This thesis explores how multinational corporations of different sizes create barriers to imitation and therefore sustain competitive advantage in rural and informal Base of the Pyramid economies. These markets require close cooperation with local partners in a dynamic environment that lacks imposable property rights and follows a different rationale than developed markets. In order to explore how competitive advantage is sustained by different sized multinational corporations at the Base of the Pyramid, the natural-resource-based view and the dynamic capabilities perspective are integrated. Based on this integration the natural-resource-based view is extended by identifying critical dynamic capabilities that are assumed to be sources of competitive advantage at the Base of the Pyramid. Further, a contrasting case study explores how the identified dynamic capabilities are protected and their competitive advantage is sustained by isolating mechanisms that create barriers to imitation for a small to medium sized and a large multinational corporation. The case study results give grounds to assume that most resource-based isolating mechanisms create barriers to imitation that are fairly high for large and established multinational corporations that operate at the rural Base of the Pyramid and have a high product and business model complexity. On the contrary, barriers to imitation were found to be lower for young and small to medium sized multinational corporations with low product and business model complexity that according to some authors represent the majority of rural Base of the Pyramid companies. Particularly for small to medium sized multinational corporations the case study finds a relationship- and transaction-based unwillingness of local partners to act opportunistically rather than a resource-based inability to imitate. By offering an explanation of sustained competitive advantage for small to medium sized multinational corporations at the rural Base of the Pyramid this thesis closes an important research gap and recommends to include institutional and transaction-based research perspectives.

De um total de 203 mulheres adultas entrevistadas, foram avaliadas 175 (entre 16 a 73 anos), por meio de medidas antropométricas: peso, altura, prega cutânea tricipital (PCT), circunferência braquial (CB), circunferência muscular (CMB) e Índice de Massa Corpórea (IMC). Das mulheres analisadas, 84,0% (n=147) residiam na zona urbana e 16,0% (n=28) na zona rural. Verificou-se que o peso médio das mulheres não nutrizes residentes na área urbana foi de 54,1 kg; altura de 149,0 cm e o IMC de 24,4 kg/m², enquanto que a PCT, CB e a CMB forneceram valores médios de 19,2 mm, 27,5 cm e 21,5 cm, respectivamente. Na área rural, as mulheres apresentaram um peso médio de 50,4 kg, altura de 148,6 cm, IMC de 23,0 kg/cm² e a PCT, CB e a CMB foram 13,8 mm, 26,4 cm e 22,1 cm, respectivamente. Houve diferença estatisticamente significativa apenas nos valores médios da PCT entre as mulheres procedentes da área urbana e rural (p<0,05). Nas mulheres da área urbana, a ocorrência de baixo peso foi de 6,5%, 28,2% de sobrepeso e 11,3% de obesidade. Nas mulheres da área rural, a prevalência de baixo peso e obesidade foi de 4% e de sobrepeso de 16%. A maioria da população estudada situou-se no limite máximo de normalidade do IMC, e os índices representantes das reservas adiposas (PCT) foram maiores nas mulheres residentes na área urbana.

This paper aimed to evaluate the richness, abundance and frequency of sand fly occurrence in rural and urban areas American visceral Leishmaniasis -AVL is endemic in the study area of Santarém municipality, Pará state. Sand flies were collected during 1995-2000, using CDC light traps placed in neighborhoods and rural areas of the municipality. A total of 53.454 individuals and 26 species of sand flies were collected. The most abundant species in both urban and rural environments was Lutzomyia longipalpis, vector of AVL in the area. The highest species richness by capture was in rural area. In all years sampled, the largest number of species of sand fly collected was always in rural areas. The species of sand flies in urban and rural area were similar in 11 species. In the rural area other 11 species were found, a total of 22 species. Shannon-Wiener index ranged from 0.12 to 0.84 at rural areas and 0.08 to 0.34 at urban ones. In general, rural localities showed higher diversity (H') of phlebotomines than urban ones. Individual-based rarefaction curves for each area demonstrated that urban localities had the lowest expected number of phlebotomine species and the richest rural ones reach higher expected values with lower amount of individuals than urban sites. The most frequent species were Lutzomyia longipalpis, Evandromyia carmelinoi and Bichromomyia flaviscutellata.

The use of construction and demolition waste (C&DW) in the construction industry is an important contribution to attain sustainability in the sector. The roads are among the civil engineering works which can use larger quantities of C&DW recycled aggregates. In Portugal, the limit values for the properties of C&DW recycled aggregates that can be used in the roads of Portuguese Road Network are defined by two Laboratório Nacional de Engenharia Civil (LNEC) technical specifications (TS), in accordance to Portuguese Decree-law no. 46/2008 of May 12th. Municipal and rural roads and trenches have specific characteristics that can enable the use of C&DW of lower quality than those required by existing LNEC TS, and even then ensuring an adequate performance. However, given the absence of specific regulation for those applications, the Portuguese Environment Agency requires compliance with the existing LNEC TS, which represents an obstacle to recycling a significant part of the C&DW, in particular at a local government level. This paper presents guidelines for the recycling of C&DW in municipal and rural roads and in trenches, which could be considered in a new forthcoming LNEC TS. In the preparation of the guidelines, the bibliography collected and analysed, the information gathered from the application of C&DW in a municipal and rural roads of a Portuguese municipality and in the roadways of a Portuguese resort, and the results of laboratory tests carried out on samples collected in the Portuguese municipality were taken into consideration.

OBJECTIVE: To assess the frequency of cardiovascular risk factors in the rural community of Cavunge, in the Brazilian state of Bahia. METHODS: A cross-sectional study was carried out with 160 individuals (age > 19 years) randomly drawn from those listed in the population census of the Cavunge Project. The following parameters were studied: arterial hypertension, dyslipidemia, diabetes, obesity, smoking, waist-hip ratio (WHR), physical activity, and overall cardiovascular risk classified according to the Framingham score. The assessing parameters used were those established by the III Brazilian Consensus on Hypertension and the II Brazilian Consensus on Dyslipidemia. RESULTS: Of the randomly drawn individuals, 126 with a mean age of 46.6 + 19.7 years were included in the study, 43.7% of whom were males. The frequency of arterial hypertension was 36.5%; 20.4% of the individuals had cholesterol levels >240 mg/dL; 31.1% of the individuals had LDL-C levels > 130 mg/dL; 4% were diabetic; and 39.7% had a high-risk Framingham score. Abdominal obesity was observed in 41.3% of the population and in 57.7% of the females. High caloric-expenditure (HCE) physical activities were performed by 56.5% of the individuals. The HCE group had a greater frequency of normal triglyceride levels (63% vs 44%; P=0.05), no diabetes, and WHR tending towards normal (46% vs 27%, P=0.08) as compared with those in the low caloric-expenditure group. CONCLUSION: Cardiovascular risk factors, such as hypertension and hypercholesterolemia, are frequently found in rural communities. The greatest frequency of normal triglyceride levels and normal WHR in the HCE group reinforces the association between greater caloric expenditure and a better risk profile.

Una fórmula magistral es una preparación farmacéutica que se realiza a partir de una prescripción realizada por un profesional (médico u odontólogo) que tiene como características particulares que es individual y personalizada, donde se detalla la composición cuali y cuantitativa de la misma y se destina a un tratamiento determinado, debiendo el facultativo renovar dicha prescripción en el caso de ser necesaria la terapéutica por un tiempo mas prolongado. El flúor es un agente mineral el cual tiene actividad cariostática ampliamente difundida. Sin embargo en poblaciones económicamente postergadas, donde existen bajas condiciones socio culturales y otros factores epidemiológicos y sanitarios concurrentes -desnutrición, dietas poco equilibradas- la falta de fluoración en las aguas (de fuentes alternativas: pozos o ríos, de dudosa potabilidad), y el escaso uso de productos dentales (dentífricos y pastas dentales) repercute en una mayor prevalencia de caries tanto en niños como en adultos. El objetivo del proyecto es diseñar y preparar una fórmula magistral con flúor con el propósito de ser utilizada como tratamiento preventivo de las caries en una población con bajos recursos, donde trabaja el voluntariado de la UCC en forma conjunta con profesionales odontólogos.

La Municipalidad de Santa Eufemia cuenta con una población de 2.900 habitantes, posee actualmente un centro de salud municipal coordinado por un director. El centro recibe a una población de 340 pacientes por mes que concurren para ser asistidos por los médicos generalistas, brinda además servicio odontológico y de especialidades médicas tal como la ginecología. La población concurrente pertenece a este éjido municipal y desempeñan sus actividades en el área rural y en la zona aledaña, de influencia en la región. El equipo de salud estable que se desempeña en dicho centro asistencial, está constituido por un equipo multidisciplinario, constituido por médicos, psicólogos, odontólogos, enfermeras idóneas y asistente social, estos brindan atención activa y pasiva las 24 hs del día. El servicio de ortodoncia y ginecología brinda atención una vez a la semana, dado que los profesionales concurrentes pertenecen a otras localidades. Los servicios bioquímicos, de kinesiología, fonoaudiología y diagnóstico por imágenes son tercerizados a demanda. Separado del centro, pero comunicado con el mismo se encuentra una residencia de ancianos que es asistido por los profesionales del centro. El mismo alberga aproximadamente 11 personas, que no reciben ninguna otra ayuda social ni cuentan con una estructura familiar que los contenga. El intendente de la comuna considera que además de las actividades que se vienen desarrollando, se podría implementar otras, tales como: a) Capacitar al personal a fin de optimizar los recursos disponibles con la implementación de programas que permitan el diseño de estrategias de planificación en salud. b) Capacitar a la comunidad en temas vinculados a la salud a fin de disminuir los índices de morbilidad de la población y fomentar el auto-cuidado del paciente. c) Implementar un sistema de gestión (adquisición, conservación y distribución) de los medicamentos recibidos por el centro. d) Obtener datos epidemiológicos que permitan planificar y establecer la demanda presupuestaria.

FUNDAMENTO: A hipertensão, importante problema de saúde pública, representa uma das principais causas de morbidade em todo o mundo. OBJETIVO: Estimar a prevalência da hipertensão arterial e seus fatores de risco em uma comunidade rural do nordeste do estado de Minas Gerais, Brasil. MÉTODOS: Estudo transversal realizado em 2004, em Virgem das Graças, comunidade rural localizada no Vale do Jequitinhonha. A amostra era composta por 287 indivíduos, com idades entre 18 e 88 anos. Hipertensão foi definida segundo os critérios da Joint National Committee (pressão arterial sistólica > 140 mmHg e/ou pressão arterial diastólica > 90 mmHg): indivíduos que já usavam medicamentos anti-hipertensivos também foram considerados hipertensos. Usou-se a análise bivariada para testar a relação entre as variáveis independentes e hipertensão, e a regressão logística para ajustar fatores de confusão e identificar interações. A força de associação foi mensurada usando-se odds ratio (OR) e seus intervalos de confiança de 95% [IC (95%)]. RESULTADOS: A prevalência bruta da hipertensão foi de 47,0% [IC (95%): 41,1 - 53,0], a prevalência ajustada por idade foi de 43,2% [IC (95%): 35,7 - 50,7], enquanto a prevalência ajustada por escolaridade foi de 44,1% [IC (95%): 43,9 - 44,3]. De acordo com a análise multivariada, observou-se que idade, triglicerídeos, circunferência da cintura e sexo eram fatores de risco independentes associados à hipertensão. CONCLUSÃO: Os achados fornecem evidências importantes de que a hipertensão é um problema de saúde pública associado à dislipidemia e à obesidade abdominal, na área rural de Minas Gerais.

FUNDAMENTOS: Os dados sobre insuficiência cardíaca (IC) no Brasil são provenientes de centros terciários. Esses dados não podem ser extrapolados para a população rural, pois refletem características socioeconômico-culturais distintas. OBJETIVO: Estabelecer o perfil clínico-demográfico e indicadores de qualidade da IC em área rural. MÉTODOS: Estudo de coorte transversal, incluindo 166 pacientes da área rural do município de Valença-RJ. Após avaliação dos dados clínicos, laboratoriais e ecocardiográficos e utilizados o teste do qui-quadrado e o exato de Fisher para a análise das proporções, assim como o teste t de Student para as variáveis numéricas, com o intuito de estabelecer as características da população. RESULTADOS: A idade média foi de 61±14 anos, sendo 85 (51%) homens; 88 (53%) afrobrasileiros e 85 (51%) com ICFER. Comorbidades prevalentes: HAS em 151 (91%) e síndrome metabólica (SM) em 103 (62%). Etiologias mais comuns: hipertensiva em 77 (46%), isquêmica em 62 (37%). Indicadores de qualidade na IC: 43 (26%) com ecocardiograma prévio; 102 (62%) utilizavam betabloqueador; 147 (88%) receberam IECA ou BRA; e 22% dos portadores de FA utilizavam anticoagulação oral. Na ICFEN, predominou o sexo feminino p=0,001 RC 0,32 CI (0,17-0,60); SM p=0,004 RC 0,28 CI (1,31-4,78); e etiologia hipertensiva p<0,0001 RC 6,83 CI (3,45-13,5). Na ICFER, predominou o sexo masculino p=0,001, RC 0,32 CI (0,170-0,605) e etiologia isquêmica p<0,0001 RC 0,16 CI (0,079-0,330). CONCLUSÃO: Na área rural, a IC mostra semelhanças em relação ao sexo, cor e classificação da IC. A etiologia mais comum foi a hipertensiva. A ICFEN foi mais prevalente em mulheres e na SM. A ICFER associou-se a homens e etiologia isquêmica.

Energy from waste (E/W) technologies in the form o f biogas plants, CHP plants and other municipal solid waste (MSW) conversion technologies, have been gaining steady ground in the provision o f energy throughout Europe and the UK. Urban Waste Water Treatment Plants (UWWTP) are utilising much o f the same biochemical processes common to these E/W plants. Previous studies on Centralised Anaerobic Digestion (CAD) within Ireland found that the legislative and economic conditions were not conducive to such an operation on the grounds o f low energy price for electric and heat energy, and due to the restrictive nature o f the allowable feedstocks. Recent changes to the Irish REFIT tariff on energy produced from Anaerobic digestion; alterations to the regulation o f the allowable use o f animal by products(ABP); the recent enactment o f the Renewable Energy D irective (09/28/EC) and a subsequent review o f the draft Biowaste Directive (2001) required that the issue o f decentralised energy production in Ireland be reassessed. In this instance the feasibility study is based on a extant rural community, centred around the village o f Woodford Co Galway. The review found that the prevailing conditions were now such that it was technically and economically feasible for this biochemical process to provide energy and waste treatment facilities at the above location. The review also outlines the last item which is preventing this process from becoming achievable, specifically the lack o f a digestate regulation on land spreading which deals specifically with biowaste. The study finds that the implementation o f the draft EU biowaste regulations, with amendments for Cr and Hg levels to match the proposed Irish regulation for compost, would ensure that Ireland has some o f the most restrictive regulations in Europe for this application. The delay in completing this piece o f legislation is preventing national energy and waste issues from being resolved in a planned and stepwise fashion. A proposed lay out for the new Integrated Waste from Energy Plant (IW/EP) is presented. Budget economic projections and alternative revenue streams are outlined. Finally a review o f the national policies regarding the Rural Development Plan (RDP), the Rural Planning Guidelines (RPG) and the National Renewable Energy Action Plan (NREAP) are examined against the relevant EU directives.
