Background

The aim of this position statement was to inform the choice of physical activity tools for use within CF research and clinical settings.

Methods

A systematic review of physical activity tools to explore evidence for reliability, validity, and responsiveness. Narrative answers to “four key questions” on motion sensors, questionnaires and diaries were drafted by the core writing team and then discussed at the Exercise Working Group in ECFS Lisbon 2013.

Results and summary

Our current position is that activity monitors such as SenseWear or ActiGraph offer informed choices to facilitate a comprehensive assessment of physical activity, and should as a minimum report on dimensions of physical activity including energy expenditure, step count and time spent in different intensities and sedentary time. The DigiWalker pedometer offers an informed choice of a comparatively inexpensive method of obtaining some measurement of physical activity. The HAES represents an informed choice of questionnaire to assess physical activity. There is insufficient data to recommend the use of one diary over another. Future research should focus on providing additional evidence of clinimetric properties of these and new physical activity assessment tools, as well as further exploring the added value of physical activity assessment in CF.

BACKGROUND: Pregnant women are recommended to 1) perform daily moderate-intensity physical activity and 2) limit the amount of sedentary time. Many women do not meet these recommendations. Reduced physical activity and increased sedentary behavior may result from women actively intending to rest during pregnancy. The Theory of Planned Behavior (TPB) has been used to assess attitudes (e.g., positive/negative beliefs), subjective norms (e.g., perception of others' views), perceived behavioral control (PBC) (e.g., self-efficacy), and intention toward exercising while pregnant but has not been applied to aspects pertaining to resting during pregnancy.

METHODS: Pregnant women (n = 345) completed a cross-sectional questionnaire that included two TPB Questionnaires where the target behaviors were 1) being physically active and 2) resting. Bootstrapped paired t tests, ANOVA, and linear hierarchal regression analyses were performed to identify predictors of intentions and whether intentions toward the two behaviors varied at different stages of pregnancy.

RESULTS: As women progressed in their pregnancy, their attitude, PBC, and intention toward being physically active all significantly declined. A positive attitude, subjective norms, and intention toward resting all significantly increased with the advancing trimester. Self-reported health conditions predicted lower intention for physical activity but not for resting.

DISCUSSION: The significantly inverse relationship between physical activity and resting across time suggests that women feel they should focus on one behavior at the expense of the other. Finding that women generally do not perceive these behaviors as mutually compatible has implications in strategizing as to how to encourage women to be active during pregnancy.

Background Clustering of lifestyle risk behaviours is very important in predicting premature mortality. Understanding the extent to which risk behaviours are clustered in deprived communities is vital to most effectively target public health interventions. Methods We examined co-occurrence and associations between risk behaviours (smoking, alcohol consumption, poor diet, low physical activity and high sedentary time) reported by adults living in deprived London neighbourhoods. Associations between sociodemographic characteristics and clustered risk behaviours were examined. Latent class analysis was used to identify underlying clustering of behaviours. Results Over 90% of respondents reported at least one risk behaviour. Reporting specific risk behaviours predicted reporting of further risk behaviours. Latent class analyses revealed four underlying classes. Membership of a maximal risk behaviour class was more likely for young, white males who were unable to work. Conclusions Compared with recent national level analysis, there was a weaker relationship between education and clustering of behaviours and a very high prevalence of clustering of risk behaviours in those unable to work. Young, white men who report difficulty managing on income were at high risk of reporting multiple risk behaviours. These groups may be an important target for interventions to reduce premature mortality caused by multiple risk behaviours.

PURPOSE: In obesity prevention, understanding psychosocial influences in early life is pivotal. Reviews reported contradictory results and a lack of longitudinal studies focusing on underlying lifestyle factors. This study tested whether psychosocial Quality-Of-Life (QOL) was associated with pre-schoolers' lifestyle and adiposity changes over one school year and whether lifestyle moderated the latter. It was hypothesised that QOL might not impact adiposity in everybody but that this might depend on preceding lifestyle. METHOD: Longitudinal data from 291 Swiss pre-schoolers (initially 3.9-6.3 years) was available. The following measures were used in longitudinal regressions: psychosocial QOL by PedsQL, adiposity (BMI z-score, waist, fat%), diet (food frequency), sedentary time and accelerometer-based activity. RESULTS: Concerning lifestyle, low psychosocial QOL was only related to unfavourable changes in diet (less fruit β = 0.21 and more fat intake β = -0.28) and lower physical activity (β = 0.21). Longitudinal QOL-adiposity relations appeared only after moderation by lifestyle factors (beta-range 0.13-0.67). Low psychosocial QOL was associated with increased adiposity in children with an unhealthy diet intake or high sedentary time. By contrast, low psychosocial QOL was associated with decreasing adiposity in high fruit consumers or more physically active pre-schoolers. CONCLUSION: Results emphasise the need for testing moderation in the QOL-adiposity relation. An unhealthy diet can be a vulnerability factor and high physical activity a protective factor in QOL-related adiposity. Consequently, QOL and lifestyle should be targeted concurrently in multi-factorial obesity prevention. The environment should be an 'activity encouraging, healthy food zone' that minimises opportunities for stress-induced eating. In addition, appropriate stress coping skills should be acquired.

Physical inactivity poses a huge burden on Canada's health care system and is detrimental to the health of Canadians (Katzmarzyk & Janssen, 2004). Walking is a viable option for individuals to become physically active on a daily basis and is in fact the most commonly reported leisure time physical activity. It has been associated with many health benefits including weight loss/weight control, reduced risk of coronary artery disease and diabetes, lowered blood pressure, and improved psychological wellbeing (Brisson & Tudor-Locke, 2004). Specifically, individuals' stage of change, selfefficacy and health related quality of life (HRQL) are three psychological constructs that can be greatly improved with increased physical activity (Dishman, 1991; Penedo & Dahn, 2005; Poag & McAuley, 1992). Public health physical activity recommendations exist but many individuals find these difficult to meet due to overly busy lifestyles (Public Health Agency of Canada, 2003). Pedometers are inexpensive devices that can monitor individual bouts of walking so that the incorporation of physical activity into one's daily life is more plausible. They are also excellent tools for motivation, goalsetting, and immediate feedback (Brisson & Tudor-Locke, 2004). Since many people spend a large proportion of their time at their places of employment, workplaces have begun to be a common site for the development of physical activity interventions. These programs have been growing in popUlarity and have shown numerous benefits for both employees and employers (Voit, 2001). The purpose of the current study was to implement and evaluate the use of a pedometer-based physical activity intervention incorporating goal-setting and physical activity logs in a workplace setting, and to examine the relationship between different types of self-efficacy (task, barrier, and scheduling) and different phases of the intervention. Twenty male participants from a local steel manufacturing plant who exhibited health risk factors (e.g. hypertension, diabetes, etc.) were assigned to one of two groups (group A or group B). All participants were asked to wear pedometers on their waists, record their daily steps, set goals that were outlined on a step-tracking sheet (detennined by their baseline number of steps), and keep track of their work days, wakelbed time, sedentary time, and time spent doing other physical activity. Group A began the intervention immediately following the baseline measures, whereas group B continued with their regular routine for 4 weeks before beginning. Physiological measures (height, weight, blood pressure, relative body fat, waist and hip circumference, and body mass index) were taken and a battery of questionnaires that assessed barrier, task and scheduling self-efficacy, HRQL, and stage of change administered at baseline, week 5 (end of intervention for group A), week 9 (end of intervention for group B; follow-up for group A) and week 13 (follow-up for both groups). Results showed that this workplace physical activity intervention was successful at increasing the participants' daily steps, that task self-efficacy is a significant predictor of participants' exercise adherence during the initial stages of participation (intervention phase), and that the participants felt that this intervention was effective. Finally, further exploratory analyses showed that this intervention was effective for all participants, but most valuable for participants most in need of improvement - that is, those who were most sedentary prior to the intervention. This intervention is an inexpensive use of simple and effective tools (e.g. pedometers), has the potential to attract a wide variety of participants and become a pennanent part of any health promotion initiative.

Cette étude s’inscrit dans le cadre du projet « Pôle francophone africain sur le Double Fardeau Nutritionnel » (DFN) du laboratoire TRANSNUT, centre collaborateur OMS sur la transition nutritionnelle du Département de Nutrition de l’Université de Montréal, en collaboration avec ses partenaires au Burkina Faso, à savoir l’Institut de Recherche en Sciences de la Santé (IRSS) et l’Institut Supérieur des Sciences de la Population (ISSP). Elle est l’une des premières à s’intéresser au double fardeau de la malnutrition en Afrique francophone. Cette étude avait pour objectif de démontrer l’existence du double fardeau de la malnutrition parmi les adultes de Ouagadougou au Burkina Faso, d’en donner l’ampleur et d’identifier ses liens avec les facteurs du mode de vie. Plus spécifiquement, elle visait à décrire les carences nutritionnelles, les facteurs de risque cardiométabolique (FRCM), et la typologie du double fardeau de la malnutrition; examiner les caractéristiques du mode de vie des adultes et leurs liens avec le double fardeau de la malnutrition selon les conditions économique, et enfin d’examiner l’association entre inflammation subclinique, les carences nutritionnelles et les FRCM selon les facteurs du mode de vie. Ces objectifs faisaient suite à nos principales hypothèses qui stipulaient que : parmi les adultes de Ouagadougou, le phénotype de double fardeau de la malnutrition le plus fréquemment observé est l’association de surpoids/obésité avec une ou plusieurs carences nutritionnelles, surtout chez les femmes, puis qu’une alimentation de piètre qualité, en lien avec de mauvaises conditions socioéconomiques et de vie est associée tant aux FRCM qu’aux carences nutritionnelles, contribuant ainsi au double fardeau de malnutrition, et enfin qu’un état d’inflammation subclinique joue un rôle de médiateur entre le mode de vie et aussi bien les carences nutritionnelles que les FRCM. Afin de répondre à ces objectifs, une étude transversale descriptive et analytique a été conduite auprès d’un échantillon aléatoire de 330 adultes âgés de 25 à 60 ans recrutés au sein de l’Observatoire de Population de Ouagadougou, situé à la partie nord de la ville. Cet échantillon a été subdivisé en terciles du score de possessions matérielles, proxy du statut socioéconomique, avec 110 personnes respectivement dans chaque strate de niveau socioéconomique bas, moyen et élevé. Chaque participant a fourni des données sociodémographiques, anthropométriques, cliniques et comportementales; il a aussi fourni un échantillon de sang. Les principales variables de l’étude étaient les suivantes : l’âge, les conditions socioéconomiques (insécurité alimentaire, éducation et proxy du revenu), le mode de vie (les apports alimentaires et la qualité de l’alimentation, l’activité physique, la consommation d’alcool et de tabac, la perception de l’image corporelle, le stress psychosocial); l’inflammation subclinique; les FRCM [surpoids/obésité, tension artérielle élevée (TAE) ou hypertension artérielle (HTA), hyperglycémie, dyslipidémie et insulino-résistance]; les carences nutritionnelles (maigreur, anémie, carence en fer et en vitamine A). Des phénotypes de double fardeau de la malnutrition ont été identifiés en combinant FRCM et carences nutritionnelles. Les résultats ont montré une prévalence élevée de surpoids/obésité, d’obésité abdominale, d’hypertension artérielle, d’hyperglycémie, de résistance à l’insuline et du taux de lipoprotéine de haute densité (HDL-C) bas, respectivement de 24,2 %, 12,5 %, 21,9 %, 22,3 %, 25,1 % et 30,0 %. En utilisant les seuils plus sensibles de la Fédération Internationale du Diabète (FID), l’obésité abdominale, la tension artérielle élevée (TAE), l’hyperglycémie était respectivement de 23,5 %; 36,1 % et 34,5 %. Des carences nutritionnelles étaient également présentes, l’anémie, la carence en fer et en vitamine A, ainsi que la maigreur atteignant respectivement 25,5 %, 15,4 %, 12,7 % et 9,7 % de la population étudiée. Les femmes étaient significativement plus touchées que les hommes autant par les FRCM que par les carences nutritionnelles. Le double fardeau de la malnutrition touchait 23,5 % des personnes et même 25,8 % avec les seuils de la FID pour l’obésité abdominale, la TAE, et l’hyperglycémie. Les deux principaux phénotypes observés étaient : l’association de « surpoids/obésité avec au moins une carence en micronutriment », touchant 7,8 % (11,8 % ♀ vs. 3,4 % ♂) des personnes et l’association d’au moins un FRCM autre que le surpoids/obésité avec au moins une carence en micronutriment, qui touchait 9.0 % (12,4 % ♀ vs. 5,4 % ♂) des personnes. La prévalence de ces phénotypes était plus élevée en utilisant les seuils de la FID plutôt que les seuils de l’OMS. Près de 72,9 % des personnes ou 81,2 % (seuils de la FID) avaient au moins un FRCM. Nous avons identifié à partir de l’analyse typologique, deux schémas alimentaires; « urbain » et « traditionnel », dans cette étude. Les carences nutritionnelles étaient davantage associées au schéma alimentaire « traditionnel », alors que les FRCM se retrouvaient dans les deux schémas alimentaires. Le schéma « urbain » regroupait significativement plus d’hommes et de personnes de niveau socioéconomique élevé, alors que les personnes de niveau socioéconomique bas et les femmes étaient proportionnellement plus nombreuses dans le schéma « traditionnel ». Le temps dévolu aux activités sédentaires était significativement plus important que celui consacré aux activités d’intensité modérée à vigoureuse. L’activité physique était inversement associée à l’indice de masse corporelle (IMC), au tour de taille (TT), à la masse grasse corporelle, à la tension artérielle systolique (TAS) et diastolique (TAD), à la triglycéridémie et au taux de lipoprotéine de faible densité (LDL-C). L’IMC et le TT augmentaient en outre avec le temps de sédentarité. Ainsi, le double fardeau de malnutrition était associé au statut socioéconomique bas, au sexe féminin et à la sédentarité. Nous avons aussi trouvé que 39,4 % des personnes avaient une inflammation subclinique qui était associée de façon indépendante et positive à la ferritinémie, à l’IMC, au TT et à la masse grasse corporelle, et négativement au HDL-C. L’exploration du stress psychosocial et de l’image corporelle a révélé une association entre le stress psychosocial, l’HTA et une perception positive de l’embonpoint. Les personnes ayant peut-être accusé un retard de croissance à l’enfance (d’après l’indice de Cormic) étaient significativement plus touchées par le surpoids/obésité, l’obésité abdominale et la résistance à l’insuline. Ces résultats nous ont permis d’atteindre nos objectifs, mais aussi de vérifier nos hypothèses de recherche. Comme on peut le constater, les FRCM sont une réalité à Ouagadougou, qui se compliquent par leur coexistence avec des carences en micronutriments dont la prévalence est tout aussi importante. Une transition nutritionnelle est en cours dans cette ville et contribue au bouleversement des comportements alimentaires et du style de vie favorisant l’émergence de ce double fardeau, dans un contexte où le passé nutritionnel de la population offre des conditions idéales pour un niveau de risque particulièrement élevé pour ces FRCM. Cependant, l’évolution de cette prévalence pourrait être inversée ou tout au moins ralentie si des actions étaient entreprises dès maintenant.

Objetivo: Evaluar la efectividad de MARA (Modulo Activo Recreo Activo), sobre el tiempo de actividad física moderada vigorosa (AFMV) y de comportamiento sedentario de niños y niñas de 5º grado en 2 instituciones educativas oficiales de la ciudad de Bogotá. Materiales y métodos: Los participantes fueron 128 niños entre 10 y 12 años de edad, de 5to grado que asisten a dos colegios públicos en la localidad de San Cristóbal en Bogotá. La actividad física de los niños fue medida con acelerómetro GT3X+, durante 7 días entre julio y octubre de 2013. Uno de los colegios fue seleccionado aleatoriamente para ser intervenido por Muévete Escolar y su Módulo Activo Recreo Activo (MARA), (CIM) y otro colegio fue el grupo control (CC). El tiempo gastado en actividad física durante el día y en el momento de recreo fue medido antes y en la semana 10 después de la intervención: sedentario ( SED), actividad física leve ( AFL), actividad física moderada(AFM), actividad física vigorosa(AFV) y actividad física moderada a vigorosa (AFMV). Resultados: Posterior al análisis estadístico a través de modelos mixtos multinivel para ajustar por el efecto de conglomerado, se observó diferencia significativa entre CIM y CC (p < 0.0049) representado por incremento en los minutos de AFMV en CIM. Después de la intervención, los CMI disminuyeron los minutos de comportamiento sedentario (p= 0.0029), comparativamente con los CC. Conclusiones: El presente estudio contribuye a investigar sobre los efectos a corto plazo de modificar el momento del recreo, mediante la implementación de actividades guiadas, supervisadas y con el uso de materiales y equipos de juego. Los resultados del estudio sugieren que los efectos de la intervención con MARA fueron significativos especialmente en promover la práctica de AF diaria logrando incrementar los minutos de AFMV diaria, y así mismo disminuyendo comportamiento sedentario en el día. .

RESUMO: O estágio contemplou a preparação e implementação de um programa, programa de Tratamento da Obesidade Pediátrica (TOP), direcionado para a redução da obesidade dos adolescentes. Foi desenvolvido num contexto clínico e teve o envolvimento de uma equipa multidisciplinar. Existiu um trabalho inicial ao nível da consulta de obesidade, no Hospital de Santa Maria, onde se procedeu ao recrutamento dos participantes. Seguidamente, foram dinamizadas sessões interativas, com exercício físico, com os adolescentes participantes, na Faculdade, no âmbito de um projeto de intervenção, que teve uma duração de seis meses, tendo tido início em janeiro de 2011 e terminado em junho de 2011. A componente científica esteve presente no estágio através da avaliação objetiva do impacte da implementação do programa TOP. No presente estudo, foi avaliado o impacte nos hábitos sedentários dos adolescentes e como objetivo secundário, pretendeu-se analisar a associação entre as atividades sedentárias e a composição corporal. Os resultados do estudo indicam uma descida significativa do peso e de todos os perímetros corporais dos participantes na amostra, bem como do tempo sedentário avaliado por acelerometria, do início para o final da intervenção. Não existiram correlações entre as alterações da composição corporal e as modificações do comportamento sedentário. ABSTRACT: The training incorporated the preparation and the implementation of an exercise program — program Tratamento da Obesidade Pediátrica (TOP) — focused on the decreasing of adolescence obesity. It was developed in a clinic context and it had the involvement of a multidisciplinary team. The work began in Hospital de Santa Maria with the obesity appointment where the participants were recruited. After, the intervention was developed in the Faculty with exercise sessions. The intervention had duration of six months. It began on January and finished on June, in year 2011. The training had a scientific character through the objetive evaluation of the implementation’s impact of the program TOP. This study analysed the impact on sedentary behavior in obese adolescents. Its secondary goal was to analyse the association between the sedentary activities and the body composition. This study’s results demonstrate that the weight and all the circumferences decreased significantly in sample’s participants, as well the sedentary time estimated by accelerometry, in response to the exercise program. There were not correlations between the alterations of the body composition and the modifications of the sedentary behavior.

Background: The high prevalence of physical inactivity worldwide calls for innovative and more effective ways to promote physical activity (PA). There are limited objective data on the effectiveness of Web-based personalized feedback on increasing PA in adults. Objective: It is hypothesized that providing personalized advice based on PA measured objectively alongside diet, phenotype, or genotype information would lead to larger and more sustained changes in PA, compared with nonpersonalized advice. Methods: A total of 1607 adults in seven European countries were randomized to either a control group (nonpersonalized advice, Level 0, L0) or to one of three personalized groups receiving personalized advice via the Internet based on current PA plus diet (Level 1, L1), PA plus diet and phenotype (Level 2, L2), or PA plus diet, phenotype, and genotype (Level 3, L3). PA was measured for 6 months using triaxial accelerometers, and self-reported using the Baecke questionnaire. Outcomes were objective and self-reported PA after 3 and 6 months. Results: While 1270 participants (85.81% of 1480 actual starters) completed the 6-month trial, 1233 (83.31%) self-reported PA at both baseline and month 6, but only 730 (49.32%) had sufficient objective PA data at both time points. For the total cohort after 6 months, a greater improvement in self-reported total PA (P=.02) and PA during leisure (nonsport) (P=.03) was observed in personalized groups compared with the control group. For individuals advised to increase PA, we also observed greater improvements in those two self-reported indices (P=.006 and P=.008, respectively) with increased personalization of the advice (L2 and L3 vs L1). However, there were no significant differences in accelerometer results between personalized and control groups, and no significant effect of adding phenotypic or genotypic information to the tailored feedback at month 3 or 6. After 6 months, there were small but significant improvements in the objectively measured physical activity level (P<.05), moderate PA (P<.01), and sedentary time (P<.001) for individuals advised to increase PA, but these changes were similar across all groups. Conclusions: Different levels of personalization produced similar small changes in objective PA. We found no evidence that personalized advice is more effective than conventional “one size fits all” guidelines to promote changes in PA in our Web-based intervention when PA was measured objectively. Based on self-reports, PA increased to a greater extent with more personalized advice. Thus, it is crucial to measure PA objectively in any PA intervention study.

Introduction: Sedentary behavior is characterized by individuals who designed much of the day sitting or lying down activities, even if such conduct regular physical activity. Over the years people tend to be less active, worsening the quality of life. For healthy aging is necessary for the individual to be constantly active, and moving activities improve mood and quality of life, though little is known of the influence of sedentary behavior in these variables. Aim: To investigate the association between sedentary time, quality of life and mood states of elderly women engaged in physical activity. Methodology: The study included 68 elderly women who belong to the Physical Activity Program for the Elderly (PROFIT), for at least 6 months. These answered the Sedentary Behaviour Questionnaire, the Pentacle Welfare and IPAQ Short Version. The Pep States List Reduced and Illustrated (LEA-RI) was applied before and after a workout and before and after three times of sedentary behaviors. To evaluate the difference of sedentary behavior time average for each domain of quality of life was conducted the analysis of the 95% confidence interval for the STATA program version 12.0 and to the moods list data a non-parametric test - test binomial using SPSS version 17.0 program and has adopted a p≤0,05. Results: A moderate intensity physical activity was the most practiced (939.5 ± 650.2 min / week) and the activity most frequently performed in sedentary time was watching television (187.6 ± 96,4min / day). In general, the quality of life for the elderly is considered good (98.39%) and the disease is most prevalent hypertension (29.47%). After a physical activity older felt less useless, less shy and less fear and after a sedentary behavior, felt less agitated and less useless. In relation between the domains of QOL and sedentary behavior time, there were no statistical differences. Conclusion: Elderly active even after a sedentary behavior...

Study 1: Schools provide a range of opportunities for youth to be active, however, over the past decade, these opportunities have been declining. Sports teams are a promising venue to promote physical activity yet limited research has examined the gender an ethnic differences in sport participation. The purpose of this study is to examine trends in sport participation from 1991-2009 among US high school students. Secondly, we examined the association between gender and ethnicity with sports over time. This serial cross-sectional study used surveillance data from the Youth Risk Behavior Survey, a probability based sample weighted to represent gender and race/ethnic subpopulations of US high school students. The findings of this paper reveal persistent gender and ethnic disparities for sports participation among US youth. Since sports teams may provide a substantial source of physical activity, greater efforts should be undertaken to increase the participation of girls, especially minorities, in sports teams. ^ Study 2: Sports team participation is congruent with teaching and supporting healthy eating, yet limited research has examined the association between sports participation and dietary behaviors. This study aims to determine the association between youth sports participation and dietary behaviors among elementary-aged children. Significant dose-response associations were observed between number of sports teams and consumption of most fruits and vegetables. The likelihood of eating fruit for boys increased with the number of sports teams (1 team: OR=1.89; 3 teams: OR=3.44, p<0.001) and the likelihood of consuming green vegetables for girls was higher with the number of sports teams (1 team: OR=1.50; 3 teams: OR=2.39; p<0.001). For boys, the odds of consuming fruit-flavored drinks was higher ( p=0.019) and the odds of drinking soda was lower (p=0.018) with participation in increasing number of sports teams whereas for girls, sports participation was positively associated with diet soda consumption (p=0.006). ^ Study 3: Parents and peers have been shown to have a strong influence over the physical activity, dietary, and sedentary behaviors of youth. Youth sports teams have the potential to offer physical activity, displace sedentary behaviors, and promote a healthy diet. The purpose of this study is to assess how peer and parental support for physical activity and healthy eating, coupled with sport participation, is associated obesity related risk factors including diet and sedentary behaviors. A secondary analysis of data from the School Physical Activity and Nutrition study, a state-representative survey, was conducted. Eighth (n=3,931) and 11th (n=2,785) grade students were categorized into four groups based upon the level of peer and parental support derived from a three item scale and their participation in sports (sports/high support, sports/low support, no sports/high support, no sports/low support). Linear models were conducted to determine the difference in means between these groups for the following outcome variables: previous day fruit and vegetable intake, scores for an unhealthy and healthy food index, and hours spent watching television, playing video games, and working on a computer. Eighth graders had significantly greater levels of parental support for healthy eating and physical activity compared to 11th grade. Both 8 th and 11th graders in the sport/high support for healthy eating from peers and parents scored significantly higher on the healthy food index than other groups. Eighth and 11th graders in the sport/high support for physical activity from peers participated in fewer hours of sedentary behaviors than any other group (p ≤ 0.032). Although it is thought that sport participation may offer opportunities to support a healthy diet and displace sedentary time by offering providing physical activity, our study found that parental and peer support for activity and healthy eating may further attenuate this association. Parents and peer support should be an important target when developing strategies to improve healthy diets and reduce sedentary time among youth, especially in the context of youth sports. Introdução: O deslocamento ativo tem estreita relação com problemas de saúde pública da atualidade e sua promoção pode contribuir para melhorias quanto à mobilidade urbana, estado de saúde e proteção do meio ambiente. Entretanto, a maior parte das pesquisas sobre o tema tem sido desenvolvida em países de renda alta. A presente tese busca ampliar a investigação sobre o deslocamento ativo no Brasil. Objetivos: i) Descrever a frequência, a distribuição e a variação temporal de indicadores do deslocamento ativo em populações brasileiras; ii) Avaliar o impacto de mudanças no padrão de transporte da população sobre o deslocamento ativo, o tempo sedentário e desfechos de saúde em populações brasileiras. Métodos: Tese composta por sete manuscritos. O primeiro apresenta revisão sistemática de estudos com informações sobre a prática de deslocamento ativo na América Latina e Caribe; o segundo descreve estimativas representativas da população brasileira sobre a prática de deslocamento ativo para o trabalho; o terceiro e o quarto descrevem a frequência e tendência temporal do deslocamento ativo na Região Metropolitana de São Paulo (ciclistas e escolares); o quinto discute a questão da mobilidade urbana e do direito à cidade em São Paulo; o sexto e o sétimo avaliam o impacto de mudanças no padrão de mobilidade da metrópole paulistana sobre a prática de deslocamento ativo, tempo não-ativo de deslocamento e tempo total de deslocamento, bem como sobre a poluição do ar e saúde da população. Resultados: A prevalência mediana de deslocamento ativo encontrada em diferentes locais do Brasil foi de 12 por cento , variando entre 5,1 por cento em Palmas (Tocantins) a 58,9 por cento em Rio Claro (São Paulo) (Manuscrito 1). Um terço dos homens e das mulheres desloca-se a pé ou de bicicleta de casa para o trabalho no país. Em ambos os sexos, esta proporção diminui com o aumento da renda e da escolaridade e é maior entre os mais jovens, entre os que residem em áreas rurais, e na região Nordeste. Em todas as regiões metropolitanas estudadas, o quinto das pessoas de menor renda apresenta uma maior frequência de deslocamento ativo (Manuscrito 2). Entre os anos de 2007 e 2012, observamos redução no número de ciclistas em São Paulo e diferenças expressivas na proporção de ciclistas entre homens e mulheres (9,7 por mil habitantes versus 1,4 por mil habitantes em 2012) (Manuscrito 3). Também verificamos uma queda na proporção de crianças que se deslocam ativamente para a escola entre os anos de 1997 e 2012 (Manuscrito 4). O cenário epidemiológico do deslocamento ativo no país é resultante da disputa pelo direito à cidade, com repercussões na transição de mobilidade humana e na saúde e qualidade de vida da população, como podemos observar no caso de São Paulo (Manuscrito 5). A construção de uma São Paulo mais inclusiva, com menores distâncias para os deslocamentos cotidianos e maior frequência de caminhada e bicicleta, levaria à substancial redução do tempo total e do tempo sedentário despendidos nos deslocamentos, sem diminuir a duração do deslocamento ativo (Manuscrito 6). Traria também ganhos à saúde da população, sobretudo pelo aumento da prática de atividade física e da redução da poluição do ar (Manuscrito 7). Conclusões: A prática de deslocamento ativo no Brasil apresenta marcadas diferenças segundo região e características sociodemográficas. De um modo geral, esta prática vem diminuindo no país, o que deve contribuir negativamente para a saúde da população. A promoção de cidades mais inclusivas e compactas, com o favorecimento a modos ativos de deslocamento, pode contribuir para reverter esta preocupante tendência.
