Purpose: Mossy fiber sprouting (MFS) is a frequent finding following status epilepticus (SE). The present study aimed to test the feasibility of using manganese-enhanced magnetic resonance imaging (MEMRI) to detect MFS in the chronic phase of the well-established pilocarpine (Pilo) rat model of temporal lobe epilepsy (TLE). Methods: To modulate MFS, cycloheximide (CHX), a protein synthesis inhibitor, was coadministered with Pilo in a subgroup of animals. In vivo MEMRI was performed 3 months after induction of SE and compared to the neo-Timm histologic labeling of zinc mossy fiber terminals in the dentate gyrus (DG). Key Findings: Chronically epileptic rats displaying MFS as detected by neo-Timm histology had a hyperintense MEMRI signal in the DG, whereas chronically epileptic animals that did not display MFS had minimal MEMRI signal enhancement compared to nonepileptic control animals. A strong correlation (r = 0.81, p < 0.001) was found between MEMRI signal enhancement and MFS. Significance: This study shows that MEMRI is an attractive noninvasive method for detection of mossy fiber sprouting in vivo and can be used as an evaluation tool in testing therapeutic approaches to manage chronic epilepsy.

PURPOSE. Vascular endothelial growth factor (VEGF) is an important signal protein in vertebrate nervous development, promoting neurogenesis, neuronal patterning, and glial cell growth. Bevacizumab, an anti-VEGF agent, has been extensively used for controlling pathological retinal neovascularization in adult and newborn patients, although its effect on the developing retina remains largely unknown. The purpose of this study was to investigate the effect of bevacizumab on cell death, proliferation, and differentiation in newborn rat retina. METHODS. Retinal explants of sixty 2-day-old Lister hooded rats were obtained after eye enucleation and maintained in culture media with or without bevacizumab for 2 days. Immunohistochemical staining was assessed against proliferating cell nuclear antigen (PCNA, to detect cell proliferation); caspase-3 and beclin-1 (to investigate cell death); and vimentin and glial fibrillary acidic protein (GFAP, markers of glial cells). Gene expressions were quantified by real-time reverse-transcription polymerase chain reaction. Results from treatment and control groups were compared. RESULTS. No significant difference in the staining intensity (on immunohistochemistry) of PCNA, caspase-3, beclin-1, and GFAP, or in the levels of PCNA, caspase-3, beclin-1, and vimentin mRNA was observed between the groups. However, a significant increase in vimentin levels and a significant decrease in GFAP mRNA expression were observed in bevacizumab-treated retinal explants compared with controls. CONCLUSIONS. Bevacizumab did not affect cell death or proliferation in early developing rat retina but appeared to interfere with glial cell maturation by increasing vimentin levels and downregulating GFAP gene expression. A intensificação de atividades inovadoras nas organizações tem desencadeado o surgimento de projetos de alto risco e com determinado nível de complexidade e estimulado a busca por modelos que possam tornam gerenciável as incertezas e riscos destes projetos. Metodologias tradicionais já não são suficientes para asseguram o sucesso destes projetos. A premissa de que um conjunto de ferramentas e técnicas padronizadas possa ser aplicável a todos os tipos de projetos tem sido fortemente questionada, dadas às diferenças fundamentais existentes entre eles. Este artigo apresenta uma revisão e análise da literatura de gerenciamento de riscos projetos inovadores a partir de uma perspectiva contingencial. Foram levantados artigos nas bases de dados científicas considerando- se duas grandes vertentes: tipologia de projetos e metodologias para gerenciamento de riscos de projetos inovativos e complexos. Com base na análise crítica da literatura, realizou-se uma proposição de um modelo estruturado para o gerenciamento das incertezas e riscos de projetos inovativos e complexos

La valutazione progressiva e il monitoraggio di una ferita difficile impongono misurazioni periodiche per valutare in modo qualitativo e quantitativo l'esistenza e l'entita della riepitelizzazione in rapporto alle pratiche terapeutiche effettuate. L'entità della guarigione nei primi 1530 giorni di trattamento, intesa come riduzione di area percentuale, nelle lesioni ulcerative di origine venosa permette secondo alcuni autori della letteratura scientifica, di avere delle informazioni prognostiche di guarigione completa a sei mesi. Altri autori invece mostrano casistiche analoghe indicando come nelle ulcere venose non si possa predire con facilità la guarigione a sei mesi.Le metodiche di misurazione delle ferite possono essere suddivise in bidimensionali e tridimensionali: nel primo gruppo troviamo la misurazione semplice, le metodiche di tracciamento, planimetria, analisi fotografica analogica e digitale. Queste metodiche misurano i diametri, il perimetro e l'area delle ferite. Nel secondo gruppo si collocano l'uso di righello Kundin, le metodiche di riempimento con alginato o con soluzione fisiologica, le metodiche stereofotogrammetriche e l'uso di strumenti laser. Queste metodiche permettono di calcolare con varie approssimazioni anche il volume delle ferite studiate. La tesi ha preso in esame un gruppo di 17 pazienti affetti da ulcere venose croniche effettuando misurazioni con lo strumento più accurato e preciso disponibile (minolta Vivid 900 laser + sensore) e acquisendo i dati con il software Derma. I pazienti sono stati misurati al primo accesso in ambulatorio, dopo 15 giorni e dopo sei mesi. Sono stati acquisiti i dati di area, perimetro, volume, profondità e guarigione a 6 mesi. L'analisi statistica condotta con modalità non parametriche di analisi dei ranghi non ha associato nessuno di questi valori ne' valori derivati ( delta V /delta A; Delta V/ Delta P) alla guarigione a 6 mesi. Secondo le analisi da noi effettuate, sebbene con l'incertezza derivata da un gruppo ridotto di pazienti, la guarigione delle lesioni ulcerative venose non è predicibile a 6 mesi utilizzando sistemi di misurazione di dimensioni, area e volume.

PURPOSE: Assessment of language dominance with functional magnetic resonance imaging (fMRI) and neuropsychological evaluation is often used prior to epilepsy surgery. This study explores whether language lateralization and cognitive performance are systematically related in young patients with focal epilepsy. METHODS: Language fMRI and neuropsychological data (language, visuospatial functions, and memory) of 40 patients (7-18 years of age) with unilateral, refractory focal epilepsy in temporal and/or frontal areas of the left (n = 23) or right hemisphere (n = 17) were analyzed. fMRI data of 18 healthy controls (7-18 years) served as a normative sample. A laterality index was computed to determine the lateralization of activation in three regions of interest (frontal, parietal, and temporal). RESULTS: Atypical language lateralization was demonstrated in 12 (30%) of 40 patients. A correlation between language lateralization and verbal memory performance occurred in patients with left-sided epilepsy over all three regions of interest, with bilateral or right-sided language lateralization being correlated with better verbal memory performance (Word Pairs Recall: frontal r = -0.4, p = 0.016; parietal r = -0.4, p = 0.043; temporal r = -0.4, p = 0.041). Verbal memory performance made the largest contribution to language lateralization, whereas handedness and side of seizures did not contribute to the variance in language lateralization. DISCUSSION: This finding reflects the association between neocortical language and hippocampal memory regions in patients with left-sided epilepsy. Atypical language lateralization is advantageous for verbal memory performance, presumably a result of transfer of verbal memory function. In children with focal epilepsy, verbal memory performance provides a better idea of language lateralization than handedness and side of epilepsy and lesion.

Conventional skin cancer prevention programs appeal to limited populations, and the middle aged male population responds less frequently. Our objective was to establish a complementary health promotion campaign tool for skin cancer prevention. Internet-based education, instruction for self assessment and teledermatological evaluation of skin lesions by an expert commission of dermatologists was used. Compliance and clinical diagnosis was assessed in a subgroup. 12,000 users visited the educational website. There was strong interest among the middle aged male population (53% (N = 262): male; mean age: 42). 28.5% of examined lesions (N = 494) were considered suspicious. Email requests, sent to the group whose lesions where considered suspicious, were answered by 46.0% of females (N = 29) and 59.7% of males (N = 46) with a female distribution predominantly in younger ages (52.6% of females with known age: < 30 years). Males were predominantly represented over 30 years (86.2% of all males). According to user's declarations, at least 8 (8.5%) malignant lesions (1 melanoma in situ, 1 squamous cell carcinoma, 4 basal cell carcinomas, 2 malignant lesions without declared diagnosis) were finally diagnosed by physicians. We conclude that internet-based, interactive, educational programs, in addition to existing health promotion campaigns, can enhance public participation in the middle aged male population in skin cancer prevention.

Proximal humerus fractures (pHF) are common. In this retrospective study intra-operative and postoperative data and complications of patients stabilized with conventional semirigid techniques (pins, n=30; helix wire, n=19) or a novel semirigid technique, the intramedullary claw (IMC, n=82) were compared. The type and frequency of postoperative complications differed between the groups (p<0.001). The IMC is a novel semirigid technique to stabilize pHF and seems to result in fewer complications than pins or helix wire. The frequency and relevance of a loss of repositioning in patients after IMC implantation need to be elucidated in long-term studies.
