The Minister for Health requested the Chief Medical Officer to prepare a Report for him on issues that arose following a Primetime Investigates programme relating to Portlaoise Hospital Maternity Services (PHMS) on 30th January 2014. This Report provides a preliminary assessment of PHMS focusing on perinatal deaths (2006-date) and related matters. Through a series of recommendations it sets out the need for further examination or actions where the findings of this preliminary assessment suggest such a need. It also makes clear who should be responsible for these further examinations or actions. Â Download the Report

Foram realizadas análises epidemiológicas entre pacientes com tuberculose atendidos no Hospital Dr. Baptista de Sousa de São Vicente. Os dados foram registados pelo Programa de Luta contra Tuberculose no período de 1997 a 2005 e as análises não envolveram a participação directa dos indivíduos. Os reportes foram estatisticamente analisados usando programas computorizados (SPSS). Os resultados demonstraram uma prevalência e incidência anual de 61,6 e 50,49 casos/100000 hab, respectivamente. A presença de surtos foi destacada, com leve aumento depois de 2001, mas com tendência à manutenção das taxas de prevalência. As maiores incidências foram reportadas em 1997, 2001 e 2003, contudo, verifica-se estabilidade da tendência. Da mesma forma, a tuberculose pulmonar foi mais frequente (93,7%), sendo reportada mais em 2001 (12,89%), destacando-se também uma inter-relação entre a tuberculose pulmonar e a extrapulmonar. A tendência à cura foi aumentada consideravelmente nos anos recentes do estudo. Baixas taxas de abandono (8,7%) e de transferências (14,5%) também foram verificadas. A taxa de mortalidade foi alta no início da pesquisa (7,4%) com posterior aumento, contudo com tendência ao declínio nos anos recentes. Também pode ser identificado que em 33,3% da amostra o aumento da população levou à possibilidade de contacto com o agente e, por conseguinte, a novos casos. Em este grupo a forma clínica mais comum foi a pulmonar, a procura de tratamentos é grande e também se regista uma baixa taxa de abandono do tratamento.

Vital tissue provided by fresh frozen tissue banking is often required for genetic tumor profiling and tailored therapies. However, the potential patient benefits of fresh frozen tissue banking are currently limited to university hospitals. The objective of the present pilot study--the first one in the literature--was to evaluate whether fresh frozen tissue banking is feasible in a regional hospital without an integrated institute of pathology.

BACKGROUND Avoidable hospitalizations (AH) are hospital admissions for diseases and conditions that could have been prevented by appropriate ambulatory care. We examine regional variation of AH in Switzerland and the factors that determine AH. METHODS We used hospital service areas, and data from 2008-2010 hospital discharges in Switzerland to examine regional variation in AH. Age and sex standardized AH were the outcome variable, and year of admission, primary care physician density, medical specialist density, rurality, hospital bed density and type of hospital reimbursement system were explanatory variables in our multilevel poisson regression. RESULTS Regional differences in AH were as high as 12-fold. Poisson regression showed significant increase of all AH over time. There was a significantly lower rate of all AH in areas with more primary care physicians. Rates increased in areas with more specialists. Rates of all AH also increased where the proportion of residences in rural communities increased. Regional hospital capacity and type of hospital reimbursement did not have significant associations. Inconsistent patterns of significant determinants were found for disease specific analyses. CONCLUSION The identification of regions with high and low AH rates is a starting point for future studies on unwarranted medical procedures, and may help to reduce their incidence. AH have complex multifactorial origins and this study demonstrates that rurality and physician density are relevant determinants. The results are helpful to improve the performance of the outpatient sector with emphasis on local context. Rural and urban differences in health care delivery remain a cause of concern in Switzerland.
