A cicatrização de feridas cutâneas visa restaurar a integridade da pele, objetivando um fechamento rápido da lesão e a formação de uma cicatriz funcional e esteticamente satisfatória. Diversos modelos in vivo têm sido estudados a fim de caracterizar diversos componentes e mecanismos envolvidos no reparo tecidual e avaliar os efeitos de potenciais compostos terapêuticos que aceleram o processo de cicatrização. O óleo extraído da gordura subcutânea de capivara possui certas propriedades, entre elas está o seu uso em ferimentos externos com o intuito de acelerar o processo cicatricial, embora tal evento não seja confirmado em pesquisas experimentais. O presente estudo teve como objetivo investigar os possíveis efeitos da aplicação tópica de óleo de capivara em feridas cutâneas induzidas em camundongos Swiss, avaliando sua interferência macro e microscópica no processo de cicatrização das lesões. A lesão foi realizada no dorso dos animais, que receberam aplicação tópica do óleo de capivara (0,1 ml) durante 3, 7, 14 e 21 dias de pós-operatório (PO), conforme protocolo estabelecido. Ao final dos dias, foi realizada eutanásia dos animais e fragmentos de lesão e pele adjacente foram coletados e processados para a microscopia de luz. Cortes histológicos da amostra foram corados com hematoxilina e eosina, azul de toluidina, picro sirius red, resorcina fucsina de Weigert, e imunomarcadas para detecção de antígeno nuclear de proliferação celular. Foram analisados macroscopicamente o aspecto geral da lesão, contração da lesão e reepitelização, e microscopicamente a espessura da neoepiderme, quantidade de leucócitos polimorfonucleares (PMN) e mastócitos, proliferação celular na epiderme e derme, e avaliação das fibras de colágeno e do sistema elástico-microfibrilar na matriz extracelular da derme cicatricial. Os animais tratados topicamente com o óleo de capivara mostraram, comparados ao grupo controle que não recebeu tratamento, melhor desenvolvimento da crosta de fibrina nas fases iniciais da cicatrização e desaparecimento da mesma antes do 14 dia PO; maior contração e reepitelização da área da lesão, bem como neoepiderme mais espessa em 7 dias PO; aumento do número de leucócitos PMN em 3 dias PO e sua gradativa redução nos dias subsequentes e diminuição do número de mastócitos em 21 dias PO. Além disso, no grupo tratado com óleo de capivara foi observada uma benéfica modulação das fibras colágenas e elásticas presentes na matriz extracelular da derme, apresentando aceleração da deposição de fibras de colágeno do tipo I e melhor organização dessas fibras no tecido, bem como aceleração da elastogênese com o aparecimento das fibras do sistema elástico-microfibrilar a partir do 7 dia PO. Mesenchymal stem cells (MSCs) are currently under investigation as repair agents in the preservation of cardiac function following myocardial infarction (MI). However concerns have emerged regarding the safety of acute intracoronary (IC) MSC delivery specifically related to mortality, micro-infarction and microvascular flow restriction post cell therapy in animal models. This thesis aimed to firstly identify an optimal dose of MSC that could be tolerated when delivered via the coronary artery in a porcine model of acute MI (AMI). Initial dosing studies identified 25x106 MSC to be a safe MSC cell dose, however, angiographic observations from these studies recognised that on delivery of MSC there was a significant adverse decrease in distal blood flow within the artery. This observation along with additional supportive data in the literature (published during the course of this thesis) suggested MSC may be contributing to such adverse events through the propagation of thrombosis. Therefore further studies aimed to investigate the innate prothrombotic activity of MSC. Expression of the initiator of the coagulation cascade initiator tissue factor (TF) on MSC was detected in high levels on the surface of these cells. MSC-derived TF antigen was catalytically active, capable of supporting thrombin generation in vitro and enhancing platelet-driven thrombus deposition on collagen under flow. Infusion of MSC via IC route was associated with a decreased coronary flow reserve when delivered but not when coadministered with an antithrombin agent heparin. Heparin also reduced MSC-associated in situ thrombosis incorporating platelets and VWF in the microvasculature. Heparin-assisted MSC delivery reduced acute apoptosis and significantly improved infarct size, left ventricular ejection fraction, LV volumes, wall motion and scar formation at 6 weeks post AMI. In addition, this thesis investigated the paracrine factors secreted by MSC, in particular focusing on the effect on cardiac repair of a novel MSC-paracrine factor SPARCL1. In summary this work provides new insight into the mechanism by which MSC may be deleterious when delivered by an IC route and a means of abrogating this effect. Moreover we present new data on the MSC secretome with elucidation of the challenges encountered using a single paracrine factor cardiac repair strategy.

Retinal ischaemic disorders such as diabetic retinopathy and retinal vein occlusion are common. The hypoxia-related stimuli from oxygen-deprived neural and glial networks can drive expression of growth factors and cytokines which induce leakage from the surviving vasculature and/or pre-retinal and papillary neovascularisation. If left untreated, retinal vascular stasis, hypoxia or ischaemia can lead to macular oedema or fibro-vascular scar formation which are associated with severe visual impairment, and even blindness. Current therapies for ischaemic retinopathies include laser photocoagulation, injection of corticosteroids or VEGF-antibodies and vitreoretinal surgery, however they carry significant side effects. As an alternative approach, we propose that if reparative intra-retinal angiogenesis can be harnessed at the appropriate stage, ischaemia could be contained or reversed. This review provides evidence that reperfusion of ischaemic retina and suppression of sight-threatening sequelae is possible in both experimental and clinical settings. In particular, there is emphasis on the clinical potential for endothelial progenitor cells (EPCs) to promote vascular repair and reversal of ischaemic injury in various tissues including retina. Gathering evidence from an extensive published literature, we outline the molecular and phenotypic nature of EPCs, how they are altered in disease and provide a rationale for harnessing the vascular reparative properties of various cell sub-types. When some of the remaining questions surrounding the clinical use of EPCs are addressed, they may provide an exciting new therapeutic option for treating ischaemic retinopathies. Objectives: Unlike adult dermal wounds, the oral mucosa demonstrates preferential healing characterized by rapid remodeling and re-epithelialisation, with minimal scar formation. Secretory leukocyte protease inhibitor (SLPI) is an epithelial-derived factor with potential for promoting scarless repair. The aims of this study were to: (i) investigate the directed migratory (chemotaxis) response of oral and skin fibroblasts to various concentrations of SLPI; and (ii) compare migratory speed of the two cell types. Methods: Paired oral and skin fibroblasts were seeded at 2x104 cells in six well plates containing glass coverslips, and cultured in DMEM supplemented with 10% FCS for 48hours. Following a period of serum starvation (18hours in DMEM plus 0.5% BSA), coverslips were incorporated within a Dunn chemotaxis chamber containing DMEM with 0.5% BSA +/- SLPI gradients at 0.5, 1 or 2µM concentrations. Using microscopy, the migratory behaviour of cells was digitally captured every 10mins for 18hours, traced with JCell tracking software and resulting co-ordinates statistically analysed using Mathmatica software. Results: At all concentrations SLPI was a significant chemoattractant (p<0.01) for both cell types. However, skin fibroblasts migrated significantly faster than oral cells at each SLPI concentration, with greatest effect observed at the highest dose (skin: 32.0±0.47µm/hr, oral: 13.6±0.23µm/hr). Conclusion: SLPI is a chemoattractant for both oral and skin fibroblasts, and may play an important role in fibroblast recruitment during wound healing. Les urodèles amphibiens, dont fait partie l’axolotl (Ambystoma mexicanum), ont la capacité de régénérer leurs organes et membres suite à une amputation, tout au long de leur vie. La patte est l’organe dont le processus de régénération est le mieux caractérisé et ce dernier est divisé en deux phases principales. La première est la phase de préparation et commence immédiatement suite à l’amputation. Elle renferme des étapes essentielles au processus de régénération comme la guérison de la plaie et la formation d’une coiffe apicale ectodermique. Par la suite, les fibroblastes du derme et certaines cellules musculaires vont revenir à un état pluripotent via un processus appelé dédifférenciation cellulaire. Une fois dédifférenciées, ces cellules migrent et s’accumulent sous la coiffe apicale pour former le blastème. Lors de la phase de redéveloppement, les cellules du blastème se divisent puis se redifférencient pour régénérer la partie amputée. Fait intéressant, la régénération d’un membre ou la guérison d’une plaie chez l’axolotl ne mène jamais à la formation d’une cicatrice. Afin d’en apprendre plus sur le contrôle moléculaire de la régénération, les gènes Heat-shock protein-70 (Hsp-70) et Transforming growth factor-β1 (Tgf-β1) ont été sélectionnés. Ces gènes jouent un rôle important dans la réponse au stress et lors de la guérison des plaies chez les mammifères. HSP-70 est une chaperonne moléculaire qui est produite pour maintenir l’intégrité des protéines cellulaires lorsqu’un stress se présente. TGF-β1 est une cytokine produite suite à une blessure qui active la réponse inflammatoire et qui stimule la fermeture de la plaie chez les amniotes. Les résultats présentés dans cette thèse démontrent que Hsp-70 est exprimé et régulé lors du développement et de la régénération du membre chez l’axolotl. D’autre part, nos expériences ont mené à l’isolation de la séquence codante pour Tgf-β1 chez l’axolotl. Nos résultats montrent que Tgf-β1 est exprimé spécifiquement lors de la phase de préparation dans le membre en régénération. De plus, le blocage de la voie des Tgf-β avec l’inhibiteur pharmacologique SB-431542, lors de la régénération, mène à l’inhibition du processus. Ceci démontre que la signalisation via la voie des Tgf-β est essentielle à la régénération du membre chez l’axolotl.

Nos études ont démontrées que la formation de la cicatrice et la guérison sont associées avec l’apparition de cellules de type myocytes cardiaques nestine(+) dans la région péri-infarcie. Présentement, l’étude examine le mécanisme, tel que l’hypoxie ou les hormones neuronales, possiblement impliqué dans leur recrutement et de dévoiler leur origine cellulaire. La présence de ces cellules a été détectée dans les coeurs infarcies d’une semaine et maintenue après neuf mois suite à une sujétion coronaire complète. Aussi, ces cellules de type myocytes cardiaques nestine(+) ont été observées dans le coeur infarci humain. L’hypoxie représente un événement prédominant suite à un infarctus de myocarde, mais l’exposition des rats normaux à un environnement hypoxique n’a pas pu promouvoir l’apparition de ces cellules. Autrement, l’infusion de l’agoniste -adrénergique non-sélectif isoprotérénol (ISO) dans les rats adultes Sprague-Dawley a augmenté la protéine nestine dans le ventricule gauche et a été associé avec la réapparition de cellules de type myocytes cardiaques nestine(+). Cela représente possiblement un effet secondaire suite à la nécrose des myocytes cardiaques par l’administration d’isoprotérénol. Dernièrement, on a identifié une sous-population de cellules nestine(+) dans le coeur normal du rat qui co-exprime les marqueurs de cellules cardiaques progénitrices Nkx-2.5 et GATA-4. Cette sous-population de cellules nestine/Nkx-2.5/GATA-4 pourrait représenter des substrats cellulaires qui puissent se différentier en cellules de type myocytes cardiaques nestine(+) suite à une ischémie. Suite à un infarctus du myocarde, la formation d’une cicatrice, nommée fibrose de réparation, représente un processus adaptatif et essentiel empêchant la rupture du myocarde. La cicatrice est constituée de myofibroblastes, de cellules vasculaires, de fibres sympathiques ainsi que de cellules souches neuronales cardiaques exprimant la nestine. Une perturbation au niveau de ces constituants cellulaires résulte en une formation maladaptative de la cicatrice et éventuellement, une diminution de la fonction cardiaque. La compréhension des événements cellulaires ainsi que les mécanismes sous-jacents participant à cette fibrose est alors d’une importance primordiale. Cette thèse est axée sur l’identification du rôle du système sympathique et des cellules souches neuronales cardiaques exprimant la nestine dans la formation de la cicatrice ainsi que leur interaction potentielle. Nos travaux examinent l’hypothèse que les cellules souches neuronales exprimant la nestine sont endogènes au cœur et que suite à un dommage ischémique, elles contribuent à la réponse angiogénique et à la réinnervation sympathique du tissu lésé. Les cellules souches neuronales exprimant la nestine sont retrouvées dans les cœurs de différentes espèces incluant le cœur infarci humain. Elles sont résidentes dans le cœur, proviennent de la crête neurale lors du développement et sont intercalées entre les cardiomyocytes n’exprimant pas la nestine. Suite à leur isolation de cœurs infarcis de rats, les cellules souches neuronales cardiaques prolifèrent sous forme de neurosphères et, dans des conditions appropriées in vitro, se différencient en neurones exprimant le neurofilament-M. Suite à un infarctus du myocarde, les niveaux de l’ARNm de nestine sont significativement augmentés au niveau de la région infarcie et non-infarcie. Nos résultats suggèrent que cette augmentation de l’expression de nestine dans la cicatrice reflète en partie la migration des cellules souches neuronales cardiaques exprimant la nestine de la région non-infarcie vers la région infarcie. Lors de la fibrose de réparation, ces cellules représentent un substrat cellulaire pour la formation de nouveaux vaisseaux et contribuent aussi à la croissance des fibres sympathiques dans la région infarcie. Finalement, nous démontrons que la formation de la cicatrice est associée à une innervation sympathique de la région infarcie et péri-infarcie. De plus, les fibres sympathiques présentes dans la région infarcie sont observées à proximité de vaisseaux de petits calibres. Ces données suggèrent indirectement que l’innervation de la cicatrice par les fibres sympathiques peut jouer un rôle dans la réponse angiogénique suite à un infarctus du myocarde. Suite à l’administration du corticostéroïde dexaméthasone, nous détectons un amincissement de la cicatrice, associé à une réduction significative des fibres sympathiques exprimant le neurofilament-M dans la région infarcie et péri-infarcie. La diminution de la densité de ces fibres par le dexaméthasone peut être reliée à une diminution de la prolifération des myofibroblastes et de la production de l’ARNm du facteur neurotrophique nerve growth factor.

L’infarctus du myocarde est une des conséquences possibles de l’ischémie cardiaque; il se traduit par la mort des cardiomyocytes se situant en aval du blocus coronaire, puis par la formation d’une cicatrice formée essentiellement de dépôts de matrices extracellulaires sécrétées par les myofibroblastes. Nestine est une protéine filamenteuse intermédiaire de classe VI couramment associée à la prolifération et à la migration cellulaire. Chez l’homme et les rongeurs, à la suite d’un infarctus du myocarde, une sous-population de cardiomyocytes localisée à la zone infarcie/péri-infarcie exprimait la forme striée de nestine. Le but principal de cette étude était de déterminer la source cellulaire des cardiomyocytes nestine (+) observée dans le cœur infarci ainsi que le mécanisme de signalisation cellulaire sous-jacent impliqué dans l’expression de nestine. L’utilisation de souris transgénique a révélé que l’augmentation des cardiomyocytes nestine (+) dans le cœur infarci des souris n’était pas attribuable à la différenciation de cellules souches/progénitrices nestine (+) en cardiomyocytes nestine (+). Le traitement des cardiomyocytes ventriculaires de rats néonataux avec l’activateur des protéines kinases C PDBu et l’inhibition concomitante des voies p38 MAPK a mené à l’augmentation du nombre de ces cellules exprimant nestine. De plus, une population importante de cardiomyocytes ventriculaires de rats néonataux a incorporé la bromodéxoxyuridine, signe d’une capacité à réentrer dans le cycle cellulaire et à synthétiser de l’ADN. Sur la base de ces observations, l’apparition de cardiomyocytes nestine (+) dans le cœur infarci des rongeurs et des hommes pourrait possiblement refléter une sous-population de cardiomyocytes en prolifération tentant de régénérer le cœur infarci.

With the exception of the cleft lip, developmental defects (DD) of the lip are rare. The upper lip originates from the ectomesenchyme and is formed by the merging of the nasal medial and lateral processes with the maxillary process. Disturbances during this formation period can cause DD with functional and/or esthetic repercussions. We present a case of DD of the upper lip in a patient with a history of progressive growth of the left lateral portion of the upper lip that occurred from the time of birth until the age of 22 years. Clinical examination revealed hypertrophy of the area from the left philtral columns to the left commissure of the lip, extending the portion of the surface mucosa creating a flaccid and asymptomatic tissue mass. All other buccal structures appeared to be within normal limits and without any evidence of defects or deformities. In the surgical planning we decided to carry out corrective surgery in two phases. The first phase accomplished a conservative excision of the total abnormal labial tissue mass with a CO2 laser radiation (5 W in continuous mode, bunch diameter φ = 0.6 mm with a power density of 768 W/cm2 and fluency of 0.231 J/cm2) being careful to preserve the vermilion portion of the lip. Postsurgical clinical evaluations were done every three days until the skin sutures were removed and then every seven days until two months post surgery. While the entire mass of excessive tissue could not be completely removed, the removal of the excessive mucosal tissue produced a very good outcome relative to lip function, with a good esthetic result without scarring, and good tissue mobility. The results showed that the CO2 laser is an extremely useful instrument that can provide excellent control of the surgical field and allow for healing that produces excellent functional and esthetic results. © 2005 Taylor & Francis.

Purpose: To determine the prevalence of trachoma in Sao Gabriel da Cachoeira (SGC), the only urban community of the upper Rio Negro Basin of the Amazon state in Brazil, near the Colombian border, and to investigate the risk factors associated with the active forms of the disease. Methods: A total of 1702 people (440 children up to 9 years and 1069 adults aged 15 years and above) were examined. The sample was selected from a probabilistic household sampling procedure based on census data and a previous study of trachoma prevalence in Sao Gabriel da Cachoeira. A two-stage probabilistic household cluster sample was drawn. Household units were randomly selected within each cluster. A variety of socioeconomic and hygiene variables were studied in order to determine the risk factors for active trachoma in a household. Results: The total prevalence of trachoma was 8.9%. Prevalence of active trachoma (TF and/or TI) in children aged 1-9 years was 11.1% and trachomatous trichiasis in adults aged 15 years and above was 0.19%. Trachomatous scarring reached a peak of 22.4% for subjects between 50 to 60 years of age. Corneal opacity occurred in subjects aged 50 years and older with a prevalence of 2.0%. No sex effect was found on the overall prevalence of trachoma in SGC. Risk factors associated with active trachoma were mainly related to poor socioeconomic indicators. Conclusions: Despite the ubiquitous presence of water, the analysis of the risk factors associated with the active forms of the disease supports the idea that a low personal standard of hygiene and not water availability per se, is the key factor associated with trachoma. In this study, biomembrane of natural latex was utilized to replace a section of the stomach wall of New Zealand rabbits, adult and non-castrated males (n=12), in order to evaluate the tissue repair process in regards to its biocompatility, scar formation ability and possible complications. The animals were euthanized at fifteen, 30 and 60 days post operation, by use of sodium thiopental (200mg kg-1), followed by macroscopic and histopathological analysis of the implant interface with the native tissue. Macroscopically, at fifteen, 30, and 60 days post operation adherence was observed in the serosal wall. At 60 days post operation, the biomembrane is not in the stomach. Under light microscopy, at fifteen and 30 days, discontinuity of muscle layer and mucosa layer, and presence of polimorfonuclear population of inflammatory cells was observed. New vessels and muscle fibers were observed. At 60 days, the mucosa and muscle layers were complete reconstituted. The implants were biocompatible and had provided the mainframe for orientation and development of the tissue layers through repairing processes, thus reestablishing the organ structure.
