Stage IV non-small cell lung cancer is a fatal disease, with a median survival of 14 months. Systemic chemotherapy is the most common approach. However the impact in overall survival and quality of life still a controversy. OBJECTIVES: To determine differences in overall survival and quality of life among patients with stage IV non-small cell lung cancer non-metastatic to the brain treated with best supportive care versus systemic chemotherapy. PATIENTS: From February 1990 through December 1995, 78 eligible patients were admitted with the diagnosis of stage IV non-small cell lung cancer . Patients were divided in 2 groups: Group A (n=31 -- treated with best supportive care ), and Group B (n=47 -- treated with systemic chemotherapy). RESULTS: The median survival time was 23 weeks (range 5 -- 153 weeks) in Group A and 55 weeks (range 7.4 -- 213 weeks) in Group B (p=0.0018). In both groups, the incidence of admission for IV antibiotics and need of blood transfusions were similar. Patients receiving systemic chemotherapy were also stratified into those receiving mytomycin, vinblastin, and cisplatinum, n=25 and those receiving other combination regimens (platinum derivatives associated with other drugs, n=22). Patients receiving mytomycin, vinblastin, and cisplatinum, n=25 had a higher incidence of febrile neutropenia and had their cycles delayed for longer periods of time than the other group. These patients also had a shorter median survival time (51 versus 66 weeks, p=0.005). CONCLUSION: In patients with stage IV non-small cell lung cancer, non-metastatic to the brain, chemotherapy significantly increases survival compared with best supportive care.

RESUMO - Introdução: O encerramento das minas de urânio em Portugal tem suscitado preocupação no que respeita aos eventuais efeitos que as radiações emitidas e os agentes químicos presentes terão na saúde das populações. Para esclarecer a existência de tal efeito foi delineado um programa de investigação em que o presente estudo se enquadra. Sendo conhecido que as populações expostas a minas de urânio, nomeadamente os mineiros, têm risco acrescido de sofrer de neoplasias, especialmente de «neoplasias da traqueia, dos brônquios e do pulmão», foi este o grupo de neoplasias seleccionado para constituir a hipótese principal do estudo. Assim, o estudo pretendeu verificar se existe associação entre exposição a minas de urânio e suas escombreiras (especialmente à mina da Urgeiriça, no concelho de Nelas) e a mortalidade por alguns grupos de neoplasias malignas, nomeadamente as «neoplasias malignas da traqueia, dos brônquios e do pulmão». Material e métodos: Trata-se de um estudo «ecológico» em que se consideraram «expostos» os residentes no concelho de Nelas, bem como no conjunto de concelhos com minas de urânio, e «não expostos» os residentes nos restantes concelhos das NUTS Dão-Lafões e Beira Interior Norte e Serra da Estrela. A análise centrou-se no cálculo, para cada concelho ou grupo de concelhos, das razões padronizadas de mortalidade (RPM) por «neoplasias malignas da traqueia, dos brônquios e do pulmão», por «neoplasia maligna do estômago» e por «todas as neoplasias malignas » no período de vinte anos compreendido entre 1980 e 1999. Resultados: Tomando os dois sexos em conjunto, o concelho de Nelas teve a RPM mais elevada para as «neoplasias malignas da traqueia, dos brônquios e do pulmão» (RPM = 133; p = 0,003). Teve também o valor mais elevado no sexo masculino (RPM = 126, não significativo) e o segundo mais elevado no sexo feminino (RPM = 142, não significativo). A razão das RPM concelho de Nelas/concelhos limítrofes de Nelas foi 1,46, p = 0,002 (homens: 1,50 p = 0,003; mulheres: 1,27, não significativo). As razões das RPM concelho de Nelas/concelhos com minas (1,94, p = 0,001) e concelho de Nelas/concelhos sem minas (1,57, p = 0,001) foram claramente superiores a 1. Pelo contrário, as RPM por «neoplasia maligna do estômago » foram sobretudo elevadas nos concelhos da NUTS Beira Interior Norte, que inclui grande parte do distrito da Guarda, (Trancoso: 154, p = 0,000; Sabugal: 146, p = 0,000), embora se tenham observado valores elevados em alguns concelhos da NUTS Dão-Lafões (Vila Nova de Paiva: 154, p = 0,001). Saliente-se que os dois valores mais baixos ocorreram nos concelhos de Tábua (RPM = 57; p = 0,000) e de Nelas (RPM = 60; p = 0,000). Para o conjunto de «todas as neoplasias malignas» as RPM dos vários concelhos variaram entre 62 e 100 sem que a distribuição desses valores sugerisse qualquer associação positiva relevante. Discussão: Os resultados mostraram que a população do concelho de Nelas teve, no período estudado, um risco acrescido de morrer por «neoplasias malignas da traqueia, dos brônquios e do pulmão» quando comparado com a dos concelhos limítrofes e restantes concelhos das NUTS estudadas. A existência da mina da Urgeiriça e da sua escombreira é uma possível causa desse excesso de mortalidade. Ele poderá ter sido gerado, por um lado, através da existência de uma percentagem elevada de ex-mineiros, bem como, por outro lado, através de uma exposição ambiental geral, facto este que é sustentado pela ocorrência de excesso de mortalidade tanto em homens como em mulheres. O excesso de mortalidade por aquele grupo de neoplasias pode ainda ter origem noutras exposições cujo potencial papel é discutido.

A rare case of thumb metastasis from small cell lung cancer is presented. The patient underwent local radiotherapy with complete palliation of symptoms. She died 4 months later with disseminated disease. Considerations about incidence, treatment, and physiopathology of this kind of dissemination are made. Conservative treatment of finger metastasis with radiation may be considered due to the poor outcome of these patients.

OBJECTIVE: Macrolide antibiotics have anti-inflammatory properties in lung diseases. The aim of this study was to investigate the effect of clarithromycin in pulmonary cellular inflammatory response in mice. METHOD: Eight adult Swiss mice were studied. All animals received an intranasal challenge (80 µL) with dead Pseudomonas aeruginosa (1.0 x 10(12) CFU/mL). Bronchoalveolar lavage was performed 2 days later, with total cell count and differential cell analysis. The study group (n = 4) received clarithromycin treatment (50 mg/kg/day, intraperitoneal) for 5 days. Treatment was initiated 2 days before intranasal challenge. RESULTS: There was no significant difference in total cell count between the groups (mean: 2.0 x 10(6) and 1.3 x 10(6), respectively). In both groups, there was a predominance of neutrophils. However, the study group had a higher percentage of lymphocytes in the bronchoalveolar lavage than the control group (median of 19% vs 2.5%, P = .029). CONCLUSION: Clarithromycin alters the cytological pattern of bronchoalveolar lavage of Swiss mice with neutrophil pulmonary inflammation, significantly increasing the percentage of lymphocytes.

OBJECTIVE: To analyze surgical and pathological parameters and outcome and prognostic factors of patients with nonsmall cell lung cancer (NSCLC) who were admitted to a single institution, as well as to correlate these findings to the current staging system. METHOD: Seven hundred and thirty seven patients were diagnosed with NSCLC and admitted to Hospital do Cancer A. C. Camargo from 1990 to 2000. All patients were included in a continuous prospective database, and their data was analyzed. Following staging, a multidisciplinary team decision on adequate management was established. Variables included in this analysis were age, gender, histology, Karnofsky index, weight loss, clinical stage, surgical stage, chemotherapy, radiotherapy, and survival rates. RESULTS: 75.5% of patients were males. The distribution of histologic type was squamous cell carcinoma 51.8%, adenocarcinoma 43.1%, and undifferentiated large cell carcinoma 5.1%. Most patients (73%) presented significant weight loss and a Karnofsky index of 80%. Clinical staging was IA 3.8%, IB 9.2%, IIA 1.4%, IIB 8.1%, IIIA 20.9%, IIIB 22.4%, IV 30.9%. Complete tumor resection was performed in 24.6% of all patients. Surgical stage distribution was IA 25.3%, IB 1.4%, IIB 17.1%, IIIA 16.1%, IIIB 20.3%, IV 11.5%. Chemotherapy and radiotherapy were considered therapeutic options in 43% and 72%, respectively. The overall 5-year survival rate of nonsmall cell lung cancer patients in our study was 28%. Median survival was 18.9 months. CONCLUSIONS: Patients with NSCLC who were admitted to our institution presented with histopathologic and clinical characteristics that were similar to previously published series in cancer hospitals. The best prognosis was associated with complete tumor resection with lymph node dissection, which is only achievable in earlier clinical stages.

Purpose Congenital diaphragmatic hernia (CDH) is characterized by a developmental defect in the diaphragm, pulmonary hypoplasia and pulmonary hypertension. NPAS3 is a PAS domain transcription factor regulating Drosophila tracheogenesis. NPAS3 null mice develop pulmonary hypoplasia in utero and die after birth due to respiratory failure. We aimed to evaluate NPAS3 expres- sion during normal and abnormal lung development due to CDH. Methods CDH was induced by administering 100 mg/ml nitrofen to time-pregnant dams on embryonic day (E) 9 of gestation. Lungs were isolated on E15, E18 and E21 and NPAS3 localization was determined by immunohisto- chemistry and quantified using Western blotting. Results We found that only E21 hypoplastic CDH lungs have reduced expression of NPAS3 in the terminal sac- cules. Western blotting confirmed the down-regulation of NPAS3 protein in the nitrofen-induced hypoplastic lungs. Conclusions We demonstrate for the first time that ni- trofen-induced hypoplastic CDH lungs have reduced NPAS3 expression in the terminal saccules during the later stages of abnormal lung development. Our findings suggest that NPAS3 is associated with pulmonary hypoplasia in CDH.

Congenital diaphragmatic hernia (CDH) is characterised by a spectrum of lung hypoplasia and consequent pulmonary hypertension, leading to high morbidity and mortality rates. Moreover, CDH has been associated with an increase in the levels of pulmonary neuroendocrine factors, such as bombesin and ghrelin, and a decrease in the action of retinoic acid (RA). The present study aimed to elucidate the interaction between neuroendocrine factors and RA. In vitro analyses were performed on Sprague-Dawley rat embryos. Normal lung explants were treated with bombesin, ghrelin, a bombesin antagonist, a ghrelin antagonist, dimethylsulfoxide (DMSO), RA dissolved in DMSO, bombesin plus RA and ghrelin plus RA. Hypoplastic lung explants (nitrofen model) were cultured with bombesin, ghrelin, bombesin antagonist or ghrelin antagonist. The lung explants were analysed morphometrically, and retinoic acid receptor (RAR) α, β and γ expression levels were assessed via Western blotting. Immunohistochemistry analysis of RAR was performed in normal and hypoplastic lungs 17.5 days post-conception (dpc). Compared with the controls, hypoplastic lungs exhibited significantly higher RARα/γ expression levels. Furthermore considering hypoplastic lungs, bombesin and ghrelin antagonists decreased RARα/γ expression. Normal lung explants (13.5 dpc) treated with RA, bombesin plus RA, ghrelin plus RA, bombesin or ghrelin exhibited increased lung growth. Moreover, bombesin and ghrelin increased RARα/γ expression levels, whereas the bombesin and ghrelin antagonists decreased RARα/γ expression. This study demonstrates for the first time that neuroendocrine factors function as lung growth regulators, sensitising the lung to the action of RA through up-regulation of RARα and RARγ.

Hyaluronan (HA) shows promise for detecting cancerous change in pleural effusion and urine. However, there is uncertainty about the localization of HA in tumor tissue and its relationship with different histological types and other components of the extracellular matrix, such as angiogenesis. We evaluated the association between HA and degree of malignancy through expression in lung tumor tissue and sputum. Tumoral tissue had significantly increased HA compared to normal tissue. Strong HA staining intensity associated with cancer cells was significant in squamous cell carcinoma compared to adenocarcinoma and large cell carcinoma. A significant direct association was found between tumors with a high percentage of HA and MVD (microvessel density) in tumoral stroma. Similarly significant was the direct association between N1 tumors and high levels of HA in cancer cells. Cox multivariate analysis showed significant association between better survival and low HA. HA increased in sputum from lung cancer patients compared to cancer-free and healthy volunteers and a significant correlation was found between HA in sputum and HA in cancer tissue. Localization of HA in tumor tissue was related to malignancy and reflected in sputum, making this an emerging factor for an important diagnostic procedure in patients suspected to have lung cancer. Further study in additional patients in a randomized prospective trial is required to finalize these results and to validate our quantitative assessment of HA, as well as to couple it to gold standard sputum cytology.

OBJECTIVE: To analyze clinical and histologic findings of 50 patients with primary neoplams of the heart in a tertiary referral center. METHODS: From 1980 to 1998, we retrospectively analyzed 50 patients, 32 of whom were females, whose ages ranged from 9 to 73 years (mean age = 44.16±18 years). RESULTS: Most tumors were located in the left side of the heart (72%), myxoma being the most common (84%) histologic type. The other histologic types found were as follows: fibroma (4%), lipoma (2%), rhabdomyosarcoma (2%), hemangioma (2%), sarcoma (2%), angiosarcoma (2%), and lymphoma (2%). Diagnosis was established by echocardiography in 94% of the cases. Clinical findings were as follows: dyspnea (36%), weight loss (20%), palpitations (18%), chest pain (16%), fever (8%), and arthralgia (6%). All patients with thromboembolic phenomena (10%) had left atrial myxoma. Approximately 20% of the patients were asymptomatic at the initial clinical assessment. CONCLUSION: Primary cardiac tumors are a rare entity with diverse clinical and histologic findings, requiring, therefore, a high level of clinical suspicion.

ჩატარებულია რადონის განაწილების რაოდენობრივი შეფასება დასავლეთ საქართველოს ცალკეულ რაიონებში. მიღებული მონაცემები მოწმობს, რომ 100-ზე მეტი წყლის სინჯში აღინიშნება რადონის მაღალი შემცველობა. ამ უბნებთანაა დაკავშირებული ბინებში რადონის დაგროვების მაღალი მაჩვენებლები.ჩვენს მიერ ჩატარებული კვლევა კიდევ ერთხელ ადასტურებს კორელაციურ კავშირს რადონის კონცენტრაციასა და ფილტვის კიბოს გავრცელებას შორის.

Studies carried out in Sw outbred mice showed that there is no correlation between the degree of lung granulomatous reaction and the level of acquired resistance against S. mansoni infection induced by BCG.
