Background Haemodialysis (HD) nursing is characterised by frequent, intense interactions with patients over long periods of time resulting in a unique nurse-patient relationship. Due to the life-limiting nature of end-stage renal failure, nurses are likely to have repeated exposures to the death of patients with whom they have formed relationships. Repeated exposure to patient death translates into frequent grief experiences. There is scant literature on the psychological impact of patient death for nurses working in the HD setting. Aims To explore HD nurses experiences of patient death and coping mechanisms used. Methods A sequential mixed method study investigating job satisfaction, stress and burnout found that HD nurses had high levels of stress and burnout. These results were explored in more detail during 8 semi-structured interviews with HD nurses. Interviews were audio-recorded, transcribed verbatim and subjected to thematic analysis. Results Three themes were identified that highlight the stress experienced by nurses when a haemodialysis patient dies. The first theme, “quazi-family” describes the close relationship which forms between nurses and patients. The “complicated grief” theme outlines the impact of death on HD nurses, and the final theme, “remembrance” explains some of the coping mechanisms used in the grieving process. Conclusion Nurses develop individual coping mechanisms to accommodate the grief and loss experienced when a “close” patient dies. The grieving process caused by the death of patient’s needs to be recognised by nurses and nurse managers as causing psychological stress and strain.

Caring for someone with dementia can be demanding, particularly for spouses living with the care recipient. The main goal of this study was to clarify differences in the experience of caregivers who were husbands and wives with respect to burden, health, healthy behaviors, presence of difficult care recipient behaviors, social supports, and the quality of the premorbid relationship. The results of this study support research demonstrating a difference between the caregiving experiences of women and men. It is becoming increasingly apparent that female gender is a marker that places them at increased risk of high burden and less support.

L’exposition à la violence dans le secteur de la santé affecte les employés et a également des implications sur la qualité des soins offerts. Les agressions en milieu de travail peuvent engendrer divers émotions ou comportements chez le personnel soignant, tels que la peur et l’évitement des patients (Gates, Gillespie, & Succop, 2011). La présente étude phénoménologique tente de comprendre et de décrire l’expérience des intervenants d’un hôpital psychiatrique lorsque ceux-ci ont été victimes d’un acte de violence grave de la part d’un patient, ainsi que les impacts engendrés sur les services offerts. L’approche phénoménologique permet de porter un nouveau regard sur cette problématique en plongeant dans l’univers de chaque intervenant, comprendre l’interprétation donnée à l’acte de violence vécu. L’emploi de cette approche donne accès à davantage de contenu et permet de préciser de quelle façon leur quotidien est altéré par ce phénomène. Trente entrevues semi-structurées ont été réalisées, soit deux auprès de chacun des 15 participants (11 femmes) provenant de divers domaines professionnels œuvrant dans un hôpital psychiatrique. Les analyses sont basées sur la technique « Empirical Phenomenological Psychological » de Karlsson (1993). Une attention particulière a été portée quant à la possibilité d’expériences différentes selon le sexe des intervenants. L’analyse a fait ressortir quatre thèmes principaux qui sont présents indépendamment du sexe des intervenants, soit: l’hypervigilance, le caring, la peur spécifique du patient agresseur, puis la peur généralisée à tous les patients. Un état d’hypervigilance est retrouvé chez tous les intervenants qui ont été victimes d’agression de la part d’un patient. Comparativement aux intervenants qui ont assisté à l’escalade d’agressivité d’un patient, ceux ayant été agressés par surprise rapportent des répercussions de cette vigilance qui s’étendent jusqu’à leur vie personnelle. Une approche caring est présente chez la majorité des participants. Ceci implique une bienveillance et une authenticité envers le patient soigné. Mettant le patient au cœur de son intervention, l’intervenant « soignant » développe un lien de confiance et agit comme un agent de changement. Un sentiment de peur est également exprimé chez les participants. Celui-ci est modulé par la présence ou l’absence de caring. Les intervenants démontrant du caring ont développé une peur spécifique à leur agresseur, tandis que ceux ne manifestant peu ou pas de caring ont développé une peur généralisée de la clientèle. Suite à un évènement de violence, les intervenants étant caring le demeurent, alors que ceux n’étant peu ou pas caring seraient plutôt portés à se désinvestir et à se désengager des relations avec les patients. Engendrées par la violence subie en milieu hospitalier psychiatrique, l’hypervigilance et la peur, qu’elle soit spécifique ou généralisée, ont toutes deux des impacts sur la qualité des soins offerts. Un intérêt considérable devrait être porté au caring, qui vient moduler cette peur et les effets qui en découlent. Des recherches pourraient porter un éclairage sur l’origine du caring – est-ce que le caring est appris ou découle-t-il d’une vocation? Finalement, ces études pourraient établir des manières de renforcer ou de développer le caring.

In der vorliegenden Dissertation geht es um die Dokumentation, theoretische Begründung und Auswertung des in 25 Jahren Praxis entwickelten Curriculums der Bewusstseinsschulung und -weitung der Orgodynamik. Dabei geht es insbesondere um den Vergleich und die forschungsorientierte Verknüpfung verschiedener Traditionen der Bewusstseinsbildung, der ihnen zugrunde liegenden Konzepte und anthropologischen Dimensionen im Schnittfeld pädagogischer, psychologischer und spiritueller Perspektiven. In Anlehnung an das von Fuhr/Dauber (2002) entwickelte Modell, der Praxisentwicklungsforschung, welche die Verflechtung von Theorie und Praxis ansteuert, wird der orgodynamische Ansatz wissenschaftlich dokumentiert und theoretisch begründet. Über eine induktive Vorgehensweise werden die historischen Wurzeln konzeptionell dargelegt, die verborgenen Paradigmen herausgearbeitet, sowie das Curriculum erläutert und ausgewertet. In einem ersten theorieorientierten Kapitel wird das orgodynamische Methodenspektrum in seinem Grundmodell und den vier zentralen Dimensionen (mentale, körperliche, emotionale, energetische Dimension) aufgezeigt und mit theoretischen Hintergrundkonzepten verglichen und verknüpft. Die vier sich überlappenden Methodengruppen der mental, körperlich, emotional und energetisch orientierten Bewusstseinsarbeit werden differenziert dargestellt und in ihrer Beziehung zueinander diskutiert. Anhand eines Modells (Methodenrad) wird die multi-dimensionale Perspektive des Methodenspektrums, in einer nichthierarchischen Zuordnung sichtbar. Im zweiten theorieorientierten Hauptteil werden zunächst die zentralen vier Paradigmen der Orgodynamik (Präsenz, Multidimensionalität, Flow/Fließendes Gewahrsein, Bezogenheit) vorgestellt, theoretisch und praxisbezogen entfaltet und in einer Paradigmen-Landkarte zueinander in Beziehung gesetzt. Dabei werden die kategorialen Ausführungen durchgehend an Praxisbeispielen veranschaulicht und im Blick auf drei vorgestellte Zugänge zur Bewusstseinsweitung (Immersion, Integration und Dekonstruktion) exemplarisch didaktisch kommentiert. Im dritten Hauptteil wird das Curriculum im Zusammenhang mit einer Auswertungsmatrix erläutert. Diese dient als Überprüfungsinstrument. Mit ihrer Hilfe werden die verschiedenen methodischen Zugangsweisen und Arbeitsformen dieses Ansatzes, exemplarisch anhand von 2 Ausbildungswochen, im Blick der Multidimensionalität dokumentiert. Damit wird diese multidimensional angelegte Praxis exemplarisch bis in methodische Details nachvollziehbar und in dialogisch-selbstreflexiver Form überprüfbar. Exemplarisch werden in einem Exkurs erste Itemvorschläge gemacht, welche die wissenschaftliche Anschlussfähigkeit an neuere Forschung im transpersonalen Bereich aufzeigen. Das innere Anliegen der vorliegenden Arbeit zeigt in der Verschränkung von Theorie und Praxis, dass die Paradigmen der Orgodynamik, Präsenz, Multidimensionalität, fließendes Gewahrsein und bewusste Bezogenheit vier pädagogisch umgesetzte Paradigmen für eine Bewusstseinserforschung in der Erwachsenenbildung sind. There are several aggressive factors in Intensive Care Units (ICU), which reach not only patients, but the nursing caregivers, since they participate in complex procedures and death of patients. Nursing caregivers may have difficulties on their daily work routine that can influence the way of care. The goal of this study was to identify the aspects of nursing caregivers working in adult ICU. It is a describe-exploratory study with qualitative approach, developed among 21 ICU adult nursing caregivers of a school hospital in Paraná. The data were collected in May and July, 2009 by recorded and transcribed semi-structured interviews. Four categories for analysis were identified: the aspects of ICU assistance, the meaning of healthcare for the nursing caregivers, the understanding of healthcare positive aspects and disclosing the difficulties of caring. The results revealed that caring is related to some factors such as mixed feelings, the mental and physical damage caused by stress; the understanding of total caring, scientific-technique procedures, family engagement in the assistance and humanization. The positive aspects are related to the welfare due to satisfaction in the work done and recognition of work. The difficulties involved death situations, psychological and biological damages, establishing links with patients and the uncaring toward the nursing caregivers.

Objective:To identify aspects that affect the quality of life of nursing caregivers and their relationship with care in an Intensive Care Unit for Adults (A-ICU). Methods:This was a descriptive study with qualitative approach, taking as subjects 21 professionals who constitute the nursing staff of the A-ICU of a school hospital in Maringá-PR. Unstructured interview was used as a strategy to collect data, conducted between May and June 2009. Data analysis was based on the method of content analysis. The categories identified were: overlooking improvement in quality of life related to the resources in an A-ICU; the quality of life influencing the form of care; interpersonal relationships into the health team reflecting on the quality of life and care. Results:The analysis of caregivers’ speech and the results of the observation showed that there is correlation between the aspects they consider influential in their quality of life and the way of caring for patients in an A-ICU.Conclusion: The findings indicate that, among the influential aspects, the stressful factors overlap the enhancing ones. From this perspective, dealing with caregiver’s suffering might be the starting point for the improvement in quality of care in an A-ICU.

This paper takes as its starting point recent work on caring for distant others which is one expression of renewed interest in moral geographies. It examines relationships in aid chains connecting donors/carers in the First World or North and recipients/cared for in the Third World or South. Assuming predominance of relationships between strangers and of universalism as a basis for moral motivation I draw upon Gift Theory in order to characterize two basic forms of gift relationship. The first is purely altruistic, the other fully reciprocal and obligatory within the framework of institutions, values and social forces within specific relationships of politics and power. This conception problematizes donor-recipient relationships in the context of two modernist models of aid chains-the Resource Transfer and the Beyond Aid Paradigms. In the first, donor domination means low levels of reciprocity despite rhetoric about partnership and participation. The second identifies potential for greater reciprocity on the basis of combination between social movements and non-governmental organizations at both national and trans-national levels, although at the risk of marginalizing competencies of states. Finally, I evaluate post-structural critiques which also problematize aid chain relationships. They do so both in terms of bases-such as universals and difference-upon which it might be constructed and the means-such as forms of positionality and mutuality-by which it might be achieved.

The postpartum period is a sensitive time due to the presence and demands of the developing infant. The care provided by a mother to her infant during this period may be compromised if she is suffering from postnatal depression or postpartum psychosis. Evidence has been emerging which suggests that postnatal depression and postpartum psychoses have adverse effects on the quality of the mother-infant relationship and also on the infants subsequent cognitive and emotional development. Presented is a review of the literature relating to how these conditions impact on parenting and infant outcomes, what measures are in place to detect these conditions and evidence-based models of best clinical practice are proposed.

The purpose of this study was to understand and analyse the meanings given to caring of patients on the isolation ward by nursing students, focusing particularly on the aspects of communication and interpersonal relationships. The data were collected from individual interviews with 18 nursing students who were performing nursing practice on the isolation ward. The results, analysed and interpreted according to existential phenomenology, describe the structure of the phenomenon 'taking care on the isolation ward' from a relational perspective. The students described their difficulties and anxieties, as well as their willingness to take care of isolated patients, resulting in the overcoming of obstacles and in contacting and becoming involved when taking care of these patients.

The nursing care for patients who are pronounced brain-dead but kept alive to serve as organ donors demands technical-scientific skills and the ability to handle situations that are often in conflict with the traditional concepts of nursing care. Based on the phenomenological approach in this article, essential themes of the lived experience of caring for these patients, including the technical and specific nursing care, the relationship with organ donors and their families, and the nurses' perception of themselves in this professional situation are described. The results point to the contradictions and ambiguities of this type of nursing, especially in regards to the affective and philosophical aspects.

OBJECTIVE: Caring for a loved one with Alzheimer disease is a highly stressful experience that is associated with significant depressive symptoms. Previous studies indicate a positive association between problem behaviors in patients with Alzheimer disease (e.g., repeating questions, restlessness, and agitation) and depressive symptoms in their caregivers. Moreover, the extant literature indicates a robust negative relationship between escape-avoidance coping (i.e., avoiding people, wishing the situation would go away) and psychiatric well-being. The purpose of this study was to test a mediational model of the associations between patient problem behaviors, escape-avoidance coping, and depressive symptoms in Alzheimer caregivers. METHODS: Ninety-five spousal caregivers (mean age: 72 years) completed measures assessing their loved ones' frequency of problem behaviors, escape-avoidance coping, and depressive symptoms. A mediational model was tested to determine if escape-avoidant coping partially mediated the relationship between patient problem behaviors and caregiver depressive symptoms. RESULTS: Patient problem behaviors were positively associated with escape-avoidance coping (beta = 0.38, p < 0.01) and depressive symptoms (beta = 0.26, p < 0.05). Escape-avoidance coping was positively associated with depressive symptoms (beta = 0.33, p < 0.01). In a final regression analysis, the impact of problem behaviors on depressive symptoms was less after controlling for escape-avoidance coping. Sobel's test confirmed that escape-avoidance coping significantly mediated the relationship between problem behaviors and depressive symptoms (z = 2.07, p < 0.05). CONCLUSION: Escape-avoidance coping partially mediates the association between patient problem behaviors and depressive symptoms among elderly caregivers of spouses with dementia. This finding provides a specific target for psychosocial interventions for caregivers.

The aim of the paper is to introduce the challenges of the international care debate of the last ten years in order to grasp basic social needs, to analyse their treatment in the public and private sphere and to look at the orientation of professional answers by the care-professions. The concept of care enhances the societal dealing with - or ignoring of - different forms of dependency on informal and formal personal and social services throughout the life-cycle (child-care, nursing sick or handicapped persons, supporting the elderly) and in special life situations (from help to lone mothers and their children, via help to drug-addicts to help for homeless people). All societies have different approaches to deal with these life-situations, they do so by employ-ing various mixtures of: familial support, mostly provided by women, social politics, organized by the state, public and/or private social services. This welfare-mix shows different combinations of private and public obligations, paid and und unpaid work, professional and laymen's tasks based on a specific understanding of mo-rality and justice embedded in the gender structure and intergenerational relationships. The importance of social work as a profession in this context differs according to the historical developments and cultural traditions. Characteristic for the profile of social work is the rele-vance of a care ethics and the existence of social rights, the tension of mothering and profes-sional methods, the relationship between help, denial and punishment and the ways of institu-tionalisation. The actuality of this debate is closely intertwined with the restructuring of societal bonds in the face of globalisation, the political reorganisation of states, the changes in the living to-gether of different generations and both sexes and the consequences for the organisation and contents of welfare. Looking at Germany and Eastern Europe two new phenomena of social relevance for the dis-cussion of care work and care needs can be taken as an example: the extent of cheap illegal women laborers travelling between east and west, especially Polish women working intermit-tendly in private care for old people and the highly organized traffiking of women from Russia to Germany to work in the sex business. The care debate entails a reframing of welfare issues in the light of social justice between classes, ethnicities and gender groups.

Background: Qualitative research has suggested that spousal carers of someone with dementia differ in terms of whether they perceive their relationship with that person as continuous with the premorbid relationship or as radically different, and that a perception of continuity may be associated with more person-centered care and the experience of fewer of the negative emotions associated with caring. The aim of the study was to develop and evaluate a quantitative measure of the extent to which spousal carers perceive the relationship to be continuous. Methods: An initial pool of 42 questionnaire items was generated on the basis of the qualitative research about relationship continuity. These were completed by 51 spousal carers and item analysis was used to reduce the pool to 23 items. The retained items, comprising five subscales, were then administered to a second sample of 84 spousal carers, and the questionnaire's reliability, discriminative power, and validity were evaluated. Results: The questionnaire showed good reliability: Cronbach's α for the full scale was 0.947, and test-retest reliability was 0.932. Ferguson's δ was 0.987, indicating good discriminative power. Evidence of construct validity was provided by predicted patterns of subscale correlations with the Closeness and Conflict Scale and the Marwit-Meuser Caregiver Grief Inventory. Conclusion: Initial psychometric evaluation of the measure was encouraging. The measure provides a quantitative means of investigating ideas from qualitative research about the role of relationship continuity in influencing how spousal carers provide care and how they react emotionally to their caring role. © 2012 International Psychogeriatric Association.
