (1992-1993). We have demonstrated that the MD due to N.meningitidis serogroup C strains in RS and SC States occurring in the last 4 years were caused mainly by one clone of strains (ET 40), with isolates indistinguishable by serogroup, serotype, subtype and even by ribotyping. One small number of cases that were not due to an ET 40 strains, represent closely related clones that probably are new lineages generated from the ET 40 clone referred as ET 11A complex. We have also analyzed N.meningitidis serogroup C strains isolated in the greater São Paulo in 1976 as representative of the first post epidemic year in that region. The ribotyping method, as well as MEE, could provide useful information about the clonal characteristics of those isolates and also of strains isolated in south Brazil. The strains from 1976 have more similarity with the actual endemic than epidemic strains, by the ribotyping, sulfonamide sensitivity, and MEE results. In conclusion, serotyping with monoclonal antibodies (C:2b:P1.3), MEE (ET 11 and ET 11A complex), and ribotyping by using ClaI restriction enzyme (Rb2), were useful to characterize these epidemic strains of N.meningitidis related to the increased incidence of MD in different States of south Brazil. It is mostly probable that these N.meningitidis serogroup C strains have poor or no genetic corelation with 1971-1975 epidemic serogroup C strains. The genetic similarity of members of the ET 11 and ET 11A complex were confirmed by the ribotyping method by using three restriction endonucleases.

From March 1991 to April 1992, serum samples for IgM detection were collected from 112 clinical measles cases reported to the Health Department of Niterói, State of Rio de Janeiro. The positivity exceeded 90% for specimens collected from the 5th to the 29th day after the onset of the disease. After day 30 a decline in IgM detection was observed, although positivity has been detected up to 90 days after the onset of the symptoms. Forty-four patients (48.9%) with an IgM response had a history of prior measles vaccination. In 5 of the 22 measles-IgM negative cases the infection was due to other agents (rubella: 4 cases, dengue: 1 case). These results show that sensitivity of the test employed for confirming suspected measles cases is high, even in vaccinated patients.

A seroepidemiologic survey about hepatitis A virus (HAV) infection was carried out in a group comprising 310 children, ranging in age from 3 months to 9 years, from day-care centers, in Goiania, a middle sized city in the central region of Brazil. The biomarkers employed in the investigation of previous infection include total IgG and IgM anti-HAV antibodies, and for the detection of more recent infection, IgM anti-HAV antibodies were analyzed. The study was performed in 1991 and 1992. According to the results, 69.7% of the children presented total IgG/IgM anti-HAV antibodies, with 60% of the group in the age range of 1 to 3 years. Among 10 day-care centers analyzed, the prevalence of the biomarker IgM anti-HAV was 3.2%, with an uniform distribution of the cases in the group of children ranging in age from 1 to 4 years. Multi-variate analysis was performed to investigate the sociodemographic factors that could influence the results. It was verified that the risk for the infection increased with the length of the attendance in the day-care centers, i.e., the risk for children with attendance of one year or more was 4.7 times higher, when compared with children with one month attendance (CI 95% 2.3-9.9). According to the results, hepatitis A is an endemic infection in day-care centers in the study area. The length of attendance in the day-care settings was demonstrated to be a risk factor for the HAV infection. Such findings suggest that if hepatits A vaccination becomes available as a routine policy in our region, the target group should be children under one year. Moreover, those children should receive the vaccine before they start to attend the day-care centers.

Serovars of a total of 5,490 Salmonella strains isolated during the period of 1991-95, from human infections (2,254 strains) and from non-human materials (3,236 strains) were evaluated. In the studied period, 81 different serovars were determined among human isolates. Salmonella Enteritidis corresponded to 1.2% in 1991, 2% in 1992, 10.1% in 1993, 43.3% in 1994, and 64.9% in 1995 of all isolates. A significant rise on the isolation of this serovar was seen since 1993 linked to food poisoning outbreaks. It is reported also an increase on the isolation of S. Enteritidis from blood cultures, associated mainly with patients with immunodeficiency syndrome. S. Enteritidis was prevalent among one hundred and thirty different serovars isolated from non-human sources. Increasing number of isolation of this serovar was seen from shell eggs, breeding flocks and from environmental samples. It is also reported a contamination of commercial feed stuffs by S. Enteritidis which represents a major concern for Brazilian poultry industry.

In order to evaluate the distribution of dermatophytes in Porto Alegre, the capital of the state of Rio Grande do Sul, Brazil, they were isolated from the skin, hairs and nails samples and retrospectively analyzed from June 1981 to June 1995, in two different institutions in the city of Porto Alegre: (i) the Serviço de Micologia do Instituto de Pesquisas Biológicas Jandyr Maya Faillace, da Secretaria de Saúde e Meio Ambiente do Rio Grande do Sul which attends the low income population (low and middle classes) and, (ii) Laboratório Weinmann, a clinical pathology laboratory which attends predominantly the higher income population (middle and upper classes), both which attend in the metropolitan area of Porto Alegre. The dermatophyte predominance of Trichophyton rubrum was confirmed (55.33%) followed by T. mentagrophytes (21.46%). The data obtained were compared with the existing prevalence data which were collected in the interior of the state over a period of 32 years (1960-1992). T. verrucosum, T. simii, Microsporum persicolor, T. schöenleinii, M. nanum and M. cookei were isolated in the interior and have not been found in the capital so far. On the other side, T. violaceum was, isolated in the capital and has not been found in the interior so far.

Serological survey was performed to detect IgG antibodies anti-Taenia solium metacestodes in blood donors of Hemocentro Regional de Uberlândia, Minas Gerais, Brazil. A total of 1133 sera from blood donors coming from four cities of Triângulo Mineiro area were analyzed by the indirect fluorescence antibody test (IFAT) and the enzyme linked immunosorbent assay (ELISA). Specific IgG antibodies were found in 5.6% of the studied population, showing differences in the positive rates according to their origin: Araguari (13.5%), Tupaciguara (5.0%), Monte Alegre de Minas (4.8%) and Uberlândia (4.7%). The results indicate the probable endemicity of cysticercosis in this population.

In São Paulo State, Brazil, the epidemic increase in isolation of Salmonella Enteritidis has been observed since 1994. A total of 105 S. Enteritidis strains (72 from human and 33 from non-human sources) isolated during the period 1975-1995, previously characterized by phage typing, was analyzed by antimicrobial susceptibility, plasmid profile, and ribotyping. Over 70% of the strains were susceptible to all antimicrobial agents tested, however, multiple resistance to antimicrobials was observed among the studied strains, mainly those from hospitalized patients. Phage type 8 (PT-8) was predominant among the strains isolated during the period of 1975-1992, but in the following years, PT-4 was the most frequent phage type identified. Seven different plasmid profiles were detected and 96% of the isolates harbored a plasmid of approximately 36 MDa. Ribotyping discriminated fourteen ribotypes (R1 to R14) among the strains examined. By analysis of dendrogram the strains were included in three groups with similarity level of 60%. The obtained results indicate that, a single ribotype (R11), determined for PT-4 strains isolated from 1993, characterizes the epidemic clone of S. Enteritidis in our region.

Dermatophytosis are superficial mycoses caused by fungi that can invade stratum corneum and keratinized tissues. In order to study the frequency of dermatophytes species and the clinical manifestations caused by these fungi, in São Paulo, SP, Brazil, the authors analyzed cultures isolated at the Mycology Laboratory from a selected population (15,300 out-patients of the Hospital das Clínicas, Department of Dermatology, Faculty of Medicine of University of São Paulo) from January 1992 to June 2002. The most prevalent dermatophyte was Trichophyton rubrum (48.7%), followed by Microsporum canis (20.9%), Trichophyton tonsurans (13.8%), Trichophyton mentagrophytes (9.7%), Epidermophyton floccosum (4.1%), and Microsporum gypseum (2.5%). These agents determined more than one clinical manifestation, i.e., tinea corporis (31.5%), tinea capitis (27.5%), tinea unguium (14.8%), tinea cruris (13.9%), tinea pedis (9.9%), and tinea manuum (1.9%). Clinical variants of dermatophytosis and their relationship to the etiologic agents were studied and the results were compared to those obtained in previous studies in other regions of Brazil and in other countries.

The most important health-threatening scorpions found in Turkey are; Androctonus crassicauda, Leiurus quinquestriatus, Mesobuthus gibbosus and M. eupeus species, all of which belong to the Buthidae family. The epidemiological and clinical findings of scorpion stings in Turkey were evaluated between the years 1995 and 2004 based on data recorded in the National Poison Information Center (NPIC). A total of 930 cases were recorded. The cases mostly occurred in the month of July. The gender distribution was 50.22% female and 45.48% male. It was shown that the 20-29 age group presented more scorpion stings. Most of the stings occurred in Central Anatolia and Marmara regions of Turkey. Patients at the hospital showed signs of localized (pain, hyperemia, edema and numbness) and systemic effects (hyperthermia, nausea and vomiting, tachycardia, shivering and lethargy) but no lethality was notified. According to records, 33% of the poisoned patients were treated with antivenin in healthcare facilities.

A gravidade das queimaduras eléctricas resulta de factores diversos como são: a voltagem, o tempo de contacto, o arco eléctrico, a resistência dos tecidos, etc. Na Unidade de Queimados do Hospital dc Dona Estefânia foram internados de Janeiro de 1992 a Janeiro de 1995, um total de 454 doentes, sendo 24 por choque eléctrico; três sofreram queimadumas da boca, 15 de uma ou ambas as mãos e seis queimaduras múltiplas. Em 19 doentes as areas foram < 1%. Descrevem-se cinco casos considerados graves, ocorridos em crianças do sexo masculino, de idades compreendidas entre os nove e os 13 anos, fora de casa, com voltagens diferentes, em actividades consideradas como lúdicas ou de experiência: dois no telhado de habitação, dois num ramal ferroviário e um num cabo eléctrico de zona portuária. Nestes, as áreas queimadas oscilam entre os 4% e os 70%, sendo todas de 2° e 3° graus; 0 tempo de internamento variou de 36 a 116 dias. Além do tratamento cirúrgico e de reabilitação precoces e coordenados, segundo as necessidades, citam-se as sequelas cutâneas (cicatrizes deformantes, bridas), neurológicas e psicológicas, com realce para um doente que sofreu amputação media da perna esquerda e dos 4° e 5° raios do pé direito. Conclui- se com a importância da acção educativa na prevenção destes acidentes, assim como legislação adequada. Destaca-se a necessidade de uma equipa multidisciplinar e centro especializado para o tratamento destes doentes.

Introdução: A doença pulmonar obstrutiva crónica (DPOC) tem sido, ao longo dos anos, uma importante causa de morbilidade e mortalidade no mundo. Em 1995, a implementação de um rastreio da função respiratória pareceu a forma mais adequada para alertar para os sintomas respiratórios negligenciados e sensibilizar para os rastreios espirométricos. Em 2002, foram criadas novas normas consensuais de diagnóstico e o reconhecimento de que a prevalência da DPOC depende dos critérios de definição de obstrução das vias aéreas. O objetivo deste estudo foi revisitar estes 2 estudos e publicar alguns dos resultados e respetivas metodologias. Métodos: Dos 12 684 indivíduos que constavam da base de dados do Pneumobil, apenas os indivíduos com 40 e mais anos (n = 9061) foram considerados para esta análise. No estudo de 2002 foi incluída uma amostra aleatorizada e representativa de 1384 indivíduos, com idades entre os 35 e os 69 anos. Resultados: A prevalência da DPOC foi de 8,96% no estudo Pneumobil e de 5,34% no estudo de 2002. Em ambos os estudos, a presença da DPOC foi superior no sexo masculino, tendo-se verificado uma associação positiva entre a presença da DPOC e os grupos etários mais velhos. Nos fumadores e ex-fumadores encontrou-se maior proporção de casos com DPOC. Conclusões: A prevalência em Portugal é mais baixa do que noutros países europeus, o que pode estar relacionado com uma menor prevalência de tabagismo. De um modo geral, os fatores de risco mais importantes que mostraram a associação com a DPOC foram a idade maior do que 60 anos, o sexo masculino e a exposição tabágica. Todos os aspetos e as limitações que se referem a diferentes critérios de definição e a metodologias de recrutamento realçam a necessidade de métodos padronizados para determinar a prevalência da DPOC e os fatores de risco associados,cujos resultados possam ser comparados entre países, como acontece no projeto BOLD.
