BACKGROUND: Although medical and travel plans gathered from pre-travel interviews are used to decide the provision of specific pre-travel health advice and vaccinations, there has been no evaluation of the relevance of this strategy. In a prospective study, we assessed the agreement between pre-travel plans and post-travel history and the effect on advice regarding the administration of vaccines and recommendations for malaria prevention. METHODS: We included prospectively all consenting adults who had not planned an organized tour. Pre- and post-travel information included questions on destination, itineraries, departure and return dates, access to bottled water, plan of bicycle ride, stays in a rural zone, and close contact with animals. The outcomes measured included: agreement between pre- and post-travel itineraries and activities; and the effect of these differences on pre-travel health recommendations, had the traveler gone to the actual versus intended destinations for actual versus intended duration and activities. RESULTS: Three hundred and sixty-five travelers were included in the survey, where 188 (52%) were males (median age 38 years). In 81(23%) travelers, there was no difference between pre- and post-travel history. Disagreement between pre- and post-travel history were the highest for stays in rural zones or with local people (66% of travelers), close contact with animals (33%), and bicycle riding (21%). According to post-travel history, 125 (35%) travelers would have needed rabies vaccine and 9 (3%) typhoid fever vaccine. Potential overprovision of vaccine was found in <2% of travelers. A change in the malaria prescription would have been recommended in 18 (5%) travelers. CONCLUSIONS: Pre-travel history does not adequately reflect what travelers do. However, difference between recommendations for the actual versus intended travel plans was only clinically significant for the need for rabies vaccine. Particular attention during pre-travel health counseling should focus on the risk of rabies, the need to avoid close contact with animals and to seek care for post-exposure prophylaxis following an animal bite.

The choice to adopt risk-sensitive measurement approaches for operational risks: the case of Advanced Measurement Approach under Basel II New Capital Accord This paper investigates the choice of the operational risk approach under Basel II requirements and whether the adoption of advanced risk measurement approaches allows banks to save capital. Among the three possible approaches for operational risk measurement, the Advanced Measurement Approach (AMA) is the most sophisticated and requires the use of historical loss data, the application of statistical tools, and the engagement of a highly qualified staff. Our results provide evidence that the adoption of AMA is contingent on the availability of bank resources and prior experience in risk-sensitive operational risk measurement practices. Moreover, banks that choose AMA exhibit low requirements for capital and, as a result might gain a competitive advantage compared to banks that opt for less sophisticated approaches. - Internal Risk Controls and their Impact on Bank Solvency Recent cases in financial sector showed the importance of risk management controls on risk taking and firm performance. Despite advances in the design and implementation of risk management mechanisms, there is little research on their impact on behavior and performance of firms. Based on data from a sample of 88 banks covering the period between 2004 and 2010, we provide evidence that internal risk controls impact the solvency of banks. In addition, our results show that the level of internal risk controls leads to a higher degree of solvency in banks with a major shareholder in contrast to widely-held banks. However, the relationship between internal risk controls and bank solvency is negatively affected by BHC growth strategies and external restrictions on bank activities, while the higher regulatory requirements for bank capital moderates positively this relationship. - The Impact of the Sophistication of Risk Measurement Approaches under Basel II on Bank Holding Companies Value Previous research showed the importance of external regulation on banks' behavior. Some inefficient standards may accentuate risk-taking in banks and provoke a financial crisis. Despite the growing literature on the potential effects of Basel II rules, there is little empirical research on the efficiency of risk-sensitive capital measurement approaches and their impact on bank profitability and market valuation. Based on data from a sample of 66 banks covering the period between 2008 and 2010, we provide evidence that prudential ratios computed under Basel II standards predict the value of banks. However, this relation is contingent on the degree of sophistication of risk measurement approaches that banks apply. Capital ratios are effective in predicting bank market valuation when banks adopt the advanced approaches to compute the value of their risk-weighted assets.

Digital information generates the possibility of a high degree of redundancy in the data available for fitting predictive models used for Digital Soil Mapping (DSM). Among these models, the Decision Tree (DT) technique has been increasingly applied due to its capacity of dealing with large datasets. The purpose of this study was to evaluate the impact of the data volume used to generate the DT models on the quality of soil maps. An area of 889.33 km² was chosen in the Northern region of the State of Rio Grande do Sul. The soil-landscape relationship was obtained from reambulation of the studied area and the alignment of the units in the 1:50,000 scale topographic mapping. Six predictive covariates linked to the factors soil formation, relief and organisms, together with data sets of 1, 3, 5, 10, 15, 20 and 25 % of the total data volume, were used to generate the predictive DT models in the data mining program Waikato Environment for Knowledge Analysis (WEKA). In this study, sample densities below 5 % resulted in models with lower power of capturing the complexity of the spatial distribution of the soil in the study area. The relation between the data volume to be handled and the predictive capacity of the models was best for samples between 5 and 15 %. For the models based on these sample densities, the collected field data indicated an accuracy of predictive mapping close to 70 %.

RÉSUMÉ L'Organisation Mondiale de la Santé (OMS) recommande d'administrer l'oxygène par concentrateurs dans les pays en voie de développement (PVD), les cylindres posant des problèmes logistiques et financiers trop importants. Cette technologie a été proposée aux enfants d'un hôpital sénégalais (Ndioum) qui présentaient les critères d'oxygénation de l'OMS. Les bénéfices cliniques et financiers ont été majeurs. En revanche, les connaissances des soignants sur les diverses techniques d'administration d'oxygène ainsi que les prestations du service de maintenance étaient insuffisantes. L'implantation de concentrateurs doit être encouragée dans les PVD, mais doit respecter une stratégie englobant enseignement, maintenance et suivi de l'opération. Diverses actions correctrices ont été entreprises à Ndioum où plusieurs concentrateurs fonctionnent désormais régulièrement. Summary: The World Health Organisation (WHO) recommends supplying oxygen in developing countries by concentra¬tors because cylinders pose considerable logistic and financial problems. This technology was employed to treat children in a hospital in Ndioum, Senegal, who met the WHO oxygenation criteria. There were clear clinical and financial benefits, but neither the nurses' knowledge of the various techniques of oxygen supply nor the maintenance service were satisfactory. The use of concentra¬tors should be encouraged in developing countries. A strategy including technical training, maintenance and monitoring should be adopted. Corrective actions were undertaken in Ndioum, and several concentrators are now being used on a regular basis.

RESUME DE LA THESE Le but de cette thèse est d'analyser la réforme générale de l'Etat au Cameroun et au Gabon par les institutions de Bretton Woods sur une période de vingt ans (1987-2007). Pour ce faire, mon travail consiste à étudier le rôle majeur joué par les différentes mesures économiques impulsées dans les pays par le Fonds Monétaire International (FMI) et la Banque Mondiale. Il s'agit de voir si les différentes réformes d'ajustement que ces institutions ont imposé au Cameroun et au Gabon ont produit des effets positifs en vue de solution la crise économique qui s'y installait durablement et remettre ces Etats sur le sentier de la croissance et du développement. L'avènement de la crise économique qui sévissait dans les pays au milieu des années 1980 montrait que le modèle développementaliste expérimenté par ces Etats n'avait expérimenté contribué qu'à provoquer la paralysie de l'appareil de production. Les causes profondes résidaient dans l'omniprésence de l'Etat qui privilégiait une logique administrative au dépend de celle de production au point de nuire gravement aux politiques interventionnistes étatiques. A cela, s'ajoutaient d'autres facteurs endogènes et exogènes aux Etats comme la mauvaise gestion couplée à la forte corruption et au clientélisme politique, la contrebande et le dumping, l'effritement du secteur agricole dû au vieillissement des plantations et à la sous-mécanisation, l'explosion démographique, la détérioration des termes de l'échange, le chômage endémique et galopant, etc. En ayant recours aux réformes d'ajustement structurel du FMI et de la Banque Mondiale, les gouvernements camerounais et gabonais entendaient lutter contre les rigidités institutionnelles et les distorsions structurelles qui empêchaient leurs économies de répondre aux signaux du marché et d'accroître l'offre d'une part. Et, d'autre part, les réformes d'ajustement devaient permettre de renforcer la concurrence des produits sur les marchés, accroître les capacités d'adaptation des marchés de facteurs et améliorer l'efficacité et l'efficience du secteur public. Mais avant d'analyser les réformes d'ajustement dans les pays j'ai d'abord présenté dans une première partie, l'évolution sociopolitique et économique des deux pays. Cette partie donne à l'analyse une profondeur historique indispensable et rend compte de l'évolution des politiques sectorielles des Etats marquée par une phase de croissance (1960-1984), puis par une phase de décroissance ou de crise qui commence à partir de 1985. La seconde partie met en évidence les politiques d'ajustement structurelle principalement axées sur la régulation monétaire, financière et budgétaire, la réforme de la fonction publique, la réforme agricole, le désengagement de l'Etat du secteur productif national et les privatisations. Je termine cette seconde partie par un bilan contrasté que je dresse de l'impact de ces réformes dans les économies camerounaises et gabonaises. La troisième partie met en évidence les nouvelles orientations stratégiques des institutions de Bretton Woods en partant de l'agenda politique des réformes au Cameroun et au Gabon. Elle fait intervenir une batterie de réformes portant sur l'initiative Pays Pauvres Très Endettés (PPTE), les Documents de Stratégie pour la Réduction de la Pauvreté (DSRP), l'Evaluation Politique et Institutionnelle du Pays (EPIP) et la «Bonne Gouvernance». Cette thèse s'achève par une conclusion exhaustive mettant en évidence les insuffisances des modèles théoriques et conceptuels fondant l'ajustement structurel et la remise en cause des nouvelles stratégies de lutte contre la pauvreté impulsées par le FMI et la Banque Mondiale dans les pays d'une part. D'autre part, j'observe une faiblesse institutionnelle de l'Etat dans ces pays qui se résume à la mauvaise gouvernance ; à l'absence d'un cadre et des outils appropriés pour la gestion économique et stratégique du développement à long terme ; à l'impunité généralisée et à l'inefficacité des mesures de lutte contre la pauvreté ; à l'absence d'un Etat de droit et à l'autoritarisme des régimes en place.
