Une proportion significative de mères d’enfants pris en charge par la protection de la jeunesse souffrirait d’un trouble de personnalité limite (TPL). En effet, les difficultés parentales associées à ce trouble prédisposent davantage à des situations d’abus ou de négligence envers leurs enfants. Cependant, il existe peu d’information sur les difficultés et les besoins de ces mères dans ce contexte particulier. Cette étude qualitative a été réalisée auprès de 30 mères avec un TPL et dont au moins un enfant était suivi par le Centre jeunesse de Montréal-Institut universitaire. La présente étude a permis de mieux décrire les difficultés des mères en lien avec leur TPL, leurs enfants et leur situation conjugale. Leurs expériences des services en protection de la jeunesse ont révélé des suivis instables et peu adaptés à leurs difficultés. Les mères ont abordé l’importance des qualités relationnelles des intervenants à l’établissement d’une collaboration de la part des intervenants et l’importance de mettre en place des services spécifiquement dédiés aux parents. Il semblerait pertinent d’adapter les services de protection de la jeunesse afin d’inclure des interventions qui ciblent les difficultés spécifiques des parents qui ont un TPL, notamment en ce qui a trait à leurs habiletés parentales.

Introducción: el Trastorno Límite de la Personalidad afecta del 2% al 6% de los adultos en Estados Unidos. Es una condición de alta relevancia dentro de las patologías psiquiátricas debido a características como impulsividad, inestabilidad en las relaciones interpersonales, disregulación en el estado de ánimo y comportamiento agresivo. Esto determina un impacto negativo en la funcionalidad del individuo siendo la agresividad contra sí mismo o contra otras personas uno de sus componentes claves. Métodos: Revisión sistemática de la literatura de artículos de bases de datos y búsqueda manual de revistas relacionadas que aportaran la mejor evidencia con el fin de encontrar estudios que evaluaran, con instrumentos objetivos, los tratamientos farmacológicos disponibles para el manejo de la agresividad en el TLP .Se evaluó calidad metodológica y los estudios se organizaron en tablas de evidencia. Resultados: La búsqueda arrojo 1081 artículos de los cuales se seleccionaron 52 como potenciales y cinco fueron incluidos en esta revisión. Se clasificaron como nivel de evidencia Ib. El topiramato, el aripiprazol, el divalproato y la fluoxetina mostraron mejores resultados que el placebo especialmente en agresividad e impulsividad. El topiramato fue asociado con pérdida de peso. Los medicamentos fueron seguros y bien tolerados. Discusión: Los medicamentos evaluados mostraron ser mejores que placebo. La diversidad en las escalas utilizadas genera complejidad en la interpretación de resultados. Conclusión: La evidencia sugiere que el tratamiento farmacológico es efectivo en síntomas como agresividad e impulsividad comparado con placebo. Deben considerarse estudios que evalúen combinaciones de fármacos y psicoterapia.

The objectives of this study are: (a) to analyze if the relationship between parenting dimensions and children adjustment regarding coping strategies are similar in Argentinian and Spanish samples within poverty contexts; (b) to compare parental dimensions in the two groups studied, and (c) to determine the efficiency of parenting through the study of their influence in children coping strategies. The Graffar-Méndez Castellano Scale (Méndez-Castellano & Méndez, 1994), that brings a socioeconomic description of the population; an Argentinian Scale of Children Perception of Parental Relationships for 8 to 12 years of age (Richaud, 2007a), and the Argentine Questionnaire of Coping for children (Richaud, 2006) were administered to a sample of 458 Spanish and Argentinian children from 8 to 12 years old. Correlations were carried out to analyze the relationships between parenting dimensions and children coping strategies in both groups, and MANOVA, to study if there were different parental dimensions in the two groups —Argentina and Spain—, and to analyze if there were differences in children coping strategies. The results indicate that correlational pattern is similar in both groups, but parental dimensions are different for each culture, being the Argentinian parents more neglectful than Spanish parents. At the same time, Argentinian children adopt coping strategies less efficient that the Spanish children ones, involving in that way a greater emotional conflict.

Estudio de la eficacia de los programas de atención temprana en niños prematuros y/o con bajo peso al nacer y su relación con el control del estrés de los padres. Muestra inicial formada por 128 niños/as y sus progenitores de los cuales 53 formaron el grupo experimental y tenían la condición de pertenecer a un grupo de riesgo biológico (prematuros y/o bajo peso) y 75 constituían el grupo de control y eran niños/as nacidos a término. Los instrumentos utilizados para recoger la información: Escala NBAS (Neonatal Behavioral Assesment Scale) de Brazellon (1995/97). Escalas de Evaluación del Desarrollo Infantil de Bayley II (Bayley, 1993). Parenting Stress Index (PSI), Abidin 1995. Escala de Observación del Desarrollo (EOD, Secadas, 1992). Perinatal Risk Inventory (Scheiner y Sexton, 1991). Una entrevista semiabierta a los padres sobre la vida y la salud del niño y una encuesta sobre el grado de satisfacción con el Servicio de Atención Temprana, ambas elaboradas por el autor. Se evaluó a los niños prematuros en su primer mes de vida, a los nacidos a término y a los padres de ambos grupos. Para comparar el potencial efecto de mejora de los programas de AT sin hacer depender dicha mejora de los índices de desarrollo, se compararon los resultados del niño consigo mismo en distintos momentos temporales constituyéndose un índice de mejora individual.. La condición biológica de nacimiento no siempre predice la evolución de los niños de forma que parecen concurrir variables ambientales sobre todo familiares, condiciones de crianza, de estimulación ambiental que pueden intervenir decisivamente en este progreso. A sample of 147 mother-infant dyads was recruited from a peri-urban settlement outside Cape Town and seen at 2- and 18-months postpartum. At 18 months, 61.9% of the infants were rated as securely attached (B); 4.1% as avoidant (A); 8.2% as resistant (C); and 25.8% disorganized (D). Postpartum depression at 2 months, and indices of poor parenting at both 2 and 18 months, were associated with insecure infant attachment. The critical 2-month predictor variables for insecure infant attachment were maternal intrusiveness and maternal remoteness, and early maternal depression. When concurrent maternal sensitivity was considered, the quality of the early mother-infant relationship remained important, but maternal depression was no longer predictive. Cross-cultural differences and consistencies in the development of attachment are discussed.

We present an integrative review of the development of child anxiety, drawing on a number of strands of research. Family aggregation and genetic studies indicate raised vulnerability to anxiety in offspring of adults with the disorder (e.g. the temperamental style of behavioural inhibition, or information processing biases). Environmental factors are also important; these include adverse life events and exposure to negative information or modelling. Parents are likely to be key, although not unique, sources of such influences, particularly if they are anxious themselves. Some parenting behaviours associated with child anxiety, such as overprotection, may be elicited by child characteristics, especially in the context of parental anxiety, and these may serve to maintain child disorder. Emerging evidence emphasizes the importance of taking the nature of child and parental anxiety into account, of constructing assessments and interventions that are both disorder specific, and of considering bidirectional influences.

The postpartum period is a sensitive time due to the presence and demands of the developing infant. The care provided by a mother to her infant during this period may be compromised if she is suffering from postnatal depression or postpartum psychosis. Evidence has been emerging which suggests that postnatal depression and postpartum psychoses have adverse effects on the quality of the mother-infant relationship and also on the infants subsequent cognitive and emotional development. Presented is a review of the literature relating to how these conditions impact on parenting and infant outcomes, what measures are in place to detect these conditions and evidence-based models of best clinical practice are proposed.
The effects of maternal social phobia on mother-infant interactions and infant social responsiveness

Background: Social phobia aggregates in families. The genetic contribution to intergenerational transmission is modest, and parenting is considered important. Research on the effects of social phobia on parenting has been subject to problems of small sample size, heterogeneity of samples and lack of specificity of observational frameworks. We addressed these problems in the current study.Methods: We assessed mothers with social phobia (N = 84) and control mothers (N = 89) at 10 weeks in face-to-face interactions with their infants, and during a social challenge, namely, engaging with a stranger. We also assessed mothers with generalised anxiety disorder (GAD) (N = 50). We examined the contribution to infant social responsiveness of early infant characteristics (neonatal irritability), as well as maternal behaviour. Results: Mothers with social phobia were no less sensitive to their infants during face-to-face interactions than control mothers, but when interacting with the stranger they appeared more anxious, engaged less with the stranger themselves, and were less encouraging of the infant's interaction with the stranger; infants of index mothers also showed reduced social responsiveness to the stranger. These differences did not apply to mothers with GAD and their infants. Regression analyses showed that the reduction in social responsiveness in infants of mothers with social phobia was predicted by neonatal irritability and the degree to which the mother encouraged the infant to interact with the stranger.Conclusions: Mothers with social phobia show specific parenting difficulties, and their infants show early signs of reduced social responsiveness that are related to both individual infant differences and a lack of maternal encouragement to engage in social interactions.

Although the impact of autism spectrum disorders (ASDs) on the family is well recognized, the way mothers attempt to make sense of the diagnosis is largely unexplored. However, in other disabilities, attributions have been shown to predict a variety of outcomes including maternal wellbeing and engagement in treatment. Using Weiner's (198S) three-dimensional model, 16 mothers were interviewed to examine the nature and impact of their beliefs about their child's ASD using semi-structured interviews and measures of depression, parenting stress and expectations for their child's future. The findings suggested that mothers made a diverse and complex range of attributions that were consistent with Weiner's dimensions of locus of cause, stability and controllability. The nature of their attributions reflected particular difficulties associated with ASDs, such as uncertainties regarding cause and prognosis. Taking account of mothers' search for meaning will better enable professionals to support families following diagnosis.

To investigate sources of influences connecting mothers' and their children's anxious cognitions, 65 children (aged 10 to 11 years) completed self-report measures of anxiety. Children and mothers responded to an ambiguous scenario questionnaire and measures of parenting style and life events. Mothers also reported expectations about their child's reaction to ambiguous situations. Mothers' and children's threat cognitions were significantly correlated (r = .31), and partially mediated by mothers' expectations about their child. Mothers' anticipated distress was associated with expectations for their child's distress, which was associated with the child's own anticipated distress. Parenting and life events were significantly associated with children's interpretative bias, but did not mediate the intergenerational association in interpretative bias. The results suggest influences on children's 'anxious cognitive style' and potential targets for preventing and reducing maladaptive cognitions in children.

Anxiety disorders are common among parents of anxious children and have been found to impede child treatment outcomes, yet it is unclear whether it is parental anxiety that needs to be targeted in therapy or associated parental behaviours. Twenty-two children (6-12 years) with a current anxiety disorder and their mothers received cognitive-behavioural treatment (CBT) for child anxiety. In addition, of the 12 mothers who met criteria for a current anxiety disorder, 6 received CBT for their own disorder. Assessments were made of the mother-child interaction. The main findings were: (1) children did less well from treatment where their mothers had a current anxiety disorder; (2) treatment of maternal anxiety disorder did not improve child treatment outcome; and (3) maternal overinvolvement and expression of fear was associated with child treatment outcome. The results suggest that in the context of maternal anxiety disorder, child treatment outcome may be improved by specifically targeting parenting behaviours. Objective To assess the efficacy of an intervention designed to improve the mother-infant relationship and security of infant attachment in a South African peri-urban settlement with marked adverse socioeconomic circumstances. Design Randomised controlled trial. Setting Khayelitsha, a peri-urban settlement in South Africa. Participants 449 pregnant women. Interventions The intervention was delivered from late pregnancy and for six months postpartum. Women were visited in their homes by previously untrained lay community workers who provided support and guidance in parenting. The purpose of the intervention was to promote sensitive and responsive parenting and secure infant attachment to the mother. Women in the control group received no therapeutic input from the research team. Main outcome measures Primary outcomes: quality of mother-infant interactions at six and 12 months postpartum; infant attachment security at 18 months. Secondary outcome: maternal depression at six and 12 months. Results The intervention was associated with significant benefit to the mother-infant relationship. At both six and 12 months, compared with control mothers, mothers in the intervention group were significantly more sensitive (6 months: mean difference=0.77 (SD 0.37), t=2.10, P<0.05, d=0.24; 12 months: mean difference=0.42 (0.18), t=-2.04, P<0.05, d=0.26) and less intrusive (6 months: mean difference=0.68 (0.36), t=2.28, P<0.05, d=0.26; 12 months: mean difference=-1.76 (0.86), t=2.28, P<0.05, d=0.24) in their interactions with their infants. The intervention was also associated with a higher rate of secure infant attachments at 18 months (116/156 (74%) v 102/162 (63%); Wald=4.74, odds ratio=1.70, P<0.05). Although the prevalence of maternal depressive disorder was not significantly reduced, the intervention had a benefit in terms of maternal depressed mood at six months (z=2.05, P=0.04) on the Edinburgh postnatal depression scale). Conclusions The intervention, delivered by local lay women, had a significant positive impact on the quality of the mother-infant relationship and on security of infant attachment, factors known to predict favourable child development. If these effects persist, and if they are replicated, this intervention holds considerable promise for use in the developing world. Trial registration Current Controlled Trials ISRCTN25664149.
