On 11 October 2013, the CEOs of 10 large European energy utilities issued a warning that the European energy infrastructure is “in jeopardy” and called for an end to support for renewables on grounds that wind and solar were mature technologies that no longer required such support. Given the unlikelihood, however, that EU decision-makers would renege on their decarbonisation or renewable energy targets, Fabio Genoese asks in this commentary whether it would not be a better strategy for conventional generators to explore new business models built around a ‘reliability pricing system’, in which nearly 100% reliability would be guaranteed for base load but not for peak demand.

Gypsum grains were identified in Miocene-Pleistocene sediment cores from two deep-water ODP sites, Site 918 off the SE Greenland margin and Site 887 in the Gulf of Alaska, and in Holocene sediment cores from shallow-water localities in Disenchantment Bay and Muir Inlet in southern Alaska. Although initial morphologic and textural observations suggested a complex system in which the gypsum may have had more than one origin, quantitative sulfur isotope analyses of the gypsum provide evidence of its detrital nature. d34S values in gypsum from southern Alaska range between +0.0 and +7.1 per mil. Gypsum has d34S values between -27.1 and -27.5 per mil in the Gulf of Alaska and values between -28.5 and +0.2 per mil off the SE Greenland margin. All of these isotopic signatures are too highly depleted in d34S to have precipitated from seawater, present or past. In addition there is no significant change in d34S values for gypsum crystals with differing physical characteristics (abraded vs. unabraded) from the same stratigraphic horizon, suggesting all the gypsum is detrital regardless of the degree of abrasion. The isotopic and physical evidence, in combination with the onshore geology the environmental setting, and site characteristics of the gypsum-bearing marine localities, lead us to propose that the ultimate source of the gypsum is precipitation from freeze-induced terrestrial sediment or soil brines. Furthermore the combined evidence suggests that the subsequent occurrence of gypsum in glacimarine sediments results from ice-rafting (by icebergs or sea ice) of the frozen regolith and/or, in the proximal glacimarine setting of southern Alaska, very rapid burial via turbidity currents.

In principle, the world and life itself are the contexts of theatrical events. The term context is broad and thus seems hardly usable. It only makes sense to use the term when terminologies and methodologies determine which parts of their contexts are to be incorporated and analysed for which theatrical event. This presentation exemplifies a method which is particularly suitable for sensibly selecting the most important contexts for research in theatre history. The complexity of the representation increases continuously from The Presentation of Self in Everyday Life to Brecht’s “Street Scene” and “Everyday Theatre”, portrayals of rulers in feasts and parades to Hamlet productions by the Royal Shakespeare Company or a Wagner opera in Bayreuth. The different forms of theatre thus constitute a continuum which spans from “everyday theatre” to “art theatre”. The representation of the world in this continuum is sometimes questioned by the means of theatre, for example when Commedia dell’arte takes a critical stance towards the representative theatre of the humanists or when playful devices such as reversal, parody and fragmentation challenge the representative character of productions, which is applied by the Vice character for instance. There is a second component that has an impact on the continuum without a theatrical device: attitude, opinion, norms and bans which originate from society. As excerpts of contexts, they refer to single forms of theatre in the continuum. This results in a complex system of four components which evolves from the panorama between the antipodes “everyday theatre” and “art theatre” as well as both spheres of influence of which only one uses theatrical devices. All components interact in a specific time frame in a specific place in a specific way in each case, which can then be described as the theatricality in this time frame. This presentation will deal with what the concept is capable of doing.

Objective. To improve quality of in-hospital care of patients with acute coronary syndromes using a multifaceted quality improvement program. Design. Prospective, before and after study of the effects of quality improvement interventions between October 2000 and August 2002. Quality of care of patients admitted between 1 October 2000 and 16 April 2001 (baseline) was compared with that of those admitted between 15 February 2002 and 31 August 2002 (post-intervention). Setting. Three teaching hospitals in Brisbane, Australia. Study participants. Consecutive patients (n = 1594) admitted to hospital with acute coronary syndrome [mean age 68 years (SD 14 years); 65% males]. Interventions. Clinical guidelines, reminder tools, and educational interventions; 6-monthly performance feedback; pharmacist-mediated patient education program; and facilitation of multidisciplinary review of work practices. Main outcome measures. Changes in key quality indicators relating to timing of electrocardiogram (ECG) and thrombolysis in emergency departments, serum lipid measurement, prescription of adjunctive drugs, and secondary prevention. Results. Comparing post-intervention with baseline patients, increases occurred in the proportions of eligible patients: (i) undergoing timely ECG (70% versus 61%; P = 0.04); (ii) prescribed angiotensin-converting enzyme inhibitors (70% versus 60%; P = 0.002) and lipid-lowering agents (77% versus 68%; P = 0.005); (iii) receiving cardiac counselling in hospital (57% versus 48%; P = 0.009); and (iv) referred to cardiac rehabilitation (17% versus 8%; P < 0.001). Conclusions. Multifaceted approaches can improve care processes for patients hospitalized with acute coronary syndromes. Care processes under direct clinician control changed more quickly than those reliant on complex system factors. Identifying and overcoming organizational impediments to quality improvement deserves greater attention.

The new Australian Computational Earth Systems Simulator research facility provides a virtual laboratory for studying the solid earth and its complex system behavior. The facility's capabilities complement those developed by overseas groups, thereby creating the infrastructure for an international computational solid earth research virtual observatory.

Study Design. Prospective clinical case series. Objective. To evaluate the clinical outcome of anterior endoscopic instrumention for scoliosis using the SRS-24 questionnaire and to examine how these scores change over a 2-year follow-up period. Summary of Background Data. Anterior endoscopic instrumentation correction has several advantages compared with open procedures. However, the clinical results of this technique using a validated outcome measure have rarely been reported in the literature. Methods. A total of 83 consecutive patients underwent endoscopic anterior instrumentation performed at a single unit. Patients completed the SRS-24 questionnaire before surgery and at 3, 6, 12, and 24 months after surgery. The SRS-24 scores were compared between each of the follow-up intervals. Results. The pain, general self-image, and function from back condition domains improved after surgery (P < 0.05). Activity level significantly improved between 3 and 6 months, and both function domains improved between 6 and 12 months (P < 0.05). None of the domains increased significantly after 1 year. Conclusions. Endoscopic anterior instrumentation for scoliosis significantly improved pain, self-image, and function. The greatest improvement in function occurred between 6 and 12 months after surgery. The SRS-24 scores at 1 year from surgery may provide a good indicator of patient outcome in the long-term.

Systems biology is based on computational modelling and simulation of large networks of interacting components. Models may be intended to capture processes, mechanisms, components and interactions at different levels of fidelity. Input data are often large and geographically disperse, and may require the computation to be moved to the data, not vice versa. In addition, complex system-level problems require collaboration across institutions and disciplines. Grid computing can offer robust, scaleable solutions for distributed data, compute and expertise. We illustrate some of the range of computational and data requirements in systems biology with three case studies: one requiring large computation but small data (orthologue mapping in comparative genomics), a second involving complex terabyte data (the Visible Cell project) and a third that is both computationally and data-intensive (simulations at multiple temporal and spatial scales). Authentication, authorisation and audit systems are currently not well scalable and may present bottlenecks for distributed collaboration particularly where outcomes may be commercialised. Challenges remain in providing lightweight standards to facilitate the penetration of robust, scalable grid-type computing into diverse user communities to meet the evolving demands of systems biology.

This paper presents an approach for optimal design of a fully regenerative dynamic dynamometer using genetic algorithms. The proposed dynamometer system includes an energy storage mechanism to adaptively absorb the energy variations following the dynamometer transients. This allows the minimum power electronics requirement at the mains power supply grid to compensate for the losses. The overall dynamometer system is a dynamic complex system and design of the system is a multi-objective problem, which requires advanced optimisation techniques such as genetic algorithms. The case study of designing and simulation of the dynamometer system indicates that the genetic algorithm based approach is able to locate a best available solution in view of system performance and computational costs.

Avaliação de desempenho de equipes multifuncionais é um conceito que tem sido abordado mais freqüentemente nas últimas duas décadas. Diversas teorias e conceitos foram desenvolvidos para tornar as avaliações menos qualitativas ou fundamentadas em percepções, compreendendo um conjunto de diferentes técnicas interpretativas que visam descrever e decodificar os componentes de um sistema complexo de significados. Com esse foco o projeto comparou três modelos de avaliação de desempenho de equipes. O primeiro modelo baseado na personalidade do participante, afirma que os resultados das equipes são influenciados pela seleção prévia dos participantes e que as características individuais desses participantes são críticas para o desempenho da equipe. O segundo modelo fundamenta suas afirmações nas relações interconectadas de positividade e negatividade; questionamento e argumentação e atuação em equipe. O terceiro modelo baseado no processo de interação de grupos também avalia as relações que influenciam o alto desempenho de equipes multifuncionais. Os três modelos foram comparados, através de um estudo de caso único em indústria química paulista, medindo-se os resultados de correlação das equipes e as interações entre os participantes das equipes. As metas atingidas pelas equipes foram utilizadas para suportar os resultados das avaliações dos modelos como sistema de avaliação de equipes. Pode-se concluir que houve uma aproximação satisfatória confirmando a influência sugerida pelos modelos avaliados. A comparação entre os três modelos não permitiu afirmar qual dos três modelos é mais eficaz para avaliação de equipes. Entretanto, é possível, sugerir que a combinação de elementos dos três modelos permitirá a construção de um processo eficaz e prático de avaliação de equipes.(AU)

OBJECTIVES: The objective of this research was to design a clinical decision support system (CDSS) that supports heterogeneous clinical decision problems and runs on multiple computing platforms. Meeting this objective required a novel design to create an extendable and easy to maintain clinical CDSS for point of care support. The proposed solution was evaluated in a proof of concept implementation. METHODS: Based on our earlier research with the design of a mobile CDSS for emergency triage we used ontology-driven design to represent essential components of a CDSS. Models of clinical decision problems were derived from the ontology and they were processed into executable applications during runtime. This allowed scaling applications' functionality to the capabilities of computing platforms. A prototype of the system was implemented using the extended client-server architecture and Web services to distribute the functions of the system and to make it operational in limited connectivity conditions. RESULTS: The proposed design provided a common framework that facilitated development of diversified clinical applications running seamlessly on a variety of computing platforms. It was prototyped for two clinical decision problems and settings (triage of acute pain in the emergency department and postoperative management of radical prostatectomy on the hospital ward) and implemented on two computing platforms-desktop and handheld computers. CONCLUSIONS: The requirement of the CDSS heterogeneity was satisfied with ontology-driven design. Processing of application models described with the help of ontological models allowed having a complex system running on multiple computing platforms with different capabilities. Finally, separation of models and runtime components contributed to improved extensibility and maintainability of the system.

Age related macular degeneration (AMD) is the leading cause of blindness in individuals older than 65 years of age. It is a multifactorial disorder and identification of risk factors enables individuals to make lifestyle choices that may reduce the risk of disease. Collaboration between geneticists, ophthalmologists, and optometrists suggests that genetic risk factors play a more significant role in AMD than previously thought. The most important genes are associated with immune system modulation and the complement system, e.g., complement factor H (CFH), factor B (CFB), factor C3, and serpin peptidase inhibitor (SERPING1). Genes associated with membrane transport, e.g., ATP-binding cassette protein (ABCR) and voltage-dependent calcium channel gamma 3 (CACNG3), the vascular system, e.g., fibroblast growth factor 2 (FGF2), fibulin-5, lysyl oxidase-like gene (LOXL1) and selectin-P (SELP), and with lipid metabolism, e.g., apolipoprotein E (APOE) and hepatic lipase (LIPC) have also been implicated. In addition, several other genes exhibit some statistical association with AMD, e.g., age-related maculopathy susceptibility protein 2 (ARMS2) and DNA excision repair protein gene (ERCC6) but more research is needed to establish their significance. Modifiable risk factors for AMD should be discussed with patients whose lifestyle and/or family history place them in an increased risk category. Furthermore, calculation of AMD risk using current models should be recommended as a tool for patient education. It is likely that AMD management in future will be increasingly influenced by assessment of genetic risk as such screening methods become more widely available. © 2013 Spanish General Council of Optometry.

The economy is communication between Man and Nature. It is an interaction-network between our outside and inside Nature, that is, the external Nature surrounding us and the internal nature expressing our human essence. Money is an institution of the society, an infrastructure that ensures division of labour, enables the flow of information and material between the participants. The concept of regional material and financial circular flow will be more important with the oncoming peak-oil and post-carbon era. We should describe in time the outlines of closed or semi-closed loops economy. The fundamentals of Input-Output will flourish once again; it could help us formulate the link between the efficiency and resiliency of a regional complex system.

The process mapping is an important tool in the management of organizations, allowing better levels of productivity, quality and reciprocity concerning the implementation and decision-making. It benefits in a clear way the process organization and assures the manager a macro vision of the system. In this sense, the objective of this study is to describe the mapping process as it occurs in the Januario Cicco Maternity School–MEJC. The bibliographic and descriptive research included interviews, observations and researches in many databases. The study analyzed the limiting points of the detected flow trying to find a better comprehension of the flow of patients in the maternity in order to offer the gestors the information necessary to improve the quality of the assistance as well as a general view of the flow of patients and any change detected there. As a result, we believe that the process mapping may become a relevant factor to the organizational management of MEJC, in view that it is an element that can contribute to ease up the management of information studied, considering that the perfect change of information inside a complex system will produce improvements in all spheres of this institution.
