The authors developed an evaluation scale for sit-stand from the ground for children with Duchenne muscular dystrophy (DMD) and tested its reliability. The construction occurred in stages: (a) the characterization of the movement in healthy children, (b) the characterization of the movement in children with DMD, (c) the elaboration of the 1st version of the scale and the manual, (d) the evaluation by experts and readjustments, and (e) the analysis of inter- and intraexaminer reliability and correlation with the Vignos Scale, age, and time for the execution of the activity. The scale comprehended 3 phases for sitting and 5 for the standing. A very good repeatability of the measures of sitting and standing (ICC = 0.89 and 0.84, respectively) and excellent reproducibility (ICC = 0.93 and 0.92, respectively) was demonstrated. The Kappa coefficient for the 8 phases in the interexaminer analysis varied from 0.77 to 1.00 (excellent reliability), and in the intraexaminer analysis varied from 0.80 to 1.00 (excellent reliability). Good correlation was found between the variables on the Vignos Scale (age: r = 0.58; stand: r = 0.56). The scale is a reliability instrument that allows evaluation of the activity of sitting and standing in children with DMD.

Ullrich congenital muscular dystrophy (UCMD) is a disabling and life-threatening disorder resulting from either recessive or dominant mutations in genes encoding collagen VI. Although the majority of the recessive UCMD cases have frameshift or nonsense mutations in COL6A1, COL6A2, or COL6A3, recessive structural mutations in the COL6A2 C-globular region are emerging also. However, the underlying molecular mechanisms have remained elusive. Here we identified a homozygous COL6A2 E624K mutation (C1 subdomain) and a homozygous COL6A2 R876S mutation (C2 subdomain) in two UCMD patients. The consequences of the mutations were investigated using fibroblasts from patients and cells stably transfected with the mutant constructs. In contrast to expectations based on the clinical severity of these two patients, secretion and assembly of collagen VI were moderately affected by the E624K mutation but severely impaired by the R876S substitution. The E624K substitution altered the electrostatic potential of the region surrounding the metal ion-dependent adhesion site, resulting in a collagen VI network containing thick fibrils and spots with densely packed microfibrils. The R876S mutation prevented the chain from assembling into triple-helical collagen VI molecules. The minute amount of collagen VI secreted by the R876S fibroblasts was solely composed of a faster migrating chain corresponding to the C2a splice variant with an alternative C2 subdomain. In transfected cells, the C2a splice variant was able to assemble into short microfibrils. Together, the results suggest that the C2a splice variant may functionally compensate for the loss of the normal COL6A2 chain when mutations occur in the C2 subdomain.

This study seeks to assess the effect of inspiratory muscle training (IMT) on pulmonary function, respiratory muscle strength, and endurance in morbidly obese patients submitted to bariatric surgery. Thirty patients were randomly assigned to sham muscular training, or to IMT with a threshold device (40% of maximum inspiratory pressure, MIP), for 30 min/day, from the 2nd until 30th postoperative (PO) day. All of them were submitted to a standard respiratory kinesiotherapy and early deambulation protocol. Data on spirometry, maximum static respiratory pressures, and respiratory muscle endurance were collected on the PO days 2, 7, 14, and 30 in a blinded matter. IMT enabled increases in PO MIP and endurance, and an earlier recovery of the spirometry parameters FEV(1), PEF, and FEF(25-75%). Comparing to preoperative values, MIP was increased by 13% at the 30th PO day in the trained group, whereas control group had a reduction of 8%, with higher values for the IMT group (30th PO, IMT-130.6 +/- 22.9 cmH(2)O; controls-112.9 +/- 25.1 cmH(2)O; p < 0.05). Muscular endurance at the 30th PO day was increased in the trained group comparing to preoperative value (61.5 +/- 39.6 s vs 114.9 +/- 55.2 s; p < 0.05), a finding not observed in the control group (81.7 +/- 44.3 vs 95.2 +/- 42.0 s). IMT improves inspiratory muscle strength and endurance and accounts for an earlier recovery of pulmonary airflows in morbidly obese patients submitted to bariatric surgery.

Background: Spinal muscular atrophy is a common autosomal recessive neuromuscular disorder caused by mutations in the SMN1 gene. Identification of spinal muscular atrophy carriers has important implications for individuals with a family history of the disorder and for genetic counseling. The aim of this study was to determine the frequency of carriers in a sample of the nonconsanguineous Brazilian population by denaturing high-performance liquid chromatography (DHPLC). Methods: To validate the method, we initially determined the relative quantification of DHPLC in 28 affected patients (DHPLC values: 0.00) and 65 parents (DHPLC values: 0.49-0.69). Following quantification, we studied 150 unrelated nonconsanguineous healthy individuals from the general population. Results: Four of the 150 healthy individuals tested (with no family history of a neuromuscular disorder) presented a DHPLC value in the range of heterozygous carriers (0.6-0.68). Conclusions: Based on these results, we estimated there is a carrier frequency of 2.7% in the nonconsanguineous Brazilian population, which is very similar to other areas of the world where consanguineous marriage is not common. The vesicle-associated membrane protein/synaptobrevin-associated membrane protein B (VAPB) Pro56Ser Mutation has been identified in Brazilian families showing various motor neuron syndromes. However, the neurophysiological characteristics of these patients have not been detailed, and some questions Still need to be solved, such as the possible presence of myotonia and the origin of the abdominal protrusion seen in most patients. The eventual finding of suggestive electrophysiological characteristics would be helpful not only for clinical diagnosis but also to selection of the appropriate DNA test. To clarify these questions we carried out sensory and motor conduction Studies, including symphatetic skin response, and needle examination in six genetically proven affected members. The electromyographic findings were those of a slowly progressive motor neuron disorder. Topographically, the abdominal muscles were severely affected, but the facial and laryngeal muscles were preserved or very mildly involved. Sensory conduction studies and sympathetic Skin responses were normal. No myotonic discharge was recorded. These findings are indistinguishable from those of other motor neuron disorders, although the predominant involvement of the proximal limbs and of the abdominal muscles may be of some help in the appropriate clinical setting.

Dogs suffering from Golden Retriever muscular dystrophy (GRMD) present symptoms that are similar to human patients with Duchenne muscular dystrophy (DMD). Phenotypic variability is common in both cases and correlates with disease progression and response to therapy. Physical therapy assessment tools were used to study disease progression and assess phenotypic variability in dogs with GRMD. At 5 (TO), 9 (T1), 13 (T2) and 17 (T3) months of age, the physical features, joint ranges of motion (ROM), limb and thorax circumferences, weight and creatine kinase (CK) levels were assessed in 11 dogs with GRMD. Alterations of physical features were higher at 13 months, and different disease progression rates were observed. Passive ROM decreased until 1 year old, which was followed by a decline of elbow and tarsal ROM. Limb and thorax circumferences, which were corrected for body weight, decreased significantly between TO and T3. These measurements can be used to evaluate disease progression in dogs with GRMD and to help discover new therapies for DMD patients. (C) 2011 Elsevier Ltd. All rights reserved.

O Sistema de Registro Eletr??nico de Ponto (SREP) representou um importante avan??o nos instrumentos de prote????o e seguran??a dos trabalhadores e empresas que utilizam o ponto eletr??nico, por coibir fraudes de altera????o dos hor??rios efetivamente registrados, de supress??o ou de impedimento na marca????o de horas extras. Ap??s investiga????o das modalidades de fraudes e dos sistemas que propiciavam as adultera????es, a equipe t??cnica do Minist??rio do Trabalho e Emprego (MTE) desenvolveu o modelo de regulamenta????o, consolidado na Portaria n?? 1.510/2009, que garante as marca????es dos hor??rios de jornada de trabalho sem possibilidade de adultera????o. O SREP ?? composto de um hardware e um software. O hardware, Registrador Eletr??nico de Ponto (REP), preserva as marca????es de ponto e fornece ao trabalhador o seu comprovante. O software opera nos computadores das empresas, permitindo o tratamento seguro dos dados para pagamento das horas trabalhadas. S?? em 2010, o SREP recuperou uma sonega????o de R$ 1,5 bilh??o em sal??rios e R$ 446,6 milh??es em contribui????es previdenci??rias e Fundo de Garantia do Tempo de Servi??o (FGTS)

Sistema desenvolvido e aplicado durante a Opera????o Tormenta para identificar suspeitos de fraudar as provas objetivas de concursos p??blicos. O Sistema de Prospec????o e An??lise de Desvios em Exames (Spade-PRO) serviu de base para auditar os concursos p??blicos realizados pela Pol??cia Federal nos anos de 2001, 2004 e 2009. Em raz??o da efetividade do sistema, a sua utiliza????o foi estendida para os concursos p??blicos realizados pelas seguintes institui????es: Ag??ncia Nacional de Avia????o Civil (Anac), Ag??ncia Brasileira de Intelig??ncia (Abin), Tribunal de Justi??a do Estado de S??o Paulo (TJSP) e para o Exame da Ordem dos Advogados do Brasil (OAB). Os dados obtidos, utilizados nas a????es criminais que apuraram a responsabilidade pelas fraudes, foram fundamentais para evitar o ingresso ilegal de pessoas no servi??o p??blico. O sistema ser?? utilizado pela Pol??cia Federal para auditar os pr??ximos concursos do ??rg??o e est?? dispon??vel para ser aplicado por qualquer institui????o que solicite a realiza????o da auditoria em seus processos seletivos

O Educacenso ?? um sistema eletr??nico de coleta de informa????es educacionais composto por um aplicativo web, que permite a coleta, migra????o e altera????o de dados educacionais das escolas em todo o territ??rio nacional, e por um banco de dados relacional, que armazena de forma sistem??tica todas estas informa????es. O Educacenso representa uma inova????o, pois se trata de um banco de dados ??nico, de base nacional, alimentado por dados e informa????es que v??m diretamente das escolas, por meio da internet. Constitui-se no mais completo cadastro de escolas, alunos e docentes do pa??s. A constru????o de um banco de dados com informa????es individualizadas amplia as possibilidades de comunica????o com outras bases de dados do governo federal. O novo desenho metodol??gico e a utiliza????o de recursos de tecnologia da informa????o permitiram ampliar a precis??o e a fidedignidade dos dados do Censo Escolar, o que possibilita realiza????o de ajustes na pol??tica educacional e maior efetividade do gasto p??blico com a educa????o b??sica

O Sistema de Controle Log??stico de Medicamentos Antirretrovirais (Siclom) ?? uma ferramenta criada com o objetivo de gerenciamento log??stico dos medicamentos antirretrovirais (ARV) e de aprimoramento da qualifica????o da dispensa????o. Esse sistema permite que o departamento de DST, Aids e Hepatites Virais, secretarias estaduais e municipais de Sa??de mantenham-se atualizados em rela????o ao fornecimento, dispensa????o de medicamentos aos pacientes em tratamento conforme as recomenda????es existentes no Consenso Terap??utico Brasileiro, al??m do controle dos estoques de cada medicamento nas v??rias regi??es do pa??s. As informa????es de consumo e estoques s??o fundamentais para que o adequado suprimento aos estados ocorra evitando, assim, a ruptura dos estoques locais e preju??zo no atendimento aos pacientes. Atualmente utilizam o sistema 670 unidades dispensadoras de medicamentos, distribu??das em todo o Brasil; 27 coordena????es estaduais de DST/Aids; 61 almoxarifados; e 936 maternidades

O Sindec consolida os registros em bases locais e forma um banco nacional de informa????es sobre problemas enfrentados por consumidores. Essas informa????es podem ser consultadas por consumidores, pelos Procons integrados e por toda a sociedade, por meio do s??tio www.mj.gov.br/sindec. O Sindec re??ne informa????es de mais de 80 Procons (entre estaduais, municipais e do Distrito Federal), est?? presente em mais de 110 cidades de todas as regi??es brasileiras e consolida registros de demandas de mais de 2 milh??es de consumidores contra mais de 10 mil fornecedores

O Inmetro desenvolveu e implementou modelo de avalia????o de desempenho individual, instrumento de gest??o capaz de promover nova cultura baseada no m??rito, exig??ncia, motiva????o e reconhecimento. Adota remunera????o vari??vel vinculada ao desempenho, com crit??rios objetivos e regras claras, elaborados a partir de amplo debate com os servidores; a avalia????o ?? realizada por comit??s com maioria dos membros externos ?? institui????o, rompendo com a pr??tica limitada da avalia????o exclusivamente pelas chefias. O ciclo ?? anual e o processo consta basicamente da pactua????o de um plano de trabalho do servidor com sua chefia ao in??cio do ciclo, vinculado ao conjunto de objetivos institucionais. A an??lise, ao final do ciclo, do relat??rio de atividades do servidor e do parecer de sua chefia ?? conduzida pelo Comit?? de Avalia????o. A participa????o dos servidores nos dois primeiros ciclos foi irrestrita, com excelente receptividade, independente dos resultados
