La classificació dels interns penats en algun dels graus penitenciaris previstos a l’ordenament jurídic espanyol és una de les funcions i tasques de l’Administració penitenciària. La classificació inicial en règim obert és una d’aquestes possibles modalitats de classificació per a un determinat conjunt de persones que complien els requisits. Aquesta modalitat de classificació ha estat fins ara poc coneguda i sovint incompresa per part de la societats, ja que s’associa amb la llibertat definitiva de l’intern sense cap tipus de control. La finalitat de la recerca és precisament donar a conèixer els resultats d’aquesta aplicació en el conjunt de penats que l’any 2001 varen accedir a aquesta modalitat i se’ls va seguir fins a octubre de 2005, per saber la seva trajectòria i s’hi havia reincidit. També es va constituir un grup de contrast amb interns classificats en segon grau, per contrastar les diferències entre ambdós col·lectius.

The Western Alpine Are has been created during the Cretaceous and the Tertiary orogenies. The interference patterns of the Tertiary structures suggest their formation during continental collision of the European and the Adriatic Plates, with an accompanying anticlockwise rotation of the Adriatic indenter. Extensional structures are mainly related to ductile deformation by simple shear. These structures developed at a deep tectonic level, in granitic crustal rocks, at depths in excess of 10 km. In the early Palaeogene period of the Tertiary Orogeny, the main Tertiary nappe emplacement resulted from a NW-thrusting of the Austroalpine, Penninic and Helvetic nappes. Heating of the deep zone of the Upper Cretaceous and Tertiary nappe stack by geothermal heat flow is responsible for the Tertiary regional metamorphism, reaching amphibolite-facies conditions in the Lepontine Gneiss Dome (geothermal gradient 25 degrees C/ km). The Tertiary thrusting occurred mainly during prograde metamorphic conditions with creation of a penetrative NW-SE-oriented stretching lineation, X(1) (finite extension), parallel to the direction of simple shear. Earliest cooling after the culmination of the Tertiary metamorphism, some 38 Ma ago, is recorded by the cooling curves of the Monte Rosa and Mischabel nappes to the west and the Suretta Nappe to the east of the Lepontine Gneiss Dome. The onset of dextral transpression, with a strong extension parallel to the mountain belt, and the oldest S-vergent `'backfolding'' took place some 35 to 30 Ma ago during retrograde amphibolite-facies conditions and before the intrusion of the Oligocene dikes north of the Periadriatic Line. The main updoming of the Lepontine Gneiss Dome started some 32-30 Ma ago with the intrusion of the Bergell tonalites and granodiorites, concomitant with S-vergent backfolding and backthrusting and dextral strike-slip movements along the Tonale and Canavese Lines (Argand's Insubric phase). Subsequently, the center of main updoming migrated slowly to the west, reaching the Simplon region some 20 Ma ago. This was contemporaneous with the westward migration of the Adriatic indenter. Between 20 Ma and the present, the Western Aar Massif-Toce culmination was the center of strong uplift. The youngest S-vergent backfolds, the Glishorn anticline and the Berisal syncline fold the 12 Ma Rb/Sr biotite isochron and are cut by the 11 Ma old Rhone-Simplon Line. The discrete Rhone-Simplon Line represents a late retrograde manifestation in the preexisting ductile Simplon Shear Zone. This fault zone is still active today. The Oligocene-Neogene dextral transpression and extension in the Simplon area were concurrent with thrusting to the northwest of the Helvetic nappes, the Prealpes (35-15 Ma) and with the Jura thin-skinned thrust (11-3 Ma). It was also contemporaneous with thrusting to the south of the Bergamasc (> 35-5 Ma) and Milan thrusts (16-5 Ma).

Many patients with Chagas disease live in remote communities that lack both equipment and trained personnel to perform a diagnosis by conventional serology (CS). Thus, reliable tests suitable for use under difficult conditions are required. In this study, we evaluated the ability of personnel with and without laboratory skills to perform immunochromatographic (IC) tests to detect Chagas disease at a primary health care centre (PHCC). We examined whole blood samples from 241 patients and serum samples from 238 patients. Then, we calculated the percentage of overall agreement (POA) between the two groups of operators for the sensitivity (S), specificity (Sp) and positive (PPV) and negative (NPV) predictive values of IC tests compared to CS tests. We also evaluated the level of agreement between ELISAs and indirect haemagglutination (IHA) tests. The readings of the IC test results showed 100% agreement (POA = 1). The IC test on whole blood showed the following values: S = 87.3%; Sp = 98.8%; PPV = 96.9% and NPV = 95.9%. Additionally, the IC test on serum displayed the following results: S = 95.7%; Sp = 100%; PPV = 100% and NPV = 98.2%. Using whole blood, the agreement with ELISA was 96.3% and the agreement with IHA was 94.1%. Using serum, the agreement with ELISA was 97.8% and the agreement with IHA was 96.6%. The IC test performance with serum samples was excellent and demonstrated its usefulness in a PHCC with minimal equipment. If the IC test S value and NPV with whole blood are improved, then this test could also be used in areas lacking laboratories or specialised personnel.

ABSTRACT: We evaluated the impact of a nurse program for hepatitis B virus vaccination in a center from the Swiss HIV Cohort Study. Immunity (anti-HBs >10 IU/mL) increased from 32% to 76% in the intervention center (n = 238) where vaccine management was endorsed by nurses, but only from 33% to 39% in control centers (n = 2712, P < 0.001) where management remained in charge of physicians. Immunity against HBV in the HIV population is insufficient in Switzerland. Specific nurse vaccination program may efficiently improve health care.

Scopo di questo studio è la disamina e la divulgazione dei pregi letterari e del valore concettualmente innovativo del Discorso dell 'amore verso la patria di Ludovico Zuccolo. L'analisi dei contenuti dell'opera, unitamente alla puntualizzazione delle sue caratteristiche formali e stilistiche, permette di vedere come questo scrittore proponga, utilizzando la forma letteraria del discorso politico, una riuscita codificazione della tematica dei diritti e dei doveri dei cittadini verso la patria; operazione in cui si sostanzia, a mio avviso, l'apporto innovativo dello Zuccolo. Dopo l'iniziale riscoperta, da parte di Benedetto Croce, del capitolo delle Considerazioni sulla Ragion di Stato, l`opera dello Zuccolo ha generato un certo interesse critico fra gli studiosi di letteratura politica del "900" senza tuttavia divenire oggetto di contributi esaustivi malgrado, a nostro parere, la presenza di elementi di originalità di pensiero nel Discorso dell 'amore verso la patria. Inoltre, l'opinione degli specialisti non è mai stata unanime riguardo al valore e ai contenuti delle opere di questo autore. Se per il Croce, ne << La Critica >> del 1926, egli appare come colui che ha prodotto, «Lo scritto più acuto e originale sull'argomento [della Ragion di Stato], composto in quel seco1o.>> (p. 301), per altri egli si presenta come un dotto estensore di trattati politici nei quali sfoggia abilmente la sua erudizione classica e Luigi Firpo arriva a suggerire addirittura che lo Zuccolo sia colpevole di plagio proprio laddove tratta il tema della Ragion di Stato : Al punto in cui siamo, una cosa è certa, e cioè che i conti non tornano : non riesco a credere che uno scrittore inzeppi centinaia e centinaia di pagine di luoghi comuni, di erudizione d"accatto, di oziosità accademiche, e poi un bel mattino, morso dalla tarantola o baciato in fronte da Minerva, metta in carta il piccolo capolavoro, le pagine meditate e profonde, e perciò lungamente soffeite, che pure gli appartengono per una paternità incontestata e certa. Dico questo, perché in un caso del genere, non al miracolo s"avrebbe da credere, ma, semmai, al plagio. (1). Catherine Pitiot, nel suo saggio La retorica politica nell 'opera utopica di Ludovico Zuccolo, coglie invece in questo autore unicamente l"utopista che << lntende allontanarsi dalla realtà contemporanea per modificarla, correggerla e presentare un'in1magine che sia fondamentalmente diversa, a livello strutturale. >> (2). Di parere diametralmente opposto Rodolfo de Mattei, che attribuisce allo Zuccolo lo status di anti-utopista per eccellenza, tipico di uno scrittore che, Non ha voluto usufruire della facile libertà della fantasia per sovvertire l'ordine storico e per alterare arbitrariamente la natura umana, cioé per proporre un ordinamento mirante ad una radicale trasformazione della società e quindi di assai dubbia realizzazione. (3). Altri studiosi, fra cui Bruno Nediani, si sono dedicati alla descrizione della personalità dello Zuccolo, attingendo alle sue lettere - che lo stesso Nediani ha riscoperto - oltre che ai brevi accermi che di sé fa lo Zuccolo negli scritti. Dal saggio del Nediani, La personalità di Ludovico Zuccolo (1969), emerge la figura di un uomo tormentato e insoddisfatto, ossessionato dal sospetto dell"autorità ecclesiastica, costretto a procacciarsi impieghi inadeguati presso i potenti, all'inseguimento dell'obiettivo di una carriera che, comunque, finisce con il risultargli sempre, prima o poi, insopportabile. Più recentemente, Sergio Bertelli, nel suo contributo alla Storia della letteratura di Cecchi e Sapegno, ha aperto una nuova prospettiva sullo Zuccolo proponendolo come colui che, Spezza finalmente il cerchio moralistico costruito dal Botero attorno al pensiero machiavelliano e la ragion di stato cessa di essere giudicata vera o falsa, buona o malvagia, interessando in sé e per sé, cioé nei suoi presupposti e nei suoi fini esclusivamente politici. Lactotransferrin, also known as lactoferrin, is an iron binding glycoprotein that displays antiviral activity against many different infectious agents, including human immunodeficiency virus (HIV)-1. Lactotransferrin is present in the breast milk and in the female genitourinary mucosa and it has been hypothesised as a possible candidate to prevent mother-to-child HIV-1 transmission. To verify if two functional polymorphisms, Thr29Ala and Arg47Lys, in the lactotransferrin encoding gene (LTF) could affect HIV-1 infection and vertical transmission, a preliminary association study was performed in 238 HIV-1 positive and 99 HIV-1 negative children from Brazil, Italy, Africa and India. No statistically significant association for the Thr29Ala and Arg47Lys LTF polymorphisms and HIV-1 susceptibility in the studied populations was found. Additionally LTF polymorphisms frequencies were compared between the four different ethnic groups.

To enhance the prevention and intervention efforts of childhood obesity, there is a strong need for the early detection of psychological factors contributing to its development and maintenance. Rather than a stable condition, childhood obesity represents a dynamic process, in which behavior, cognition and emotional regulation interact mutually with each other. Family structure and context, that is, parental and familial attitudes, activity, nutritional patterns as well as familial stress, have an important role with respect to the onset and maintenance of overweight and obesity. Behavioral and emotional problems are found in many, though not all, obese children, with a higher prevalence in clinical, treatment-seeking samples. The interrelatedness between obesity and psychological problems seems to be twofold, in that clinically meaningful psychological distress might foster weight gain and obesity may lead to psychosocial problems. The most frequently implicated psychosocial factors are externalizing (impulsivity and attention-deficit hyperactivity disorder) and internalizing (depression and anxiety) behavioral problems and uncontrolled eating behavior. These findings strengthen the need to further explore the interrelatedness between psychological problems and childhood obesity.

Epidemiological studies have shown that obesity is associated with chronic kidney disease and end stage renal disease. These studies have used creatinine derived equations to estimate glomerular filtration rate (GFR) and have indexed GFR to body surface area (BSA). However, the use of equations using creatinine as a surrogate marker of glomerular filtration and the indexation of GFR for BSA can be questioned in the obese population. First, these equations lack precision when they are compared to gold standard GFR measurements such as inulin clearances; secondly, the indexation of GFR for 1.73 m(2) of BSA leads to a systematic underestimation of GFR compared to absolute GFR in obese patients who have BSA that usually exceed 1.73 m(2). Obesity is also associated with pathophysiological changes that can affect the pharmacokinetics of drugs. The effect of obesity on both renal function and drug pharmacokinetics raises the issue of correct drug dosage in obese individuals. This may be particularly relevant for drugs known to have a narrow therapeutic range or excreted by the kidney.
