Die Metalloproteasen Meprin α und β übernehmen Schlüsselfunktionen in vielen (patho-) physiologischenrnProzessen. So sind sie beteiligt an der Umstrukturierung der extrazellulären Matrix, an immunologischenrnReaktionen oder an entzündlichen Gewebserkrankungen. Die beiden Enzyme kommenrnhauptsächlich in den Bürstensaummembranen von Niere und Darm sowie in der Haut von Vertebratenrnvor. Für die Erforschung der biologischen Aktivität der Meprine wurde in dieser Arbeit der ModellorganismusrnDanio rerio verwendet, der vor allem durch die Möglichkeit der gentechnischen Manipulationrnprädestiniert ist. Im Fisch konnten drei homologe Enzyme (Meprin α1, α2 und β) nachgewiesenrnwerden. Während mRNA-Analysen eine nahezu ubiquitäre Verteilung der Meprine offenbarten,rnkonnte ich mittels spezifischer Antikörper die Expression auf Proteinebene nachweisen. WährendrnMeprin α1 und β verstärkt im Darmepithel und in der Epidermis lokalisiert sind, konnte Meprinrnα2 ausschließlich in der Lamina propria des Darms identifiziert werden.rnDer Hauptteil der vorliegenden Arbeit zielt auf die spezifische Reduzierung des Expressionslevels derrnMeprine in Embryonen des Zebrabärblings. Dies wurde durch die Mikroinjektion von sogenanntenrnMorpholinos in die Zygote erzielt. Morpholinos sind RNA-Moleküle, die spezifisch an die mRNA desrnZielproteins binden können und die Translation verhindern. Die auftretenden Effekte durch das Fehlenrnder Meprine lassen so Rückschlüsse auf ihre physiologische Funktion zu. Nach der Injektion vonrnMorpholinos gegen Meprin α1 zeigten sich lediglich leichte epidermale Deformationen. Bei Meprin βrnhingegen kam es zu einer massiven Fehlbildung von Organen im Rumpf- und Schwanzbereich. Diesesrnführte zu erheblichen Defekten; die Embryonen starben innerhalb der ersten 24 Stunden nach derrnBefruchtung. Demzufolge müssen Meprin α1 und Meprin β insbesondere an der Gewebsdifferenzierungrnbeteiligt sein. Dies korreliert mit verschiedenen Experimenten, u.a. an knockout Mäusen, ausrndenen hervorgeht, dass die Prozessierung und Aktivierung der Cytokine Interleukin-1β oder Interleukin-rn18 durch Meprin β erfolgen kann.rnDie Injektion von Meprin α2-Morpholinos erbrachte ein weiteres, eindrucksvolles Ergebnis: Das Blutgefäßsystemrnvon injizierten Embryonen war vollständig unterbrochen und es sammelten sich Erythrozytenrnim Bereich der Caudalvene an. Diese Phänotypen gleichen den knockdown-Experimenten mitrndem vascular endothelial growth factor VEGF-A, dem entscheidenden Wachstumsfaktor in der Angiogenesern(Blutgefäßbildung). Eine Inkubation des humanen VEGF-A mit (humanem) rekombinantemrnMeprin α bzw. β führte zu einer differenzierten Prozessierung des Moleküls. Diese Ergebnisse legenrnnahe, dass Meprin α pro-angiogenetisch wirkt, indem es VEGF-A prozessiert und damit die Gefäßbildungrnaktiviert. Aus den Daten dieser Arbeit wird die hohe Signifikanz der Meprine für die Proliferationrnund Differenzierung spezieller Gewebe deutlich, welche somit eine wichtige Grundlage für Studienrnan höheren Vertebraten darstellt.

Numerose evidenze sperimentali hanno dimostrato il contributo delle cellule staminali (SC) di derivazione midollare nei processi di rigenerazione epatica dopo danno tissutale. E’ cresciuto pertanto l’interesse sul loro potenziale impiego in pazienti con cirrosi. Questo studio si proponeva di valutare la fattibilità e la sicurezza della reinfusione intraepatica di cellule staminali midollari autologhe CD133+ in 12 pazienti con insufficienza epatica terminale. Previa mobilizzazione nel sangue periferico mediante somministrazione di granulocyte-colony stimulating factor (G-CSF) alla dose di 7,5 mcg/Kg/b.i.d. e raccolta per leucoaferesi (solo se la concentrazione di CD133 + SC era > 8/μL), le cellule CD133+ altamente purificate sono state reinfuse in arteria epatica a partire da 5x104/Kg fino a 1x106/kg. Nei tre giorni successivi è stato somministrato G-CSF per favorire l’espansione e l’attecchimento delle cellule. Durante la fase della mobilizzazione e quella della reinfusione sono stati eseguiti saggi biologici quali: caratterizzazione fenotipica delle SC circolanti, saggi clonogenici, valutazione della concentrazione sierica del Hepatocyte Growth Factor (HGF), Stromal-Derived Factor-1 (SDF-1) ed il Vascular-Endotelial Growth Factor (VEGF) e caratterizzazione fenotipica delle CD133+SC purificate. Fino ad oggi sono stati reinfusi 12 pazienti. Questi dati preliminari suggeriscono che è possibile mobilizzare e reinfondere un numero considerevole di SC autologhe CD133+ altamente purificate in pazienti con ESLD . Gli studi biologici mostrano che: il numero di progenitori ematopoietici ed endoteliali circolanti è aumentato dopo il trattamento con G–CSF; le SCs CD133+ altamente purificato esprimono marcatori emopoietici ed endoteliali; la concentrazione sierica di HGF, SDF-1, VEGF e la capacità clonogenica di progenitori emopoietici sono aumentati durante la mobilitazione e nelle fasi di reinfusione; il potenziale clonogenico dei progenitori endoteliali mostra espressione variabile.

Die Förderung der Zelladhäsion durch sogenannte biomimetische Oberflächen wird in der Medizin als vielversprechender Ansatz gesehen, um Komplikationen wie z. B. Fremdkörperreaktionen nach der Implantation entgegenzuwirken. Neben der Immobilisierung einzelner Biomoleküle wie z. B. dem RGD-Peptid, Proteinen und Wachstumsfaktoren auf verschiedenen Materialien, konzentriert man sich derzeit in der Forschung auf die Co-Immobilisierung zweier Moleküle gleichzeitig. Hierbei werden die funktionellen Gruppen z. B. von Kollagen unter Verwendung von nur einer Kopplungschemie verwendet, wodurch die Kopplungseffizienz der einzelnen Komponenten nur begrenzt kontrollierbar ist. Das Ziel der vorliegenden Arbeit war die Entwicklung eines Immobilisierungsverfahrens, welches die unabhängige Kopplung zweier Faktoren kontrolliert ermöglicht. Dabei sollten exemplarisch das adhäsionsfördernde RGD-Peptid (Arginin-Glycin-Asparaginsäure) zusammen mit dem Wachstumsfaktor VEGF (Vascular Endothelial Growth Factor) auf Titan gebunden werden. In weiteren Experimenten sollten dann die pro-adhäsiven Faktoren Fibronektin, Kollagen, Laminin und Osteopontin immobilisiert und untersucht werden. rnDie Aminofunktionalisierung von Titan durch plasma polymerisierte Allylaminschichten wurde als Grundlage für die Entwicklung des nasschemischen Co-immobilisierungsverfahren verwendet. Für eine unabhängige und getrennte Anbindung der verschiedenen Biomoleküle stand in diesem Zusammenhang die Entwicklung eines geeigneten Crosslinker Systems im Vordergrund. Die Oberflächencharakterisierung der entwickelten Oberflächen erfolgte mittels Infrarot Spektroskopie, Surface Plasmon Resonance Spektroskopie (SPR), Kontaktwinkelmessungen, Step Profiling und X-Ray Photoelectron Spektroskopie (XPS). Zur Analyse der Anbindungsprozesse in Echtzeit wurden SPR-Kinetik Messungen durchgeführt. Die biologische Funktionalität der modifizierten Oberflächen wurde in vitro an Endothelzellen (HUVECs) und Osteoblasten (HOBs) und in vivo in einem Tiermodell-System an der Tibia von Kaninchen untersucht.rnDie Ergebnisse zeigen, dass alle genannten Biomoleküle sowohl einzeln auf Titan kovalent gekoppelt als auch am Bespiel von RGD und VEGF in einem getrennten Zwei-Schritt-Verfahren co-immobilisiert werden können. Des Weiteren wurde die biologische Funktionalität der gebundenen Faktoren nachgewiesen. Im Falle der RGD modifizierten Oberflächen wurde nach 7 Tagen eine geförderte Zelladhäsion von HUVECs mit einer signifikant erhöhten Zellbesiedlungsdichte von 28,5 % (p<0,05) gezeigt, wohingegen auf reinem Titan Werte von nur 13 % beobachtet wurden. Sowohl VEGF als auch RGD/VEGF modifizierte Proben wiesen im Vergleich zu Titan schon nach 24 Stunden eine geförderte Zelladhäsion und eine signifikant erhöhte Zellbesiedlungsdichte auf. Bei einer Besiedlung von 7,4 % auf Titan, zeigten VEGF modifizierte Proben mit 32,3 % (p<0,001) eine deutlichere Wirkung auf HUVECs als RGD/VEGF modifizierte Proben mit 13,2 % (p<0,01). Die pro-adhäsiven Faktoren zeigten eine deutliche Stimulation der Zelladhäsion von HUVECs und HOBs im Vergleich zu reinem Titan. Die deutlich höchsten Besiedlungsdichten von HUVECs konnten auf Fibronektin mit 44,6 % (p<0,001) und Kollagen mit 39,9 % (p<0,001) nach 24 Stunden beobachtet werden. Laminin zeigte keine und Osteopontin nur eine sehr geringe Wirkung auf HUVECs. Bei Osteoblasten konnten signifikant erhöhte Besiedlungsdichten im Falle aller pro-adhäsiven Faktoren beobachtet werden, jedoch wurden die höchsten Werte nach 7 Tagen auf Kollagen mit 90,6 % (p<0,001) und Laminin mit 86,5 % (p<0,001) im Vergleich zu Titan mit 32,3 % beobachtet. Die Auswertung der Tierexperimente ergab, dass die VEGF modifizierten Osteosyntheseplatten, im Vergleich zu den reinen Titankontrollen, eine gesteigerte Knochenneubildung auslösten. Eine solche Wirkung konnte für RGD/VEGF modifizierte Implantate nicht beobachtet werden. rnInsgesamt konnte gezeigt werden, dass mittels plasmapolymerisierten Allylamin Schichten die genannten Biomoleküle sowohl einzeln gebunden als auch getrennt und kontrolliert co-immobilisiert werden können. Des Weiteren konnte eine biologische Funktionalität für alle Faktoren nach erfolgter Kopplung in vitro gezeigt werden. Wider Erwarten konnte jedoch kein zusätzlicher biologischer Effekt durch die Co-immobilisierung von RGD und VEGF im Vergleich zu den einzeln immobilisierten Faktoren gezeigt werden. Um zu einer klinischen Anwendung zu gelangen, ist es nun notwendig, das entwickelte Verfahren in Bezug auf die immobilisierten Mengen der verschiedenen Faktoren hin zu optimieren. rn

Introduction: Throughout follicular growth and subsequent corpus luteum formation the leukocyte number increases and follicular vascularisation changes. These processes are enhanced under exogenous stimulation with gonadotropins. Cytokines released by leukocytes contribute to further recruitment and vascularisation of the follicle, and they play an important role in regulating ovarian steroidogenesis by influencing theca and granulosa–lutein cell function. Changes in cytokine and vascular endothelial growth factor (VEGF) concentrations in the ovary as a consequence of gonadotropin stimulation may negatively influence oocyte quality. In this project we have compared the intrafollicular production of inflammatory cytokines and growth factors between natural IVF cycles (NC) and classical, gonadotropin-stimulated IVF cycles (gsIVF). Material and Methods: Serum on the day of oocyte retrieval and follicular fluid (FF) were collected in 37 NC and 39 gsIVF cycles. Thirteen women within this population underwent one NC and one gsIVF cycle each. A total of 14 cytokines from Bio-Plex panels I and II were determined in matched serum and FF samples using Luminex xMAP technology on the Bio-Plex(R) platform, using the serum protocol. Results: Tumour necrosis factor-alpha, RANTES, eotaxin and interferon-gamma-induced protein-10 levels were lower in FF than in serum, and thus not further investigated. Interleukin (IL)-6, -8, -10, -15, -18, monocyte chemotactic protein-1 (MCP-1), VEGF and leukaemia inhibitory factor (LIF) showed higher median concentrations in FF than in serum, indicating possible ovarian production. Moreover, most of these showed higher evels in the gsIVF than in the NC groups in the serum, but not in the follicular fluid. IL-8 was reduced in gsIVF cycles. Conclusion: The fact that serum but not FF levels of the studied cytokines were higher in the stimulated than in the natural cycles can be attributed to the increased number of active follicles present after controlled ovarian stimulation.

The heparin-binding epidermal growth factor-like growth factor (HB-EGF) has been implicated in wound-healing processes of various tissues. However, it is not known whether HB-EGF may represent a factor implicated in overstimulated wound-healing processes of the retina during proliferative retinopathies. Therefore, we investigated whether human retinal pigment epithelial (RPE) cells, which are crucially involved in proliferative retinopathies, express and respond to HB-EGF. RPE cells express mRNAs for various members of the EGF-related growth factor family, among them for HB-EGF, as well as for the EGF receptors ErbB1, -2, -3, and -4. The gene expression of HB-EGF is stimulated in the presence of transforming and basic fibroblast growth factors and by oxidative stress and is suppressed during chemical hypoxia. Exogenous HB-EGF stimulates proliferation and migration of RPE cells and the gene and protein expression of the vascular endothelial growth factor (VEGF). HB-EGF activates at least three signal transduction pathways in RPE cells including the extracellular signal-regulated kinases (involved in the proliferation-stimulating action of HB-EGF), p38 (mediates the effects on chemotaxis and secretion of VEGF), and the phosphatidylinositol-3 kinase (necessary for the stimulation of chemotaxis). In epiretinal membranes of patients with proliferative retinopathies, HB-EGF immunoreactivity was partially colocalized with the RPE cell marker, cytokeratins; this observation suggests that RPE cell-derived HB-EGF may represent one factor that drives the uncontrolled wound-healing process of the retina. The stimulating effect on the secretion of VEGF may suggest that HB-EGF is also implicated in the pathological angiogenesis of the retina.

Eph receptor tyrosine kinases and their cell-surface-bound ligands, the ephrins, regulate axon guidance and bundling in the developing brain, control cell migration and adhesion, and help patterning the embryo. Here we report that two ephrinB ligands and three EphB receptors are expressed in and regulate the formation of the vascular network. Mice lacking ephrinB2 and a proportion of double mutants deficient in EphB2 and EphB3 receptor signaling die in utero before embryonic day 11.5 (E11.5) because of defects in the remodeling of the embryonic vascular system. Our phenotypic analysis suggests complex interactions and multiple functions of Eph receptors and ephrins in the embryonic vasculature. Interaction between ephrinB2 on arteries and its EphB receptors on veins suggests a role in defining boundaries between arterial and venous domains. Expression of ephrinB1 by arterial and venous endothelial cells and EphB3 by veins and some arteries indicates that endothelial cell-to-cell interactions between ephrins and Eph receptors are not restricted to the border between arteries and veins. Furthermore, expression of ephrinB2 and EphB2 in mesenchyme adjacent to vessels and vascular defects in ephB2/ephB3 double mutants indicate a requirement for ephrin-Eph signaling between endothelial cells and surrounding mesenchymal cells. Finally, ephrinB ligands induce capillary sprouting in vitro with a similar efficiency as angiopoietin-1 (Ang1) and vascular endothelial growth factor (VEGF), demonstrating a stimulatory role of ephrins in the remodeling of the developing vascular system.

Sprouting of new capillaries from pre-existing blood vessels is a hallmark of angiogenesis during embryonic development and solid tumor growth [1]. In addition to the vascular endothelial growth factor (VEGF) and its receptors, the Tie receptors and their newly identified ligands, the angiopoietins, have been implicated in the control of blood vessel formation [2,3]. Although 'knockouts' of the gene encoding the Tie2 receptor, or its activating ligand angiopoietin-1 (Ang1), result in embryonic lethality in mice due to an absence of remodeling and sprouting of blood vessels [4,5], biological activity in vitro has not yet been described for this receptor-ligand system. In an assay in which a monolayer of endothelial cells were cultured on microcarrier beads and embedded in three-dimensional fibrin gels, recombinant Ang1 (0.5-10 nM) induced the formation of capillary sprouts in a dose-dependent manner that was completely inhibited by soluble Tie2 receptor extracellular domains. In contrast with VEGF, which also induced sprouting of capillaries, Ang1 was only very weakly mitogenic for endothelial cells. Suboptimal concentrations of VEGF and Ang1 acted synergistically to induce sprout formation. Thus, the biological activity of Ang1 in vitro is consistent with the specific phenotype of mice deficient in Tie2 or Ang1. The data suggest that, like in other developmental systems, blood vessel formation requires a hierarchy of master-control genes in which VEGF and angiopoietins, along with their receptors, are amongst the most important regulators.

Retinae of aged humans show signs of vascular regression. Vascular regression involves a mismatch between Angiopoietin-2 (Ang-2) and vascular endothelial growth factor (VEGF) expression. We used heterozygous Ang-2 deficient (Ang2LacZ) mice to evaluate murine retinal vascular changes and gene expression of growth factors. Vascular changes were assessed by quantitative retinal morphometry and gene expression levels of growth factors were measured by quantitative PCR. The numbers of endothelial cells and pericytes did not change in the Ang2LacZ retinae with age, whereas they decreased throughout the age spectrum studied in the wild type retinae. Moreover, vascular regression significantly decelerated in the heterozygous Ang2LacZ retinae (200% to 1 month), while the formation of acellular capillaries was significantly increased at 13 months in the wild type retinae (340% to 1 month). Gene expression analysis revealed that VEGF, Ang-1, PDGF-B and Ang2 mRNA levels were decreased in the wild type retinae at 9 month of age. However, the decrease of Ang-2 was smaller compared with other genes. While VEGF levels dropped in wild type mice up to 60% compared to 1 month, VEGF increased in heterozygous Ang-2 deficient retinae at an age of 9 months (141% to 1 month). Similarly, Ang-1 levels decreased in wild type mice (45% to 1 month), but remained stable in Ang2LacZ mice. These data suggest that Ang-2 gene dose reduction decelerates vasoregression in the retina with age. This effect links to higher levels of survival factors such as VEGF and Ang-1, suggesting that the ratio of these factors is critical for capillary cell survival.

OBJECTIVE: Vasa vasorum (VV) have been implicated to play a role in the pathogenesis of atherosclerosis. This study was designed to describe the distribution of VV density in different vascular beds in humans and to investigate the association between VV density and the known distribution of atherosclerosis in human arteries. METHODS: Forty-two human arteries, harvested at autopsy or after explantation, were analyzed by three-dimensional microscopic-computed tomography (micro-CT). VV density, endothelial-surface-fraction (Sigma VV endothelial-surface-area/vessel-wall-volume) and vascular-area-fraction (Sigma VV area/vessel-wall-area) were calculated for coronary, renal and femoral arteries. Representatively five coronary, renal and femoral arteries were stained for endothelial cells (von Willebrand-Factor), macrophages (CD68), vascular endothelial growth factor (VEGF) and collagen (Sirius Red). RESULTS: Coronary arteries showed a higher VV density compared to renal and femoral arteries (2.12+/-0.26 n/mm(2) versus 0.61+/-0.06 n/mm(2) and 0.66+/-0.11 n/mm(2); P<0.05 for both) as well as a higher endothelial-surface-fraction and vascular-area-fraction. Histology showed a positive correlation between histologically derived VV density and CD68-positive cells/area (r=0.54, P<0.01), VEGF-immunoreactivity/area (r=0.55, P<0.01) and a negative correlation between VV density and collagen I content (r=0.66, P<0.05). CONCLUSION: This micro-CT study highlights a higher VV density in coronary than in peripheral arteries, supporting the relation between VV density and the susceptibility to atherosclerosis in different vascular beds in humans.

BACKGROUND & AIMS: Little is known about how endothelial cells respond to injury, regulate hepatocyte turnover and reconstitute the hepatic vasculature. We aimed to determine the effects of the vascular ectonucleotidase CD39 on sinusoidal endothelial cell responses following partial hepatectomy and to dissect purinergic and growth factor interactions in this model. METHODS: Parameters of liver injury and regeneration, as well as the kinetics of hepatocellular and sinusoidal endothelial cell proliferation, were assessed following partial hepatectomy in mice that do not express CD39, that do not express ATP/UTP receptor P2Y2, and in controls. The effects of extracellular ATP on vascular endothelial growth factor (VEGF), hepatocyte growth factor (HGF), and interleukin-6 responses were determined in vivo and in vitro. Phosphorylation of the endothelial VEGF receptor in response to extracellular nucleotides and growth factors was assessed in vitro. RESULTS: After partial hepatectomy, expression of the vascular ectonucleotidase CD39 increased on sinusoidal endothelial cells. Targeted disruption of CD39 impaired hepatocellular regeneration, reduced angiogenesis, and increased hepatic injury, resulting in pronounced vascular endothelial apoptosis, and decreased survival. Decreased HGF release by sinusoidal endothelial cells, despite high levels of VEGF, reduced paracrine stimulation of hepatocytes. Failure of VEGF receptor-2/KDR transactivation by extracellular nucleotides on CD39-null endothelial cells was associated with P2Y2 receptor desensitization. CONCLUSIONS: Regulated phosphohydrolysis of extracellular nucleotides by CD39 coordinates both hepatocyte and endothelial cell proliferation following partial hepatectomy. Lack of CD39 activity is associated with decreased hepatic regeneration and failure of vascular reconstitution.

Crosstalk between elements of the sinusoidal vasculature, platelets and hepatic parenchymal cells influences regenerative responses to liver injury and/or resection. Such paracrine interactions include hepatocyte growth factor (HGF), vascular endothelial growth factor (VEGF), IL-6 and small molecules such as serotonin and nucleotides. CD39 (nucleoside triphosphate diphosphohydrolase-1) is the dominant vascular ectonucleotidase expressed on the luminal surface of endothelial cells and modulates extracellular nucleotide signaling. We have previously shown that integrity of P2-receptors, as maintained by CD39, is required for angiogenesis in Matrigel plugs in vivo and that there is synergism between nucleotide P2-receptor- and growth factor-mediated cell proliferation in vitro. We have now explored effects of CD39 on liver regeneration and vascular endothelial growth factor responses in a standard small animal model of partial hepatectomy. The expression of CD39 on liver sinusoidal endothelial cells (LSEC) is substantially boosted during liver regeneration. This transcriptional upregulation precedes maximal sinusoidal endothelial cell proliferation, noted at day 5-8 in C57BL6 wild type mice. In matched mutant mice null for CD39 (n=14), overall survival is decreased to 71% by day 10. Increased lethality occurs as a consequence of extensive LSEC apoptosis, decreased endothelial proliferation and failure of angiogenesis leading to hepatic infarcts and regenerative failure in mutant mice. This aberrant vascular remodeling is associated with biochemical liver injury, elevated serum levels of VEGF (113.9 vs. 65.5pg/ml, p=0.013), and decreased circulating HGF (0.89 vs. 1.43 ng/ml, p=0.001) in mice null for CD39. In agreement with these observations, wild type LSEC but not CD39 null cultures upregulate HGF expression and secretion in response to exogenous VEGF in vitro. CD39 null LSEC cultures show poor proliferation responses and heightened levels of apoptosis when contrasted to wild type LSEC where agonists of P2Y receptors augment cell proliferation in the presence of growth factors. These observations are associated with features of P2Y-desensitization, normal levels of the receptor tyrosine kinase VEGFR-1 (Flt-1) and decreased expression of VEGFR-2 (FLK/KDR) in CD39 null LSEC cultures. We provide evidence that CD39 and extracellular nucleotides impact upon growth factor responses and tyrosine receptor kinases during LSEC proliferation. We propose that CD39 expression by LSEC might co-ordinate angiogenesis-independent liver protection by facilitating VEGF-induced paracrine release of HGF to promote vascular remodeling in liver regeneration.

BACKGROUND: The expression of vascular endothelial growth factor (VEGF) is characteristic of differentiated thyroid cancer and is associated with aggressive tumor behavior and a poor clinical outcome. Motesanib diphosphate (AMG 706) is a novel oral inhibitor of VEGF receptors, platelet-derived growth-factor receptor, and KIT. METHODS: In an open-label, single-group, phase 2 study, we treated 93 patients who had progressive, locally advanced or metastatic, radioiodine-resistant differentiated thyroid cancer with 125 mg of motesanib diphosphate, administered orally once daily. The primary end point was an objective response as assessed by an independent radiographic review. Additional end points included the duration of the response, progression-free survival, safety, and changes in serum thyroglobulin concentration. RESULTS: Of the 93 patients, 57 (61%) had papillary thyroid carcinoma. The objective response rate was 14%. Stable disease was achieved in 67% of the patients, and stable disease was maintained for 24 weeks or longer in 35%; 8% had progressive disease as the best response. The Kaplan-Meier estimate of the median duration of the response was 32 weeks (the lower limit of the 95% confidence interval [CI] was 24; the upper limit could not be estimated because of an insufficient number of events); the estimate of median progression-free survival was 40 weeks (95% CI, 32 to 50). Among the 75 patients in whom thyroglobulin analysis was performed, 81% had decreased serum thyroglobulin concentrations during treatment, as compared with baseline levels. The most common treatment-related adverse events were diarrhea (in 59% of the patients), hypertension (56%), fatigue (46%), and weight loss (40%). CONCLUSIONS: Motesanib diphosphate can induce partial responses in patients with advanced or metastatic differentiated thyroid cancer that is progressive. (ClinicalTrials.gov number, NCT00121628.)

Although chemotherapy for breast cancer can increase inflammation, few studies have examined predictors of this phenomenon. This study examined potential contributions of demographics, disease characteristics, and treatment regimens to markers of inflammation in response to chemotherapy for breast cancer. Thirty-five women with stage I-III-A breast cancer (mean age 50 years) were studied prior to cycle 1 and prior to cycle 4 of anthracycline-based chemotherapy. Circulating levels of inflammatory markers with high relevance to breast cancer were examined, including C-reactive protein (CRP), tumor necrosis factor-alpha (TNF-alpha), Interleukin-1 receptor antagonist (IL1-RA), vascular endothelial growth factor (VEGF), soluble intercellular adhesion molecule-1 (sICAM-1), Interleukin- (IL-6), soluble P-selectin (sP-selectin), and von Willebrand factor (vWf). Chemotherapy was associated with elevations in VEGF (p < or = 0.01), sICAM-1 (p < or = 0.01), sP-selectin (p < or = 0.02) and vWf (p < or = 0.05). Multiple regression analysis controlling for age and body mass index (BMI) showed that higher post-chemotherapy levels of inflammation were consistently related to higher pre-chemotherapy levels of inflammation (ps < or =0.05) as well as to certain disease characteristics. Post-chemotherapy IL-6 levels were higher in patients who had larger tumors (p < or = 0.05) while post-chemotherapy VEGF levels were higher in patients who had smaller tumors (p < or = 0.05). Post-chemotherapy sP-selectin levels were highest in women who had received epirubicin, cytoxan, 5-fluorouracil chemotherapy (p < or = 0.01). These findings indicate that chemotherapy treatment can be associated with elevations in certain markers of inflammation, particularly markers of endothelial and platelet activation. Inflammation in response to chemotherapy is most significantly related to inflammation that existed prior to chemotherapy but also potentially to treatment regimen and to certain disease characteristics.
